1.Guide and prediction value of partial pressure of end-tidal carbon dioxide during cardiopulmonary resuscitation

Objective To study the clinical significance of end-tidal carbon dioxide partial pressure (PetCO2 ) during cardiopulmonary resuscitation (CPR) and seek the fixed value according which to decide if we should and when to give up.Methods This was a prospective,observational study.A total of 124 patients with cardiac arrest in or out-of-hospital from may 2003 to March 2009 in emergency department of our hospital were selected.All of them had definite etiological factors.Changes of PetCO2 in 124 cardiac arrest patients during CPR were tracked.Results The gender,age,rescue time in seventy-one patients with the return of spontaneous circulation (ROSC) after endotracheal intubation have a significant difference with that in fifty-three patients without ROSC (P <0.01 ).The PetCO2 of the survival were higher than that of patients without ROSC or with ROSC,but finally died (P <0.01 ).A fixed point 14.4 mmHg of PetCO2 after 20 minutes' CPR can be used as a reference value to guide CPR or predict prognosis.Conclusions Monitoring PetCO2 during CPR has a predictive value on the success of resuscitation.
2.Long-term efficacy of calcitonin and bisphosphonates on renal osteopathy in maintenance hemodialysis patients

Objective To observe the efficacy of ealcitonin and bisphosphonates on renal osteopathy of maintenance hemodialysis (MHD)patients. Methods Forty-three MHD patients were raindomly divided into two groups: A group and B group. All the patients were routinely received oral calcium carbonate 1.0 g tid and calcitriol 0.25 μg qd. Calcitonin (20U) hypodermic injection was given three times a week additionally during hemodialysis in A group. Patients in B group received bisphosphonates 70 mg once a week based on the therapy of A group. Serum levels of intact parathyroid hormone (iPTH), calcium, phosphorus, alkaline phosphatase (AKP), bone mass density (BMD) of lumbar spine and femoral neck, and the degree of bone ache (visual analogue scale, VAS) were assessed before the therapy and 3, 6 and 12 months after treatment. The adverse reactions were recorded during treatment. Results The levels of AKP and iPTH in both two groups decreased significantly after treatment. The above values of pre-treatment and 12 months after treatment were as follows: AKP(U/L)of A group 244.05±41.99 and 148.35±27.71,of B group 245.60±40.86 and 143.40±28.03;PTH(ng/L) of A group 697.5±119.7 and 267.4±45.9,of B group 708.2±120.3 and 277.6±41.9 (all P<0.05). While the levels of calcium and phosphorus did not change obviously during treatment (P>0.05). BMD was not improved at 3, 6 mouths and became better at 12 mouths after treatment. As compared to pre-treatment, BMD of lumbar spine(g/cm2) in A group was 1.062±0.223 vs 1.202±0.251 ,in B group 1.033±0.152 vs 1.189±0.225; BMD of femoral neck (g/cm2)in A group was 0.993±0.108 vs 1.067±0.095,in B group 0.947±0.083 vs 1.018 ±0.217 (all P<0.05). The scores of VAS also decreased significantly at 3, 6, 12 months after treatment(P<0.05). No severe adverse reaction was found during the treatment. Conclusions Utilization of calcitonin and combination with bisphosphonates during bemodialysis can effectively preserve the BMD and prevent bone loss in MHD patients and is well tolerated. No significant difference of therapeutic effect is observed between using ealcitonin or combination with bisphosphonates.
3.The evaluation of insulin resistance and ?-cell function in patients with systemic lupus erythematosus

Objective To evaluate insulin resistance and ?-cell function in patients with systemic lupus erythematosus(SLE).Methods The study group included thirty-two female SLE patients who were hospitalized or received outpatient treatment in our hospital;the control group consisted of twenty-two healthy females.Their fasting plasm glucose and serum insulin were all tested.Homeostasis model assessment(HOMA) was used to calculate HOMA-IR and HOMA-?-cell function,and these parameters were compared between the study group and control group.Results Compared with control group,the SLE patients had significantly higher fasting insulin levels(12.13?4.32 vs.8.84?2.19,P0.05).Conclusion Patients with SLE have a higher risk of insulin resistance and abnormal insulin secretion than controls.
4.Effects of azithromycin on the formation of nontypeable haemophilus influenzae biofilm on endotracheal tubes

