1.Study on the in vitro release of tetanus toxo id from polylactide microspheres and the influence of additives on the drug release

Objective: To study on in vitro release of tetanus toxoid contained in polylactide microsp heres and the influence of additives on the drug release. Methods: Some parameters affecting the rate of release were asse ssed during the in vitro experiments lasted 84 days: (1)the molecular mass of the poly mers , (2)the protein loading of the microspheres, (3)the particle sizes, (4)co-enca p sulation of additives. Results: The polylactide microspheres con taining tetanus toxoid presented considerable sustained release effect. Release kinetics of the microspheres fitted in with zero order equations. The rate of release was shown to dep end on the four parameters mentioned above. Conclusion: The poly lactide microsph eres investigated may be prospective for the development of controlled release v accines.
2.Progression on the medical nutrition therapy of type 2 diabetes mellitus

Medical nutrition therapy (MNT)is the basic and long-term treatment of diabetes patients. At present,the dietary structure of type 2 diabetes mellitus(T2DM)patients in China has yet to be further improved. This article focused on the effect,dietary structure,food selection and guarantee of compliance of MNT in T2DM.We recommended that the dietary pattern should be designed based on the traditional dietary habits around the country,combined with people living and working reality. The rational utilization of network platform can improve the efficiency and compliance of nutrition education.
3.Significance of liquid-based thinprep cytology test and human papillomavirns in cervical lesions screening

Objective To evaluate the clinical value of liquid-based thinprep cytology test (TCT) and human papillomavirus (HPV) in cervical lesions diagnosis.Methods 525 patients with TCT was inflammation and abnormalities had HPV testing and colposcopy with biopsy analysis.Results Histopathological diagnosis as CIN Ⅰ or CIN Ⅰ above cases percentage of TCT of ASCUS,LSIL,HSIL,and cancer cases were 55 % (160/290),88 % (99/113),92 % (24/26) and 100 % (4/4),Pathological diagnosis as inflammation and glandular phosphate in 441 cases of this TCT-positive patients were 148 patients,false positive 34 %.TCT-negative patients,pathologically confirmed of CIN in 84 cases patients with moderate to severe cervical erosion or cervical polyps were 22 cases,26 % TCT false negative.They had statistically significance (x2 =815.4,P =0.000).74 % HPV positive patients diagnosed as CIN Ⅰ or CIN Ⅰ above (x2 =104.94,P =0.000).Diagnosed as CIN Ⅰ cases in less than 30 years old accounted for 62 % (61/98),CIN Ⅱ and above for only 38 % (37/98) and without cancer.But the CIN Ⅰ in above 30 age group was significantly reduced to only 37 % (81/217),CIN Ⅱ and above was significantly increased to 63 % (136/217),and cancer rate increased (x2 =63.71,P =0.012).Histopathological diagnosed as CIN Ⅰ case were 26 % (83/315) of cervical smooth,74 % (232/315) of cervical erosion; 14 cases of cancer occurred in patients with cervical erosion.It was statistically significant (x2 =39.9,P =0.014).Conclusion TCT and HPV for screening of cervical lesions play an important role in cervical lesions screening.
4.Optimization of in vitroamplification of peripheral blood γδ T cells with cytokines

Objective To optimize in vitro amplification of human γδ T cells with cytokines for tumor adoptive immunotherapy.Methods On the basis of the immobilized anti-TCR γδ antibody plus IL-2 system, other γ chain receptor family cytokines, including IL-7, IL-15 and IL-21, were tested to amplify human peripheral blood γδ T cells either alone or in diversity combination.The percentage of γδ T cells was measured by flow cytometry, and the proliferation efficiency of γδ T cells was calculated.The expression of proliferation-or cytotoxicity-related molecules on γδ T cells was examined by flow cytometry in order to explore the relevant mechanisms.The cytotoxicity of γδ T cells to Daudi cells was detected by lactate dehydrogenase.Results IL-15 alone but not IL-7 or IL-21 increases the γδ T cell purity, amplification efficiency and cytotoxicity to reach comparable levels to those of IL-2.IL-2 plus IL-15 up-regulates the expression of CD69 on γδ T cells and significantly increases their amplificationefficiency (P<0.05).IL-2 plus IL-21 enhanced the cytotoxicity of γδ T cells against Daudi cells by increasing the expression of granzyme A (P<0.001).The combination of IL-2, IL-15 and IL-21 significantly improves cytotoxicity of γδ T cells but reduces their amplification efficiency.In addition, when IL-21 was applied for a short time, it also enhanced the cytotoxicity of γδ T cells (P<0.05).Conclusions The combination of IL-2 and IL-15 as well as a short time addition of IL-21 is the best cytokine recipe to amplify human peripheral blood γδ T cells in vitro with immobilized anti-TCR γδ antibody, which can increase both the proliferation efficiency and the cytotoxicity to tumor cells of γδ T cells.
5.Bone mineral density and biochemistry data of health checkup receivers

