1.Evaluation on levator ani muscle injuries after vaginal delivery with MRI

Objective To explore the MRI finding of female normal levator ani muscle and the levator ani muscle injuries and to evaluate the correlation between childbirth and levator ani muscle injuries. Methods One hundred asymptomatic nulliparous women(control group)and 200 vaginally primiparous women(study group)were selected as the object of this study.Moreover,the study group was divided into two subgroups:group A(100 cases)with stress incontinence,group B(100 cases)without clinical symptoms.Multiplanar proton density magnetic resonance images were obtained at 0.5 cm intervals from these study individuals.All images were used to analyze the differentiation of MRI features between normal levator ani muscle and levator ani muscle injuries.Results No levator ani injuries were identified in the control group.Fifty-four primiparous women(27%)had visible injuries in their levator ani muscles,42 in group A and 12 in group B.Injuries were identified in the puborectalis muscle in 49 cases and in the iliococeygeus muscle in 5 cases(X~2=41.447,P
2.Effect of shenhuang ointment in promoting the rehabilitation of postoperative gastrointestinal dysfunction patients of Qi stagnation blood stasis syndrome: a clinical observation.

OBJECTIVETo observe the effect and safety of plastering Chinese Compound Shenhuang Ointment (CSO) at Shenque (RN8) in promoting the rehabilitation of postoperative gastrointestinal dysfunction patients of qi stagnation blood stasis syndrome (QSBSS).
METHODSA prospective, multi-centered, randomized, double-blinded, controlled trial was conducted in 220 postoperative gastrointestinal dysfunction patients of QSBSS. They were randomly assigned to two groups, the CSO group (110 cases) and the placebo group (110 cases). CSO was plastered at Shenque (RN8) for 5 days after operation. The time of exhaustion, defecation, the recovery of intestinal peristalsis, integrals of TCM syndrome, and serum levels of motilin (MOT)and somatostatin (SS) were observed.
RESULTSCompared with the placebo group, the condition of exhaustion and defecation, the recovery of intestinal peristalsis on the 3rd day after operation was all improved (P < 0.05). The integrals of TCM syndrome at day 2, 3, and 4 were more significantly lowered in the CSO group than in the placebo group (P < 0.01, P < 0.05). The total effective rate of TCM syndrome was 95.3% in the CSO group, better than that in the placebo group (91.8%, P < 0.05). Compared with the placebo group, the serum MOT level increased and the serum SS level decreased at day 5 after operation in the CSO group (P < 0.05).
CONCLUSIONSThe plastering of CSO at Shenque (RN8) could advance the time of exhaustion and defecation, and improve patients' clinical symptoms. And patients could tolerate well.

3.Effect of allyl glycoside extracted from Herba Rhodiolae on activity and proliferation of cultured Schwann cells under high glucose condition.

OBJECTIVETo observe changes in activity and proliferation of cultured Schwann cells under the high glucose condition and effect of allyl glycoside extracted from Herba Rhodiolae (AG) on these changes for exploring the possible mechanism of diabetic peripheral nerve injury and the pharmacologic action of AG on them.
METHODSAdopting refined Brokes method, Schwann cells were isolated from the sciatic nerve tissue of newborn Wistar rats and purified. Their activity and proliferative capability were determined using XTT method and 3H-TdR incorporative method respectively.
RESULTSHigh glucose showed marked inhibitory effect on both activity and proliferative capability of Schwann cells, and the inhibition could be markedly improved by AG.
CONCLUSIONDiabetic peripheral nerve injury is possibly related to inhibitory effect of high glucose on activity and proliferative capability of Schwann cells. Effect of AG in improving these inhibitory changes provides the experiment basis for clinical use of traditional Chinese medicine, Herba Rhodiolae.

4.The expression of Maspin protein in oral squamous cell carcinoma and its significance.

