1.Effect of one-dose multiple point scanning PRP treatment on patients with NPDR

AIM: To investigate the clinical effect of one-dose multiple point scanning panretinal photocoagulation (PRP) in the treatment of non-proliferative diabetic retinopathy (NPDR).METHODS:Selected 78 patients 132 eyes with NPDR in our hospital from January 2015 to June 2016,the patients were randomly divided into observation group (42 patients of 72 eyes) and control group (36 patients of 60 eyes).The observation group was given one-dose multi-point scanning PRP.The control group was given a single point scanning,3 to 4 times to complete the PRP.To observethe therapeutic effect in two groups,the average visual field threshold,the flash electroretinogram (F-ERG) a,b wave amplitude,the laser energy and so on were observed.RESULTS:The effective rate of the observation group and the control group were 84.7% and 83.3%,the difference was not statistically significant (P>0.05).In the observation group and the control group,at 6mo after treatment,the leakage area of retinal neovascularization was lower than that before treatment (P<0.05).The visual field threshold and F-ERG a wave amplitude differences were not statistically significant (P>0.05).F-ERG b wave amplitude were 221.94±70.18mV and 219.82±69.56mV in the observation group and the control group at 6mo after treatment which were significantly lower than that before treatment (P<0.05).The laser energy of the observation group was 541.23 ± 56.39mW,significantly higher than the control group 326.39±78.83mW (P<0.05),while the energy density was 0.34±0.14mW·ms/mm2,significantly less than the control group 2.01±0.97mW·ms/mm2(P<0.05).The incidence of complications in the observation group and the control group were 8.3% and 15.0%,the difference was statistically significant (P<0.05).CONCLUSION:The clinical effect of multiple point scanning PRP in the treatment of NPDR is better than single point multiple PRP with advantages of lower energy density and less laser damage.
2.Expression of VEGF165 and VEGF165b in human retinal pigment epithelial cells in hypoxia and high glucose environment

·AIM:Through the expression of VEGF165and VEGF165bin human retinal pigment epithelial cells in vitro in artificial simulated hypoxia and high glucose environment, to discuss their roles in the development of diabetic retinopathy and the relationship between each other. ·METHODS: After normal inoculation and cultivation of human retinal pigment epithelial cells (RPE) in vitro, the cells was divided into the normal group (5. 56mmol/L glucose,without CoCl2),the hypoxia group(5.56mmol/L glucose + 150μ mol/L CoCl2), the high glucose group (25mmol/L glucose, without CoCl2), the combination group (25mmol/L glucose + 150μ mol/L CoCl2),a total of four groups. The RNA of each group was extracted respectively in 12h,24h,36h,and 48h. We used the MTT colorimetry to detect cell vitality and growth trend; RT-PCR method to detect VEGF165and VEGF165brelative expression of mRNA of RPE cells in four different time points. ·RESULTS:Hypoxia and high sugar environment limited proliferation of RPE cell division and cell vitality. After comparing cells of the same group in different time points, in the normal group there was no statistically significant different expression over time (P>0.05); the expression in the hypoxia group, the high glucose group and the combination group increased over time, the difference was statistically significant(P<0. 05). At the same time,differences of the expression between groups was not statistical significant in 12h (P > 0. 05); the difference was statistically significant in 24h,36h,48h(P<0.05). ·CONCLUSION: Cultured RPE cells can express VEGF165b normal. Lack of oxygen and high glucose can induce the increase of VEGF165 mRNA, at the same time reduces the VEGF165bmRNA expression.
3.Neuroprotection of sevoflurane against the ischemia injury in hippocampal slices of rat.

AIMTo study the protective effects and the mechanisms of sevoflurane on ischemic cerebral neurons.
METHODSWith electrophysiological microelectrode recoding technique, the OPS of hippocampal slices deprived with oxygen and glucose (OGD) and injured from toxicity of glutamate (Glu) in the control group, 2% sevoflurane group and 4% sevoflurane group were observed. The changes of ultrastructure in the three groups were also observed respectively.
RESULTSIn the control group and 2% sevoflurane group it didn't show the improvement of recovery in OPS of hippocampal slices injured from OGD and Glu. In 4% sevoflurane group the recovery degree and the recovery rate of OPS were obversely. With electricmicroscope, it was founded that in the control group and 2% sevoflurane group, the pyramidal neurons in CA1 regions deprived with glucose and oxygen and exposured by Glu were damaged. Intercellular edema were severe, the nucleus membranes were not complete, the chromatin formed mass, the endoplasmic reticulum in the cytoplasm were degenerate, mitochondrion swelled. In 4% sevoflurane group, the pyramidal neurons in CA1 regions did not swell obviously, the nucleus was clear, the nucleus membranes were complete and the mitochondria swelled lightly.
CONCLUSION4% sevoflurane could protect hippocampal neurons deprived with glucose and oxygen from the damage. The probable mechanism is 4% sevoflurane reduced the excitatory of Glu.

