Objective To compare the clinical characteristics of community-acquired acute kidney injury (CA-AKI) and hospital-acquired acute kidney injury (HA-AKI) patients.Methods Hospital network system was employed to screen the clinical data of adult patients in the First Affiliated Hospital in Xinjiang Medical University in January to July 2013.A total of 19 528 patients were screened,and 544 AKI patients were identified based on KIDGO (Kidney Disease:Improving Global Outcomes) AKI guidelines.Three hundred and thirty patients were included in HA-AKI group and 214 patients in CA-AKI group.Clinical variables including mortality were analyzed retrospectively.Results The incidence of AKI in hospitalized patients was 2.8％ (544/19 528):1.7％ in CA-AKI group and 1.1％ in HA-AKI group.The mean age in CA-AKI group was significantly older than that in HA-AKI group [(62.9 ± 16.8) years vs (56.6± 15.9) years].Medical patients in CA-AKI group accounted for 62.4％,and surgical patients in HA-AKI group accounted for 64.1％.The co-morbid diseases were cardiac disease,hypertension,diabetes and chronic liver disease.Majority of AKI was caused by pre-renal etiologies.The length of hospitalization was significantly shorter in CA-AKI group compared to that in HA-AKI group [12(8,20) days vs 19 (12,27) days,P ＜ 0.01].Compared to that in HA-AKI group,all-cause mortality was significantly lower in CA-AKI group (11.5％ vs 20.1％,P=0.005).Results by multivariate logistic regression analysis demonstrated that the common independent risk factors of AKI in both groups were ICU hospitalization and shock.The independent risk factor of AKI in CA-AKI group was diabetes (OR=3.019).In contrary,the independent risk factors of AKI in HA-AKI group were elderly (≥65 years) (OR=3.303),oliguria (24 h urine volume ＜ 400 ml) (OR=6.906),use of antiinflammatory drugs (OR=13.079) and multiple organ dysfunction syndrome (OR=17.778).Conclusions The incidence of AKI in hospitalized patients is not rare,among which both communityacquired and hospital-acquired AKI are mainly caused by pre-renal etiologies.All-cause mortality is lower in community-acquired AKI compared to that in hospital-acquired AKI and the independent risk factors are different between CA-AKI and HA-AKI.

OBJECTIVE:To study the chemical compositions of n-butabol extract from Solanum lyratum. METHODS:Glucan LH-20 column chromatography,silica gel column chromatography and TLC were adopted to separate and purity the chemical com-positions,physicochemical property and spectral evidence to identify their structures. RESULTS:Totally 10 chemical compositions were separated from n-butabol extract,namely apigenin-7-O-β-D-apiofuanosyl(1→2)-β-D-glucose (1),apigenin-7-O-β-D-glucose (2),adenosine(3),3-methoxy-4-hydroxy-5-[(8′S)-3′-methoxy-4′-hydroxyl-phenyl-alcohol]-E-cinnamic-phenylpropyl alcohol-4-O-β-D-glucoside (4),N-(4-amino-butyl)-3-(3-hydroxy-4-methoxy-phenyl)-E-acrylamide (5),N-(4-amino-butyl)-3-(3-hydroxy-4-me-thoxy-phenyl)-Z-acrylamide (6),resveratrol (7),naringenin (8),quercetin (9) and dioscin (10). CONCLUSIONS:Compound 1-8 are first separated from S. lyratum,the study can lay a foundation for quality evaluation of S. Lyratum.

Objective:To assess the surgical outcome of patients with mesenchymal chondrosarcoma (MCS) treated in our insti-tute. This study was also designed to describe the clinical characteristics, treatment, and outcome of MCS to provide a better understand-ing of its clinical management. Methods:A total of 27 patients with MCS were treated in Peking University People's Hospital, Beijing, China from October 1997 to March 2011. Demographic information and follow-up data were obtained and statistically analyzed. Re-sults:Among the 27 patients, 9 were males and 18 were females with a mean age of 30.4 years (ranging from 14 years to 51 years). The median follow-up time was 42.6 months (ranging from 6 months to 104 months). Among the total number of tumor cases, 22 and 5 were detected in bone tissues and extra-skeletal sites, respectively. A total of 25 patients underwent surgery, but only 17 achieved the standard surgical margin of wide excision. Among these patients, 16 and 13 were subjected to chemotherapy and irradiation. The three-and five-year survival rates were 65%and 49.5%, respectively. Conclusion:MCS is a rare tumor resulting in morbidity with local recur-rences and long-term metastases. In this study, standard multimodal regimens were proposed to treat MCS. The results recommended wide resection with suitable surgical margins as the preferred treatment. However, further studies should be conducted because the infor-mation about the benefits of chemotherapy and radiotherapy for the control of local or systemic symptoms of MCS remains insufficient.

Objective To study the effects of nitrobenzene on macrophage function and lymphocyte proliferation in mice. Methods ICR mice were divided into groups and treated with nitrobenzene by gavage,once a day,at doses of 2,20 and 200 mg/kg respectively,for 21 consecutive days.The mice were killed after 21 days of treatment and then the effects of nitrobenzene on the organs index,the maerophage function and the lymphocyte proliferation were determined.Results The maerophage function and the lymphocyte proliferation decreased as the increase of the dose of nitrobenzene.Conclusion The results of the present paper show that nitrobenzene may inhibit the immunity function of mice.

