Object To investigate the effects of Panax notoginseng saponins (PNS) on myocardial Gs? mRNA expression of seriously scalded rat. Methods A 30% skin full thickness scald model was produced by immersing rat in 95 ℃ water for 10 s. The effects of PNS on myocardial Gs? mRNA level were observed with dot blotting hybridization and in situ hybridization technique; effects on cAMP and adenyl cyclase (AC) activities were determined with radioimmunoassay. Results Myocardial Gs? mRNA, AC activity and cAMP content were reduced significantly 3 h after scalding. PNS (100, 200 mg/kg) could markedly increase the level of myocardial Gs? mRNA expression. Objective:To observe the sensitivity of transcription mediated amplification (TMA),and to compare its performance with real-time reverse transcription polymerase chain reaction (real-time RT-PCR) in detecting human immunodeficiency virus RNA (HIV RNA).Methods:TMA system was established with TaqMan probes,specific primers,moloney murine leukemia virus (MMLV) reverse transcriptase,T7 RNA polymerase,and reaction substrates.The sensitivity of TMA was evaluated by amplifying a group of 10-fold diluted HIV RNA standards which were transcribed in vitro.A total of 60 plasma of HIV infected patients were measured by TMA and Cobas Amplicor HIV-1 Monitor test to observe the positive rate.The correlation and concordance of the above two technologies were investigated by linear regression and BlandAltman analysis.Results:TMA system was established successfully and HIV RNA transcribed standards at concentration of equal or more than 10 copies/mL could be detected by TMA technology.Among 60 samples of plasma from HIV infected patients,46 were positively detected and 12 were negatively amplified by both TMA and Cobas reagents;2 samples were positively tested by Cobas reagent but negatively tested by TMA system.The concordance rate of the two methods was 97.1% and the difference of positive detection rate between the two methods was not statistically significant (P>0.05).Linear regression was used for 46 samples which were positively detected by both TMA and Cobas reagents and showed an excellent correlation between the two reagents (r=0.997,P<0.001).Bland-Altma analysis revealed that the mean different value ofHIV RNA levels for denary logarithm was 0.02.Forty-four samples were included in 95% of credibility interval of concordance.Conclusion:TMA system has the potential of high sensitivity.TMA and real-time RT-PCR keep an excellent correlation and consistency in detecting HIV RNA.
3.Dosimetric evaluation of four adaptive IMRT strategies for head-neck cancer

Objective To investigate the effects of numerous re-planning strategies on the anatomic and dosimetric outcomes of target volume and organs at risk (OARs) in patients with head and neck cancer receiving fractionated radiotherapy.Methods From 2015 to 2016,28 patients with head and neck cancer were enrolled in this study with Shandong Cancer Hospital,consisting of 19 patients with nasopharyngeal carcinoma, 4 patients with laryngocarcinoma, and 5 patients with carcinoma of the maxillary sinus.All of them received conventionally fractionated radiotherapy.Each patient had six weekly cone-beam CT (CBCT) scans, which were performed on the first day of every week, to obtain reference images.A virtual CT image was generated by registration of planning CT and each weekly CBCT image.The four re-planning strategies were used for the reconstruction of re-planned dose, while the initial planning was used as a reference.The weekly doses calculated using virtual CT were summed together to obtain the actual dose.The actual and initial planned doses were evaluated.The nonparametric Friedman test was used to evaluate the differences between multiple groups, and the differences between any two groups were analyzed by paired t test.Results The sizes of planning target volume, clinical target volume, and left/right parotid glands (PGs) changed significantly within the six weeks (P=0.041, 0.046, 0.024, and 0.017, respectively).For these four re-planning strategies, there were significant differences between the actual dose and the initial planned dose to the PGs (all P<0.05), with average values decreased by 5.02%, 11.17%, 12.08%, and 13.19%, respectively, compared with that in the reference strategy.Conclusions Re-planning during treatment course could ensure the sparing of OARs and allow for sufficient dose to the target volume.The higher the number of re-planning strategies, the more the actual dose is close to the initial planed dose;the efficiency of two re-planning strategies is the highest.
4.Research on the effect of optimizing protocol activity on the cardiac function in patients with hypertension