4.Case-control study on superior labrum from anterior to posterior repair and biceps tenodesis for the treatment of type II SLAP injury.

OBJECTIVETo compare clinical outcomes of superior labrum from anterior to posterior (SLAP) repair and biceps tenodesis in treating type I SLAP injury.
METHODSFrom March 2009 to March 2012, 38 patients with type II SLAP injury were treated with SLAP repair and biceps tenodesis, and all patients were unilateral SLAP injury. Sixteen patients treated with biceps tenodesis included 8 males and 7 females with an average age of (49.3±3.7) years old (ranged, 45 to 54); 10 cases were on the left side and 6 cases on the right side; 10 cases were caused by falling down, 2 cases were caused by throwing damage and 4 cases were caused by daily life damage; the time from injury to operation were from 3 to 8 weeks. Twenty-two patients treated with SLAP repair included 14 males and 8 females with an average age of (49.0±2.8) years old (ranged, 44 to 56); 13 cases were on the left side and 9 cases were on the right side; 14 cases were caused by falling down, 5 cases were caused by throwing damage and 3 cases were caused by daily life damage; the time from injury to operation were from 3 to 7 weeks. Preoperative, postoperative at 6 months, 1 year and 2 years' UCLA and SST score were compared between two groups.
RESULTSThere was no significant differences in UCLA and SST score between two groups before operation. At 6 months after operation, UCLA and SST score in biceps tenodesis group was higher than SLAP group, and action,range of anteflexion, strength of anteflexion, degree of satisfaction in biceps tenodesis group was higher than SLAP group. There was no significant meaning in SST and UCLA score between two groups at 1 and 2 years after operation.
CONCLUSIONShort-term efficacy of biceps tenodesis for SLAP injury is better than SLAP repair, but long-term efficacy is fairly.

5.Effects of rosiglitazone on aortic function in rats with insulin resistant-hypertension.

Rosiglitazone (ROSI), thiazolidione peroxisome proliferator-activated receptor-gamma (PPAR-gamma) activator, reduces insulin resistance in patients with type 2 diabetes (T2DM). It also improves vascular reactivity in T2DM patients and some animal models by unclear mechanisms. In order to investigate the effect of ROSI on aortic systolic and diastolic function of insulin resistant-hypertensive rats (IRHR) and the underlying mechanism, male Sprague-Dawley (SD) rats were fed with high fructose (HF) for 8 weeks to induce IRHR model. To verify IRHR model, systolic blood pressure (SBP), fasting blood sugar (FBS), fasting serum insulin (FSI) were measured respectively in each group, and insulin sensitive index (ISI) was also calculated. Subsequently, the vascular function test was performed. The thoracic aortic ring of SD rats was mounted on a bath system. The effect of rosiglitazone on the contraction elicited by L-phenylephrine (PE) and potassium chloride (KCl) and the relaxation induced by acetylcholine (ACh) and sodium nitroprusside (SNP) were measured. To explore the mechanism, nitric oxide synthase (NOS) inhibitor N(omega)-nitro-L-arginine methyl ester (L-NAME) was used and serum nitric oxide (NO) was measured. The results obtained were as follows: (1) Rosiglitazone reduced the level of SBP, serum insulin and improved insulin resistance in IRHRs. (2) The contractive responses of thoracic aortic rings to PE and KCl were enhanced and the relaxation response to ACh was depressed significantly in the HF group, and the effect was reversed by ROSI. (3) After pretreatment with L-NAME, the relaxation response to ACh was further impaired in the HF group, this effect was partly reversed by ROSI. (4) Sodium nitroprusside (SNP)-induced vasodilator responses did not differ significantly among the groups. (5) Aortic systolic and diastolic function of the control group was not affected markedly by ROSI. (6) Compared with the control group, serum nitric oxide was significantly reduced in the HF group, but after rosiglitazone treatment it was remarkably increased. These findings suggest that ROSI can improve aortic diastolic function of insulin resistant-hypertensive rats, the mechanism of this effect might be associated with an increase in nitric oxide mediated partly by NOS pathway, a decrease in the level of blood pressure, serum insulin and the improvement of insulin resistance.

6.Acquisition of multidrug resistance by overexpression of Twist1 in colorectal cancer cells SW620 and its mechanism

