1.Anesthetic management for CO2 laser treatment of laryngeal papilloma under self-retaining laryngoscope in children

Twenty-eight children with laryngeal papilloma aged 10 months -3.5 yr weighing 8-15 kg received CO2 laser treatment under serf-retaining laryngoscope from May 2003 to May 2007. There were 17 patients without laryngeal obstruction, 7 patients with 1st degree laryngeal obstruction and 4 patients with 2rid or 3rd degree laryngeal obstruction. Different techniques of anesthesia were used for patients with different degrees of laryngeal obstruction. In patients without laryngeal obstruction anesthesia was induced with intramuscular ketamine 5 mg/kg. After the patients lost consciousness midazolam 0.1 mg/kg, ketamine 1-2 mg/kg or fentanyl 2 μg/kg was given iv. Tracheal intubation was facilitated with succinyl-cboline 1.5 mg/kg. In patients with 1st degree laryngeal obstruction, ketamine 5 mg/kg was given ira. The patients kept spontaneous breathing. Tracheal intubaiion was pedormed under topical anesthesia with 1% tetracaine. In patients with 2nd and 3rd degree laryngeal obstruction tracheal intubation was performed awake without any premedication under topical anesthesia with 1% tetracaine. The trachea was intubated with the tracheal tube 1 size smaller than the regular size. Anesthesia was maintained with propofol 3-5 mg·kg1·h-1 and intermittent iv boluses of ketamine 1-2 mg/kg and vecuronium 0.05-0. 1 mg/ kg. Dexamethasone 0.2-0.3 mg/kg was given iv at the end of operation. The patients were extubated when the patients regained consciousness and SpO2≥ 96% on air. In one patient with Ist degree laryngeal obstruction emergency tracheotomy was performed during induction of anesthesia. Anesthesia was otherwise smooth and recovery was uneventful.
2.Characteristics of verbal working memory in elderly patients with amnestic mild cognitive impairment

Objective To explore the features of verbal working memory in elderly patients with anmestic mild cognitive impairment (aMCI).Methods Thirty patients with aMCI and 30 normal controls (NC), whose age were over 65 years old, were examined with verbal working memory software and the other neuropsychological tests, such as verbal fluency test (VFT) and digit span test.Results The correct rate of visual semantic verbal working memory test was significantly lower in aMCI patients (79.83% ± 3.22% )than in NC ( 87.00% ±: 1.93 % , t = - 1.03, P = 0.002).There was no significant difference of correct rate of the visual phonological verbal working memory test between the aMCI (78.92% ± 8.60% ) and NC (86.80% ±2.14%, t = - 2.34, P = 0.060).Also, aMCI cases achieved significantly lower scores than NC on VFT (22.96 ±2.31 vs 31.53 ±3.72, t = - 1.08, P =0.004) and BDS ( 1.53 ±0.86 vs 3.63 ±0.56, t = - 1.23, P = 0.027).Conclusion The visual semantic verbal working memory is impaired and visual phonetic working memory is saved in aMCI patients.Both scores of verbal fluency test and digit span test are significantly decreased in aMCI patients, too.
3.Extraction of Alsophila spinulosa(Hook.) Tryon flavonoids by orthogonal experiment and separation by macroporous resin

AIM:To study the optimum extraction process of total flavonoids in Alsophila spinulosa(Hook.) Tryon and technological parameter of macroporous resin. METHODS: The optimum parameters were chosen by ratio of total flavonoids as the assessment index such as static and dynamic adsorption of macroporous resin. RESULTS: The optimum extracting process conditions were as follows: solvent was ethanol,ratio of solid-liquid was 1(∶)12,extraction time was 1 h,temperature was at(80 ?C).The optimum technological conditions of the D104 were: the solution concentract at 1.36 mg/mL,the current velocity of adsorption 2 BV/h,the amount of saturated adsorption at 5 BV,the eluting reagent 70% ethanol,the eluting velocity 3 BV/h,eluent volume at 2 BV. CONCLUSION: The process can separate and concentrate the total flavoionds of Alsophila spinulosa(Hook.) tryon.
4.Working memory in patients with Parkinson disease

Objective To study the working memory changes in patients with Parkinson disease(PD).Methods The Smith working memory software was used and 66 cases of PD patients and 60 normal controls were enrolled.Results The results showed that the correct rate of visuo-spatial working memory was significantly decreased (P0.05) working memory impairment found in PD patients. The correct rate of visuo-spatial and visuo-object working memory in the middle stage of PD was significantly decreased than that in the early stage. The relative analysis showed that the Hoehn-Yahr stage, a negative relationship to the correct rate of visuo-spatial and visuo-object working memory.Conclusion Spatial working memory impairment without object shape and the color working memory impairment were found in the early stage of PD. As disease advanced both the visuo-spatial and object shape working memory deficits were found exacerbated.
5.Double Intervention Therapy in the Treatment of Recurrent Cancer on the Stump of Stomach After Operation of Gastric Antrum

