1.Quality Evaluation of Meta-analysis of Randomized Controlled Trials on Antivirus TCM Injections for Respiratory Tract Infections

OBJECTIVE:To provide reference for the improvement of Metaanalysis. METHODS:13 repots about Meta-analysis of randomized controlled trials(RCTs)on antivirus traditional Chinese medicine(TCM)injections for respiratory tract infections were collected and analyzed. Their qualities were evaluated using QUOROM statement,CONSORT statement,Jadad scale and the quality evaluation method for Metaanalysis posed by Sacks,etc. Disagreements were resolved by consensus. RESULTS:The obtained average mark of 13 studies was(64?10.13)with the highest mark of 84 and the lowest of 52. Quality evaluations of reports were conducted from 5 aspects including 25 items. 5 aspects were all considered in 13 studies. 60% of the 25 items were in- volved in 13 reports at least and 88% at most. CONCLUSION:Meta- analysis methods are improved significantly in China. It should be still standardized to make sure correctness and reliability of results of meta-analysis. Meta-analysis method should be stan-dardized to obtain correct and reliable results.
2.Efficacy of Nicorandil for Treating the Patients With Cardiac Syndrome X and Its Impact on Vascular Endothelial Function

To observe the clinical efficacy of nicorandil for treating the patient with cardiac syndrome X (CSX) and its impact on vascular endothelial function. Methods: A total of 140 CSX patients were randomly divided into 2 groups: Control group, the patients received conventional anti-angina therapy and Nicorandil group, based on conventional anti-angina therapy, the patients received additional oral nicorandil treatment. n=70 in each group. All patients received resting emission computed tomography (ECT) and treadmill exercise ECG stress test (TET). Blood levels of endothelin (ET-1), high-sensitivity C-reactive protein (hs-CRP) and nitric oxide (NO) were examined before and 3 months after treatment. Results: Compared with pre-treatment condition, the attack frequency of angina pectoris and positive rate of ECT were decreased after treatment in both groups, P<0.05; in Nicorandil group, the suspicious positive rate and positive rate of TET were reduced after treatment, P<0.05. Compared with Control group, Nicorandil group had the much lower suspicious positive rate and positive rate of TET after treatment, P<0.05. Blood tests indicated that compared with pre-treatment condition, ET-1 and hs-CRP were decreased, NO was increased after treatment in both groups, all P<0.05; blood levels of ET-1, hs-CRP and NO were different between 2 groups after treatment, all P<0.05. Conclusion: Nicorandil could inhibit inflammatory factors, elevate endothelial function and therefore improve micro vascular angina symptoms, increase exercise tolerance obviously.

4.Expression and significance of BCAS3 and RFX3 protein in macrophages of gout stone tissue

Objective To analyze the expression and significance of BCAS3 and RFX3 proteins in mac-rophages from gout stone tissues.Methods Thirty gout stone excision specimens in this hospital from April 2019 to December 2022 were collected as the experimental group,and during the same period 25 specimens of synovial hyperplasia accompanied by foreign body giant cell reaction after joint replacement and 5 specimens of fat necrosis accompanied by macrophage hyperplasia served as the control group.The blood uric acid level of the control group was normal.Immunohistochemical staining was applied to conduct CD68,CD163,BCAS3 and RFX3 antibodies staining in all specimens.CD68 and CD163 were used to locate macrophages in tissues. The expression levels of BCAS3 and RFX3 in macrophages were detected.The expressions of the two antibod-ies and their correlation with age,sex and blood uric acid level were analyzed.The receiver operating character-istic (ROC) curve was used to evaluate the diagnostic value of the both on gout.Results The expression lev-els of BCAS3 and RFX3 in the experimental group were significantly higher than those in the control group (P<0.05).The BCAS3 and RFX3 protein expression levels in the control group had no statistical difference between the different sexes (P>0.05).The expression level of BCAS3 protein in the experimental group was positively correlated with the RFX3 protein expression (P<0.05);the uric acid level in the experimental group was positively correlated with the expression level of BCAS3 and RFX3 protein (P<0.05).The blood uric acid level in the control group had no correlation with BCAS3 and RFX3 protein (P>0.05).The age in the two groups had no correlation with the BCAS3 and RFX3 protein (P>0.05).The area under the curve (AUC),sensitivity and specificity of RFX3 were 0.935,96.55% and 86.67% respectively,which were higher than 0.742,70.00% and 80.00% of BCAS3.RFX3 had the higher diagnostic value for gout.Conclusion High expression of BCAS3 and RFX3 proteins in macrophages may participate in the pathogenesis of gout.There may be some synergistic promoting effect between the two.The diagnostic value of RFX3 for gout is higher than that of BCAS3.
5.SR4R:An Integrative SNP Resource for Genomic Breeding and Population Research in Rice