Objective To observe the formation of biofilms on endotracheal tubes ,and to determine the effect of azithromycin on Haemophilus influenzae in biofilms .Methods One hundred and twenty Wistar rats were intubated with a silicone tube ,80 of them(40 rats were model group and 40 were medicine group) were infected by bacteria to establish the animal models of endotra-cheal tube infections the others were ues as control gruop .Formation of biofilms was detected by scanning electron microscope .Via-ble cells counting was carried out to evaluate the effect of azithromycin orally on biofilms on endotracheal tubes .Results Uneven biofilms were formed on the surface of endotracheal tubes ,and slim bodied of bacteria enclosed by a lot of extracellular matrix .Via-ble cells counting was reduced in medicine group compared with model group .Conclusion Haemophilus influenzae can form bio-films on endotracheal tubes .Azithromycin can exert inhibitory effect on haemophilus influenzae biofilms formed on endotracheal tubes .
5.Phenylephrine-induced excessive hypertrophy responses in cardiomyocytes from endothelial nitric oxide synthase gene knockout mice

AIM: The study aimed to observe the difference in hypertrophy responses in cardiomyocytes of cultured neonatal endothelial nitric oxide synthase (eNOS) gene knockout mice and wild mice by RNA quantitation detection and cell area estimation. METHODS: The experiment was performed at the Empirical Study Center of Medicine Department in Shunde Profession and Technique Institution in August 2007. ①Male or female C57BL/6 stock neonatal eNOS knockout mice and wild mice aged 1-3 days were selected. ②Cultivated Mouse cardiomyocytes were divided into experimental and control groups 72 hours later. Adrenergic receptor agonist phenylephrine was cultured with cardiomyocytes. DMEM of the same volume as the experimental group was added in the medium in the blank control group. ③DNA, RNA quantitation detection and cell area estimation methods was used to observe the responses contrast in cell proliferation and hypertrophy of cardiomyocytes in cultured neonatal eNOS gene knockout mice and wild mice. Objective To explore the roles of p53 in ionizing radiation induced MCF-7 cell cycle uncoupling. Methods The p53 knock-down models was established in MCF-7 with retrovirus packaged particles from 293T cells through calcium acid phosphate co-precipitation, then Western blot was used to detect the protein expression. Flow cytometry(FCM) was used to analyze the cell cycle uncoupling and polyploid after irradiation. Results Compared with p53+/+ group, the percentages of G0/G1 cells in p53 -/- group decreased, while those of S and G2+M increased (P < 0.01). In polyploidy analysis 2N cells decreased, whereas both 4N and 8N cells =increased (P<0.01). Compared with sham-irradiation, 4 Gy X-ray led to the decrease of G0/G1, S cells, and the increase of G2+M cells. The increase of 2N cells and decrease of 4N and 8N cells were observed in both p53+/+ and p53-/- cells. Compared with p53+/+ +IR group, the decrease of G0/G1 and S cells and the increase of C2 + M cells were significant (P < 0.01) in p53-/-+ IR groups. 2N cells decreased, 4N cells increased, but no changes in 8 N cells occurred. Conclusion Radiation might induce G2 arrest and cycle uncoupling, p53 plays a role in the regulation of G2 arrest, but no role in cycle uncoupling.
7.Clinical manifestations and pathological characteristics of 1gA nephropathy with hyperuricemia

