2.Clinical characteristic and treatment of maxillofacial trauma in the elderly

Objective To analyze the clinical characteristic and provide therapeutic techniques of maxillofacial trauma in the elderly. Methods Geriatric maxillaofacial trauma patients admitted to our hospital from January 1992 to December 2002 was retrospectively reviewed. Data were collected according to etiology, location, comorbidities, associated injuries, therapeutic methods and the outcome. The patients were compared with a control group consisting of 200 young and adult patients admitted at the same period. Results For the 126 geriatric trauma patients, the major causes were traffic accidents(61.1%) and violence assault(25.4%). A large proportion of facial injuries occurred on the lower part of the face. 69.1% of the patients presented with preexisting diseases. Most of the fractures were not treated(35.7%). 3.Investigation of the optimal surrogate for forced vital capacity in the diagnosis for chronic obstructive pulmonary disease

Objective To evaluate whether the forced expiratory volume in sixth scend(FEV6) was the optimal surrogate for forced vital capacity (FVC) in the diagnosis for chronic obstructive pulmonary disease (COPD).Methods Retrospectively analyzed the spirometric data of 142 COPD patients (7 cases with mild COPD,60 cases with moderate COPD,48 cases with severe COPD,27 cases with extremely severe COPD) admitted to Hebei Chest Hospital from October 2011 to October 2012 who had FEV6 data.FEV6,FEV5,FEV4 and FEV3 were measured on volume-time curves and the diagnostic value was analyzed.Results FEV6,FEV5,FEV4 and FEV3 were highly correlated to FVC (r =0.994,0.939,0.935 and 0.923 respectively,P <0.001).Assuming =70% as the diagnostic standard for obstruction,FEV1/FEV6 had a diagnostic rate of 92.96% with a false negative rate of 7.04%.FEV1/FEV5,FEV1/FEV4 and FEV1/FEV3 had higher false negative rates (9.86%,13.38% and 27.46% respectively) than FEV6.The value of FEV1/FEV6-FEV1/FVC had no statistical significance between the mild-moderate and the severe-extremely severe COPD groups(t =1.376,P =0.171).Conclusion There is a strong correlation between FEV6 and FVC.FEV6 may be the best surrogate for FVC in the diagnosis of chronic obstructive pulmonary disease.Elevating diagnostic critical value can reduce the false negative rate.
4.Expression of glucose transporter-1 and its correlation with basic fibroblast growth factor and proliferating cell nuclear antigen in epithelial ovarian neoplasm

