1.Construction of DNA vaccine pcD-flaB against pathogenic leptospira infection and its immune mechanism

Objective:To investigate the immume mechanism and protective effect of DNA vaccine pcD flaB against pathogenic leptospira infection. Methods: DNA vaccine pcD flaB was constructed by inserting flaB into the eucaryotic expressing plasmid pcDNA3. After guinea pigs were infected with leptospira, the protective rate was observed and specific anti leptospira antibody IgG were tested by ELISA. TNF activity was tested by cell proliferation. Results:The protective rate against leptospira infection was 100%. The specific antibody IgG generated peaked at the 6th week. Activity of TNF released by macrophage of guinea pigs given DNA vaccine was higher than that not given vaccine. Conclusion:DNA vaccine pcD flaB can protect guinea pigs from leptospira challenge infection by inducing humoral immune response and increasing TNF activity.
2.Pharmacodynamic Effects of Volatile Oil from Homalomena occulta on Adjuvant-induced Arthritis Model Rats and Its Mechanism Study

OBJECTIVE:To study the pharmacodynamic effects of volatile oil from Homalomena occulta on adjuvant-induced arthritis(AA)model rats and its mechanism. METHODS:60 rats were randomly divided into normal control group(0.5%polysor-bate 80),model control group (0.5% polysorbate 80),positive control group (Tripterygium glycosides tablets,10 mg/kg), high-dose,middle-dose and low-dose(0.08,0.04 and 0.02 ml/kg)groups of volatile oil from H. occulta. Except for normal control group,other groups were given complete Freund's adjuvant subcutaneously via right rear foot plantar to induce AA model,and giv-en relevant medicine intragastrically for 25 days,once a day,since modeling. The articular swelling degree,immune organ (thy-mus gland and spleen) index,pathological change,the contents of IL-1β and TNF-α in serum were detected. RESULTS:Com-pared with normal control group,the primary and secondary articular swelling degree and thymus gland index of rats and the serum contents of IL-1β and TNF-α increased in model control group,while spleen index decreased(P<0.05 or P<0.01);obvious tissue swelling,large amount of neutrophile granulocyte,leukomonocyte and macrophage infiltrating joint surrounding tissue,the prolifer-ation of synovial cells and obvious osteoarthritic lesion were observed in podarthrum. Compared with model control group,the pri-mary and secondary articular swelling degree and the serum content of IL-1β decreased in the volatile oil from H. occulta groups;thymus gland index increased in middle-dose and low-dose groups of the volatile oil from H. occulta;the content of TNF-α de-creased in high-dose group of the volatile oil from H. occulta(P<0.05 or P<0.01). The inflammatory cell infiltration of joint sur-rounding tissue relieved in treatment groups,synovial cells proliferation was not obvious and synovial cells morphology was im-proved. CONCLUSIONS:The volatile oil from H. occulta has the pharmacodynamic effects on AA in rat,and its mechanism might be related to the serum content reduction of IL-1β and TNF-α.
3.Investigation into the effect of infant sleep patterns on the maternal sleep quality

Objective:To explore the effects of infant sleep patterns on maternal sleep quality.Methods:Three hundred and nineteen pairs of mothers and infants from two grade three class-a hospitals in Nantong City were enrolled. Infant sleep patterns were assessed by the Brief Infant Sleep Questionnaire (BISQ). Maternal sleep quality was evaluated using the Pittsburgh sleep quality index (PSQI). The effect of infant sleep patterns on maternal sleep quality was analyzed by one-way ANOVA followed with multivariate analysis.Results:The maternal sleep quality scored 7.08±2.60. Single factor analysis pointed out that the sleep quality of women whose infants slept in bed near parents, woke up more than 3 times a night, was nocturnal wakefulness more than 60 minutes or nocturnal sleep duration less than 8 hours was significantly worse. After controlling for confounding variables, the way of baby falling asleep, numbers of night wakings, nocturnal sleep duration, maternal age, infant′s gender and primary caregiver were independent factors of maternal sleep quality.Conclusion:Infant sleep pattern is an important factor affecting maternal sleep quality. Nurses should provide information about the sleep patterns of infants to mothers and their family members to promote their babies′ sleep habits so as to improve the maternal sleep quality.
4.Prediction for hemorrhagic transformation risk after intravenous thrombolysis in acute ischemic stroke patients in different therapeutic windows: comparison of 5 scoring systems

