1.Study on Formulation Optimization and Drug-release Mechanism of Cilnidipine Sustained-release Tablet

OBJECTIVE:To optimize the formulation of Cilnidipine sustained-release tablet,and study its drug-release mecha-nism. METHODS:Solvent method was adopted to prepare the cilnidipine solid dispersion,then Cilnidipine sustained-release tablet was prepared by using hypromellose K4M(HPMC K4M)as release material. Using comprehensive scores of cumulative release de-gree in 2,6,12 h as indexes,single factor method and Box-Behnken response surface method were used to screen the amounts of HPMC K4M and ethyl cellulose (EC),lactose-microcrystalline cellulose (MCC) ratio in formulation of Cilnidipine sustained-re-lease tablet,and verification test was conducted. The drug-release mechanism of Cilnidipine sustained-release tablet was investigat-ed by model fitting way. RESULTS:The optimal formulation was as follow as 25% of cilnidipine solid dispersion,30% of HPMC K4M,10% of EC,lactose-MCC ratio of 1:1(m/m). The adhesive was 5% PVPP ethanol solution and the lubricant was 0.5%magnesium stearate. The cumulative release degrees of prepared sustained-release tablet in 2,6,12 h were(21.4±3.3)%,(62.9± 2.8)%,(85.4±0.5)%(n=3),relative error of which to predicted value 25%,60%,90%were 14.4%,4.8%and 5.1%. Release curve showed the highest fitting degree with the first-order release model,conforming to non-Fick diffusion. CONCLUSIONS:Cil-nidipine sustained-release tablet with sustained-release effect is successfully prepared by optimized formulation.
2.Study on the effect of Bazhen granules on the treatment of patients with polymyalgia rheumatica and effect on immune level

Objective To investigate the effect of Bazhen granules combined with conventional treatment in patients with polymyalgia rheumatica and effect on immune level.MethodsFrom December 2013 to May 2016 in the second hospital of Shandong University 80 cases with polymyalgia rheumatica were randomly divided into the Banzhen granule group and the conventional group, 40 cases in each group.The two groups were given basic treatment with prednisone.The Bazhen granules group were given Bazhen granules at the same time.The two group were given continuous treatment for 3 months.ResultsBefore treatment, the difference of serum ESR and CRP levels between the Bazhen granules group and the normal group was not statistically significant.After treatment, in the Bazhen granule group, serum ESR, CRP levels were lower than those in the conventional group, the difference was statistically significant (P<0.05);serum CD3+, CD4+, CD4+/CD8+, IgG, IgM, IgA levels were higher than those in the conventional group (P<0.05), CD8+ level was lower than that in the conventional group (P<0.05).The effective rate in the Bazhen granule group 85%was higher than that in the conventional group 62.50% (P<0.05).The total effect rate in the Bazhen granule group95% was higher than that in the conventional group 87.50%, but the difference was not statistically significant.ConclusionThe effect is better that Bazhen granules combined with conventional treatment on the treatment of patients with polymyalgia rheumatica, which can improve the patient's immune level.
3.Observation on the effect of clostridium butyricum powder in treating infantile indigestion diarrhea

Objective To investigate the clinical effect of Clostridium butyricum powder in treating infantile indigestion diarrhea.MethodsFrom February 2013 to October 2015 in our hospital 76 cases of pediatric indigestion diarrhea as the research object, randomly divided into experimental group and control group with 38 cases in each group, two groups were given Bao,er ning granule, combined group plus live bacteria powder for the treatment of Clostridium butyricum, three days for a course of treatment, continuous treatment of two courses.ResultsBefore treatment, there was no significant difference in clinical symptom scores between the experimental group and the control group,after treatment,the scores of loss of appetite, abdominal pain, diarrhea, abdominal distension and stool of the experimental group were lower than those of the control group, the difference was statistically significant (P<0.05);Before treatment, there was no significant difference in serum motilin, gastrin and somatostatin levels between the experimental group and the control group.After treatment, the levels of serum motilin and gastrin in the experimental group were lower than those in the control group, and the level of serum somatostatin was higher than that of the control group, the difference was statistically significant (P<0.05);After treatment, the total effective rate of experimental group was 92.11% higher than that of control group 73.68%, the difference was statistically significant (P<0.05).ConclusionClostridium butyricum powder with Bao,er ning granule in the treatment of infantile dyspeptic diarrhea effect is good, worthy of clinical application.
4.Long-term Efficacy of Radiofrequency Ablation Combined with Chemotherapy in the Treatment of Patients with Advanced Non-small Cell Lung Cancer--A Retrospective Study

