1.Clinical observation on oblique Ban-pulling tuina manipulation plus acupuncture for cervical vertigo

Objective:To explore the clinical efficacy of rotating-lifting oblique Ban-pulling tuina manipulation plus acupuncture in treating cervical vertigo (CV).Methods:Seventy-three CV patients were divided into two groups by block randomization, 50 cases in each group. The observation group was intervened by rotating-lifting oblique Ban-pulling tuina manipulation plus acupuncture, while the control group was by the same acupuncture treatment alone. The tuina treatment was given once every other day and the acupuncture treatment was given once every day. The symptom score was observed before and after 20-day treatment, and the therapeutic efficacies were compared between the two groups.Results:During the intervention, 14 subjects in the observation group and 13 subjects in the control group dropped out. Finally, 36 cases in the observation group and 37 cases in the control group were involved into the data analysis. After 20-day treatment, the symptom scores were significantly increased in both groups (P<0.05), and the symptom score in the observation group was significantly higher than that in the control group (P<0.05). The total effective rate was 91.7% in the observation group versus 83.8% in the control group, and the difference was statistically insignificant (P>0.05). The recovery and markedly effective rate was 69.4% in the observation group versus 32.4% in the control group, and the difference was statistically significant (P<0.05).Conclusion:Rotating-lifting oblique Ban-pulling tuina manipulation plus acupuncture can significantly improve the clinical symptoms of CV patients, and its clinical efficacy is superior to that of acupuncture alone.
2.Imaging findings of primary bone malignant fibrous histiocytoma in long bone

Objective To investigate the imaging findings of primary bone malignant fibrous histiocytoma (PBMFH) in long bone. Methods MRI and XV ray examinations were performed in 10 patients with PBMFH in long bone confirmed by histology. CT scans were performed in 6 cases. Results Nine cases had single and 1 case had multiple lesions of single bone. Seven cases showed osteolytic bone destruction, and 3 cases showed mixed bone destruction. Only 1 case showed periosteal reaction. Gross bony ridge was seen in 3 cases on CT scan. All the patients demonstrated isointensity or hypointensity on T1WI, and slight hyperintensity signal on T2WI. Necrosis was seen in the lesion of 10 cases. Four cases showed obvious but heterogeneous enhancement after contrast medium injection. Eight cases had soft tissue mass beyond the scope of bone destruction. Conclusions Some imaging features may suggest the diagnosis of PBMFH in long bone.
3.Compilation of the Elderly Functional Constipation Health Education Scale and its reliability and validity test

Objective To prepare the Elderly Functional Constipation(FC) Health Education Scale, and verify its reliability and validity. Methods The Elderly FC Health Education Scale was prepared by qualitative interviews, literature review, and Delphi method. The elderly patients who were over 60 years old and at Chinese PLA 181st Hospital were recruited by the purposive sampling method from January 2013 to August 2015.The diagnostic criteria of RomanⅢwas used to diagnose FC, the data of preliminary investigation and large sample test was used to form the formal scale, and its reliability and validity were further verified. Results The Elderly FC Health Education Scale was compiled with 6 dimensions and 28 items, and the Cronbach alpha coefficient which was the internal consistency reliability of the 6 factor was 0.965; the correlation coefficient analysis of equality reliability Kendall tau-b rank and various index variables score were positive correlation significantly. Both item level content validity index and scale level content validity index of the content validity were as a result of 1. The structure validity of the cumulated variance contribution ratio of the 6 factors were 60.15%. All factor loading coefficients between the items were more than 0.5, which indicated the fitting was good. Conclusions The reliability and validity of the Elderly FC Health Education Scale are good, and the scale may be used as a tool to prevent the elderly FC health education, and also be applied to the elderly FC patients in self-management and continue nursing after leaving hospital.
4.Effect of dexmedetomidine on acute kidney injury in septic rats: the relationship with HMGB1 mRNA in renal tissues

Objective To evaluate the effect of dexmedetomidine on acute kidney injury (AKI) and the relationship between the mechanism and expression of high mobility group protein B1 (HMGBl) mRNA in renal tissues of septic rats.Methods Fort-eight healthy adult male Sprague-Dawley rats,aged 8-12 weeks,weighing 240-270 g,were divided into 3 groups (n =16 each) using a random number table method:control group (C group),sepsis group (S group) and dexmedetomidine group (D group).AKI was induced by cecal ligation and puncture.The concentrations of serum cystatin C (Cys C) were measured by immunoturbidimetry at 24 and 48 h after operation.The rats were sacrificed after blood sampling,and kidney tissues were removed for determination of the expression of HMGB1 mRNA (by fluorescent quantitative real-time polymerase chain reaction) and for examination of the pathological changes.The damage to the renal tubules was scored.Results Compared with group C,the renal tubular damage score and serum Cys C concentrations were significantly increased,and the expression of HMGB1 mRNA was up-regulated in S and D groups (P<0.05).Compared with group S,the renal tubular damage score and serum Cys C concentrations were significandy decreased,the expression of HMGB1 mRNA was down-regulated (P<0.05),and the pathological changes of renal tissues were significantly attenuated in group D.Conclusion Dexmedetomidine can attenuate AKI in septic rats and the mechanism may be related to inhibiting the expression of HMGB1 mRNA in renal tissues.
5.Legal risks of face recognition technology in medical field

