2.Study on the establishment of evaluation system for population & health informatization

Objective:The present study aims at setting up an evaluation index system aiming at providing a set of measurement tools for population & health informatization developing level for administrative departments. Meth-ods: Delphi method, analytic hierarchy process, Synthetic Index Method and some mathematical or statistical meth-ods were used respectively to select the primary indexes, calculate the weight and combined weight of each index, build up the synthetic evaluation mode,and test the evaluation system. Results:According to the results of this work, the evaluation index system has been established which is composed of 6 level 1 indexes, 16 level 2 indexes, and 33 level 3 indexes, and it covers 6 aspects, such as information resources construction, information infrastructure con-struction, information technology application, informatization talents and the information policy, planning, and stand-ards. The weights of these indexes were also calculated. The synthetic evaluation model has been built. Finally, the reliability, the validity and the discrimination of the evaluation system were tested. Conclusions:The evaluation index system has been established, which is able to be used in the evaluation, and to some extent guide the work of popula-tion & health informatization construction.
3.Stability of anticoagulant peptide Hirulog-S

Objective To study the stability of anticoagulant peptide Hirulog-S and its lyophilized product, and to provide data on the storage conditions and clinical applications.Methods RP-HPLC was used to determine the content and the related substances of Hirulog-S and its lyophilized powder with influence factor test, accelerated test and long-term storage test.Results Light, temperature and humidity had no significant effect on the stability of Hirulog-S and its lyophilized powder in the influence factor test.The content and related substances of Hirulog-S and its lyophilized powder did not significantly change in the accelerated test ( 40℃, RH75%) and 24-month long-term storage test at room temperature and 4℃.Conclusion Hirulog-S and its lyophilized product are very stable, even after being stored at room temperature for two years.
4.Incidence of metabolic syndrome in incipient female patients with rheumatoid arthritis

Objective To examine the incidence of metabolic syndrome (MS) in incipient female patients with rheumatoid arthritis (RA), and study the relationship between MS and disease activity of RA. Methods The clinical data of 102 incipient female RA patients (RA group) and 125 healthy controls (control group) were retrospectively analyzed. The incidence of MS was compared between 2 groups, and the clinical and laboratory data were analyzed between patients with MS and patients without MS. Results There were no statistical differences in age, body mess index (BMI), waist circumference, systolic blood pressure, diastolic blood pressure, fasting blood glucose (FBG), triglyceride (TG) and high-density lipoprotein cholesterol (HDL-C) between 2 groups (P>0.05). The incidences of waist circumference ≥ 80 cm, TG ≥ 1.70 mmol/L, HDL-C ≤ 1.30 mmol/L, MS according to the National Cholesterol Education Program Adult Treatment Panel Ⅲ (NCEP-ATPⅢ) standard and MS according to the Chinese Diabetes Society (CDS) standard in RA group were significantly higher than those in control group: 39.2% (40/102) vs. 25.6% (32/125), 37.3% (38/102) vs. 24.0%(30/125), 34.3%(35/102) vs. 21.6%(27/125), 34.3%(35/102) vs. 22.4%(28/125) and 27.5%(28/102) vs. 16.0% (20/125)%, and there were statistical differences (P<0.05). There were no statistical differences in the incidences of BMI≥25 kg/m2, systolic blood pressure ≥130 mmHg (1 mmHg = 0.133 kPa), diastolic blood pressure ≥85 mmHg and FBG≥5.6 mmol/L between 2 groups (P>0.05). The RA patients were divided into RA with MS (35 cases) and RA without MS (67 cases) according to the MS diagnostic standard of NCEP-ATPⅢ. The erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) and blood uric acid in RA with MS patients were significantly higher than those in RA without MS patients:(68.1 ± 26.3) mm/h vs. (40.1 ± 30.6) mm/h, 4.260 mg/L vs. 1.505 mg/L and (330.5 ± 69.1)μmol/L vs. (269.0 ± 67.5)μmol/L, and there were statistical differences (P<0.05). There were no statistical differences in age, involved joint number, rheumatoid factor (RF), hemoglobin and incidence of anti-cyclic citrullinated peptide antibody (CCP antibody) between RA with MS patients and RA without MS patients (P>0.05). Conclusions The incidence of MS in incipient female patients with RA is higher than that in healthy controls, and RA patients are more prone to abdominal obesity, TG increasing and HDL-C decreasing. The inflammatory index and blood uric acid levels in RA with MS patients are significantly higher than those in RA without MS patients.
5.Preparation of conotoxin GI antiserum and determination of neutralization activity

