1.Application of bundle strategies in the management of multidrug-resistant organisms

Objective To explore the application of bundle strategies in the management of multidrug-resistant or-ganisms(MDROs).Methods From July 1 ,2013,bundle intervention measures were applied in the management of MDROs from inpatients in a tertiary first-class hospital,the implementation of isolation measures for MDRO-infec-ted/colonized patients were monitored,April-June 2013 was pre-intervention group,July-September 2013 was the first stage of intervention,October-December 2013 was the second stage of intervention,the qualified rate of each measure and incidence of MDRO infection per 1 000 patient-days before and after intervention were compared. Results From April to December 2013,a total of 3 430 non-repetitive isolates were detected.The qualified rate of doctors’ order “contact isolation ”before intervention and in the second stage of intervention were 78.14% and 95.95% respective-ly;the qualified rate of hanging bed-side isolation signs,providing bed-side quick-drying hand disinfectant,and patients wearing blue wristbands before intervention were 52.70%,66.89%,and 87.16% respectively,and in the second interven-tion stage were all 100.00%.The qualified rate of disinfection of medical supplies and environment,as well as patients’ bed assignments before intervention were 23.65% and 15.54% respectively,in the second stage were 79.79% and 77.66% respectively.Health care workers’awareness rate of hand hygiene knowledge,hand hygiene compliance rate,and hand hygiene correct rate before intervention were 69.70%,45.76%,and 76.35% respectively;in the second stage were 90.23%,87.50%,and 94.58% respectively;the qualified rate of implementation of each isolation measure before inter-vention and in different stages after intervention were all significantly different(all P <0.01).The incidence of MDRO in-fection per 1 000 patient-days before intervention,in the first and second intervention stages were 3.24‰,2.63‰,and 2.20‰ respectively.Conclusion After the monitoring and intervention in MDROs with bundle management strategies,the qualified rate of each measure improved,incidence of MDRO infection per 1 000 patient-days decreased.
3.Development of microcomputer system for SCCA marker detection

With a PII-300 computer, a microcomputer system for SCCA marker detection is developed in this paper, which is based on Windows operating system and Visual Basic 6.0. More than 1,000 cases have proved that the results from the system completely accord with the one from artificial detection. With this system, such disadvantages are conquered as high labor intensity, low working efficiency and high error rate, and a quantitative detection method for SCCA marker comes into existence.
4.Clinical effect of tiotropium bromide combined with seretide on treating moderate and severe chronic obstructive pulmonary disease patients in stable phase

Objective To investigate the clinical effect of tiotropium bromide combined with Seretide in treatment of moderate to severe chronic obstructive pulmonary disease( COPD) patients. Methods From Jan. 2012 to Oct. 2013,76 severe COPD were selected as our subjects and they were randomly divided into the control group(38 cases)and research group(38 cases). Patients in control group were used Seretide(1 time both at morning and night,the does was 50 μg/ 500 μg),and in research group were given tiotropium bromide with thiophene beside Seretide(1 time every day). The two groups were received routine treatment,including anti infection,cough and asthma,according to the change of illness. The pulmonary function,blood gas index, respiratory symptoms and 6 minutes walking distance(6 MWD)were recorded. Results Forced expiratory volume in one second( FEV1),forced vital capacity( FCV),FEV1 / FCV of patients in research group after ftreatment were(2. 22 ± 0. 48)L,(3. 28 ± 0. 32)L,(66. 23 ± 9. 22)% respectively,higher than those in the control group,and the differences were significant((1. 78 ± 0. 35)L,(2. 85 ± 0. 47)L,(56. 83 ± 7. 85)% ;t= 5. 39,4. 66,4. 78;P < 0. 01). Partial pressure oxygen(PaO2 )of patients in research group after treatment was (72. 83 ± 5. 28)mmHg,significantly higher than that of control group((65. 36 ± 3. 22)mmHg). However, partial pressure of carbon dioxide( PaCO2 )was(43. 28 ± 3. 52)mmHg,significantly lower than the control group((48. 76 ± 2. 85)mmHg;t = 7. 44,7. 45,P < 0. 01). Dyspnea( MMRC)score in research group was (1. 38 ± 0. 32),lower than the control group(1. 76 ± 0. 35),and 6 MWD was(428. 36 ± 32. 85)m,higher than that of control group((398. 65 ± 28. 38)m;t = 4. 93,4. 21,P < 0. 01). Conclusion The treatment plan of tiotropium bromide combine with Seretide on moderate and severe COPD patients is proved with the better clinical effect and it can improve the partial pressure of oxygen and then promote repair mechanism of lung injury as well as promote the quality of life of patients with ascension.
5.Efficiency of hearing screening combined with gene mutation screening in neonates with high-risk of hearing impairment

