1.COMMUNITY MANAGEMENT AND FAMILY CARE OF CHILDREN WITH INTELLECTUAL DISABILITIES

Objective To explore for mentally challenged children, their families after the implementation of com-munity-based management and care, the effect obtained performance.Methods Select mentally disabled children 150 ca-ses as the research object of this experiment, implement necessary community management intervention for children and to give the necessary family care, mainly include the children carry out health education, existence education and encourage children to participate in training activities, etc.Comparing the patient′s psychological state before and after the intervention and the quality of life scores.Results Before the intervention and post-intervention comparison found that children in ath-letic ability, the ability to understand the performance aspects of quality of life scores, and a very significant improvement (p<0.05).In terms of children with inner feelings, after giving intervention also received a significant effect, children with heart felt warm enough.Conclusion For the mentally challenged children, to give them the necessary community manage-ment and home care, improving the quality of life in children with intellectual disabilities and children with enhanced inner feelings, etc., have shown a very significant meaning, highlights the success of community management and family care Ap-plication significance.
2.STUDY ON THE BIOLOGICAL ACTIVITIES OF BRAIN NATRIURETIC PEPTIDE AND ITS ANALOG

Brain natriuretic peptide consists of 26 amino acids and has diuretic, natriuretic and hypotensive activities. We have synthesized BNP and its analog (BNP-A) (Mpr4, D-Ala613)-BNP(4-26)-NH2 using solid phase method and examined their biological activities. Our results showed that 1) BNP (10-7mol/L) dilated human basal cerebral artery which was pre-constricted with endothelin, 5-HT and noradrenaline in vitro; 2) both BNP (5 ?g/ rat) and BNP-A (5 ?g/ rat) increased regional cerebral blood flow following i.v. injection in rats; 3) intracerebroventricular injection of BNP (3 ?g/ rat) induced an obvious natriuretic effect when blood volume was extremely expanded with hypoosmotic glucose solution in rats; 4) BNP (5 ?g/ rat) infusion for 60 minutes caused a decline of plasma concentration of angiotensin II and aldosterone at the end of infusion in spontaneous hypertensive rats (SHR), and the bolus injection of BNP (10 ?g/ rat) led to a fall in blood pressure about 1.33kPa. We conclude that BNP is an important peptide in regulating water and sodium metabolism and plays an important role in regulating blood flow to various organs.
3.Clinical effect of the low molecular heparin in the treatment of oligohydramnios in late pregnancy and its in-fluence on pregnant women blood coagulation function

ObjectiveToinvestigatetheclinicaleffectofthelowmolecularheparininthetreatmentofoli-gohydramnios in late pregnancy and its influence on pregnant women blood coagulation function.Methods According to didital table 100 patients with oligohydramnios in late pregnancy were divided into the observation group and control group.The treatment group was given routine treatment,and the observation group was given low molecular heparin therapy on the basis of conventional treatment.Results The clinical curative effect score of the observation group was (8.52 ±1.48)points,which was obviously higher than (5.03 ±1.26)points of the control group,the difference was statistically significant(P<0.05).Conclusion For patients with oligohydramnios in late pregnancy,low molecular heparin can be a significant boost for the clinical treatment effect of patients,can promote the use in clinic.
4.CT diagnosis of endodermal sinus tumor of the pediatrics reproductive system

Objective To investigate the charateristic CT features of pediatircs reproductive system of endodermal sinus tumor. Methods Thirteen cases with pediatircs reproductive system of endodermal sinus tumor confirmed by surgery and biopsy pathology all underwent CT and contrast-enhanced CT scans,and theire imaging features were then ananlyzed.Results On CT scan,7 cases of testicular or vagina endodermal sinus tumor of children appeared soft tissues mass with uneven density,small patchy hypodense shadows,and ill-defined boundaries.Contrast-enhanced CT scans reveal the solid masses were obvious uneven continuous enhance-ment,but small patchy hypodense shadows were not enhanced,local envelope were enhanced.CT scans reveal that 6 cases of ovaries endodermal sinus tumor showed the large cystic solid mass,predominantly cystic mass,including 2 cases of punctate and patchy cal-cification,and contrast-enhanced CT scans showed that solid part were enhanced obviously and with obvious strengthening vessels shadows.1 case of right ovaries endodermal sinus tumor with fat density shadow and small patchy calcification.Its pathological di-agnosis was the endodermal sinus tumors with mature cystic treatomas.Conclusion CT image has an advantage in the diagnosis of endodermal sinus tumor of the children's reproductive system,especially combined with serum AFP determination.
5.Effect of psychological contract on innovative behavior in nurses

Objective To explore the effect of psychological contract on innovation behavior in nurses.Methods 266 nurses were questionnaired with Psychological Contract Scale and Innovative Behavior Scale.And then,explored the relationship between the psychological contract and innovative behavior.Results ①The average score of nurse innovative behavior was (4.58 ± 0.73),which was in the middle level.There were significant differences between the scores of innovative behavior of nurses in the different age,or work experience,or professional titles.②Pearson correlation analysis showed that nurse innovative behavior score was significantly negatively correlated with each dimension of psychological contract(r=-0.542-0.441,P<0.05).③The multiple hnear regression analysis showed that the scores of development responsibility of hospital,realistic responsibility of hospital,personal development responsibility and personal realistic responsibility were the independent influencing factors of nurse innovative behavior.Conclusions Psychological contract is the important influencing factor of nurse innovative behavior.
6.The neurodegeneration of pretectal area in the early stage of diabetes