Objective To explore the effect of serum uric acid on the clinical manifestations,pathological characteristics and prognosis of IgA nephropathy.Methods Four hundred and fifty-six cases of primary IgA nephropathy confirmed by renal biopsy from Jan 2007 to Oct 2010 in the Ji'nan Military General Hospital were reviewed retrospectively.The clinical manifestations and pathological characteristics of all the patients were analyzed,x2-test and t-test were used for statistical analysis.Results There were 127 cases with hyperuricemia in 456 IgAN patients (27.9%).The mean age,percentage of male patients,number of patients with hypertension,the serum cholesterol and triglyceride level,body mass index (BMI),serum creatinine and 24 hour urine protein level in hyperuricemia group were significantly higher than those with normal serum uric acid (P<0.01).The renal pathological changes,glomerular score (8.1 ±0.8 v 5.3t0.9 ),tubulointerstitial score (4.2±0.4 vs 2.7±0.4) and vasculopathy score ( 1.43±0.60 vs 0.76±0.29) in the hyperuricemia group were more severe than those with normal serum uric acid (P<0.01).Conclusion High level of serum uric acid can affect IgA nephropathy significantly.It is effe-ctive to delay the kindey damage and progression of IgA nephropathy by decreasing the level of uric acid and control the clinical parameters listed above.
8.Exclusive gene mapping on a Chinese familial IgA nephropathy family

Objective To initially map the gene responsible for autosomal dominant familial IgA nephropathy of a Chinese family by exclusive the five loci that had been reported with linkage analysis.Methods The genetic pattern of the familial IgA nephropathy was identified and the genomic DNA was extracted from the blood samples collected from the family members.Short tandem repeat (STR) inside the loci that had been reported was selected,such as 2q36,3p23-24,4q26-31,6q22-23,17q12-22,and the data with two-point linkage analysis were performed.Results Autosomal dominant inheritance pattern was demonstrated in phenotypes of the family and there was no linkage relationship in the above five loci of chromosomes because the maximum two-point LOD score was 0.39 at D17S1868.Conclusion Following exclusion of the loci which had been reported,there are other new pathopoiesis loci of FIgAN and it reveals that FIgAN has the genetic heterogeneity according to initial result at the same time.
9.Analysis of associations between spleen stiffness and esophageal-gastric varices in patients with HBV- related liver cirrhosis receiving anti-viral treatment

Objective To investigate the diagnosis value of spleen stiffness measurement by transient elastography (FibroScan, FS) for esophageal-gastric varices (EV) in patients with HBV-related liver cirrhosis receiving anti-viral treatment. Method Total of 41 patients from Jan 2014 to Dec 2014 diagnosed with HBV-related liver cirrhosis receiving anti-viral treatment were enrolled. All patients were evaluated for spleen and liver stiffness measurement by FS and checked by gastroscopy for diagnosis of EV. Using gastroscopy as the gold standard, the area under receiver operating characteristic curve (AUROC) were used to evaluate the value of the spleen stiffness and liver stiffness in diagnosis of EV and its degree. Results The spleen and liver FS values in patients were (40.64 ± 25.45) kPa and (20.76 ± 13.21) kPa respectively, and they showed a positive correlation (r = 0.402, P < 0.001). The spleen FS values in patients without EV were significantly lower than those in patients with mild EV and moderate-severe EV (all P < 0.05). Furthermore, they showed significantly lower in patients with mild EV than those in patients with moderate-severe EV too (P < 0.05). The AUROC of spleen FS in patients with EV was 0.863, with sensitivity of 79.4% and specificity of 83.2%. Moreover, the AUROC of spleen FS in patients with moderate-severe EV was 0.924, with sensitivity of 87.9% and specificity of 91.3%. Both of them were much higher than those of liver FS. Conclusion Spleen FS may act as a non-invasive marker to predict EV and its degree in patients with HBV-related liver cirrhosis receiving anti-viral treatment.
10.Comparative Effect of Different Media in in vitro Cultivation of Trichomonas vaginalis

Objective To investigate the optimal condition for in vitro cultivation of Trichomonas vaginalis for obtaining a better harvest of T\^vaginalis. Methods An isolate of T\^vaginalis from clinical specimens was cultivated in three different media with initial inoculation of 9\^0?10\+4/ml under pH 5\^6. Results There was distinct difference after 96h incubation in the cumulative harvest of T.vaginalis. The highest harvest was received in cysteine/liver/peptone/maltose medium, followed by the liver/peptone/maltose medium and soybean/liver/peptone/maltose medium. Conclusion The cysteine/liver/peptone/maltose medium may be a suitable environment for in vitro multiplication of T.vaginalis.
