As a clinical diagnosis and treatment of low back pain,straight leg raising test has been used most commonly more than 100 years.It is simple and practical,and has a high clinical value.As a very mature orthopedic special examination,straight leg raising test progress in non-orthopedic applications more often than orthopedic aspects.Of course,the most important thing is the diagnosis of low back pain,followed by the treatment of low back pain,and then can be used as a respiratory and circulatory aspect of diagnosis and treatment options.This article elaborated the principle of straight leg raising test and the clinical research progress in order to give the clinician some guidance.
3.Clinical study of myocardial damage after neonatal asphyxia

Objective To investigate the early diagnosis of myocardial injury after neonatal asphyxia based on the clinical manifestations of myocardial injury, electrocardiogram (ECG), cardiac enzymes and tissue Doppler echocardiography. Methods From January 1, 2013 to June 30, 2014, 101 cases of neonatal asphyxia in the neonatal intensive care unit of the First Hospital of Tsinghua University, with gestational age> 37 weeks and birth weight > 2 500 g, were enrolled. Apgar scores were used to diagnose neonatal asphyxia. Myocardial damage after neonatal asphyxia was diagnosed according to the hypoxia history, clinical presentation, ECG and cardiac enzymes. According to the umbilical arterial blood gas analysis, severe asphyxia was divided into two groups:the severe asphyxia with severe acidosis group and the severe asphyxia without acidosis group. The incidence of myocardial damage, clinical manifestations associated with myocardial damage, ECG and myocardial enzymes [creatine kinase isoenzyme MB (CK-MB) and cardiac troponin T (TnT); control group involved 50 cases for the same period of admission with newborn jaundice] and echocardiography (control group involved 30 cases for the same period with normal term delivery) were compared among the three groups [mild asphyxia (n=72), severe asphyxia with severe acidosis (n=18) and severe asphyxia without severe acidosis (n=11)]. One-way ANOVA, the LSD test, Kruskal-Wallis test for independent samples, Chi–square test and Fisher's exact test were used for statistical analysis. Results (1) The incidence of myocardial damage after asphyxia was 34.6%(35/101). It was higher in the severe asphyxia group than in the mild asphyxia group [62.1%(18/29) vs 23.6% (17/72), χ2=7.549, P=0.006]; and it was higher in the severe asphyxia with severe acidosis group than in the severe asphyxia without severe acidosis group (14/18 vs 4/11, Fisher's exact test, P=0.048). (2) Clinical manifestations: The proportion of bradycardia was greater in the severe asphyxia with severe acidosis group (13/14) than in the severe asphyxia without severe acidosis group (1/4) and the mild asphyxia group (7/17);the differences were statistically significant (Fisher's exact test, P=0.019 and 0.007). (3) ECG: Eighteen cases (51.4%, 18/35) showed ECG abnormalities. (4) Cardiac enzymes:CK-MB 48 h after birth in the severe asphyxia with severe acidosis group, severe asphyxia without severe acidosis group, mild asphyxia group and the control group were 78.72 (34.63-122.01), 31.71 (21.33-37.12), 23.11 (14.61-36.02) and 11.82 (8.64-18.93) μg/L, respectively. CK-MB in the severe asphyxia with severe acidosis group was higher than in the severe asphyxia without severe acidosis group, mild asphyxia group and the control group (H=48.425, 90.040 and 96.045, respectively, all P<0.01). After treatment for 5-7 days, there was no statistically significant difference in these four groups (H=7.165, P=0.416). TnT 48 h after birth in the four groups was 0.19 (0.12-0.39), 0.11 (0.06-0.34), 0.07 (0.05-0.13) and 0.06 (0.04-0.08) μg/L, respectively. TnT in the severe asphyxia with severe acidosis group was higher than in the other three groups (H=45.753, 44.665 and 61.215, respectively, all P < 0.01). Despite the reduced TnT level after treatment for 5-7 days, TnT in the severe asphyxia with severe acidosis group was higher than that in the other three groups (H=17.520, 21.122 and 43.286, respectively, all P<0.01). (5) Echocardiography:Twenty cases (57.1%, 20/35) showed abnormalities. The values of mitral systolic peak velocity and late diastolic peak velocity in the severe asphyxia with severe acidosis group were lower than those in the control group found by tissue Doppler echocardiography [(3.4±0.3) vs (4.8±0.3) cm/s, (4.1±0.2) vs (6.0±1.1) cm/s, respectively, t=3.293 and 2.542, both P < 0.05]. Conclusions Myocardial damage can occur after neonatal asphyxia. Cord blood pH value should be combined to determine the severity of asphyxia. Myocardial damage is more serious in the severe asphyxia with severe acidosis group. Clinical manifestations should be taken seriously, and laboratory examinations should be improved for early diagnosis and treatment.
4.Expression of SNCG and CXCR4 mRNAs in gastric adenocarcinoma and its clinical significance