ObjectiveTo investigate the significance of hearing screening combined with gene screening for neonates with high-risk of hearing impairment.MethodsNeonates admitted to the Neonatal Department of Guangdong Women and Children Hospital between July 2013 and June 2014 were enrolled in this study. They were divided into high-risk group (with high-risk for hearing impairment) (n=3 129), and control group (n=5 106). Neonate hearing screening was carried out using otoacoustic emission and automated auditory brainstem response. Blood samples were collected using a standard protocol for detecting the mutations of four common deafness genes, includingGJB2,GJB3,SLC26A4 and mitochondrial 12s rRNA.Chi-square test was used to compare the differences of the pass rate of hearing screening and positive rate of gene mutations between the two groups.ResultsThe rates of failure on otoacoustic emission, automated auditory brainstem response or both in the high-risk group were 11.92% (373/3 129), 10.32% (323/3 129) and 4.83% (151/3 129), respectively, higher than those in the control group [5.03%(257/5 106), 6.56%(335/5 106) and 2.02% (103/5 106)] (χ2=130.265, 37.354 and 51.196, allP=0.000). In the high-risk group, the overall positive rate of gene mutations was 5.63% (176/3 129), and theGJB2 andSLC26A4 gene mutation rates were 3.04% (95/3 129) and 2.40% (75/3 129)], all higher than the control group [3.15% (161/5 106), 2.04% (104/5 106) and 1.06% (54/5106)] (χ2=30.301, 8.216 and 22.517, allP<0.01). But the mitochondria 12S rRNA gene andGJB3 gene mutation rates were the same in high-risk group and control group [0.19% (6/3 129) vs 0.06% (3/5 106); 0.03% (1/3 129) vs 0.00%(0/5 106), bothP>0.05]. The rates of failure on otoacoustic emission and automated auditory brainstem response of the neonates with deafness gene mutations were 9.50% (32/337) and 10.39% (35/337), respectively, higher than the neonates without [1.14% (90/7 898) and 1.29% (102/7 898)] (χ2=154.621 and 163.399, both P=0.000).ConclusionCombined hearing screening is of clinical significance for neonates with high-risk of hearing impairment.
6.Pharmacodynamic fingerprint of Flos Lonicerea by HPLC

Objective Looking for the chemical components which have positive correlations with anti-inflammatiory activity of Flos Lonicerea to set up pharmacodynamic-fingerprint. Methods To gain the fingerprints of different parts by HPLC and to detect the pharmacological activities of anti-inflammation, then the correlationship between chemical components and pharmacological activities were detected by linear regression. Results Pharmacological activities of the methanol extracted part were best of all, so the fingerprint of the methanol part could represent the pharmacodynamic-fingerprint of Flos Lonicerea. Conclusion The fingerprint established by this way is more scientific and reasonable.
7.Development of a detection system of the enzyme-linked analyzer

In allusion to high labor intensity, low working efficiency and high error rate in hospital laboratory, a detection system of the enzyme-linked analyzer based on Multiscan EX instrument is developed. Being easy to operate, the detection system proves effective in enhancing working efficiency. It deserves popularizing because of its comprehensive applications to the detection.
8.The development of an intelligentized microcomputer system for HBV marker detection and reporting

With a PII-300 computer, an intelligentized microcomputer system for HBV marker detection and reporting is developed in this paper, which is based on Windows operating system and accomplished through Visual Basic6.0. More than 10, 000 cases have proved that the result from the system accords with the one from artificial detection completely. This system solves the problems in hospital laboratory such as high labor intensity, low working efficiency and high error rate, and thus a quantitative detection method for HBV marker comes into existence.
9.Phenylephrine-induced excessive hypertrophy responses in cardiomyocytes from endothelial nitric oxide synthase gene knockout mice