Background and objective Radiofrequency ablation (RFA) combined with chemotherapy has a certain short-term therapeutic effect for the treatment of advanced non-small cell lung cancer (NSCLC), but whether it can improve the long-term survival rate of patients is still controversy. This study retrospectively analyzed the difference of long-term effi-cacy between RFA combined with chemotherapy and chemotherapy alone in the treatment of patients with advanced NSCLC. Methods A total of 77 patients with stage IIIb and stage IV NSCLC who underwent radiofrequency ablation and chemo-therapy in the Department of Thoracic Surgery, Xuanwu Hospital, Capital University of Medical Sciences from September 2009 to December 2015 were enrolled as the treatment group. Chemotherapy with no radiofrequency ablation was performed in 56 patients with stage IIIb and stage IV NSCLC as the control group. Two groups of patients were followed up by telephone about their living conditions. "Survival" package of R software version 3.4.1 was used for statistical analysis. Two sets of data baseline levels were tested by chi-square test.The bias was processed by Cox regression model and the survival curve was plot-ted using covariate mean substitution method. Results The first-year survival rate of the treatment group was 70.74%, the two-year survival rate was 39.31% and the median survival time was 22.1 months. The one-year survival rate was 54.54% in the control group, the two-year survival rate was 19.49%, the median survival for 18.1 months. The long-term survival rate of the treatment group was better than that of the control group (P<0.05, OR=0.571). Conclusion Radiofrequency ablation of lung cancer combined with chemotherapy can significantly improve the 2-year survival rate of patients with stage IIIb and stage IV NSCLC.
5.Occurrence of fall and its influencing factors in elderly patients with knee osteoarthritis

Objective? To investigate the occurrence and influencing factors of falls in elderly patients with knee osteoarthritis (KOA), and provide scientific reference for the prevention of falls in elderly patients with KOA. Methods? Using convenience sampling method, 104 elderly patients diagnosed as KOA in orthopaedic wards of four ClassⅢ Grade A hospitals in Hangzhou from November 2017 to August 2018 were selected as subjects. General Information Questionnaire, Morse Fall Scale(MFS)and the Barthel Index(BI)for Activities of Daily Living (ADL) were applied in the investigation. Single factor analysis and multiple factor Logistic regression were used to analyze the influencing factors of falls in elderly KOA patients. Results? A total of 104 questionnaires were sent out in this study, and 95 valid questionnaires were recovered, yielding an effective recovery rate of 91.35%. The incidence of falls in elderly KOA patients was 45.26% (43/95), of which 62.79% (27/43) had fallen twice or more. There were significant differences in knee injury history, staircase climbing and falls among elderly KOA patients (P< 0.05). There were significant differences in the risk of falls between elderly KOA patients in "whether taking drugs", "whether using walking aids", "status of self-care ability", and "age" (P< 0.05). Logistic regression analysis showed that history of knee injury, frequent staircase climbing and use of walking aids were the influencing factors of falls in elderly KOA patients (OR=4.028, 25.569, 3.191; P< 0.05). Conclusions? In view of the influencing factors of falls in elderly KOA patients, individualized nursing measures should be taken to strengthen falls risk assessment, carry out fall prevention related education and improve patients' social support, which will help to reduce the incidence of falls and further improve patients' quality of life.
6.Factors influencing the sustainability of the evidence-based practice project for irritant dermatitis around urostomy: a qualitative study based on the i-PARIHS framework

Objective:To explore the influencing factors of the poor sustainability of the evidence-based practice project for irritant dermatitis around urostomy.Methods:This is a qualitative study. From April to May 2020, 12 nurses working in the Urology Surgery of Peking University First Hospital were selected by purposive sampling for the semi-structured interview. The integrated-Promoting Action on Research Implementation in Health Services framework (i-PARIHS framework) was applied to analyze and extract the obtained data.Results:The interview results found that, in terms of transform (evidence) , the localization and continuity of evidence for the evidence-based practice project of irritant dermatitis around urostomy needed to be improved, including the high matching between evidence and practice environment, and the insufficient continuity of evidence application. In terms of transform recipients, the leadership of project practitioner needed to be improved, including the conflict between the roles of practitioners and researchers, the lack of knowledge reserves of practitioners, and the conflict among the values of practitioners. In terms of organizational environment, the leadership of managers and the imformatization degree of project needed to be improved, including insufficient support from leaders and insufficient facilities.Conclusions:At the initial stage of formulating the nursing evidence-based practice project, the project implementer should fully incorporate the opinions of stakeholders for localization debugging, and consider the sustainability of the project. In addition, the leadership of managers, nurses and interns should be increased, the use and training of urostomy talents should be strengthened, the training of urostomy knowledge should be continued, the information-based follow-up system should be improved, and the intervention strategy should be continuously adjusted to ensure the project is carried out.
7.The preliminary study of Tian Jiu therapy regulating Th 1/Th2 ratio in bronchial asthma patients