Under the background of " Internet+ medical treatment" and the continuous deepening of face recognition technology research, the face recognition industry has continued to mature, and face recognition has been initially applied in medical fields such as hospital management, auxiliary medical care, and epidemic prevention and control. At the same time, face recognition technology brings problems including error risk, technical cracking risk, privacy risk, equality risk, abuse risk, and other issues in practice, which seriously threaten the personal and property rights and interests of the public. On the basis of summarizing the specific application direction of face recognition technology in hospitals, the authors sorted out the legal regulation of face recognition in China, and proposed that it should be based on technology research and development, strengthen the " gatekeeper" responsibility of medical institutions, improve legal system and recommendations for strengthening judicial leadership in order to improve the legal regulations of face recognition technology, reduce the risk of infringement by medical institutions in the application of face recognition technology, and protect the legitimate rights and interests of citizens.
6.GJB2 (connexin 26) gene mutation screen in patients with nonsydromic hearing loss in Hunan.

OBJECTIVE:
To identify the genetic characteristics in patients with nonsydromic hearing loss (NSHL) in Hunan province, to determine the prevalence and spectrum of mutations in GJB2 gene, and to explore the pathogenic mechanism.
METHODS:
A total of 140 sporadic patients with NSHL were enrolled after clinical examination. Molecular studies were performed by amplifing the coding region of GJB2 gene, purifying the PCR products, and sequencing directly. Sequences were analysed by DNAStar software to determine GJB2 mutations in the patients. Special method was designed to confirm the unreported mutation.
RESULTS:
We detected GJB2 mutation in 56 out of the 140 patients (40%, 56/140). Both of the 2 alleles were mutated in 29 patients and 1 allele in the other 27 patients, and the rate of allele mutation was 30.4%(85/280). Ten variations were detected, including 7 mutations and 3 polymorphisms. The deaf-causing mutations were nonsense mutation c.139G>T; frameshift mutation c.235delC and c.176-191del16; and missense mutation c.109G>A, c.344T>G, c.550C>T and c.571T>C. The unreported missense mutation was c.344T>G. The c.235delC mutation was the most prevalent mutation found in the 27 patients (19.3%, 27/140). The frequency of c.109G>A mutation was next to c.235delC found in 25 patients (17.9%, 25/140).
CONCLUSION
GJB2 mutation is a major cause for NSHL. The most common-spot in Chinese patients with NSHL is c.235delC. The unreported missense mutation is c.344T>G.

7.Protective effect of N-acetyl-L-tryptophan on intestinal damage after rat hepatic ischemia reperfusion

Objective To investigate the protective effects of N-acetyl-L-tryptophan (L-NAT) on intestinal damage after rat hepatic ischemia reperfusion.Methods Twenty-four healthy adult rats were divided into the sham operation group (Sham),ischemia reperfusion group (IR),ischemia reperfusion and N-acetyl-L-tryptophan group(IR+L-NAT).The hepatic ischemia reperfusion model was established by occluding the afferent vessels of the left and middle lobes.The morphological structures of the small intestine were observed by hematoxylin-eosin (HE) staining.The expressions of active caspase-3,Bax and Bcl-2 were detected by immunohistochemistry staining.Results (1) In the IR group,the structure of intestinal villis was destroyed,the intestinal mucosa showed congestion and exfoliation,the epithelial cells had degeneration and necrosis,and infiltration of inflammatory cells appeared;which could be alleviated by L-NAT.(2)The immunohistochemistry showed that compared with the Sham group,the expression of active caspase-3,Bcl-2 and Bax in the IR group was increased,after L-NAT intervention,the Bax and caspase-3 expression was decreased,while the Bcl-2 expression was further increased.Conclusion L-NAT could inhibit the apoptosis of small intestinal epithelial cells caused by liver ischemic reperfusion and attenuates intestinal epithelial damage.
8.Role of TLR2/NF-κB signaling pathway in aluminum-induced inflammatory response in GMI-R1 cells