Objective: To investigate the effect of overexpression of Twist1 on multidrug resistance( MDR) in colorectal cancer SW620 cells and its possible mechanism. Methods: The colorectal cancer SW620 cells were transfected with Twist1 overexpressing lentivirus and negative control virus, which were called experimental group ( SW620-Twist1) and control group ( SW620-NC) respectively. The protein and mRNA expression of Twist1、ABCB1 and ABCG2 in two groups were detected by Western blot and RT-qPCR, respectively. The growth inhibitory rate of 5-Fluorouracil(5-FU) and Oxaliplatin( OXA) at 24 ,48 and 72 h in two groups were determined by CCK8 assay. Cancer stem cell markers CD133,CD44 expression levels were detected by Flow cytometry. Results: The experimental group was compared with the control group, the expressions of Twsit1 gene mRNA and protein increased significantly( P<0. 01) ,Twist1 gene overexpressing SW620 cell line was successfully constructed. The inhibition rates of cell proliferation of the three chemotherapeutic drugs at 48 h and 72 h were significantly decreased ( 48 h, P<0. 05 ; 72 h, P<0. 01) . The mRNA and protein expression levels of ABCB1,ABCG2 were significantly increased( P<0. 01). The expression of CSCs markers CD133 and CD44 increased significantly (P<0. 01). Conclusion: Overexpression of Twist1 can promote the expression of ABCB1 and ABCG2 genes in SW620 cells and make the tumor cells acquire MDR. This phenomenon may be related to the promotion of stemness.
7.Genetic evolution of non-structural gene among avian influenza H5N1 viruses isolated from the boundary of Yunnan province.

OBJECTIVETo elucidate the characteristics of variation and the genetic evolution of non-structural protein (NS1, NS2) genes related to avian influenza subtype H5N1 viruses isolated from the boundary region of Yunnan province.
METHODSSwab samples were collected from foreign poultry and wild birds in the boundary regions of Yunnan province and screened by H5/N1 subtype-specific multiplex RT-PCR. The NS segment of H5N1 virus from the positive samples were amplified by RT-PCR and cloned into pMD18-T vectors for sequencing. The alignment and phylogenetic analysis on those available NS1, NS2 genes were performed with sequences of the known reference strains.
RESULTS71 positive samples were identified from 1240 samples, with the positive rate as 5.72%. Fourteen different NS segment sequences were obtained from 30 representative positive samples and could be divided into 3 distinct clades or sub-clades (I-1, I-2 and II), by phylogenetic analysis. The NS1/NS2 genes and Hemagglutinin (HA) genes of H5N1 viruses from the boundary regions of Yunnan province showed different relationships regarding the characteristics on genetic evolution. The substitution or mutation of key amino acids sites had been noticed in the nuclear location signal domains, effect domain, and other pathogenicity markers.
CONCLUSIONNS genes of H5N1 subtype viruses in boundary region of Yunnan province showed genetic divergence and the virus of clade I-2 and II had become dominant epidemic strains in this region since 2010.

A total of 14 different M2 sequences were obtained from 30 positive samples and were divided into 3 distinct clades or sub-clades(1.2.1, 1.2.2 and 2) by phylogenetic analysis, 5, 7 and 2, respectively. The M2 genes and Hemagglutinin(HA) genes of H5N1 viruses from the boundary region of Yunnan province had showed different relationship of genetic evolution. The substitution or mutation of key amino acids sites had been found among the domains of epitope, adamantane-resistance, and poultry or human original viral strains.
CONCLUSIONThe M2 genes of H5N1 subtype viruses in boundary region of Yunnan province from 2008 to 2012 showed genetic divergence and the virus of clade 1.2.2 had become dominant epidemic strain in this region.

9.Platelet count of peripheral blood is associated with invasion and metastasis of esophageal carcinoma.

OBJECTIVETo analyze the association between the platelet count of peripheral blood and clincopathologic parameters of esophageal carcinoma.
METHODSPlatelets of peripheral blood were measured in 415 cases of esophageal carcinoma and 325 healthy subjects as control. The correlation of platelet counts and clinicopathological features of cancer was analyzed.
RESULTPlatelet count in patients with esophageal carcinomas (286+/-88)x10(9)/L was significantly higher than that in the control subjects [(204+/-114)x10(9)/L, P<0.05 ]. Increased platelet counts (>300 x 10(9)/L) was significantly associated with tumor infiltration and lymph node metastasis in patients with esophageal cancer (P<0.05).
CONCLUSIONPlatelet count of peripheral blood might be associated with the development and progression of esophageal cancer.

10.Association between chronic hepatitis B virus infection anddiabetes among adults

Objective:
To explore the association between chronic hepatitis B virus infection and diabetes among adults.
Methods:
The baseline data of China Kadoorie Biobank ( CKB ) study from Tongxiang of Zhejiang Province was used for analysis. Community residents were investigated in the study from August 2004 to May 2008, including questionnaire survey, physical measurement and biological sample test. Univariate and multivariate logistic regression models were used to estimate the association of chronic hepatitis B virus infection with diabetes.
Results:
Totally 52 888 participants were included in the final analysis. The overall prevalence of HBsAg-positive was 3.55% ( N=1 877 ). The overall prevalence of diabetes was 5.17% ( N=2 733 ). The prevalence of HBsAg-positive in diabetic patients was 3.51% ( N=96 ). Both univariate and multivariate logistic regression models indicated that there was no association between chronic hepatitis B virus infection and diabetes( P>0.05 ).
Conclusion
No significant association has been found between chronic hepatitis B virus infection and diabetes among adults.