Objective:To detect expression of SNCG and CXCR4 mRNA in gastric adenocarcinoma and evaluate its function in carcinogenesis, progression, infiltration, and metastasis, as well as the correlation of these proteins. Methods:The expression of SNCG and CXCR4 mRNAs was detected using reverse transcription-polymerase chain reaction in gastric adenocarcinoma and normal gastric mucosa. Results:The expression of SNCG and CXCR4 mRNAs was obviously higher in gastric adenocarcinoma than in normal gastric mucosa (P<0.01). The expression of SNCG mRNA was closely correlated with tumor infiltration depth (P<0.05) and lymph node metas-tasis (P<0.01). The expression of CXCR4 mRNA closely correlated with lymph node metastasis (P<0.01). The expression of SNCG and CXCR4 mRNAs positively correlated in 29 cases of gastric adenocarcinoma (r=0.346, P<0.05). Conclusion:SNCG and CXCR4 may be overexpressed in tumor tissue and may have important functions in tumorigenesis, progression, infiltration, and metastasis of gastric adenocarcinoma.
5.Effect of Astragalus injection combined with low molecular weight heparin and nifedipine on levels of laboratory related indexes and pregnancy outcomes in preeclampsia

Objective To investigate the effect of Astragalus injection combined with low molecular weight heparin and nifedipine on levels of related indexes and pregnancy outcomes in patients with preeclampsia.Methods A total of 60 patients with preeclampsia from March 2013 to December 2016 in Cixi Municipal Maternal and Child Care Hospital were selected and randomly divided into the study group and control group, with 30 cases in each group.All patients were given spasmolysis, diuretic and other conventional treatment.The control group were treated with low molecular weight heparin and nifedipine while the study group were additionally treated with Astragalus injection.The blood pressure, blood coagulation and other laboratory indicators in the two groups were monitored before and after treatment , and the incidence of pregnancy complications and pregnancy outcomes in the two groups were statistically analyzed.Results There was no significant difference in blood pressure, human chorionic gonadotropin (β-hCG), alpha fetoprotein (AFP) and blood coagulation function indexes in the two groups before treatment .After treatment, the blood pressure,β-hCG, AFP, platelet count (PLT), prothrombin time (PT), activated partial thromboplastin time (APTT), plasma fibrinogen (FIB) and D-dimer (D-D) were significantly improved, the difference was statistically significant ( P<0.05).After treatment, the diastolic and systolic blood pressure, β-hCG, AFP, PLT, FIB and D-D levels in the study group were significantly lower than those in the control group, the difference was statistically significant ( P<0.05), PT and APTT were significantly longer than those in the control group, the difference was statistically significant (P<0.05).The incidence rates of pregnancy complications ( uterine inertia, premature rupture of membrane, fetal distress, placental abruption) and adverse pregnancy outcomes (cesarean section, neonatal asphyxia) in the study group were significantly lower than those in the control group, the difference was statistically significant (P<0.05).Conclusion The application of Astragalus injection combined with low molecular weight heparin and nifedipine in the clinical treatment of patients with preeclampsia can effectively control the blood pressure level and other parameters , reduce pregnancy complications and improve pregnancy outcomes.
6.The effects of strict dietary salt restriction on blood pressure and proteinuria in chronic glomerulonephritis patients

