1.Diagnostic value of 18F-FDG PET/CT in patients with pancreatic neoplasm

Objective To evaluate the diagnostic value of 18F-FDG PET/CT in patients with pancreatic neoplasm.Methods Fifty-three patients (36 males,17 females; age:(60.3±8.9) years) who underwent 18F-FDG PET/CT examination for suspected pancreatic tumor were retrospectively analyzed.Thirtytwo of them underwent dual-time point imaging.The characteristics of PET/CT images and serum CA19-9 were reviewed.Pathological results were used as a gold standard for evaluating the diagnostic value of PET/CT.Two-sample t test,paired t test and ROC curve analysis were used for data analysis.Results Thirtynine patients were finally diagnosed with pancreatic cancer and the other fourteen patients with benign disease.With SUVmax cutoff value of 3.13,the sensitivity and specificity of single-time point 18F-FDG PET/CT for diagnosis of pancreatic cancer were 92.3% (36/39) and 9/14,respectively.The SUVmax was significant different between pancreatic cancer group and benign disease group (6.16±2.89 vs 3.37± 1.58; t =4.46,P<0.01).Combined with the level of CA19-9,the diagnostic sensitivity and specificity were 69.2% (27/39)and 13/14,respectively.The SUVmax of the early scan (5.45±2.43) was significantly different from that of the delayed scan (6.87±2.93) in pancreatic cancer group (t =8.25,P<0.01),whereas no difference could be found in the benign group (3.18±1.28 vs 4.18±2.49; t=2.24,P>0.05).With the SUVmax cutoff value of 3.3,the sensitivity,specificity and accuracy in the early scan were 87.0% (20/23),6/9 and 81.2% (26/32),respectively.While with the SUVmax cutoff value of 3.0,the sensitivity,specificity and accuracy in the delayed scan were 95.7% (22/23),5/9 and 84.4% (27/32),respectively.Conclusion 18 F-FDG PET/CT is a useful tool in the differential diagnosis of benign from malignant pancreatic diseases.

OBJECTIVETo study the clinical effects of Fuzheng Huayu Gantang comprehensive therapeutic program (FHGP) on post-hepatitis B liver cirrhosis associated with glyco-metabolic abnormality (LCGA).
METHODSThe patients with LCGA enrolled in the randomized controlled clinical trial were assigned to 2 groups, the treated group (68 cases) and the control group (74 cases), they were treated respectively by FHGP and conventional TCM and Western medicine therapeutic program for 3 months. Indexes including fasting plasma glucose (FPG), 2 h postprandial plasma glucose (2 h PG), fasting insulin (FINS) were detected, homeostasis model assessment insulin resistance (HOMA-IR) was calculated, and the score of syndrome was recorded before and after treatment. Then the effects on syndrome and glyco-metabolic abnormality were evaluated through statistical analysis.
RESULTSLevel of 2 h PG after treatment was lowered in both groups (P < 0.01), but significant difference was found in the pre-treatment to post-treatment decrement of FPG and HOMA-IR between the two groups (P < 0.05). The syndrome improving rate and the total effective rate on glyco-metabolic abnormality in the treated group were significantly better than those in the control group respectively (85.3% vs 64.9% , P < 0.01; 80.9% vs 62.2%, P < 0.05).
CONCLUSIONFHGP has the capability to improve the syndrome and glyco-metabolic abnormality of patients with LCGA.

4.Evaluation of the memory function and cerebral blood flow in patients with hyperthyroidism

