Aim To investigate the effects of ginaton on tubular cell apoptosis in a rat model of renal ischemia/reperfusion(I/R) injury.Methods The model of bilateral post-ischemic renal injury was established by clamping renal pedicles for 45 minutes.To explore the effects of ginaton on activation and expression of JNK and c-Jun,p-JNK1/2,JNK,p-c-Jun,c-Jun were examined by Western blotting.Apoptosis of renal tubular epithelial cells was assessed by terminal deoxynucleotidyl transferase mediated dUTPnick end labeling(TUNEL) and flow cytometric analysis.Results Phosphorylation levels of the JNK and the c-Jun were significantly increased at 20 min after renal I/R injury.This corresponds to a significant increase of apoptotic tubular cells induced by renal I/R injury.Ginaton attenuated the increased phosphorylation of p-JUK at 20 min(vs I/R 20 min,P
3.Effect of propofol on stress response and immune balance in patients with severe trauma hemorrhage after fluid resuscitation

Objective To explore the stress response and immune balance in patients with severe trauma hemorrhage after fluid resuscitation and to clarify the clinical effect of Propofol administered with continuous intravenous infusion pump on it.Methods With prospective,randomized and control analysis,54 patients treated with fluid resuscitation following severe trauma hemorrhage admitted from October 1st,2008 to December 1st,2009 were studied.Another 20 healthy volunteers were enrolled as control group (C group). Patients were randomly divided into:conventional treatment group (R group,n =27 ) and conventional therapy combined with propofol treatment ( P group,n =27) as per gender,age,ISS score,estimated blood loss four factors and the principle ofminimum distribution imbalance index.HR,MAP,the levels of stress hormones [ norepinephrine (NE),cortisol (Cor) ],immune function ( T lymphocyte subsets Th1/Th2) and other biomarkers were observed.Mortality within 28 d between R group and P group was compared.ANOVA was used for the comparison of biomarkers,and independent samples t test was employed to compare variables between the two groups. Results Plasma cortisol (Cor),norepinephrine (NE),alanine aminotransferase (ALT),creatinine ( Cr),blood glucose,and Th1 and Th2 lymphocytes in patients were significantly higher than those in healthy volunteers of group C (P <0.01 ),and Th1/Th2 was significantly lower than those in healthy volunteers of C group (P =0.001 ).The ALT (48 h),Glu (6 h),NE (24 h and 48 h) and Cor (6 h and 24 h) of patients in P group were lower than those in R group ( P <0.05 ),and Th1 and Th2 were lower than those in R group at different intervals ( Th1:24h P =0.028,48 hP=0.002 ; Th2:6 h P=0.033,24 h P=0.007,48 h P=0.009),and Th1/Th2 ratio (48 h) was significantly higher in P group than that in R group (P =0.028),but there was no significant difference in mortality within 28d between the two groups. Conclusions Strong stress response,immune suppression and organ dysfunction can occur in the early stages of severe trauma hemorrhage after fluid resuscitation.Propofol can inhibit excessive stress response,modulate the immune balance and protect the important organs in those exsanguination patients from severe trauma.
4.Efficacy and safety of bronchoalveolar lavage through flexible fiberoptic bronchoscope in treatment of neonatal atelectasis

Objective To evaluate the clinical efficacy and safety of flexible fiberoptic bronchoscopy (FFB) and bronchoalveolar lavage (BAL) in treatment of neonatal atelectasis.Methods Eligible patients, who were diagnosed as neonatal pulmonary atelectasis and admitted consecutively to the neonatal intensive care unit of the Children's Hospital of Hebei Province from January 2013 to January 2015, were included in the study.They were randomly assigned to FFB group (n=30) and control group (n=28).Newborns in the FFB group received BAL under FFB, while those controls received tracheal irrigation after intubation.The duration of lung recruitment, oxygen exposure and antibiotic administration, hospital stay, culture results of respiratory secretions, prognosis and total expenses during hospitalization were compared between the two groups.Complications of FFB were also recorded.Chi-square test and t-test were performed for statistical analysis.Results (1) In the FFB group, atelectasis occurred in the upper fight lobe (n=26), upper lobes of both sides (n=1), lower right lobe (n=2) and lower left lobe (n=1), while in the control group, atelectasis occurred in the upper right lobe (n=26), lower left lobe (n=1) and middle right lobe (n=2) (x2=0.094, P > 0.05).(2) The positive rate of bacteria culture results showed no difference between bronchoalveolar lavage fluid in FFB group and tracheal secretions in the controls [43%(13/30) vs 32%(9/28), x2=0.770, P > 0.05].(3) The duration of lung recruitment, antibiotic administration and hospital stay of the FFB group were all shorter than those of the control group [(4.7±3.4) vs (7.4±6.6) d, (14.0±4.5) vs (20.3±10.9) d, (15.1±4.7) vs (21.8±12.3) d, t=-5.718, 8.604 and 7.733, all P < 0.05].(4) Among babies in the FFB group, nine experienced fever and returned to normal after physical cooling;three showed more shadow in chest X-ray with aggravated dyspnea during a short period, and relieved 12 hours later;two had minimal hemorrhage from tracheal mucous membrane;one showed crying hoarse.Serious complications, such as pneumothorax, massive bleeding or cardiac arrest, did not occurred.No death or refuse of treatment was reported.Conclusion FFB and BAL is much more effective than tracheal irrigation after intubation in treatment of neonatal atelectasis without any severe complications.