Objective To determine whether the antiserum produced by immunizing mice with conotoxin GI coupled with bovine serum albumin (BSA) could neutralize GI conotoxin.Methods The GI-BSA was prepared by glutaraldehyde-coupled method,and the mice were immunized with the GI-BSA to produce antiserum.The antibody neutralization assay was used to test the detoxication of the antiserum.Results The SDS-PAGE protein electrophoresis showed that the coupling reaction of GI hapten with BSA was successful.The two distinct protein bands of GI-BSA were more than 120×103.Each mouse was immunized four times with 99 μg every two weeks.After the fourth immunization,the serum neutralization titer was more than 1:64 000.After the intraperitoneal injection of the mixture of 100 or 200 μl of the antiserum and different doses of GI,75% of the mice survived in the group with 100 μl of the antiserum and 1× LD50 GI(16.3 μg/kg).The same percentage of mice also survived in the group of with 200 μl of serum and 25.8 μg/kg of GI.Conclusion The antiserum produced by immunizing mice with GI-BSA exhibits significant detoxication activity to conotoxin GI.
6.A study of the functional prognosis and influential factors of cerebral infractions in stroke unit

Objective To observe the prognostic factors of functional outcome of patients with cerebral infraction treated in stroke unit.Methods The data of patients with cerebral infractions treated in stroke unit in our hospital were prospectively collected from April 2004 to December 2005 continuously.All the patients They were fol lowed up either by their regular visits to our outpatient clinics or by phone interview.The modified Rankin scale(mRS)score was used as the index of primary outcome.The prognostic factors of the functional outcome were analyzed by monovariate and multivariate logistic regression.Results A total of 150 cases were collected,of whom 98 were with good outcome,52 cases with poor outcome.The multivariate logistic regression showed that the poor outcome was associated with aging,previous history of stroke,high NIHSS scores,complications,and time from onset of the disease to admission to stroke unit.Conclusion The results suggested that older age,having stroke history,higher NIHSS total scores,having complications and delayed hospitalized time were influence factors to functional outcome of cerebral infractions in stroke unit.
7.Development of HIS-based liver disease information and diagnosis database system and its application

Objective To help the rapid diagnosis and treatment of liver disease by intelligent diagnosis with information technology. Methods Liver disease diagnosis and treatment information database was established by screening the diagnosis and treatment indexes of liver disease based on HIS database, and different neural network modules were developed to establish the model parameters and realize the intelligent diagnosis function for liver disease. Results The different neural network modules could provide frontier, practical, and effective intelligent information for the clinical diagnosis and treatment of liver disease and its clinical research. Conclusion The HIS-based liver disease information and diagnosis database system that can effectively mine the data resources of HIS database not only im-proves the clinical diagnosis and treatment efficacy of liver disease but also meets the needs of scientific research, and is thus a success of developing HIS database.
8.Study on Anti-inflammatory Effects and Mechanism of Petroleum Ether Extract from Citrullus lanatus Vine

This article was aimed to study anti-inflammatory effects of petroleum ether extract from Citrullus lanatus vine (PEECLV) and explore the mechanism. The methods of ear swelling with xylene, increasing of peritoneal capil-lary permeability, and paw edema with egg albumin were used in the establishment of mice models, respectively. Granuloma with cotton pellet, and paw edema with carrageenan were used in the establishment of rat models, re-spectively. Observation was made on anti-inflammatory effects of PEECLV. The content of nitrous oxide (NO) and nitric oxide synthase (NOS), as well as the level of TNF-α and IL-1β in blood serum were measured among granu-loma with cotton pellet rat models. Contents of malondialdehyde (MDA), prostaglandin E2 (PGE2), 5-HT, histamine (His), and protein in inflammatory exudates from the paw edema with carrageenan rat model, as well as contents of NO and NOS in blood serum, and activity of superoxide dismutase (SOD) were measured in order to study the possi-ble anti-inflammatory effects. The results showed that PEECLV can suppress the ear swelling with xylene, the in-creasing of peritoneal capillary permeability, and the paw edema with egg albumin in mice, as well as granuloma with cotton pellet, paw edema with carrageenan in rats. It had anti-inflammatory effects at different degrees. The content of NO and NOS, as well as the level of TNF-α and IL-1β in blood serum were reduced in granuloma with cotton pellet rat models. The content of MDA, PGE2, 5-HT, His, and protein content were reduced in inflammatory exudates from the paw edema with carrageenan rat model. The increasing of NO and NOS in blood serum was inhib-ited. And the activity of SOD was increased. It was concluded that PEECLV had certain anti-inflammatory effects. Its mechanism may be related to the influence of the production of inflammatory mediators and antioxidant.
9.Development of three Drosophila melanogaster strains with different sensitivity to volatile anesthetics.