The information commons for rice (IC4R) database is a collection of 18 million single nucleotide polymorphisms (SNPs) identified by resequencing of 5152 rice accessions. Although IC4R offers ultra-high density rice variation map, these raw SNPs are not readily usable for the public. To satisfy different research utilizations of SNPs for population genetics, evolutionary anal-ysis, association studies, and genomic breeding in rice, raw genotypic data of these 18 million SNPs were processed by unified bioinformatics pipelines. The outcomes were used to develop a daughter database of IC4R–SnpReady for Rice (SR4R). SR4R presents four reference SNP panels, includ-ing 2,097,405 hapmapSNPs after data filtration and genotype imputation, 156,502 tagSNPs selected from linkage disequilibrium-based redundancy removal, 1180 fixedSNPs selected from genes exhibiting selective sweep signatures, and 38 barcodeSNPs selected from DNA fingerprinting sim-ulation. SR4R thus offers a highly efficient rice variation map that combines reduced SNP redun-dancy with extensive data describing the genetic diversity of rice populations. In addition, SR4R provides rice researchers with a web interface that enables them to browse all four SNP panels, use online toolkits, as well as retrieve the original data and scripts for a variety of population genet-ics analyses on local computers. SR4R is freely available to academic users at http://sr4r.ic4r.org/.
6.Study on neurexin 1 gene mutation in children with Tourette syndrome

Objective To explore the mutation characteristics of neurexin 1(NRXN1)gene in children with Tourette syndrome(TS).Methods A total of 524 children with TS were enrolled.DNA extracted from peripheral blood was sequenced for NRXN1 gene by using target region sequencing which was further verified by using Sanger sequencing.DNAMAN software,SIFT,PolyPhen2,Mutation Taster,FATHMM and ClinPred were used to analyze the hazard of suspected variants.Finally,the genotype and phenotype of the patients with NRXN1 gene variants were analyzed.Results We found 13 variants of the NRXN1 gene in 13 TS patients such as 11 point mutations and 2 deletion mutations including two novel point mutations:c.79G>T(p.A27S)and c.58G>T(p.G20C).The other nine point mutations and two deletion mutations were c.3523A>G(p.I1175V),c.4180A>T(p.T1394S),c.1697A>T(p.H566L),c.3715G>A(p.A1239T),c.878A>C(p.N293T),c.475C>T(p.P159S),c.320C>T(p.T107M),c.365A>G(p.Q122R),c.611T>A(p.L204Q)c.68_79del(p.G23_G26del),c.65_79del(p.G22_G26del).Bioinformatics analysis showed that the six gene variants c.58G>T,c.1697A>T,c.475C>T,c.365A>G,c.878A>C,c.79G>T were relatively harmful.There were 6 children with different parts of the tic,1 child with obsessive-compulsive symptoms,1 child with emotional instability,3 children with irritability,6 children did not have repetitive language,attention deficit,hyperactivity disorder,sleep disorder and depression.Conclusion NRXN1 gene mutation sites are detected in TS children,which expands the NRXN1 mutation spectrum.Children with different gene variants exhibit different clinical manifestations and the relationship between genotype and phenotype need further exploration.
7.Zinc finger protein A20-targeting siRNA promotes pyroptosis of human rheumatoid arthritis fibroblast-like synoviocytes

Objective To investigate the regulatory effect of small interfering RNA(siRNA)silencing zinc finger protein A20 on pyroptosis of rheumatoid arthritis(RA)fibroblast-like synoviocytes(HFLS-RA).Methods Hu-man FLS-RA cell line MH7A cells were cultivated,A20 siRNA silencing group was synthesized for knocking down the human A20 gene,and then specific A20 gene siRNA and siRNA-NC(negative control)were transfected into MH7A cells using liposome method.RT-qPCR was applied to detect the expression of NLRP3 and Caspase-1 mRNA in cells.The protein expression of NLRP3 and Caspase-1 was detected by Western blot,and IL-1β and IL-18 in cell culture medium were detected by ELISA method.Transmission electron microscopy(TEM)was used to detect pyroptosis.Results After A20 knockdown,the mRNA and expression of NLRP3 and Caspase-1 in MH7A cells in the siRNA-A20 group were significantly increased as compared with the siRNA-NC group(P<0.01).The concentration of IL-1β and IL-18 in the cell culture supernatant of the siRNA-A20 group was sig-nificantly increased compared with the siRNA-NC group(P<0.01).Compared with the siRNA-NC group,some cells in the siRNA-A20 group showed swollen and ruptured.The integrity of the cell membrane was also lost,and a large area of edema was present in the cell.In addition,a blurred depression of the local nuclear membrane was noted,while an increase in heterochromatin pyknosis was accompanied by their uneven distribution as well as their aggregation around the nuclear membrane.Conclusions Silencing of A20 gene with siRNA might promote NLRP3/Caspase-1 mediated pyroptosis in HFLS-RA,which lays an experimental foundation for new clinical treatment meth-ods of RA.
8.Factors influencing the sustainability of the evidence-based practice project for irritant dermatitis around urostomy: a qualitative study based on the i-PARIHS framework