The paper described efforts made by Guangdong province to achieve a goal of the healthcare reform Serious diseases can be treated within the county.In this reform,the province attempts such operation mechanisms as autonomous type,guided type and trusted type for the purpose of elevating the service capabilities of county-level public hospitals.To this end,four independent operation mechanisms have also been built for talent development,remuneration incentive,specialty development,and cost control.All these efforts aim at encouraging the hospitals to improve service capability and quality.
6.Clinical studies of phentolamine warm uterus paste used in postpartum refractory urinary retention

Objective To study more effective treatment and care measures to release postpartum refractory urinary retention.Methods From January 2011 to December 2012,312 cases hospitalized single head delivery of perineum after episiotomy with maternal urinary retention were selected and divided into Group A,B and C stochastically,each group contained 104 cases.Group A was given phentolamine warm uterus paste on Qihai acupuncture point,Group B was given phentolamine treatment,Group C as control group was given warm uterus paste on Qihai acupuncture point.The treatment effects and treatment time were observed in three groups.The clinical outcomes and related indexes of medical resources consumption among 3 groups were compared.Results After intervention treatment on urinary retention of the 3 groups,the treatment time and effects on Group A was significantly better than Group B and C to relieve maternal urinary retention.The comparison among 3 groups had significant differences.While the postpartum hemorrhage,urinary tract infection caused by retention of urine and length of hospital stay and hospital costs were lower than Group B and C,the differences had statistical significance.Conclusions Phentolamine warm uterus paste on Qihai acupuncture point in treatment of postpartum refractory urinary retention is quick to work,simple,economic and safe,it has significant effects,and can significantly reduce complications,hospitalization time and cost of hospitalization.

Object In order to obtain further evidence of therapeutic efficacy of sophoramine (SA) *, its effect on transplantation rejection was studied with simultaneous exploration of its mechanism of action Methods Number of T lymphocyte and its sub sets of skin allografted rat model were determined by immunofluorescence technique, and proliferation of T lymphocyte was assessed by thiazolyl blue (MTT) assay Results SA reduced the CD 3 and C 4 cell counts, lowered CD 4/C 8 ratio, inhibited T lymphocyte proliferation and prolonged the survival time of skin allografted rat Conclusion SA could inhibit transplantation rejection by inhibiting the function of T lymphocyte with simultaneous reduction of lymphocyte count
8.Effect of HBM-based home visits on blood pressure and health-related behaviors of elderly hypertensive patients

Objective To explore the effects of health belief mode (HBM)-based home visits on the blood pressure (BP) and health-related behaviors of elderly hypertensive patients.Methods One-year HBM-based home visits were paid to 114 elderly hypertensive patients over 80 years old chosen from the community.The normalization rate of blood pressure after intervention at 3,6,12 months and health-related behaviors after intervention were also compared with those of pre-intervention.Results The BP after intervention was improved significantly after intervention as compared with that of pre-intervention (P<0.01).Health-related behaviors after intervention were significantly more than those before pre-intervention (P<0.01).Conclusions The hypertensions are controlled effectively with the implementation of the HBM-based home visits to the elderly hypertensive patients.The development of disease in elderly hypertensive patients can be slowed down and their life quality can be improved.
10.Differences in gene expressions between synovioblast and skin fibroblast in patients with osteoarthritis

BACKGROUND: During recent years, mononucleotide polymorphism of some genes is possibly related to affectability of osteoarthritis (OA). However, previous researches mainly compare the gene expression of synoviocytes between OA and rheumatoid arthritis (RhA); therefore, the correlation of gene expression between synovioblast and fibroblast in other tissues should be further studied as compared with OA.OBJ ECTIVE: To observe the differences of gene expression between OA synovioblast and skin fibroblast.DESIGN: Observational contrast analysis.SETTING: People's Hospital of Shenzhen City.PARTICIPANTS: Synovium tissue was derived from OA patients who received replacement of knee joint in the Department of Orthopaedics, People's Hospital of Shenzhen City. All OA patients met the diagnostic criteria of osteoarthritis established by American College of Rheumatology in 1995. Three patients including 1 male and 2 females aged more than 65 years old and they did not have cardiac and pulmonary disease and diabetes mellitus. Three male normal volunteers who aged 25 to 35 years did not have rheumatic disease, osteoarthritis and dermatosis. All subjects provided a confirmed consent. The main reagents were detailed as follows: RPMI1640 culture medium, fetal bovine serum and TRIZOL agent (Invitrogen Life Technologies Company, USA); pGEM-T pUC (Progema Company, USA);Display PROFILE-BASIC and Display PROFILE Probe kits (Qbiogen Company, USA).METHODS: The experiment was carried out in People's Hospital of Shenzhen City from January to June 2005. Synovium of OA patients were treated with primary culture to obtain synovioblast; meanwhile, skin fibroblast treated with primary culture from normal subjects was regarded as the control group. Restricted enzyme section differential display was used to separate the different-expressed genes of synovioblast and skin fibroblast in OA patients. In addition, blast technique was used to compare the resulted ranks with Genbank ranks.MAIN OUTCOME MEASURES: Differences of gene expression between synovioblast and skin fibroblast in OA patients.RESULTS: Gene expressions of superoxide dismutase (SOD), TFPI2, CXCL2, CXCL6 and transforming growth factor (TGF) were high in synovioblast of OA patients as compared with those in skin fibroblast of normal subjects.CONCLUSION: Gene expressions of SOD, TFPI2, CXCL2, CXCL6 and TGF are high in synovioblast of OA patients as compared with those in skin fibroblast of normal subjects. This suggests that gene may play a certain role in onset of OA.