{L-End}Objective To investigate the role and mechanism of Toll-like receptor 2 (TLR2)/nuclear factor-κB(NF-κB) signaling pathway in the inflammatory response induced by aluminum in the rat GMI-R1 microglia cells. {L-End}Methods GMI-R1 cells in logarithmic growth phase were randomly divided into the control group, positive control group, and low-, medium-, and high-dose groups. The cells in the three dose groups were stimulated with maltolol aluminum at concentrations of 100, 200, and 400 μmol/L, respectively. The cells in the positive control group were stimulated with lipopolysaccharide at a mass concentration of 20 mg/L, while the cells in the control group were not treated. The morphological changes of cells were observed, and the cell survival rate was evaluated by CCK-8 method after 24 hours of culture. The secretion levels of tumor necrosis factor-α (TNF-α), interleukin (IL) -12 and IL-4 were detected by enzyme-linked immunosorbent assay. The relative protein expression of TLR2, NF-κB P65, cluster of differentiation (CD) 68 and CD206 of cells was detected by Western blotting, and the expression of CD68 and CD206 of cells was detected by immunofluorescence method. {L-End}Results The results of cell morphology showed that the number of GMI-R1 cells decreased, the number of activated cells increased, the degree of cell cytoplasm filling decreased, and the cell protrusions elongated with the increase of exposure dose, showing a dose-response relationship. The cell viability of the positive control group and the medium- and high-dose groups were significantly lower than those of the control group (all P<0.05). The secretion levels of TNF-α, IL-12 and the relative expression of TLR2 and CD68 proteins increased (all P<0.05) while the secretion level of IL-4 decreased (all P<0.05) in the cells of positive control group compared with the control group. The secretion levels of TNF-α and IL-12 increased (all P<0.05) while the secretion levels of IL-4 decreased in the cells of the three doses groups (all P<0.05), compared with the control group, and all showed a dose-effect relationship. The relative expression of TLR2 protein in the cells of the three doses groups increased (all P<0.05) compared with the control group. The relative expression of NF-κB p65 and CD68 protein in the cells of the medium- and high-dose groups increased (all P<0.05), but the relative expression of CD206 protein decreased (all P<0.05) compared with the control group. The relative expression of TLR2 and NF-κB p65 protein increased (all P<0.05) while the relative expression of CD206 protein decreased (all P<0.05) in cells of the high-dose group, compared with the low- and medium-dose groups. The average fluorescence intensity of CD68 increased (all P<0.05) while the average fluorescence intensity of CD206 decreased in the cells of high-dose group and the positive control group (all P<0.05), compared with the control group. {L-End}Conclusion Aluminum participated in and promoted the inflammatory response of GMI-R1 cells through the TLR2/NF-κB signaling pathway.
9.Efficacy observation of P-Gemox chemotherapy combined with intensity modulated radiotherapy in treatment of extranodal NK/T cell lymphoma, nasal type

Objective:To evaluate the therapeutic efficacy and side effects of P-Gemox regimen combined with intensity modulated radiotherapy in the treatment of extranodal NK/T cell lymphoma, nasal type (ENKTL).Methods:The data of 60 patients with ENKTL confirmed by pathomorphology and immunohistochemistry in Guizhou Cancer Hospital from July 2014 to October 2019 were retrospectively analyzed. All patients received P-Gemox chemotherapy combined with intensity modulated radiotherapy (at least 2 cycles), and the efficacy and adverse reactions were evaluated.Results:The complete remission rate of 60 patients was 65.0% (39/60), the partial remission rate was 25.0% (15/60), and the total effective rate was 90.0% (54/60). The main side reactions were myelosuppression, transaminase elevation and radiation mucositis; most of them were mild to moderate, which were relieved after treatment or the withdrawal of radiotherapy and chemotherapy. No treatment-related death cases were found. The overall survival rate of 1-year, 2-year, 3-year was 91%, 75% and 69%; the progression-free survival rate of 1-year, 2-year, 3-year was 86%, 68% and 62%. During the treatment, 3 cases died due to the progress of the disease and infection. Multivariate analysis showed that with and without hemophagocytic syndrome and radiotherapy dose were related to prognosis (all P < 0.05). Conclusion:P-Gemox, as the first-line induction chemotherapy regimen combined with intensity modulated radiotherapy has good short-term efficacy and safety for patients with ENKTL.
10.Spatial distribution and prediction analysis of the national AIDS epidemic in 2009 - 2020

Objective Tto analyze the spatial distribution and characteristics of the national AIDS/HIV epidemics from 2009 to 2020 to discover its distribution, aggregation, and hot spots, and provide corresponding suggestions for AIDS prevention and control. Methods Spatial autocorrelation analysis, hot spot analysis, and Kriging interpolation prediction were used to describe, analyze, and predicting the spatial distribution of AIDS epidemics across the country. Results The national AIDS incidence and mortality rate increased yearly, but the growth rate shows a downward tendency with uneven spatial distribution,focusing on the southwest and northwest regions; the average annual incidence rate of AIDS ( Moran's I> 0, P < 0. 01) and the average annual mortality rate (Moran's I> 0, P < 0. 01) of the distribution had a positive global spatial correlation, with Sichuan, Yunnan, Guangxi, Chongqing,Hunan and Guizhou being the areas with “high-high” clusters of AIDS incidence; Sichuan, Yunnan, Guangxi,Hunan,Xinjiang and Guizhou were the areas with “high-high” clusters of average annual mortality. The “hot spot” areas were mainly concentrated in the southwestern part of China, and the “cold spot” areas were mainly concentrated in the eastern coastal and northern parts of China; Kriging interpolation predicted that Xinjiang would be the new hot spot area for future epidemics. Conclusion The spatial distribution of AIDS in China is uneven, showing spatial aggregation, hot spots and cold spots coexist, and the high-risk areas will continue to expand in the future.So the prevention and control work should be carried out in a targeted and localized manner.