Objective To compare the predictive value of 5 scoring systems for hemorrhagic transformation risk after intravenous thrombolysis in patients with acute ischemic stroke (AIS) in different therapeutic windows.Methods A single-center and retrospective study was performed for 243 AIS patients who underwent intravenous thrombolysis using recombinant tissue plasminogen activator (rt-PA) in different therapeutic windows in our department during January 2014 and December 2016.Five scoring systems,including HAT model (hemorrhage after thrombolysis),MSS model (multicenter stoker survey),GRASPS model (glucose at presentation,race,age,sex,systolic blood pressure at presentation,severity of stroke at presentation),SEDAN model (baseline blood sugar,early infarct signs,hyperdense cerebral artery sign on admission CT,age,NIHSS on admission),and SITS model (safe implementation of thrombolysis in strokemonitoring study) were used to evaluate the risks for hemorrhagic transformation.The relationships between the 5 scoring systems and incidence rate of hemorrhagic transformation were analyzed among the patients in different therapeutic windows.The predictive values of the 5 scoring systems were compared using the areas (AUC) under the receiver operating characteristic (ROC) curve.Results When the AIS patients were treated by intravenous thrombolysis within 3 h,the AUC of GRASPS and HAT models were 0.698 and 0.619,respectively,higher than those of the other 3 systems.When the therapeutic window was between 3 to 4.5 h,HAT model and SEDAN model had highest AUC (0.719,0.744) than the other 3 systems (P <0.05).When the windows were >4.5 ~6 h,the HAT model had the highest AUC (0.676).Conclusion The 5 scoring systems show better predictive value for hemorrhagic transformation after intravenous thrombolysis.For the therapeutic window within 4.5 h,HAT model presents best predictive value than the other 4 scoring systems.
5.Preparation and Quality Control of Compound Premna Fulva Craib Liniments

OBJECTIVE:To prepare Premna fulva craib liniments and to establish a method for its quality control.METHODS:The liniments was prepared from yellowhairy premna stem,rhizoma drynariae,borneo and menthol in70%al-cohol solvent.Naringin in the liniments was identified qualitatively by TLC,and the content of naringin was determined by HPLC.RESULTS:Naringin was identified in TLC.The sample size of naringin showed a good linear relationship with its peak area in the range of0.51?g~2.55?g(r=0.9993).The average recovery was98.58%(RSD=1.31%).CONCLUSION:The preparation technology is practicable and the method of quality control is reliable.
6.Ultrastructural observation of dormant mouse embryos cultured in vitro after freezing-thawing

Objective The aim of this study was to investigate the differences of the cell ultrastucture of normal mouse hatched blastocysts and their dormant ones cultured in vitro after freezing-thawing, and to explore whether the dor-mant embryos have a better anti-freezing shock property than the normal hatched mouse embryos .Methods By transmis-sion electron microscopy , the ultrastructure of these two types of mouse embryos was observed and analyzed .Results By comparative analysis of their ultrastructure , the results showed that the dormant embryos before freezing are being austerity and with lower energy metabolism at a ‘ground state ’ .After freezing-thawing and culture , their cellular structure seemed to be similar to that of the normal embryos cultured in vitro before freezing.However, after freezing-thawing and culture, the number of mitochondria decreased , the nuclei were loose , and their heterochromatin also increased .Conclusions From the ultrastructural observation , compared with the normal mouse hatched embryos , the cellular state of dormant mouse em-bryos after freezing-thawing is more favorable for material storage and energy metabolism , thus, indicating that they have a better anti-freezing property than normal hatched embryos .
7.Change of ET-1 mRNA and ET AR mRNA expression in some organs in sepsis and septic shock rats

Objective: To investigate changes of the endothelin-1(ET-1) and its receptor (endothelin receptor subtype A, ET AR ) mRNA expression in some organs(kidney, lung and small intestinal mucous membrane) in the sepsis and septic shock rats. Methods: Twenty-four male rats randomized into sepsis group, septic shock group, control and normal group was infused with endotoxin(LPS) via indwelling right atria catheters except normal and control group. RT-PCR was used to detect kidney, lung and small intestinal mucous membrane tissue mRNA expression of the ET-1 , ET AR and glucose-6-phospho dehydrogenase(G-6-PD) in every group.Serum BUN, Cr, ALT and A were determined. The arterial oxygen tension (PaO 2) and arterial carbon dioxide partial pressure(PaCO 2)were measured in every group. Results: ET-1 mRNA and ET AR mRNA expression in the sepsis group and septic shock group were significantly higher than in normal group. There were significant differences between the normal/control group and sepsis/shock group in BUN,Cr,ALT,PaO 2 and PaCO 2. Conclusion: A higher expression of ET-1 mRNA and ET AR mRNA may be one of the startup factors on multiple organ dysfunction syndrome (MODS) and may play an important role on pathogenesis in sepsis and septic shock.
8.Correlations between emphysema quantification and severity of chronic obstructive pulmonary disease