Objective To investigate the curative effect and the clinical value of double intervention in treating recurrent cancer on the stump of stomach after radical operation of gastric antrum.Methods 11 patients with recurrent cancer and stricture on the stump of stomach were treated with stent implantation and celiac arterial chemotherapy. 2 patients with ascites were treated with intravenous chemotherapy at first ,3 patients with hepatometastatic carcinorma were treated by embolization with lipiodol.All patients were retreated with celiac chemotherapy according to their healthy state after three or four weeks. All patients were fllowed-up.Results After therapy 6 months,12 months and 18 months survival rate were 63 6%,45% and 18% respectively,average survive 12 months.Conclusions Celiac arterial chemotherapy and stent implantation can prolong the survival period of the patients with recurrent cancer on the stump of stomach after radical operation for carcinoma of gastric antrum and improve their living quality.
6.Cloning and characterization of promoter region of cell division autoantigen 1

Objective To clone different lengths cell division autoantigen1(CDA1) promoters(CDA1P) and measure their activities in order to provide basis for fumctional study of CDA1P.Methods Different lengths CDA1P were amplified by PCR using mouse liver genomic as template and cloned into BglⅡ digested pGL3-basic plasmid to produce recombinant plasmid pGL3-basicmCDA1P,then they were transfected into Lewis lung cell line(LLC) and RAW 264.7 cells differently,these cells were collected after transfected for 48 h,finally luciferase activities of pGL3-basic-mCDA1P were measured.Results ①Different lengths CDA1P were cloned and characterized by restriction enzymes.②Different lengths CDA1P were ligated into pGL3basic vector,and were proved with PCR and DNA sequencing .③Activities of pGL3-basic-mCDA1A were different in the two kinds of Leuwis and mononuclear macrophage cells(RAW264.7 cells).Conclusion The different activities of CDA1P may be related with different cellular types.
7.Effect of Zhengjian Granule on blood pressure and insulin resistance in spontaneously hypertensive rates

Objective: Effect of Zhengjian Granule(ZJ)(Radix Berberidis, Ramulus Euonymi, Radix Scorphulariae Rhizoma Alismatis) on blood pressure(BP) and insulin resistance(IR) in spontaneously hypertensive rats(SHR) was observed. Methods: Twenty four SHR aged 13 weeks were divided into three groups treated with large and small dosage of ZJ and nothing as compared with age matched six Wister Kyoto rats(WKY). BP, glucose(G) and insulin(I) of fasting, G/I and blood lipid were measured before and after treated 8 weeks and compared among groups. Results: While G of fasting had no difference in respective SHR groups compared with WKY, insulin of fasting was markedly higher and G/I were notably lower than WKY( P
8.A retrospective analysis of 112 cases of adverse drug reaction of Chinese drug preparations

Objective To study the characteristics and regularities of Chinese drugs preparations ADR in order to improve the rational drug use.Methods A total of 112 cases of TCM ADR reports collected in 2009~2011 which have been presented to the Guangzhou ADR Monitoring Center.Results Of the 112 cases,26 kinds of drugs were involved,quinquagenarian were the main crowd,intravenous infusion was the main route of administration,the main types of TCM ADR were lesions of skin and accessories.Conclusion Great importance should be attached to TCM ADR monitoring to promote rational drug use in the clinic.
9.Characteristics of prospective memory impairment in patients with Parkinson's disease at early and middle stage

Objective To explore the characteristics of prospective memory impairments in patients with Parkinson's disease (PD) at the early and middle stage. Methods Fifteen PD patients at the early and middle stage and 15 normal controls matched with age, sex, education and occupation were asked to complete event-based prospective memory (EBPM) and time-based prospective memory (TBPM) tasks. Results In the EBPM task (points), there was a significant difference between PD patients (3.93±2.69) and normal controls (6.00±2.17, t =-2.318,P <0.05). In the TBPM task(points), there was no significant difference between PD patients (3.47±1.96) and normal controls (3.80±1.70, t= -0.498, P >0.05). Conclusion The EBPM is impaired and the TBPM is relatively spared in early and middle stage PD patients.
10.Changes and significance of serum glial fibrillary acidic protein (GFAP) in patients with Parkinson disease

Objective To investigate changes in serum level of glial fibrillary acidic protein (GFAP) in patients with Parkinson disease (PD) and its clinical significance. Methods Serum GFAP was determined with sandwich ELISA for 82 patients with PD and acute cerebral infarction (ACI), as well as healthy normal controls. Patients with PD were then divided into two sub-groups in terms of their course duration, one with less than five years and the other with more than or equal to five years. Effects of course duration and age of the patients on their serum GFAP were analyzed. Results Serum level of GFAP was significantly higher in patients with PD [(1.628±0.104) μg/L] and ACI [(1. 637±0. 063 )μg/L] than that in healthy normal controls [ (0. 025±0. 003)μg/L, t = 82. 7, 142. 2, all P <0. 05 ]. But, there was no significant difference in serum GFAP between patients with PD and ACI ( t =0. 214, P > 0. 05 ). Serum level of GFAP in PD patients had no significant correlation with their age. There was no significant difference in serum GFAP between two sub-groups of patients with PD (P > 0.05 ). Conclusions Serum level of GFAP increased significantly in PD patients, as compared to healthy normal controls, but it is not specific. Serum GFAP in patients with PD maintained at certain high level with progression of the disease, indicating effect of astrocytes may persistently exist during the disease course.