Objective To assess the memory function of hyperthyroid patients at different disease durations and investigate the value of cerebral blood flow (CBF) imaging in the detection of memory dysfunction in related regions.Methods Thirty-seven hyperthyroid patients (10 males,27 females; mean age (39.27± 10.58) years) and 28 healthy volunteers (8 males,20 females; mean age (35.80±9.41) years) were enrolled into this prospective study.The patients were divided into two subgroups: short duration group (duration ≤ 6 months; n =15),long duration group (duration >6 months ; n =22).Wechsler memory scale was used for memory assessment,and cancellation test was used for attention assessment.Self-rating depressions scale (SDS) and self-rating anxiety scale (SAS) were used for mood disorder assessment.99Tcm-ECD SPECT CBF imaging was performed at rest for all patients and controls on the same day.SPM 2.0 was used to investigate the differences of rCBF between the two groups.Two independent samples t test was used for the comparisons of memory and attention scores between patients and controls,also between patients with short and long disease durations.Multiple stepwise regression was used for factor analysis of memory state.Results Scale total score (92.27±17.50 vs 101.75±11.70; t=-2.476)and memory quotient (91.32±17.76 vs 100.29±9.43 ; t =-2.421) were significant different between patients and controls (both P<0.05).The scale total score and memory quotient in patients with long disease duration were significant lower than those of controls(88.77±16.69 vs 101.75±11.70,t=-3.231; 86.18±16.73 vs 100.29±9.43,t=3.770,both P<0.05).The memory quotient was significant lower in patients with long disease duration than those with short disease duration(86.10± 17.13 vs 98.87± 17.00; t =2.212,P<0.05).There was no significant difference in memory quotient and scale total score between short duration group and controls (t=-0.754,0.910,both P>0.05).CBF was reduced in the limbic system of hyperthyroid patients and the involved area was larger in patients with long disease duration.Age,attention score,serum FT4 level and reduced CBF in left pulvina nuclei,left lateral geniculate body and right amygdale were the statistically significant factors for impaired memory function in hyperthyroid patients (b=-0.393-0.685,all P<0.05).Conclusions Memory impairment could be significant in hyperthyroid patients with long disease duration.CBF imaging could reflect the degree of dysfunction at specific brain regions responsible for memory impairment in these patients.
5.Clinical Efficacy and Safety of Qingyusan Capsules in Treatment of Ulcerative Colitis with Syndrome of Large Intestine Dampness-heat

ObjectiveTo evaluate the efficacy and safety of Qingyusan capsules in the long-term treatment of mild to moderate active ulcerative colitis (UC) with the syndrome of large intestine dampness-heat. MethodA randomized, controlled design was adopted, and 88 patients with mild to moderate UC and syndrome of large intestine dampness-heat were randomized into a Qingyusan (Qingyusan capsules, 0.8 g·d-1) group and a control (mesalazine, 0.4 g·d-1) group, with 44 patients in each group. Three and one patients dropped out in the control and Qingyusan groups, respectively, during the 32 weeks of treatment. The clinical remission rate, mucosal healing rate, and modified Mayo score, TCM symptom score, and short inflammatory bowel disease questionnaire (SIBDQ) score before and after treatment were compared between the two groups. The colonoscopic and pathological changes were observed, and the clinical safety was compared between the two groups. ResultAfter treatment, the clinical remission rate and mucosal healing rate in the Qingyusan group were 72.1% (31/43) and 74.4% (32/43), respectively, which were higher than those [26.8% (11/41) and 41.5% (17/41), respectively] in the control group (χ2=17.200, χ2=10.843, respectively, both P<0.01). The treatment in both groups decreased the modified Mayo score, partial Mayo score, and TCM symptom score (P<0.05), and the decreases in the Qingyusan group were higher than those in the control group (P<0.01). After treatment, the SIBDQ scores in both groups increased (P<0.05), and the increase was more pronounced in the Qingyusan group than in the control group (P<0.01). There was no difference in the incidence of adverse events between the two groups. ConclusionThe clinical efficacy of Qingyusan capsules is remarkable in the long-term treatment of UC with the syndrome of large intestine dampness-heat. Particularly, Qingyusan capsules demonstrates advantages in inducing and maintaining clinical remission, promoting mucosal healing, alleviating TCM symptoms, and enhancing the survival quality of patients, with high safety.
6.Literature review on clinical features and associated complications of dorsal pancreas agenesis

