1.A qualitative study on the cognition of the first group of nurses to protect against instrument-related stress injury in the novel coronavirus pneumonia

Objective:To explore the protective cognition of the first batch of nurses against novel coronavirus pneumonia on apparatus-related stress injury, so as to provide reference for formulating and implementing relevant training and improving management measures.Methods:Using the phenomenological method of qualitative research, from January 20, 2020 to February 17, 2020, a half structured in-depth interview was conducted among the echelon nurses who completed the first batch of nurses against novel coronavirus pneumonia on apparatus-related stress injury. A total of 13 nurses in isolation wards were interviewed for the study.Results:Using the method of Colaizzi phenomenology to analyze the interview data, the related knowledge cognition was low, the identification of risk factors was limited, the risk assessment was not carried out, the treatment measures were not timely, the awareness of protection was lacking, the management training was not perfect, and the medical protection materials were not enough.Conclusion:The first batch of nurses to fight against novel coronavirus pneumonia had a low awareness of the prevention of medical device-related stress injury. Hospital managers should strengthen the training of related knowledge, improve management strategies, enhance the awareness of protection, and identify risk factors early, take preventive measures to ensure the safety and health of front-line medical staff in a timely manner.
2.The role of multi-slice spiral CT in diagnosis and treatment of lumbar hernia

Objective To explore the imaging appearances and the value of multi-slice CT (MSCT)with post-processing tech-niques in diagnosis and treatment of lumbar hernia (LH).Methods The imaging and clinical data of 1 6 patients with LH which were confirmed by surgery were analyzed retrospectively.Results In 1 6 patients,1 7 lesions of LH were revealed by MSCT.There were superior lumbar hernias in 13 (76.5%),inferior lumbar hernias in 3 (1 7.6%)and diffuse lumbar hernia in 1 (5.9%).Hernia ring in diameter ranged from 1.5 to 9.3 cm,and the hernia sac size ranged from 1.8 cm×2.4 cm to 1 1.3 cm× 6.2 cm.MSCT showed oval or flask shaped mass of extraperitioneal fat with or without peritoneum and visceral contents protruded through the de-fecting fascia floor into lumbar triangle.Superior lumbar hernia was bounded by the 12th rib superiorly,the erector spine muscle medially and the internal oblique muscle laterally.Inferior lumbar hernia was bounded by latissimus dorsi muscle medially,the ex-ternal oblique muscle laterally and the iliac crest inferiorly.Hernia contents included colon in 3 patients and small bowel in 2 with in-carcerated hernia with small bowel obstruction in 1,and adipose tissue and mesentery in other 12.All patients received surgery treatment,and the appropriate surgical methods were selected according to the MSCT findings.The surgical findings were consistent with MSCT results.No recurrence was found during follow-up.Conclusion LH has characteristic CT manifestations.MSCT volu-metric scanning with post-processing techniques can clearly display the type of LH,the size of abdominal wall defect,hernia contents and their complications.It is helpful for diagnosis of the LH and differentiation from other diseases,which may provide important information for clinical surgery.
3.Clinical value of miR-27a and miR-1299 in predicting prognosis of patients with esophageal cancer

Objective To explore the clinical value of micro ribonucleic acid(miR)-27a and miR-1299 in predicting the long-term prognosis of patients with esophageal cancer.Methods A total of 89 patients with esophageal cancer in the hospital were enrolled between June 2020 and June 2022,and all underwent Minimally invasive thoracic and laparoscopic resection of esophageal cancer.The expression levels of miR-27a and miR-1299 were detected by real-time fluorescence quantitative polymerase chain reaction(PCR)before surgery,and level of serum ferritin(SF)was detected by enzyme linked immunosorbent assay.The relationship between miR-27a,miR-1299,SF and clinicopathology in patients with esophageal cancer was analyzed.All were followed up till February 2023 after surgery,and survival of patients was record.The relationship between miR-27a,miR-1299,SF and prognosis was analyzed.The risk factors of long-term prognosis in patients with esophageal cancer were analyzed by multivariate Cox regression model.The relationship between miR-27a,miR-1299 and serum SF was analyzed by Pearson.Results There were significant differences in miR-27a,miR-1299 and serum SF among patients with different tumor staging,lymph node metastasis and differentiation degree(P<0.05).The results of survival analysis showed that overall survival rates in high-expression miR-27a and SF groups were lower than those in low-expression miR-27a and SF groups(P<0.05),while which was higher in high-expression miR-1299 group than low-expression miR-1299 group(P<0.05).Cox regression analysis showed that miR-27a ≥2.54,miR-1299<4.18 and serum SF ≥223.78 μg/L(P<0.05)were independent risk factors of postoperative long-term prognosis of patients with esophageal cancer.Pearson correlation analysis showed that level of serum SF was positively correlated with relative expression level of miR-27a(P<0.05,r=0.612),while negatively correlated with relative expression level of miR-1299(P<0.05,r=-0.517).Conclusion The miR-27a,miR-1299 and serum SF are related to tumor staging,lymph node metastasis and differentiation degree in patients with esophageal cancer,which can be applied to evaluate postoperative long-term prognosis.
