1.Short term efficacy of glucocorticoids in patients with idiopathic interstitial pneumonia evaluated by high resolution CT semi quantitative scores
Chinese Journal of Biochemical Pharmaceutics 2017;37(7):434-435
Objective To study and analyze the short-term effect of glucocorticoid in patients with idiopathic interstitial pneumonia by analyzing the semi quantitative scores of high resolution CT.Methods 50 cases of idiopathic interstitial pneumonia treated in our hospital from May 2015 to October 2016 were selected as the research object.Clinical data were analyzed, including smoking, signs of hospitalization and dyspnea score.High resolution CT level for the right diaphragmatic top tube, bifurcation, left atrium roof, in 3 aspects from the calculation of cellular disease and shadow grid as a percentage of average, the critical point of high resolution CT scores were 10%, 20%, 30%, 40% and 50%.The accuracy, sensitivity and specificity of glucocorticoids were analyzed. The chi square test was used to examine the enumeration data. Results The accuracy was 72% when the high resolution CT score was 20% critical point. In patients treated with corticosteroids, the high resolution CT score was greater than or equal to 20% for patients and 25 for patients with a score of less than 20% for 25 patients.20% or more patients died during hospitalization, 10 cases, the mortality rate was 40%, and the effective rate of glucocorticoid treatment was 8%.Less than 20% of the patients died during hospitalization, 1 cases, the mortality rate was 4%, and the effective rate of glucocorticoid treatment was 50%.Conclusion High resolution CT semi quantitative score judgment of idiopathic interstitial pneumonia patients treated with glucocorticoid efficacy of high accuracy, high resolution 20% scores higher accuracy, and the diagnosis is well, with further clinical promotion and application significance.

Objective: To evaluate the preemptive analgesia of intravenous tramadol preoperatively in neck-surgery. Methods: Eighty patients, ASAⅠ-Ⅱ,were scheduled for neck-surgery under local anesthesia. Patients were randomly divided into 2 groups (40 cases/group).In group P intravenous tramadol was administered as a bolus dose of 3mg/kg 15min prior to skin incision. Group C received saline as a placebo. The patients-controlled analgesia(PCA) devices were used in all patients after operation. We observed BP, HR,VAS score, the time of first PCA activation, the valid number of press PCA and the total consumption of analgesic after operation. Results:The postoperative VAS was markedly lower in group P compared with the group C in 0-24h( P

Objective To investigate costimulatory pathway of breast cancer patients, we examined peripheral CD28 positive T lymphocytes and B7 molecules expression on the primary breast cancer cells in patients with breast cancer. MethodsFlow cytometry was used to analyze CD28 and B7 molecules expressed on T lymphocytes and primary breast cancer cells respectively. Results CD28 positive T lymphocytes in breast cancer patients was lower than that in control group (t=2.879,P

OBJECTIVETo observe the clinical effect of Ziyin Huoxue Granule (ZHG) combined glucocorticoids and antibiotics in treatment of radiation pneumonitis.
METHODSTotally 70 radiation pneumonitis patients were assigned to the treatment group and the control group according to random digit table, 35 in each group. All patients received glucocorticoids and antibiotics. Patients in the treatment group additionally took ZHG, one dose per day for 4 successive weeks. Watters clinical-radiologic-physiologic (CRP) score, Karnofsky Performance Status Scale (KPS) , and acute radiation injury classification [set by Radiation Therapy Oncology Group (RTOG)] were observed in the two groups before and after treatment. The application time for antibiotics and glucocorticoids was compared between the two groups.
RESULTSAll patients completed this trial, and nobody dropped out or died. There was no statistical difference in Watters-CRP scores, KPS, or RTOG between the two groups before treatment (P > 0.05). Compared with before treatment in the same group, RTOG classification was obviously improved in the two groups (P < 0.05). Compared with the control group, Watters-CRP scores decreased, KPS increased, the application time for antibiotics and glucocorticoids was reduced (P < 0.05). The efficacy of RTOG classification was better in the treatment group than in the control group, but with no statistical difference between the two groups (P > 0.05).
CONCLUSIONZHG combined glucocorticoids and antibiotics was superior in treating radiation pneumonitis to using glucocorticoids or antibiotics alone in elevating Watters-CRP scores, shortening the application time for glucocorticoids and antibiotics, and improving patients' physical conditions.

Objective To compare the clinical value of ultrasound and X-ray guided percutaneous nephrolithotomy ( PCNL) for treat-ment of kidney calculi. Methods We searched relevant clinical controlled articles in PubMed,EMBASE/Cochrane library and CNKI data-base. The outcome indicators included stone clearance rate, complication rate, puncture time, blood loss, and hospital days. All data were ana-lyzed by Revman 5. 0 software. Results There were 9 articles involved in 1693 patients. The results suggested that there was no significant difference in stone clearance rate between ultrasound group and X-ray group[OR=1. 11,95%CI(0. 83,1. 49),P=0. 47]. However, the complication [OR=0. 43,95%CI (0. 27,0. 69),P=0. 0004],time to puncture[WMD= -4. 54,95%CI( -0. 45,-8. 62),P=0. 03], blood loss[WMD= -22. 77,95%CI( -36. 57,-8. 97),P=0. 001],and hospital days[WMD= -0. 16,95%CI( -0. 28,-0. 03),P=0. 02] were better in ultrasound group than those in X-ray group. Conclusion Ultrasound and X-ray guided PCNL had an equal stone clear-ance rate, but it seemed that ultrasound was better than X-ray in time to puncture and hospital days after operation. However, the results of this meta-analysis should be treated carefully because of limitation of data and heterogeneity.