Objective To study the expression of glucose transporter-1 (GLUT_1) and its correlation with basic fibroblast growth factor (bFGF) and proliferating cell nuclear antigen (PCNA) in epithelial ovarian neoplasm. Methods Streptavidin-peroxidase complex technique was used to examine the expression of GLUT_1, bFGF and PCNA protein in six cases of normal ovarian tissue, 20 cases of benign epithelial tumors, seven cases of borderline tumor and 44 cases of epithelial ovarian carcinoma. Results In normal ovary and benign ovarian tumor, GLUT_1 was not detected,but in borderline ovarian tumor and cancer, the positive expression ratio of GLUT_1 was 6/7 and 91%(40/44), respectively. The intensity of GLUT_1 in ovarian epithelial neoplasm was significantly higher than in borderline tumors. The staining intensity of GLUT_1 was significantly correlated with the histological grade of the tumor (r_S=0.499, P=0.001), and was positively correlated with the clinical stage, cancer invasion and lymph node metastasis. GLUT_1 staining was intense in cytoplasmic membrane, and was stronger in areas far away from blood vessels and near the necrotic center. GLUT_1 expression level did not show any association with histology type (P=0.513). bFGF positive rate in tumor was 57%(25/44). Objective To evaluate early diagnostic value of quantitative analysis of contrast-enhanced ultrasound (CEUS) in acute radiation-induced liver injury.Methods Sixty female rats were divided into two groups:50 rats in an experimental group (model group) and 10 rats in a negative control group.The rats in model group were radiated with stereotactic single dose of 20Gy on their liver to establish acute radiationinduced liver injury models.Each 10 rats from model groups and 2 rats from control group were randomly selected and underwent CEUS and histopathological examination on the 3,7,14,21,28 days after radiation.The degree of injury was classified into four groups according to pathological grading:non-injured group,mild injured group,moderated injured group and severe injured group.The dynamic images of CEUS were off-line analysis and the parameters of arrival time of contrast agent to hepatic artery (HAAT),the arrival time of contrast agent to hepatic vein (HVAT),and the transit time of hepatic artery-hepatic vein (HAHVTT) were recorded.Time intensity curve (TIC) of liver parenchyma drawn by the software of quantitative analysis was used to obtain quantitative parameters including time to peak (TTP) and peak intensity (PI).Results Along with the severity degree of radiation-induced liver injury,the quantitative parameters,PI decreased while TTP extended.PI of mild injured group,moderated injured group and severe injured group were lower than that of non-injured group (P <0.05).TTP of the three liver injuried groups was higher than non-injured group (P <0.05).The quantitative parameters HA-HVTT of moderated injured group and severe injured group were decreased than the non-injured group (P <0.05),whereas the difference between mild injured group and non-injured group was not significant.Conclusions Quantitative analysis of CEUS can provide a certain value for early diagnosis in acute radiation-induced liver injury.
6.Clinical significance of serum miR-216a in diagnosis and prognosis monitoring of patients with acute pancreatitis

Objective To detect altered levels and clinical significance of serum miR-216a and prognosis monitoring in acute pancreatitis (AP) patients.Methods Serum miR-216a levels were determined by quantitative reverse-transcription PCR (qRT-PCR) assay among 80 mild acute pancreatitis (MAP) patients,80 severe acute pancreatitis (SAP) patients and 74 healthy controls.And amylase (AMY),lipase (LPS),Ca2+,glucose (Glu),hematocrit (HCT),triglyceride (TG),total cholesterol (TC) and C reactive protein (CRP) were measured by biochemical analyzer.The clinical usefulness of miR-216a for AP patients was assessed by ROC curve analysis and correlation analysis.Results Compared with healthy controls (11.12 × 10-5 [5.83 × 10-5,19.12 × 10-5],the serum miR216a levels were significantly increased in AP patients(38.49 × 10-5 [24.05 × 10-5,62.02 × 10-5],(U =-9.10,P < 0.01) . The serum miR-216a levels in MAP and SAP patients were (36.46 × 10-5 [22.29 × 10-5,55.80 × 10-5] vs 40.44 × 10-5 [25.84 × 10-5,65.48 × 10-5]),there was no significant difference between MAP and SAP patients (U =-0.96,P > 0.05).The areas under ROC curve (AUCROC) of miR-216a for differential healthy controls and AP patients was 0.870 (95% CI:0.825-0.915),cut-off value is 0.61.AUCROC of miR-216a for differential healthy controls and MAP patients was 0.865 (95% CI:0.808-0.921),cut-off value is 0.59.And AUCROC of miR-216a for differential healthy controls and SAP patients was 0.876 (95% CI:0.822-0.930),cut-off value is 0.66.Moreover,after the clinical improvement of the patients,the levels of serum miR-216a were significantly lowered from (41.88 × 10-5 [24.24 × 10-s,64.44 × 10-5]) to (20.58 × 10-5 [11.01 × 10-5,41.91 × 10-5]),the differences was significant(U =5.24,P < 0.0l).Correlation analysis showed that miR-216a was positively correlated with CRP (r =0.215,P =0.006) in AP patients.Conclusion The levels of miR-216a in serum of AP patients were increased,which is a potential biomarker for the diagnosis and prognosis monitoring of AP.