Objective To investigate the effect of optimizing protocol activity on the cardiac function in patients with hypertension.Methods One hundred and one patients with mild and moderate hypertension in Navy General Hospital were enrolled in the study.The patients took regular exercise(6 min walking activity and treadmill test),Echocardiogram was performed 24 h after 6 min walking activity and treadmill test.The cardiac function related parameters were obtained through M-mode,2DE,pulse Doppler and DTI detection to compare the effect of 6 min walking activity and treadmill test on cardiac function.Results The results showed that in the male hypertensive patients,EF and E/E' after treadmill test were superior to those after 6 min walking activity (EF:(59.33±4.46)% vs.(56.05±4.57)%;E/E':(4.12±1.66)vs.(4.95±1.79)),the differences were statistically significant(P=0.02,0.01),while among the female hypertensive patients,there were no significant differences in cardiac function(P>0.05).Conclusion Appropriate activity can improve the cardiac function in male hypertensive patients.EF and E/E' after the treadmill test performed better than those after the 6 min walking activity,while its influence on female hypertensive patients was not significant.
5.Immunohistochemical spectrum in the detection and differentiation of intrahepatic neoplasms.

OBJECTIVETo scrutinize the immunohistochemical spectrum to differentiate hepatocellular carcinoma (HCC) from intrahepatic cholangiocarcinoma (ICC) and metastatic adenocarcinoma (MAC) in the liver.
METHODSSeven antibodies including AFP, Hep Par 1, CK18, CK19, CA19-9, CD34 and pCEA were immunohistochemically stained in resected specimens of 300 HCC, 35 ICC and 30 MAC. The specificity and sensitivity of the antibodies were evaluated by comprehensive capability score (CCS), with only those with CCS > or = 8 considered as having highly diagnostic value.
RESULTSAntibodies CCS > or = 8 were observed as Hep Par 1 and CD34 in HCC, and CK19 in ICC, but none in MAC. For HCC, CCS of Hep Par 1 was higher than that of AFP (9 vs. 7) with 83.7% in sensitivity and 96.7% in specificity.
CONCLUSIONFor HCC, Hep Par 1 and CD34 can be used as the first line antibodies, AFP and pCEA as the second line ones. CK19 is the first line antibody for ICC, and CA19-9 as the second. Hep Par 1, CD34 and CK19 are definitely helpful for the routine immunohistochemical stain to differentiate HCC from ICC and MAC.

6.Clinical features of patients with autoimmune glial fibrillary acidic protein astrocytopathy

Objective To explore the clinical features through analyzing the clinical data of 5 patients with autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy and reviewing the literatures.Methods We retrospectively analyzed the general data,clinical symptoms,cerebrospinal fluid results,electroencephalogram results,imaging data and treatment results of 5 patients with GFAP astrocytopathy at our hospital and reviewed the literatures.Result The average age of onset was 57 in five patients.The main clinical symptoms were limb fatigue,memory loss,increased sleep,urine and stool disorders,fever and so on,and 1 patient was accompanied by epileptic seizure in addition to the above manifestations.Cerebrospinal fluid examination showed that protein and cell number were increased in different degrees,4 patients showed positive oligoclonal bands and specific oligoclonal bands in cerebrospinal fluid,and 24 hours intrathecal synthesis rate increased in different degrees.The electroencephalogram showed varying degrees of slow waves.Abnormal signals in the temporal lobe,lateral ventricle,basal ganglia and corpus callosum were observed by craniocerebral MRI.Abnormal signals in the medulla oblongata,cervical medulla and thoracic medulla were observed in 3 patients.All the 5 patients received hormone therapy,and their symptoms were improved to varying degrees.Conclusions The clinical symptoms of patients with GFAP astrocytosis were mainly encephalopathy,meningitis and encephalomyelitis,and the cerebrospinal fluid had different degrees of immune-mediated inflammatory reaction.Imaging examination showed that the brain,meninges,spinal cord and optic nerve were involved,and most of patients respond well to hormones.
7.Effects of Lindera aggregate of Stir-baking with Vinegar- Aucklandia lappa on Gastric Emptying and Gastrointestinal Hormones in Functional Dyspepsia Liver Depression and Qi Stagnation Model Rats

