OBJECTIVETo search for A3243G point mutations in mitochondrial DNA (mtDNA) from 10 cases of mitochondrial encephalomyopathy, lactic acidosis and strokelike episodes (MELAS).
METHODSUsing PCR-restriction analysis, we investigated A3243G point mutations in mtDNA of muscle and/or blood cells from 10 patients and their 8 maternal relatives. We also quantitated the A3243G mtDNA in samples harboring the mutation.
RESULTSA3243G point mutations were identified in all muscle and/or blood samples from 10 MELAS patients. The proportion of mutant mtDNA was 10.8%-47.8% in blood (7 cases), and 39.4%-67.7% in muscle (5 cases). This ratio was invariably higher in muscle than in blood from two patients whose blood and muscle samples were both available. Younger patients usually carried higher proportions of A3243G mutant mtDNA in blood. Eight maternal relatives from 6 families were also examined. Maternal transmission of the disease could be identified in one family. No A3243G point mutations were found in mothers' blood from 3 families and siblings' blood from 2 families.
CONCLUSIONSAll 10 MELAS patients were found to have the mtDNA A3243G mutation in their muscle and/or blood. The A3243G mutation seems to be sporadic in 5 of the families examined, suggesting the mechanism of de novo mutation for the pathogenesis of their MELAS syndrome.

Objective To explore the relationship between nutritional risk and clinical outcome in hospitalized patients with inflammatory bowel disease ( IBD ); to analyze the factors influencing the nutritional risk of IBD patients, so that to provide theoretical and practical basis for further study, prevention and treatment of the malnutrition in IBD patients. Methods A total of 93 hospitalized patients with IBD at gastroenterology department in the Third Affiliated Hospital of Soochow University, Changzhou Second Hospital affiliated to Nanjing Medical University and Changzhou Hospital affiliated to Nanjing University of Traditional Chinese Medicine from January to July in 2014 were enrolled into the study. Informed consent forms were signed and case datum of patients were collected within 24 hours after hospitalization, and patients′nutritional status were completed with Nutritional Risk Screening 2002.Results The hospitalization duration and medical expenses of group with nutritional risk were higher than that group without nutritional risk ( P<0.05);If we regarded the nutritional risk of hospitalized patients with IBD as the dependent variable, and other variables which might be associated with nutritional risk as independent variables, then conducted a binary logistic regression analysis. The results showed that: gender, diagnosis and disease severity were the independent risk factors of the nutritional risk of hospitalized patients with IBD(P<0.05), which means that women′s nutritional risk was higher than men;the nutritional risk of Crohn disease ( CD) was higher than that of ulcerative colitis ( UC ); the more serious of the disease, the higher incidence of nutritional risk; but nutritional risk had nothing to do with age and course of disease(P>0.05).Conclusions Nutritional risk can predict clinical outcomes; gender, diagnosis and disease severity are independent risk factors of the nutritional risk of hospitalized patients with IBD except age and course of disease.

International Journal of Pediatrics 2023;50(4):257-261
Due to the development of neonatal intensive care, the survival rate of very preterm infants and very low birth weight infants has been significantly improved, and the incidence of bronchopulmonary dysplasia (BPD) has been obviously increasing year by year. The pathogenesis of BPD has not been clear, it is considered that inflammation is an important link in the occurrence and development of BPD at present.Neutrophils can use their neutrophil extracellular traps (NETs) to capture and kill pathogens and reduce inflammation, but excessive formation of NETs is easy to induce inflammatory imbalance, so as to damage normal cells or tissues and participate in the pathophysiological process of BPD.This paper reviews the structure, formation, function and regulatory role of NETs in BPD, and the targeted treatment strategies and potential research directions of NETs in BPD.