Objective To analyze the correlation between emphysema extent measured by high resolution computed tomography (HRCT) and pulmonary function tests, symptom score in patients with chronic obstructive pulmonary disease ( COPD ) , and to study the value of HRCT in the emphysema quantification in the clinical evaluation of COPD patients. Methods 78 patients with stable COPD were recruited to take the HRCT scan , and emphysema extent was qualified by measuring the proportion of low attenuation area in the whole lung (LAA%). Correlations between LAA% and indices of pulmonary function test, bronchial dilation test, mMRC scale, CAT score and six minutes walking distance (6MWD) were assessed. Results LAA% was negatively correlated with FEV1/FVC and DLCO%pred, and the correlation coefficients were -0.759 and -0.589 (P <0.01), respectively. LAA% was positively related to mMRC score (r = 0.342, P < 0.01), and negatively asso-ciated with 6MWD (r = -0.365,P< 0.01). There was no association between LAA% and indices of bronchodila-tion test (⊿FVC, ⊿FVC%, ⊿FEV1, ⊿FEV1%) (P > 0.05). Conclusions The severity of emphysema measured by HRCT is well correlated with the clinical symptoms , pulmonary function tests and exercise capacity in COPD patients. It can be used to diagnose emphysema early and to evaluate the severity of the disease com-prehensively. Thus, the risk factors of COPD can be controlled and the prognosis of the patients can be im-proved.
9.Association between parent child relationship and rebelliousness in boarding school students

Objective:
To explore the relationship between parent child relationship and rebelliousness of junior middle school students in boarding schools and to provide a scientific basis for preventing the occurrence of youth rebellion.
Methods:
A cluster sampling method was used to investigate parent child separation, family intimacy, and rebellion among 1 790 junior high school students in one boarding middle school in Lujiang County, Hefei City, Anhui Province in January 2020, and the influence of parent child relationship on students rebelliousness was analyzed by Chi square test and binary Logistic regression.
Results:
The detection rate of rebelliousness among participants was 21.45%(384/ 1 790 ). Junior middle school students who were in senior grade (second and third grades), reported higher household economic status and no communication with parents during the past month were more likely to have rebelliousness( OR=1.83, 1.89, 1.80, 1.77 , P <0.05); Junior middle school students with higher parental warmth and intimacy were less likely to have rebelliousness( OR=0.52, 0.71, P <0.05); Gender, length of boarding, maternal education, frequency of meeting parents during boarding, and childhood experience of separation with parents showed no statistical significance on rebelliousness( P >0.05).
Conclusion
The detection rate of rebellious psychology among junior high school students in boarding schools is high, and good parent child relationship is the protective factor of rebellious psychology. Parents should pay attention to contact with their boarding children and strengthen communication to reduce rebelliousness among them.
10.Central venous catheter-related thrombosis in critically ill patients: a prospective observational study by ultrasonography

Objective Doppler ultrasonography was used to screen the incidence of central venous catheter (CVC) thrombosis in severe patients to observe the incidences of catheter-related thrombosis (CRT) at subclavian (SC) and internal jugular (IJ) venous insertion sites, and to analyze the factors affecting the thrombosis. Methods One hundred and twenty three adult patients with IJ or SC CVC admitted to the Department of Intensive Care Unit (ICU) of Zhongnan Hospital of Wuhan University from May to December 2015 were enrolled to be the research objects, they were divided into an IJCVC group (35 cases) and a SCCVC group (88 cases) according to different catheterization sites; they were divided into an operation group (85 cases) and a non-operation group (38 cases) according to whether operation was performed or not; and they were also divided into an anticoagulation group (25 cases) and a non-anticoagulation group (98 cases) according to whether anticoagulation therapy was used or not. Doppler ultrasonography was performed every day to observe the incidences of CRT during ICU stay. Results One hundred and twenty-three patients were included in this study. CRT was detected in 11 (8.9%) patients, with an incidence of 22.1 per 1 000 catheter-days. All the 11 cases with CRT were presented within 3 days after the insertion, with 9 cases (81.8%) on the first day and 2 cases (18.2%) on the third day. The incidence of CRT in SCCVC group was significantly lower than that in IJCVC group [5.7% (5/88) vs. 17.1% (6/35), P < 0.05], with the rates of 12.6 and 59.4 per 1 000 catheter-days, respectively. There were no statistical significant differences in the incidences of CRT between operation group and non-operation group [11.8% (10/85) vs. 2.6% (1/38)], and between anticoagulation group and non-anticoagulation group [8.0% (1/25) vs. 9.2% (2/98), both P > 0.05]. Conclusions The incidence of CRT at IJCVC site is estimated to be 3-times higher than that at SCCVC site, anticoagulants or surgical operation may have impacts on the incidence of CRT, although there were no statistically significant differences. The CRT usually occurs within 3 days after the catheter insertion. Frequent bedside ultrasonography in the first 3 days after catheterization can confirm the diagnosis and guide clinical treatment.