Objective To analyze the expression of BDNF, NGF,GFAP in Wistar rat pretectel area and determine the relation between neuron damage and neurotrophin change of pretectal area and retinal neurodegeneration in the early stage of diabetes. Methods Male Wistar rats weighing about 200 g were used to create diabetic animal model by injecting STZ. Wistar rats were killed by heart perfusion with 4% FDP at 1-week, 2-week and 1-month after diabetes. The dissected pretectal area was used to determine the expression of BDNF, NGF and GFAP with immunohistochemistry. Results In pretectal area of diabetic rat, the expression of BDNF and NGF began to decrease from 1-week DM, the expression of GFAP began to increase from 2-week DM. Conclusions There are neuron damage and neurotrophin decrease in the pretectal area in the early stage of diabetes. Neurodegeneration and neurotrophin decrease in pretectal area, which is the target tissue of retina, might be one of the reasons resulting in retinal neurodegeneration at the early stage of diabetes.
7.Analysis of prognostic factors of epithelial ovarian carcinoma

Objective To assess prognostic factors impacted on overall survival in patients with epithelial ovarian carcinoma. Methods Totally 170 patients with stages Ⅰ-Ⅳ epithelial ovarian carcinoma admitted in Shanxi Provincial Cancer Hospital from Jan 2002 to Dec 2005 were analyzed by retrospective analysis. Results The results showed that the prognostic factors of epithelial ovarian carcinomas were related to age, stage, histological type, pathological differential grade, the size of residues lesions and the number of course of chemotherapy (P<0.01). The univariate analysis showed that family history was not related to the survival of epithelial ovarian carcinoma (P > 0.05). Compared with stage Ⅳ,the risk of mortality was 0.005 for stage Ⅰ (95% CI, 0.001-0.024), 0.106 for stage Ⅱ (95% CI,0.038-0.297) and0.361 (95% CI, 0.181-0.718)for stage Ⅲ (P<0.01). The risk of mortality was 0.307 (95% CI, 0.176-0.536) for the patients with residual diameter >2 cm, in comparison with the residual ≤2 cm (P < 0.01). The risk of mortality in patients received < 6 courses of chemotherapy was 8.191 times higher than that in patients received ≥6 courses of chemotherapy (95% CI, 4. 666-14. 379;P < 0.01). Conclusions The major independent prognostic variables for epithelial ovarian carcinoma are stage, the size of residual tumor lesions and the number of courses of chemotherapy. Therefore, the earlier diagnosis, the earlier surgery, sufficient cycles and timely assistant chemotherapy are the key point to improve the survival rates of epithelial ovarian carcinoma.

To improve the survival quality of patients and extend their life time as far as possible, and according to the recurrence epithelia ovarian carcinoma's different situation, we can selectively perform surgery, and combined it with appropriate chemotherapy and biological treatment. This article summarized its treatment from surgery, chemotherapy and biological therapy, these three aspects.
9.Yigong powder with Yupingfeng powder for syndrome of spleen deficiency in children with mycoplasma pneumoniae pneumonia

ObjectiveTo evaluate the clinical efficacy ofYigongpowder withYupingfengpowder for syndrome of spleen deficiency in children with mycoplasma pneumoniae pneumonia.MethodsA total of 60 patients who had spleen deficiency after azithromycin injection 5 days were randomly divided into treatment group and control group according to the random number table, with 30 in each group. The patients in both received azithromycin suspension for 2 weeks, while the patients in the treatment group received azithromycin suspension and traditional Chinese medicineYigongpowder withYupingfengpowder for 2 weeks. The symptoms score was used to evaluate the clinical efficacy.ResultsThe scores of appetite loss (0.65 ± 1.00vs. 2.32 ± 1.28,t=5.631), abdominal distention (0.73 ± 0.23vs.1.33 ± 0.97,t=3.297) and fatigue (0.72 ± 0.32vs. 1.42 ± 1.18,t=3.136) in the treatment group were significantly lower than those in the control group (all P<0.05). The total effective rate for syndrome of spleen deficiency in the treatment group was significantly higher than thatin the control group(90.0%vs.66.7%;χ2=4.812,P<0.05).ConclusionYigongpowder withYupingfeng powder can significantly improve the symptoms of syndrome of spleen deficiency in children with mycoplasma pneumoniae pneumonia.
10.Progression on the medical nutrition therapy of type 2 diabetes mellitus

Medical nutrition therapy (MNT)is the basic and long-term treatment of diabetes patients. At present,the dietary structure of type 2 diabetes mellitus(T2DM)patients in China has yet to be further improved. This article focused on the effect,dietary structure,food selection and guarantee of compliance of MNT in T2DM.We recommended that the dietary pattern should be designed based on the traditional dietary habits around the country,combined with people living and working reality. The rational utilization of network platform can improve the efficiency and compliance of nutrition education.