OBJECTIVE: To investigate the effects of Lindera aggregate of stir-baking with vinegar-Aucklandia lappa on gastric emptying and gastrointestinal hormones in functional dyspepsia liver depression and qi stagnation (FDLDQS) model rats. METHODS: Totally 60 SD rats were randomly divided into blank group (n=10) and modeling group (n=50); FDLDQS model was induced by chronic stress restraint or food deprivation or excessive fatigue in modeling group. After modeling, model rats were randomly divided into model group (normal saline), L. aggregate of stir-baking with vinegar group (1.62 g/kg, calculated by crude drug), A. lappa group (1.62 g/kg, calculated by crude drug), L. aggregate of stir-baking with vinegar-A. lappa group (1 ∶ 1, m/m,1.62 g/kg, calculated by crude drug) and mosapride group (positive control, 1.35 mg/kg), with 10 rats in each group. Administration groups were given relevant medicine intragastrically once a day; blank group and model group were given constant volume of normal saline intragastrically, for consecutive 14 d. 2 h after last medication, gastric emptying rate and small intestinal propulsion rate of rats in each group were measured by phenol red content method. After HE staining, the morphological changes of antrum tissue were observed under microscope. The contents of motilin (MTL), gastrin (GAS) and cholecystokinin (CCK) in serum were determined by ELISA method. RESULTS: Compared with blank group, gastric emptying rate and small intestinal propulsion rate of rats were decreased significantly in model group (P<0.01); the serum contents of MTL and GAS were decreased significantly (P<0.01), while the content of CCK was increased significantly (P<0.01). No organic damage was found in all model groups. Compared with model group, L. aggregate stir-baking with vinegar group and A. lappa group, gastric emptying rate and small intestinal propulsion rate of rats were increased significantly in L. aggregate of stir-baking with vinegar-A. lappa group and mosapride group (P<0.05 or P<0.01); serum contents of MTL and GAS were increased significantly (P<0.01), while the content of CCK was decreased significantly (P<0.01). CONCLUSIONS: L. aggregate of stir-baking with vinegar-A. lappa can accelerate gastric emptying and small intestinal propulsion rate of FDLDQS rats, increase serum contents of MTL and GAS but decrease the content of CCK; its effects are better than that of them alone.
8.Improvement of Skin Barrier and Anti-inflammatory Mechanism of Huangliansan on Atopic Dermatitis in Mice

ObjectiveTo explore the therapeutic effect of Huangliansan on atopic dermatitis (AD) model mice induced by 2, 4-dinitrochlorobenzene (DNCB). MethodA total of 42 male BALB/c mice were randomly divided into normal group, model group, hydrocortisone group, low, medium, and high-dose groups (0.3, 0.6, 1.2 g·kg-1) of Huangliansan oil, and water extract group (0.6 g·kg-1) of Huangliansan. In addition to the normal group, DNCB was applied on the back of mice in other groups to establish the AD model. On the 15th day after DNCB stimulation, each group was given the corresponding drug or solvent, and the changes in skin lesions, dermatitis score, and frequency of scratching were observed and recorded. Hematoxylin-eosin (HE) staining was used to observe the histopathological changes in the skin and spleen. Real-time fluorescent quantitative polymerase chain reaction (Real-time PCR) was used to detect mRNA levels of filaggrin (FLG), lorophane (LOR), and involucrin (IVL) in skin, as well as immunoglobulin E (lgE), interleukin-4 (IL-4), interleukin-6 (IL-6), interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), and interferon-γ (IFN-γ) in spleen. ResultCompared with the normal group, the model group showed symptoms of skin swelling and scab, and the score of dermatitis, the frequency of scratching, and the spleen index were increased (P<0.05). The expression levels of FLG, LOR, and IVL in skin tissue were significantly decreased (P<0.01), and the mRNA expressions of IgE, IL-4, IL-6, IL-1β, and TNF-α in the spleen were significantly increased, while the expression level of IFN-γ was significantly decreased (P<0.01). Compared with the model group, the symptoms of skin erythema, scaly, and scab of mice in each drug group were alleviated to varying degrees, and the score of dermatitis, the frequency of scratching, and the spleen index were decreased (P<0.05, P<0.01). In addition, the expression levels of FLG, LOR, and IVL in the skin of mice in the drug group were increased (P<0.05, P<0.01), and the mRNA expression of IgE, IL-4, IL-6, IL-1β, and TNF-α in spleen were decreased. IFN-γ was increased (P<0.05, P<0.01), and the lesions of the skin and spleen were improved to varying degrees. The medium-dose group of Huangliansan oil and hydrocortisone group had the most obvious manifestations (P<0.05, P<0.01). The indexes in the medium-dose group of Huangliansan oil were better than those in the water extract group of Huangliansan. ConclusionHuangliansan may improve the expression level of skin barrier protein, inhibit the expression of helper T cell 2 (Th2)-related inflammatory factors, increase the expression of helper T cell 1 (Th1) inflammatory factors, restore the skin barrier function and Th1/Th2 balance in the spleen, regulate the inflammatory response in the spleen of AD mice, and thus relieve AD. Huangliansan oil is more effective than water extract.