Objective To investigate whether Tian Jiu therapy can regulate Th1/Th2 ratio effectively for bronchial asthma patients.Methods A total of 30 bronchial asthma patients who were treated with Tian Jiu therapy in the hospital were enrolled in the study as patients group and 20 healthy individuals were enrolled as control group.The mRNA of T-bet,GATA3,Foxp3 and STAT-6 were detected by using realtime-PCR,while IFN-γ and IL-4 were detected by using ELISA for control group and patents group before and after treatment. Flow cytometry was used to detect Th1/Th2 ratio.Results After Tian Jiu therapy,Th2 proportion decreased and Th1/Th2 ratio increased,secretion of IFN-γ decreased,mRNA of T-bet and GATA3 significantly in-creased,while STAT-6 decreased significantly(P<0.05).Conclusion Tian Jiu therapy has therapeutic effect on bronchial asthma patients through decreasing Th2 proportion and regulating Th1/Th2 ratio.
8.Obstacles factors of the compliance of nurses in clinical application of catheter fixation device: a qualitative study based on theoretical domain framework

Objective:To find out the obstacle factors in its clinical application according to the clinical situation of the application of urinary catheter fixation device.Methods:From September 2018 to October 2018, 19 nurses working in the urology ward of Peking University First Hospital were selected for a semi-structured interview. Based on the theoretical domain framework, the interview outline was formulated and the content was analyzed, and the topic was extracted again.Results:This study identified obstacle factors related to 5 areas of the theoretical domain framework, namely "knowledge, social or professional role and identity, motivation and goals, environmental factors and social influence". The themes were extracted again, respectively: "the system level of evidence implementation (organizational factors, training, product categories) needs to be optimized, the level of willingness of users (patients) for evidence implementation needs to be improved and the sustainability of evidence application needs to be strengthened".Conclusions:The factors that affect the clinical application of urinary catheter fixation devices are multiple, and future research needs to formulate targeted intervention plans based on evidence according to obstacles identified.

The permanent or temporary stoma left in patients undergoing neostomy affects the patients both physiologically and psychologically. The Registered Nurses Association of Ontario (RNAO) released the clinical practice guidelines of Supporting Adults Who Anticipate or Live with an Ostomy, second edition in 2019. This article interprets the recommendations of this guideline, and focuses on preoperative and postoperative care planning, parastomal hernia prevention, and the quality of life of patients undergoing neostomy, in order to provide clinical practitioners with effective stoma care advice.
10.Improvement of Skin Barrier and Anti-inflammatory Mechanism of Huangliansan on Atopic Dermatitis in Mice

ObjectiveTo explore the therapeutic effect of Huangliansan on atopic dermatitis (AD) model mice induced by 2, 4-dinitrochlorobenzene (DNCB). MethodA total of 42 male BALB/c mice were randomly divided into normal group, model group, hydrocortisone group, low, medium, and high-dose groups (0.3, 0.6, 1.2 g·kg-1) of Huangliansan oil, and water extract group (0.6 g·kg-1) of Huangliansan. In addition to the normal group, DNCB was applied on the back of mice in other groups to establish the AD model. On the 15th day after DNCB stimulation, each group was given the corresponding drug or solvent, and the changes in skin lesions, dermatitis score, and frequency of scratching were observed and recorded. Hematoxylin-eosin (HE) staining was used to observe the histopathological changes in the skin and spleen. Real-time fluorescent quantitative polymerase chain reaction (Real-time PCR) was used to detect mRNA levels of filaggrin (FLG), lorophane (LOR), and involucrin (IVL) in skin, as well as immunoglobulin E (lgE), interleukin-4 (IL-4), interleukin-6 (IL-6), interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), and interferon-γ (IFN-γ) in spleen. ResultCompared with the normal group, the model group showed symptoms of skin swelling and scab, and the score of dermatitis, the frequency of scratching, and the spleen index were increased (P<0.05). The expression levels of FLG, LOR, and IVL in skin tissue were significantly decreased (P<0.01), and the mRNA expressions of IgE, IL-4, IL-6, IL-1β, and TNF-α in the spleen were significantly increased, while the expression level of IFN-γ was significantly decreased (P<0.01). Compared with the model group, the symptoms of skin erythema, scaly, and scab of mice in each drug group were alleviated to varying degrees, and the score of dermatitis, the frequency of scratching, and the spleen index were decreased (P<0.05, P<0.01). In addition, the expression levels of FLG, LOR, and IVL in the skin of mice in the drug group were increased (P<0.05, P<0.01), and the mRNA expression of IgE, IL-4, IL-6, IL-1β, and TNF-α in spleen were decreased. IFN-γ was increased (P<0.05, P<0.01), and the lesions of the skin and spleen were improved to varying degrees. The medium-dose group of Huangliansan oil and hydrocortisone group had the most obvious manifestations (P<0.05, P<0.01). The indexes in the medium-dose group of Huangliansan oil were better than those in the water extract group of Huangliansan. ConclusionHuangliansan may improve the expression level of skin barrier protein, inhibit the expression of helper T cell 2 (Th2)-related inflammatory factors, increase the expression of helper T cell 1 (Th1) inflammatory factors, restore the skin barrier function and Th1/Th2 balance in the spleen, regulate the inflammatory response in the spleen of AD mice, and thus relieve AD. Huangliansan oil is more effective than water extract.