ObjectiveTo analyze the correlations between bone mineral density and biochemistry data of health checkup receivers.MethodsA total of 3025 sets of medical measurements,including blood glucose,lipid profiles,blood uric acid,blood pressure and bone mineral density,were collected from January 2008 to November 2010 in Medical Examination Center of Peking Union Medical College Hospital.Bone mineral density of lumbar vertebra 1 - 4 (L1-4) and hip were recorded.Linear correlation analysis was used to investigate the correlation between bone mineral density and other factors. Results In multiple regression analysis,age,body weight,and alkaline phosphatase level were found to be related to male hip bone mineral density; age,body weight,alkaline phosphatase,and glutamyltranspetidase level were related to male L2-4 bone mineral density; age,body weight,alkaline phosphatase,and glutamyltranspetidase level were indicated to be related to female hip bone mineral density; age,body weight,alkaline phosphatase,and the ratio of neutrophile granulocyte were related to female L2-4 bone mineral density.ConclusionIn thisstudygroup, bodyweightwaspositivelycorrelatedwithbonemineraldensity, although alkalinephosphatase was negatively associated with bone mineral density.The role of blood glucose and lipid in the regulation of bone mineral density remains inconclusive.
6.X-ray and Clinical Characteristics of the Adult Contagious Atypical Pneumonia

Objective To enhance the cognition about the adult contagious atypical pneumonia for an expectation of earlier diagnosis, prevention and treatment.Methods We analyzed and summarized X-ray lung and clinical characteristics of 34 patients with contagious atypical pneumonias who were admitted to our hospital from Feb to April. 2003.Results X-ray findings of lung were characterized by mostly involving unilateral and presenting ill defined cloud or patches-form opacities. These lesions progressed and varied rapidly. The clinical manifestations were characterized by the onset of fever (34/34), mostly associated with systemic and joint aching pains (29/34) and dry coughs (29/34). Leucocyte counts showed normal or lower than normal. Patient with a continuing high fever might tend to develop ARDS (2/34). All the 34 patients recovered after combined therapies (anti-inflammation, anti-virus and promoting immunity ) and were discharged from hospital. The average in-hospital days were (10.6?4.2) days. Conclusion X-ray examination is the main method for the early diagnosis and observing state-variation. The predominant clinical features of this disease are fever, dry cough, aching pain in extremities, unconspicuous pulmonary signs as well as hemogram not rising, a continuing high fever may lead to the tendency of ARDS.
7.Association of gastric hyperplastic polyps with synchronous colorectal neoplasia

Objective To evaluate the association of gastric hyperplastic polyps with colorectal neo-plasia.Methods Data of consecutive patients who underwent esophagogastroduodenoscopy (EGD)and colonoscopy between January 2011 and December 2013 were reviewed retrospectively.They were compared with patients without gastric polyps who also underwent EGD in the same period.The relationship between gastric polyps and colorectal neoplasia was analyzed.Results The incidence of colorectal neoplasia in gas-tric hyperplastic polyps group was higher than that of the control group [24.0% (46 /192)VS 10.4%(40 /384),P =0.000].An increased incidence of colorectal adenomas in gastric hyperplastic polyps group was found,but there was no difference in the incidence of colorectal cancer in gastric hyperplastic polyps group and control group[2.1%(4 /192)VS 2.3%(9 /384),P =1.000].Stratification analysis suggested that the incidence of colorectal neoplasia in gastric hyperplastic polyps group who aged over 50 was signifi-cantly higher than that in the control group[28.5%(43 /151)VS 10.6%(29 /274),P =0.017],regard-less of different genders and the number of gastric hyperplastic polyps.Conclusion The incidence of color-ectal neoplasia in gastric hyperplastic polyps has significantly increased,especially in those aged over 50 years,regardless of different genders and the number of gastric hyperplastic polyps.Such patients should undergo colonoscopy to detect colorectal neoplasia.