8.Advanced sensate proper digital artery escalating flaps for repair of fingertip skin defect

Objective To investigate the feasibility of advanced sensate proper digital artery escalating flaps in reconstruction of fingertip skin defect.Methods Between January 2012 and February 2015,15 cases (17 fingers) of fingertip skin defect accompanied with exposed tendon or phalange were treated,including 9 males and 6 females,with mean 41 years of age (range,7-55 years).Injury resulted from hit by heavy objects in 5 cases,knife cutting in 4,machine twist injury in 4 and mass resection in 2.Fingertip defects were covered with the flaps carrying proper digital artery and nerves in order to one-stage reconstruct finger pulp epicritic sensibility and avoid onychogryphosis.Joint function was assessed with the standard set up by Chinese Medical Association Hand Surgery Branch.Results All flaps survived.One case had ankylosis of the middle interphalangeal joint caused by poor weight-bearing exercise and was improved after exercise.Two cases with large defect in the distal finger pulp had slight onychogryphosis after operation.Period of follow-up was 6-14 months (mean,8 months).The injured fingers showed good appearance and sense,with two-point discrimination of the flap ranging from 6-8 mm.There was no linear scar or tenderness in the finger tip.Hypertrophic scar was not obvious in the flap donor area.Function assessment was excellent in 16 fingers and good in 1 finger.Conclusion The flaps result in good blood supply,good sense,no onychogryphosis and no linear scar and are easy to be operated,indicating an ideal method to reconstruct fingertip defect.
9.Current situation and countermeasures of smoking cessation service system construction in China

Abstract
In order to achieve the goal of reducing the smoking rate among Chinese adults at ages of 15 years and above to below 20% by 2030, smoking cessation services require to be reinforced. The current Chinese smoking cessation service system includes short-term smoking cessation interventions and smoking cessation hotlines at the population level, and smoking cessation clinics at the individual level; however, these smoking cessation services suffer from problems of low awareness, low accessibility and low utilization rate. Based on the publications pertaining to smoking cessation services and released policy documents in China until June 2022, this review analyzes the current status, problems and causes of smoking cessation services, and proposes suggestions for improving the smoking cessation service system construction in China.
10.Analysis of cognitive function and blood glucose level in patients with Parkinson disease

Objective To study the relationship between level of plasma glucose and cognitive function in patients with Parkinson's disease.Methods Two hundred PD patients were assessed cognitive function using Mini-Mental State Examination (MMSE),Montreal Cognitive Assessment (MoCA),Wechsler Intelligence Scale and Wechsler Memory Scale.The patients were divided into cognitive normal group (n=91) and cognitive impairment group (n=109).One hundred twenty-six normal subjects were enrolled as control group (n=126).The levels of fasting plasma glucose (FPG),postprandial plasma glucose (2hPPG),glycosylated hemoglobin (HbAlc) and the prevalence of diabetes mellitus were compared among the groups.The effect of blood glucose level on the cognitive function of PD patients was analyzed by Binary Logistic Regression.Results The levels of FPG,HbAlc and the prevalence of diabetes mellitus [5.19 (0.72),5.7% (0.5%),14%] were significantly higher than those in the normal control group [4.85(0.79),5.6% (0.5%),6%] (P<0.05).The levels of FPG in PD patients with cognitive impairment [5.21 (1.32)] was significantly higher than that in PD patients with cognitive normal group [4.81 (0.95)] (P<0.05).Although 2hPPG and HbAlc increased slightly in PD patients with cognitive impairment,the difference did not reach an significant level (P>0.05).Binary Logistic Regression analysis showed that FPG(OR:1.764;95% CI:0.06-3.244;P=0.068) was not associated with the impaired cognitive function in PD patients.Conclusion The present study has not revealed an association between the incidence of cognitive impairment in patients with PD and plasma glucose level although high plasma glucose may be a high risk factor for PD patients.