Objective To analysis the clinical features of dorsal pancreas agenesis ( DPA) and the associated diabetes, pancreatitis and other congenital organ malformations.Methods Chinese databases of Sinomed, CQVIP and CNKI using the term of short pancreas, pancreas agenesis, bulbar pancreas and dorsal pancreas, and English databases of PubMed using the term of dorsal pancreas agenesis, short pancreas and pancreas hypoplasia were searched.The clinical manifestation, pancreatic head characteristics and associations with diabetes, pancreatitis and other congenital organ malformations were analyzed.Results Six related publications from Chinese databases were searched and 21 patients were included with 2 cases excluded.Sixty-one publications from English database were searched and 71 patients were included.Thus, a total of 91 patients with DPA were analyzed.Abdominal pain was the most common manifestation, which was reported by 61.5% of the patients. 15.3% patients were identified during regular physical examination. Other manifestations including jaundice, fatigue, abdominal discomfort and diabetes were rare.After removing cases with insufficient information, 39 patients (61.9%) carried abnormal pancreatic head.Prevalence of diabetes or impaired glucose tolerance was 56.7% and the percentage of insulin-dependent diabetes in patients with abnormal glycaemia was 47.3%(n=18).20 patients (26.7%) were associated with pancreatitis, including 15 patients (75.0%) with acute pancreatitis, 1 patient (5.0%) with recurrent pancreatitis, and 4 patients (20.0%) with chronic pancreatitis. Thirty-three patients ( 36.2%) suffered other congenital organ malformations, including 21 patients (63.6%) with splenic malformation, 8 patients (24.2%) with heart malformation, and 17 patients (51.5%) with multi-organs malformations like gastrointestinal malformation, azygos vein and inferior cava vena fusion, duodenal and biliary atresia and renal absence.Conclusions The main diagnostic criteria of DPA was the absence of dorsal pancreatic duct.Diabetes was the most common complication followed by pancreatitis.
7.Effects of suxiao jiuxin pill on patients with acute coronary syndrome undergoing early percutaneous coronary intervention.

OBJECTIVETo investigate the intervention effects of Suxiao Jiuxin Pill (SJP) on patients with acute coronary syndrome (ACS) undergoing early percutaneous coronary intervention (PCI).
METHODSSixty ACS patients were randomly assigned to the treatment group (treated by SJP and Western medicine) and the control group (treated by Western medicine alone), 30 in each group. Coronary arteriography and early PCI were performed in all patients. The effects of SJP on the blood flow rate, the collateral artery patency, and perioperative myocardial infarction incidence were observed.
RESULTSThe coronary blood flow rate was better in the treatment group than in the control group either pre- or post-PCI. [pre-PCI thrombolysis in myocardial infarction (TIMI) level III: 16/30 vs 11/30, P < 0.01; post-PCI TIMI level III: 14/14 vs 13/19, P < 0.05)]. In patients with ITMI level 0 - I , more patients in the treatment group had collateral artery protective function than those in the control group (5/6 vs 3/13, P < 0.05). The incidence of perioperative myocardial infarction was obviously lower in the treatment group than in the control group (8/30 vs 15/30, P < 0.05).
CONCLUSIONSJP could improve the pre- and post-PCI coronary artery flow rate, increase the collateral artery patency, and reduce the incidence of perioperative myocardial infarction of ACS patients.

8.Liver histopathological features of patients with drug-induced liver injury due to Chinese herbal medicine or Western medicine: A comparative analysis of 50 cases