OBJECTIVETo investigate the role of Maspin protein in the development of oral squamous cell carcinoma (OSCC), with the effort to realize practical diagnosis use of them.
METHODSThe expression status of Maspin protein in 45 cases of OSCC, 33 cases of paratumor tissue and 15 cases of normal tissue was examined by immunohistochemical staining.
RESULTSThe positive rates of Maspin protein were 86.67% (13/15), 72.73% (24/33) and 37.78% (17/45) in normal tissue, paratumor tissue and OSCC. The expression of Maspin protein increased respectively in OSCC, paratumor tissue and normal tissue. The expression were different between OSCC and normal tissue, OSCC and paratumor tissue, paratumor tissue and normal tissue (P<0.05). The expression of Maspin protein was correlated with lymph node metastasis, histological grade (P<0.05), but not with TNM staging (P>0.05).
CONCLUSIONMaspin protein may play an important role in the development of OSCC. The expression level of Maspin protein detected in early time might be a useful prognostic marker for the diagnosis of lymph node metastasis.

6.Resources and application of She's nationality wild medicinal plants.

To make a thorough investigation of the common She's nationality wild medicinal plants resources in our country, including the species, the distribution, the folk application and the endemic medicinal plant species, Field surveyed was conducted with 25 She people mainly lived area (county, district or city) throughout the country, the folk prescription and treatment cases provided by She's medical personnel, the drug usage and dosage, the commonly used traditional She's medicine and drug samples were collected. And the distribution, growing environment of these plants were investigated, their characteristics, photographs, GPS data and track were record , and the fresh wax leaf or plants specimens were collected. In total 1 600 varieties of folk medicine of She's nationality, 450 disease names and 1 016 prescriptions were collected. 520 kinds of these medicinal plants were commonly used, growing mainly distributed in the southeastern China, about 200 meters above sea level to 1 500 meters. There are 5 First-Grade State protection wild plants (medicinal), 15 second-Grade State protection wild plants (medicinal), and 11 She characteristic medicinal plants in our study, they belong to 144 families, 312 genera 494 species, 2 subspecies, 17 varieties, 3 forms and 1 cultivated varieties of She's nationality. Folk medicine usage is different from the traditional Chinese medicine and ethnic medicine. This survey finds out the common She's nationality wild medicinal plants resources in China, including the species, the distribution, the folk application and commonly used drugs, and found the rare and endangered medicinal plants and the She's nationality endemic medicinal plants, which provides a basis for further development and use the traditional She's medicine resources.

7.Clinical Characteristics of Alien Hand Syndrome:Eight Cases Report

Objective To report the clinical characteristics of alien hand syndrome(AHS). Methods Eight AHS inpatients from July,2015 to July,2017 were reviewed from the etiology,clinical manifestations, imaging characteristics and outcome. Results The etiology of the patients was cerebrovascular disease. The clinical features included alien hand (8 cases), movement disorder(8 cases),cognitive disorder(5 cases),speech disorder(4 cases)and affective disorder(3 cas-es).Imaging indicated the lesions involved callosum(8 cases),frontal lobe(2 cases),parietal lobe(3 cases),tem-poral lobe(2 cases)and basal ganglial region(1 case).There were four cases healing,two improving,two of re-mission after medicine and rehabilitation. Conclusion ASH is mainly resulted from cerebrovascular disease, in which callosum and frontal lobe are mostly in-volved,features in different forms,and prognoses well.
8.Comparative study between dynamic MRI and pelvic organography in diagnosis of pelvic floor disorders.