Objective To study the effect of oleic acid on the morphology of cultured human umbilical vein endothelial cell (HUVEC) and on the secretive function of endothelial cell mediated by insulin or leptin. Methods The cell morphological characteristics were studied by phase microscopic observation. The changes of endothelial cell secretive function were studied by detecting the concentrations of nitric oxide (NO) and endothelin (ET-1) after endothelial cell was pretreated with oleic acid of different concentrations. Results With the increased concentration of stimulating insulin, the concentration of NO increased and ET-1 decreased gradually in a dose-dependent fashion. The profile of endothelial cell pretreated with oleic acid became darker than before. As the concentration of pretreatment oleic acid increased, the concentration of NO secreted by endothelial cell which was mediated by insulin and leptin gradually decreased in a dose-dependent fashion, but the concentration of ET-1 secreted by endothelial cell mediated by leptin decreased significantly only in the 50～100 ?mol?L -1 of pretreatment oleic acid. Conclusion Oleic acid damages endothelial cell directly and inhibits endothelial cell mediated by insulin and leptin to secrete NO. Oleic acid and insulin can directly inhibit release of ET-1 by cultured endothelial cell.

Objective To study the relationship between periodontitis and chronic kidney disease (CKD) among Uygur adults of Xinjiang. Methods Data of 1650 Uygur adult residents (age＞18 years) in the Moyu county, Xinjiang were analyzed. The subjects were sampled randomly with stratify capacity from 15 villages among 364 villages. All the subjects received the questionnaire and the oral examination. The markers and risk factors of chronic renal injury were inspected. The subjects were categorized as periodontitis group and non-periodontitis group according to chronic periodontitis diagnostic criteria. The periodontitis group was further divided into mild, moderate and severe periodontitis. Results The data of 1415 subjects were completed. The prevalence of chronic periodontitis was 65.2% (95%CI:65.0-65.4). The prevalence of CKD was 5.2% (95%CI:5.1-5.3). Albuminuria was found in 4.2% (95%CI:4.1-4.3) of subjects. 1.3%(95%CI:1.3-1.4) of individuals had renal insufficiency. In periodontitis group, the prevalence of CKD was 6.4%, which was higher than that in non-periodontitis group 2.9% (χ2=7.841 ,P=0.005).Univariate regression analysis showed that severe periodontitis was risk factor of CKD (OR =3.2,95% CI:2.0-5.2). Multivariate logistic regression analysis showed that severe periodontitis was independently associated with CKD (OR = 1.9, 95%CI: 1.1-3.3). Conclusions In Uygur adults of rural area of Xinjiang, the prevalence of CKD is higher in periodontitis group as compared to non-periodontitis group. Severe periodontitis is an independent risk factor of CKD.

Objective To investigate the effects of a training program based on virtual reality on static and dynamic balance perfor-mance in patients with Parkinson's disease (PD). Methods From June, 2014 to June, 2016, 46 patients with PD were randomly divided into control group (n=23) and experimental group (n=23). The control group received routine balance training, while the experimental group re-ceived balance training of virtual reality, for six weeks. They were assessed with Unified Parkinson's Disease Rating Scale part 3 (UP-DRS3), Berg Balance Scale (BBS), TimedUp and GoTest (TUGT) and Hamilton Depression Scale (HAMD) before and after training. The envelope area, anteroposterior standard deviation (AP-SD), mediolateral standard deviation (ML-SD) of centre of pressure (COP) were also measured with posturography. Results The scores of BBS, TUGT and HAMD improved in both groups after training (t>2.657, P<0.05), and improved more in the experimental group than in the control group (t>2.426, P<0.05). The score of UPDRS3 and the parameters of pos-turography improved in the experimental group (t>2.626, P<0.05), and improved more than that in the control group (t>2.112, P<0.05). Con-clusion Virtual reality rehabilitation is more effective than routine balance training on the static and dynamic balance function in patients with PD, and may release their depression.

Abstract A novel luminol electrochemiluminescence strategy based on titanium dioxide/carbon nanotubes ( TiO2/CNTs) nanocomposites for detection of glucose was developed. First, the TiO2/CNTs nanocomposites were prepared by a sol-gel method and modified on the glassy carbon electrode. The electrochemiluminescence ( ECL) signal could be greatly enhanced when the electrode was established by the nanocomposites, which finally resulted in the increased sensitivity. Glucose oxidase calalyzed the oxidation of glucose to form H2 O2 , and the H2 O2 reacted with luminol to produce the ECL signal. Thus the above system was proved to be efficient for glucose detection. The modified electrode exhibited excellent ECL signals and a good linear range of 1. 0í10-7-5. 0í10-6 mol/L with a detection limit of 5. 2í10-8 mol/L towards glucose detection. This strategy was successfully demonstrated as a sensitive, rapid, simple and cost-effective method to detect glucose. Meanwhile, the TiO2/CNTs nanocomposites offered a novel material for the signal enhancement in electrochemiluminescence sensor.