ObjectiveTo compare the liver histopathological features of drug-induced liver injury (DILI) caused by Chinese herbal medicine (HM) and that caused by Western medicine (WM), and to investigate the pathological features of HM-DILI. MethodsA total of 50 patients with drug-induced liver injury(DILI) who were diagnosed and treated in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from March 2014 to June 2019. All the patients with DILI were enrolled and divided into HM-DILI group and WM-DILI group. A retrospective analysis was performed for the clinical features and the pathological features of the two groups, including the clinical type of DILI, severity of liver injury, and liver histopathological features, especially location and grade of liver inflammation, type and degree of steatosis, and iron deposition. The t test and Mann-Whitney U test was used for comparison of continuous data between two groups; the chi-square test was used for comparison of categorical data between two groups; the Mann-Whitney U test was used for comparison of ranked data. ResultsOf all 50 patients, 20 (40.0%) had HM-DILI and 30 (60%) had WM-DILI. There were no significant differences in sex, age, course of disease, and biochemical parameters of liver function between the two groups (all P>0.05). The main clinical types of the two groups were hepatocellular injury type and the degree of injury which was mainly grade l, and there were no significant differences in the clinical type of DILI and the severity of liver injury between the two groups (both P>0.05). As for liver histopathological features, both groups had varying degrees of focal necrosis of hepatocytes, steatosis (microvesicular, macrovesicular, and mixed), and fibrous tissue proliferation at the portal area, and compared with the WM-DILI group, the HM-DILI group had significantly greater lymphocyte/plasma cell infiltration in the portal area (χ2=3.860, P<0.05) and iron deposition in Kupffer cells (χ2=4787, P<0.05). ConclusionThere are no significant differences in clinical manifestations between HM-DILI and WM-DILI, but patients with HM-DILI have more obvious inflammation in the portal area and iron deposition in Kupffer cells than those with WM-DILI.
9.The precision of glomerular filtration rate determined by Gates method and compared with the results from renal pathological changes

Objective To evaluate the precision of GFR using Gates method and compared with the results from renal pathological changes. Methods Twenty-seven patients whose 99Tcm-DTPA renograms had no obvious uptake phase were enrolled in Group A, and 27 patients whose 99Tcm-DTPA renograms had obvious uptake phase were enrolled in Group B. The measurement of GFR by Gates method was compared to the creatinine clearance measured and predicted by Cockroft-Gault (C-G), modification of diet in renal disease (MDRD) and SCr level. Renal pathological changes in two groups were compared using Pearson correlation and t test analysis. Results In Group A, GFR determined by Gates method did not show correlation with that estimated by C-G or 1/SCr (r = 0. 357,0. 376, both P >0.05), but was significantly correlated with GFR estimated by MDRD(r = 0. 440, P < 0.05). In Group B, GFR determined by Gates method showed significantly correlation among GFR estimated by MDRD, C-G, and 1/SCr (r =0. 471, 0. 527,0. 452, all P < 0.05). Renal tubulointerstitial damage score in Group A was higher than that in Group B (7.15±2.32, 3.70±3.06, t=4.66, P <0.001). Conclusions GFR determined by Gates method is less precise when 99Tcm-DTPA renogram has no obvious uptake phase than that when 99Tcm-DTPA renogram has obvious uptake phase. Renal tubulointerstitial damage is a strong indicator of no obvious uptake phase in 99Tcm-DTPA renogram.
10.Traditional Chinese medicine in the treatment of ulcerative colitis in 2013-2023:a bibliometric analysis

OBJECTIVE To analyze the current research status and hotspots of traditional Chinese medicine (TCM) in the treatment of ulcerative colitis (UC) via bibliometrics method. METHODS The clinical, basic, and pharmacological research of TCM in the treatment of UC in the CNKI core journals database and Web of Science core collection database were searched from January 2013 to April 2023. The CiteSpace 6.1. R6 software was adopted to analyze the number of authors, institutions and keywords. What’s more, a visual map was plotted. RESULTS Overall, 1 060 Chinese articles and 555 English articles were included. In 2013-2023, the number of relevant research documents issued by TCM in the treatment of UC has shown a significant upward trend. Among the included literature, there are 59 core authors in Chinese and 33 core authors in English. A stable group centered on multiple key scholars has been formed. Chinese research hotspots focus on the experience of famous doctors, classical famous prescriptions, etc.; English research hotspots center on cells, expression, activation,signaling pathway, etc. The study of intestinal flora, inflammatory factors, and other mechanisms has attracted much attention in both Chinese and English literature. CONCLUSIONS In recent years, some progress has been made in the study of treating UC with TCM. In the future, the top-level design of clinical research protocols based on maintaining the characteristics of TCM should be strengthened. Moreover, the long- term efficacy evaluation of TCM in the treatment of UC should be emphasized, and new technologies such as metabolomics and protein sequencing should be actively adopted to explore the scientific connotation of TCM compatibility in treating diseases.