Objective:To explore the influencing factors of the poor sustainability of the evidence-based practice project for irritant dermatitis around urostomy.Methods:This is a qualitative study. From April to May 2020, 12 nurses working in the Urology Surgery of Peking University First Hospital were selected by purposive sampling for the semi-structured interview. The integrated-Promoting Action on Research Implementation in Health Services framework (i-PARIHS framework) was applied to analyze and extract the obtained data.Results:The interview results found that, in terms of transform (evidence) , the localization and continuity of evidence for the evidence-based practice project of irritant dermatitis around urostomy needed to be improved, including the high matching between evidence and practice environment, and the insufficient continuity of evidence application. In terms of transform recipients, the leadership of project practitioner needed to be improved, including the conflict between the roles of practitioners and researchers, the lack of knowledge reserves of practitioners, and the conflict among the values of practitioners. In terms of organizational environment, the leadership of managers and the imformatization degree of project needed to be improved, including insufficient support from leaders and insufficient facilities.Conclusions:At the initial stage of formulating the nursing evidence-based practice project, the project implementer should fully incorporate the opinions of stakeholders for localization debugging, and consider the sustainability of the project. In addition, the leadership of managers, nurses and interns should be increased, the use and training of urostomy talents should be strengthened, the training of urostomy knowledge should be continued, the information-based follow-up system should be improved, and the intervention strategy should be continuously adjusted to ensure the project is carried out.
9.Retrospective analysis of endoscopic retrograde cholangiopancreatography after pancreaticoduodenectomy

Objective:To evaluate the efficacy and safety of endoscopic retrograde cholangiopancreatography (ERCP) after pancreaticoduodenectomy and endoscopic selection strategies.Methods:Clinical data of 34 patients treated with ERCP after pancreaticoduodenectomy at the Endoscopic Center of the First Affiliated Hospital of Air Force Medical University from January 2013 to December 2021 were retrospectively analyzed. The success rates of endoscopic insertion, diagnosis, treatment and ERCP, and the incidence of adverse events were analyzed.Results:Fifty ERCP treatments were performed in 34 patients. The success rates of endoscopic insertion, diagnosis, treatment, and ERCP after pancreaticoduodenectomy were 92.0% (46/50), 93.5% (43/46), 88.4% (38/43) and 76.0% (38/50), respectively. The success rates of ERCP assisted with colonoscope and balloon-assisted enterosocpe were 76.0% (19/25) and 75.0% (18/24), respectively. There were 3 adverse events, including 1 case of anastomotic mucosa tear during surgery, 1 case of cardiopulmonary arrest and 1 case of postoperative cholangitis.Conclusion:ERCP is effective and safe after pancreaticoduodenectomy in general. ERCP assisted with colonoscope and balloon-assisted colonoscope shows similar success rate after pancreaticoduodenectomy.
10. Clinical and bacteriological analysis of lymphoid tissue neoplasms patients with bacteria bloodstream infections

Objective:
To investigate the clinical status of lymphoid tissue neoplasms patients with bacteria bloodstream infections, bacteriology and drug susceptibility results, and provide the basis for rational clinical anti-infection option.
Methods:
A retrospectively analysis of clinical data and bacterial susceptibility test results of patients with bacteria bloodstream infections from September 2010 to December 2014 was conducted.
Results:
A total of 134 cases including 107 patients with bloodstream infections were enrolled. 84 cases were male, 50 cases were female, the median age was 31 (12-71) years old. 112 cases were agranulocytosis, and 106 cases were severe agranulocytosis (ANC<0.1×109/L) . 27 cases underwent hematopoietic stem cell transplantation, 100 cases received chemotherapy[33 cases with VD (I) CP±L (vincristine+daunorubicin/idarubicin + cyclophosphamide + prednison±asparaginasum) induction chemotherapy, 41 cases with intensive chemotherapy of Hyper-CVAD/MA or MA (mitoxantrone+cytarabine) , 26 cases with other chemotherapy regimens], and 7 cases were infected without chemotherapy. 10 patients discharged from hospital owing to treatment abandoning, 120 cases were cured through anti-infective therapy, 2 patients died of bacteria bloodstream infections, 1 patient died of sudden cardiac, and 1 patient died of GVHD after allogenic hematopoietic stem cell transplantation. A total of 144 strains were isolated, including 108 strains (75.0%) of Gram-negative bacteria and 36 strains (25.0%) of Gram-positive cocci. The susceptibility of Gram-negative bacteria to the carbapenems was 98.00%, and the adjustment treatment rate of carbapenems was 3.0%. The susceptibility of Gram-negative bacteria to the other antibiotics was 60.30%, and the adjustment treatment rate was 90.5%. The susceptibility of Grampositive cocci to the carbapenems was 49.3%, and to glycopeptides and linezolid was 100.0%. Comparing all patients’empirical use of antimicrobial agents with the drugs susceptibility results of blood cultures, 80.1% of the patients’initial drug selection was sensitive.
Conclusion
The lymphoid neoplasms patients experienced bacteria bloodstream infections most often after receiving the chemotherapy regimens of treating acute lymphoblastic leukemia. The majority type of bacteria was Gram-negative bacteria. Drug susceptibility test showed that susceptibility of Gram-negative bacteria to the carbapenems was the highest, and the treatment adjustment rate was obviously lower. The susceptibility of Gram-positive cocci to glycopeptides and linezolid was high, and which could be applied to the patients with Gram-positive cocci sepsis on basis of susceptibility results in general.