AIM: The study aimed to observe the difference in hypertrophy responses in cardiomyocytes of cultured neonatal endothelial nitric oxide synthase (eNOS) gene knockout mice and wild mice by RNA quantitation detection and cell area estimation. METHODS: The experiment was performed at the Empirical Study Center of Medicine Department in Shunde Profession and Technique Institution in August 2007. ①Male or female C57BL/6 stock neonatal eNOS knockout mice and wild mice aged 1-3 days were selected. ②Cultivated Mouse cardiomyocytes were divided into experimental and control groups 72 hours later. Adrenergic receptor agonist phenylephrine was cultured with cardiomyocytes. DMEM of the same volume as the experimental group was added in the medium in the blank control group. ③DNA, RNA quantitation detection and cell area estimation methods was used to observe the responses contrast in cell proliferation and hypertrophy of cardiomyocytes in cultured neonatal eNOS gene knockout mice and wild mice. BACKGROUND: The complex prescription of lujiao prescription is made up of antle, malytea scurfpea fruit, korean epimedium herb, dogwood, glossy privet fruit, and Chinese eaglewood wood. It is characterized by its positive myodynamia as well as promoting diuresis and expanding vessels. OBJECTIVE: To observe the effect of lujiao prescription in reversing left ventricular remodeling in rats with pressure overload. DESIGN: A randomized controlled trial. SETTING: Department of Integrated Traditional Chinese Medicine and Western Medicine, Nanjing General Hospital of Nanjing Military Area Command of Chinese PLA; Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine; Postdoctor Institute of Hebei Medical University.MATERIALS: The experiment was conducted at the Laboratory of Shanghai University of Traditional Chinese Medicine from January to December 1998. Totally 153 male Wistar rats were provided by the Experimental Animal Center of Shanghai Medical College. There were five experiments with 36 rats selected for each experiment except 9 rats for the fourth one.METHODS: Totally 36 Wistar male rats selected for each experiment were randomly divided into sham operation group, model group and lujiao prescription group. Sham-operation group: The rats' abdominal aortas were separated, but not narrowed by silver clip. After four weeks, the rats were treated with twice-distilled water (15 mL/mg, ig), once a day for 4 consec utive weeks. Model group: Four weeks after modeling, the rats were treated with twice-distilled water (15 mL/mg, ig) once a day for 4 consecutive weeks. Lujiao prescription group: Four weeks after rat modeling, the ratswere given orally liquid decoction of lujiao prescription (15 mL/mg, ig),once a day for 4 weeks. Observations on left ventricular mass index (LVMI): Collagen type Ⅰ and Ⅲ was measured by immunohistochemistry. mRNA expression of collagen type Ⅰ and Ⅲ was measured by RT-PCR. Ang Ⅱ, ANF and ALD were measured by radioimmunoassay. MAIN OUTCOME MEASURES: LVMI, left ventricular myocardium collagen type Ⅰ and Ⅲ, mRNA expression of collagen type Ⅰ and Ⅲ, and changes of plasm, myocardium Ang Ⅱ, plasm ANF and serum aldosterone.RESULTS: Totally 153 rats entered the final analysis. During the detection of mRNA expression of collagen type Ⅰ , 3 rats in sham-operation group died and 1 in lujiao prescription group died. The other 29 rats en tered the final analysis. During the detection of mRNA expression of collagen type Ⅲ, 3 rats in sham-operation group died, 3 in lujiao prescription group died, and 6 in model group died. The other 26 rats entered the final analysis. All rats were lost because of operation injury. ① LVMI: It was lower in sham-operation group and lujiao prescription group than in model group [(2.24±0.12)/1 000, (2.60±0.44) /1 000, (3.15±0.47)/1 000,t=2.959-6.499, P < 0.05]. ② Changes of absorbency of collagen type Ⅰ :It was lower in sham-operation group and lujiao prescription group than in model group [(0.56±0.23, 0.57±0.19, 2.79±2.00), t=0.661-3.964,P < 0.01]. ③ Changes of absorbency of collagen type Ⅲ: It was lower insham-operation group and lujiao prescription group than in model group [(0.48±0.10,0.55±0.09,0.84±0.27), t=2.898-3.560, P < 0.01]. ④ mRNA expression of collagen type Ⅰ: It was lower in sham-operation group and lujiao prescription group than in model group [58.0±36.0)%, (79.1±18.6)%,(139.0±29.2)%, t=3.027, P < 0.05]. ⑤ Changes of myocardium Aug Ⅱ: It was lower in sham-operation group and lujiao prescription group than in model group [(2.49±0.57, 2.64±0.57,3.96±0.77) pg/mL, t=4.773-5.315,P < 0.001]. ⑥ Changes of serum aldosterone: It was lower in sham-operation group and lujiao prescription group than in model group [(501.6±94.6,476.6±85.7, 647.8±72.2) ng/mL, t=4.256-5.292, P < 0.001].CONCLUSION: Indexes of integrating absorbency of myocardial collagen type Ⅰ and type Ⅲ, mRNA expression of collagen type Ⅰ , myocardial Aug Ⅱ and serum aldosterone are all similar to the changes LVMI. Lujiao prescription can reverse left ventricular remodeling and myocardial fibrosis.