9.Effects of berberine on the tumor-associated macrophages of intestinal polyps in Apc (Min/+) mice

Objective To investigate the effects of berberine on tumor-associated macrophages (TAM)and the expression of cyclooxygenase-2 (COX-2)of intestinal polyps in Apc(Min/+) mice.Methods A total of 20 Apc(Min/+) mice,four weeks old,were equally divided into the control group and the berberine group,10 in each group.The mice of the control group drank plain water,while the mice of berberine group drank water with 0.1 % berberine.After 12 weeks,all the mice were sacrificed.The intestine and colon were isolated,and the numbers of polyps were counted.The expression of F4/80,inducible nitric oxide synthase-2 (iNOS),macrophage mannose receptor (MR)and COX-2 was detected by immunohisto-chemistry method.The relative expression of COX-2 at protein level was measured by Western blotting. The t test was performed for comparison between two independent groups.Results The total number of intestinal polyps,the number of small intestinal polyps and the number of colon polyps of the berberine group (11 .50±2.05 ,10.50±1 .77 and 1 .00±0.46,respectively)were all less than those of the control group (30.63±1 .69,28.00±2.00 and 2.63±0.74,respectively),and the differences were statistically significant (t=16.727,16.952 and 3.162,P =0.001 ,0.001 and 0.010,respectively).The percentage of F4/80 positive cells in the stroma of polyps of the berberine group ((17.40 ±4.23 )%)was less than that of the control group ((31 .24±6.34)%),and the difference was statistically significant (t =5 .327, P =0.043).The percentage of iNOS positive cells in the stroma of polyps of the berberine group ((7.43± 1 .78 )%) was higher than that of the control group ((2.72±0.68)%), and the difference was statistically significant (t=7.335 ,P =0.004).The percentage of MR positive cells in stroma of polyps of the berberine group ((19.52±1 .54)%)was less than that of the control group ((12.63±0.68)%),and the difference was statistically significant (t=5 .634,P =0.016).The percentage of COX-2 positive cells in stroma of polyps of berberine group ((3.38 ± 0.51 )%)was less than that of the control group ((7.60±0.57 )%),and the difference was statistically significant (t = 7.234,P = 0.001 ).The relative expression of COX-2 at protein level of polyps of the berberine group was lower than that of the control group. Conclusion Berberine may take the role in inhibiting the growth of intestinal polyps in Apc(Min/+) mice through interfering the differentiation of TAM in polyps and suppression the expression of COX-2.
10.Prevalence of hyperuricemia in health check-up population of Beijing suburb

Objective To investigate the prevalence of hyperuricemia in health check-up population of Beijing suburb.Methods Total 1 336 rural residents in Nankou Township of Beijing received health check-up from July to Aug 2014,including 686 subjects aged 20-59 years (young/middle-aged group) and 650 subjects aged 60-96 years (elderly group).The blood pressure and body mass index (BMI) were measured;serum uric acid (SUA),fasting blood glucose (FBG) and blood lipids (TG,TC,HDL-C,LDL-C) were determined.The SUA levels > 420 μmol/L for male and > 360 μmol/L for female were defined as hyperuricemia.Results The four quartiles of SUA levels were 27.00-254.59 μmol/L (Q1),254.60-302.35 μmol/L (Q2),302.36-359.78 μmol/L(Q3) and 359.79-702.0 μmol/L (Q4).The prevalence of hyperuricemia was significantly higher in young/middle-aged group than that in elderly group [20.41% (140/686) vs.13.85% (90/650),x2 =10.08,P =0.001 5],the systolic blood pressure [SBP,(126.8±15.7) vs.(116.7±12.0)mmHg(1 mmHg=0.133 kPa),t=2.76,P=0.008],FBG [(7.40±4.10) vs.(6.11 ±2.03)mmol/L,t=2.12,P=0.036],TC [(5.52±1.10) vs.(5.23±1.00)mmol/L,t =2.04,P =0.045],LDL-C [(3.5 ±0.7) vs.(2.4 ±0.9)mmol/L,t =2.21,P =0.029]in young/middle-aged group were significantly higher than those in elderly group.BMI,FBG were significantly higher in Q4 than those in other quartiles [BMI:(26.44 ± 3.88) vs.(24.19 ± 3.37),(25.49±3.42) and (25.61 ±3.49)kg/m2,t =2.78,P=0.008;FBG:(8.19 ±1.52) vs.(6.34±1.34),(6.09 ± 1.51) and (6.40 ± 1.98) mmol/L,t =2.80,P =0.007].The triglyceride (TG) levels in group Q3 and Q4 [(1.85 ± 0.90) and (1.92 ± 0.44) mmol/L] were higher than those in Q1 and Q2 [(1.37 ±0.76) and (1.70 ±0.84) mmol/L,t =2.1,P =0.035].Only 9.57% subjects (22/230)with hyperuricemia was not combined with metabolic disorder;subjects combined with one and two metabolic disorders accounted for 20.87% (48/230) and 69.57% (160/230),respectively.Conclusion Screening for hyperuricemia is important for comprehensiye treatment and management of hyperuricemia in rural residents,especially in the young and middle-aged population.