BACKGROUNDThe mechanisms of action for volatile anesthetics remain unknown for centuries partly owing to the insufficient or ineffective research models. We designed this study to develop three strains derived from a wild-type Drosophila melanogaster with different sensitivities to volatile anesthetics, which may ultimately facilitate molecular and genetic studies of the mechanism involved.
METHODSMedian effective doses (ED(50)) of sevoflurane in seven-day-old virgin female and male wild-type Drosophila melanogaster were determined. The sensitive males and females of percentile 6 - 10 were cultured for breeding sensitive offspring (S(1)). So did median ones of percentile 48 - 52 for breeding median offspring (M(1)), resistant ones of percentile 91 - 95 for breeding resistant offspring (R(1)). Process was repeated through 31 generations, in the 37th generation, S(37), M(37) and R(37) were used to determine ED(50) for enflurane, isoflurane, sevoflurane, desflurane, halothane, methoxyflurane, chloroform and trichloroethylene, then ED(50) values were correlated with minimum alveolar concentration (MAC) values in human.
RESULTSFrom a wild-type Drosophila melanogaster we were able to breed three strains with high, median and low sevoflurane requirements. The ratio of sevoflurane requirements of three strains were 1.20:1.00:0.53 for females and 1.22:1.00:0.72 for males. Strains sensitive, median and resistant to sevoflurane were also sensitive, median and resistant to other volatile anesthetics. For eight anesthetics, ED(50) values in three strains correlated directly with MAC values in human.
CONCLUSIONSThree Drosophila melanogaster strains with high, median and low sensitivity to volatile anesthetics, but with same hereditary background were developed. The ED(50) are directly correlated with MAC in human for eight volatile anesthetics.

10.Multicentric randomized double blinded clinical study of Yiqi Tongmai Oral Liquid against angina pectoris in patients with coronary heart disease

OBJECTIVE: To study the efficacy and safety of Yiqi Tongmai Oral Liquid (YQTM), a traditional compound Chinese herbal medicine, in treating angina pectoris in patients with coronary heart disease. METHODS: A multicentric, randomized, double blinded and paralleled controlled trial was conducted on 110 patients in trial group treated with YQTM, and 109 patients in control group treated with Shuxin Oral Liquid (SX). Cure and effective rates in both groups were evaluated. Frequency and duration of angina attack were counted and measured. Coronary angiography (CAG), electrocardiogram (ECG) and flat exercise test were taken in both groups. Blood lipid indexes, such as cholesterol (CH), triglyceride (TG), low-density lipoprotein (LDL), high density lipoprotein (HDL), were determined at pre- and post-treatment. The hemodynamic indexes, such as whole blood viscosity (J2), high-shear reduced viscosity (Eh), low-shear reduced viscosity (Ei), red cell aggregation index (Lb), red cell rigidity index (Rh), fibrinogen (Fb), blood sedimentation rate (BSR) and hematocrit (HCT), were determined at pre-and post-treatment. The indicated scores of symptoms and signs of traditional Chinese medicine (TCM) pattern, such as chest pain, chest constriction, breath shortness, palpitation, fatigue, dim complexion, spontaneous perspiration and tongue proper, tongue coating were evaluated in week 0, 1, 2, 3, 4 during the treatment course. The safety indexes, such as body temperature, pulse, respiration and blood pressure were observed. Routine tests of blood, urine and stool, hepatic function test and renal function test were taken at pre- and post-treatment. RESULTS: There was no significant difference between the total effective rate of the trial group and that of the control group, which were 91.82% and 85.32%, respectively (P>0.05). Trial groups percentile of cure rate is significantly higher than that of the control group (P<0.01). The frequency and duration of angina attack, the positive ratio of CAG and flat exercise test of both groups were lowered, while the effect of the trial group on frequency and duration of angina attack was better. No significant difference was found in ECG features between the two groups (P>0.05). The levels of CH, TG and LDL of both groups were lowered significantly (P<0.05). The effect of lowering CH, TG and LDL of the trial group was stronger than that of the control group (P<0.05). The hemodynamic indexes, such as J2, Eh, Ei, Lb, Rh, Fb, BSR and HCT were improved significantly in both groups (P<0.05). The improvements of J2, Eh, Ei, Lb, Rh, Fb and SR in the trial group were greater than those of control group (P<0.05). The TCM symptoms and signs, such as chest pain, chest constriction, breath shortness, palpitation, fatigue, dim complexion, spontaneous perspiration were improved significantly in both groups (P<0.05). The improvements of chest constriction, palpitation, fatigue and spontaneous perspiration in the trial group were greater than those of the control group (P<0.05). The total indicated score of TCM symptoms and signs was lowered more significantly than that of the control group (P<0.01). No significant changes were found at pre- and post-treatment in safety indexes, such as routine tests for blood, urine and stool, hepatic function test and renal function test. There was no significant difference in safety features of both groups (P>0.05). CONCLUSION: Yiqi Tongmai Oral Liquid bears good therapeutic effect on angina pectoris without adverse reaction, and is superior to Shuxin Oral Liquid. Yiqi Tongmai Oral Liquid is a new effective and safe medicine for the treatment of angina pectoris in patients with coronary heart disease.