OBJECTIVETo evaluate the clinical value of simultaneously combined pelvic floor dynamic MRI and pelvic organography in diagnosing female pelvic floor disorders and search for the best imaging model for diagnosing pelvic floor disorders.
METHODSThirty women with pelvic floor disorders received pelvic floor dynamic MRI and simultaneously combined pelvic organography including cystourethrography, peritoneography, vaginography and defecography. Clinical diagnostic value was compared between this two methods.
RESULTSThe diagnostic accuracy of pelvic floor dynamic MRI and simultaneously combined pelvic organograph for cystocele,anorectal junction abnormal descent, pelvic floor hernia,uterocervical prolapse was 100%, 95.2 %, 86.7%, 85.7% respectively. Rectocele and prolapse of rectal were diagnosed by pelvic organograph in 12 and 28 cases respectively, while only 6 and 0 cases were diagnosed by pelvic floor dynamic MRI respectively. The mean examining time of pelvic floor dynamic MRI and simultaneously combined pelvic organography was (16 +/- 3)min, (34 +/- 9)min respectively (P< 0.01).
CONCLUSIONPelvic floor dynamic MRI combined with defecography is the best imaging model for diagnosing pelvic floor disorders.

9.Effects of pravastatin, fosinopril and their combination on myocardium TNF-alpha expression and ventricular remodeling after myocardial infarction in rats.

OBJECTIVETo investigate the effects of pravastatin, fosinopril and their combination on ventricular remodeling, cardiac function, tumor necrosis factor-alpha (TNF-alpha) mRNA expression, and matrix metalloproteinases (MMPs) activities after myocardial infarction (MI) in rats.
METHODSAcute myocardial infarction (AMI) was established by ligation of the anterior descending coronary artery in male Sprague-Dawly (SD) rats. Twenty-four hours after the procedure, the 48 surviving rats were grouped randomly as AMI control, fosinopril (10 mg.kg(-1).d(-1)), pravastatin (20 mg.kg(-1).d(-1)) and a combined use of the 2 drugs. Sham-operated group (n = 8) was taken randomly as non-infarction control. Six weeks after treatment with the drugs by gastric gavage, heart function and left ventricular remodeling were assessed. Left ventricular weight (LVW)/body weight (BW) ratio was determined. The relative expression of myocardium TNF-alpha mRNA was assessed by reverse transcription-polymerase chain reaction. Left ventricular myocardium MMPs activities were assessed by Zymography.
RESULTSThere were no significant differences among the four AMI groups in infarction size (P > 0.05). In comparison with the AMI group, left ventricular end-diastolic pressure, left ventricular end-diastolic diameter, LVW/BW all decreased significantly (P < 0.05 - 0.01); while dp/dtmax, dp/dtmin, fractional shortening (FS) and ejection fraction (EF) increased significantly in all three drug-treated groups (P < 0.05 - 0.01); increments of FS, LVEF and dp/dtmax were more evident in the combination group than either the fosinopril or pravastatin group (P < 0.05). The levels of TNF-alpha mRNA in AMI rats treated with fosinopril, pravastatin and their combination reduced 29%, 26% and 33%, respectively (P < 0.01); MMP-2 activity reduced 25%, 30% and 35%, respectively (P < 0.01); MMP-9 activity reduced 20%, 18% and 24%, respectively (P < 0.01). There were no significant differences in other variables among the 3 treatment groups (P > 0.05).
CONCLUSIONPravastatin, fosinopril and their combination showed favorable effects on left ventricular remodeling after AMI in rats and demonstrated improved cardiac function. The combined treatment group yielded better results in the context of improving left ventricular systolic function. These effects could be relevant to the attenuation of increased MMP-2 and MMP-9 activities and left ventricular expression of TNF-alpha.

10. Treatment for latent tuberculosis infection in China: advantages or disadvantages

Tuberculosis remains one of the top ten causes of death worldwide, which seriously endangered human health. Latent tuberculosis infection(LTBI) refers to the fact that there no active tuberculosis after human body is infected, but there is the risk of developing active tuberculosis. This article elaborates the advantages and disadvantages of LTBI treatment, and based on the reality of tuberculosis in China, it is suggested that the prevention and treatment of LTBI should be improved step by step and the treatment of LTBI should be promoted from are to point in China,so as to eliminate tuberculosis in latent infection, reduce the incidence of tuberculosis,and achieve the goal of END TB strategy by the World Health Organization by 2035.