Objective To evaluate the effects of strict dietary salt restriction on blood pressure and proteinuria in chronic glomerulonephritis (CGN) patients. Methods From October 2007 to April 2009, 32 CGN inpatients were enrolled. Among them, 15 patients followed a strict dietary salt restriction menu (sodium 100 mmol/d, potassium 50 mmol/d, protein (0. 8-1. 0) g · kg~(-1) · d~(-1) , calorie (105-125) KJ · kg~(-1) · d~(-1) ) for 7 days, while the other 17 patients were fed freely offered by hospital as controls. 24 h urinary sodium excretion (24h-UNa) was used to monitor the salt intake. No changes of drug therapy were made during the study. Blood pressure was monitored every day. 24-hour urinary protein and serum biochemical parameter were measured before and after the study. Results There was no significant difference of baseline 24h-UNa between the two groups [(135.1 ±50.4) mmol/d vs (137.4 ±28.6) mmol/d) ]. During the study, the average 24h-UNa of patients with strict dietary intervention was (97. 2 ± 8.6) mmol/d. Both SBP [ (117. 7 ± 10. 0) mm Hg( 1 mm Hg=0. 133 kPa) vs (106.2 ±9.9) mm Hg] and DBP [ (76. 3 ± 6. 1 ) mm Hg vs (67. 5 ± 5. 5 ) mm Hg] decreased significantly ( P < 0. 001 ) . Proteinuria decreased significantly too [ 1. 57 (0. 3-3. 0) g/d vs 0. 57 (0. 16-2. 72) g/d,P = 0. 006]. The reduction of SBP was positively correlated with the reduction of 24h-UNa (r =0. 572, P =0. 026) , while the reduction of proteinuria correlated with both the reduction of SBP (r = 0. 568, P = 0. 027) and 24h-UNa (r =0. 525, P =0. 044). In the control group, only SBP decreased significantly [ ( 122. 6 ± 15. 5) mm Hg vs (115.8 ±10.4) mm Hg, P = 0.02] without significant changes of DBP and proteinuria When comparing the subgroups who took ACEI/ARB from both groups, the reduction of proteinuria wasmore prominent of those from the study group than the control group [ - 0. 4 ( -0. 95-0. 07) vs 0. 07 ( - 0. 39-0. 42), P = 0. 014 ]. Conclusion Strict dietary salt restriction is effective in reducing blood pressure and proteinuria in CGN patients.
7.Primary culture of human eutopic and ectopic endometrial glandular epithelial cells and stromal cells

Objective:To explore the way to separate and culture eutopic and ectopic endometrial glandular cells and their stromal cells,providing an in vitro cell model of endometriosis for study of its mechanism. Methods:Digestion,filtration and sedimentation were used to isolate and culture the eutopic and ectopic endometrial glandular cells and their stromal cells. The estrogen level was imatated to study the way of promoting cell growth. Morphological characters of eutopic and ectopic endometrial cells were examined using optical microscope. Results:The success rate of separation and culture of normal control endometrial glandular cells and its stromal cells was 91.7%(11/12);of eutopic endometrial glandular cells and its stromal cells of endometriosis was 93.8%(15/16);of etopic endometrial glandular cells and its stromal cells of endometriosis was 75.0%(12/16) . Conclusion:The cultured eutopic and ectopic endometrial cells is more like human body feature than the endometriosis model of animals. So the isolation and culture of eutopic and ectopic endometrial glandular cells and their stromal cells may serve as an in vitro experimental model.

9.Comparison of laparoscopic versus laparotomic operation for cervical carcinoma in elderly Chinese women: a meta-analysis

Objective To compare the curative effect of laparoscopic and laparotomic operation for elderly Chinese women with cervical cancer. Methods The randomized controlled trials (RCT) and case controlled trials (CCT) were collected by searches of WanFang database, CNKI, VIP, China Biology Medicine (CBM) , PubMed. Data were extracted from these trials and data analysis was performed by RevMan 5.2.9. Results There were no RCTs, however, a total of 10 CCTs met the inclusion criteria and had data extracted for this review. The baseline characteristics of the laparoscopic group were similar to those of the laparotomic group. Comparing to laparotomic group, the laparoscopic group have longer operation time (MD =32.60, 95%CI: 5.65~59.55, P = 0.020), less amount of bleeding (MD = -94.01, 95%CI: -130.65 ~ -57.37, P = 0.000), smaller number of lymph node dissection(MD = 1.69, 95%CI: 0.67 ~ 2.72, P = 0.001), earlier anus exhaustion (MD=-17.09, 95%CI: -21.19 ~ -12.98, P = 0.000) and shorter hospital stays (MD = -4.30, 95%CI: -5.57 ~ -3.02, P = 0.000). There was significant difference between the two groups. But there was no statistical significance in postoperative indwelling catheter time (MD = -0.67, 95%CI: -1.92 ~ 0.58, P = 0.290) and surgical complications incidence (OR=0.62, 95%CI: 0.27 ~ 1.42, P = 0.260) between the two groups. Conclusion Laparoscopic management has the advantages of less traumatic and recovered quickly, and did not increase the incidence of complications. Laparoscopic operation is an ideal procedure for elderly Chinese women with cervical carcinoma.
10.Effects of function-oriented training on the balance of stroke survivors and their ability in the activities of daily living

Objective:To observe any effect of function-oriented training on the balance of recovering stroke survivors and on their ability in the activities of daily living.Methods:Sixty stroke survivors in recovery were randomly divided into an observation group ( n=30) and a control group ( n=30). The control group received routine rehabilitation training, while the observation group underwent function-oriented training for 6 weeks. Before and after the intervention, both groups were evaluated using the Berg balance scale (BBS), the Fugl-Meyer lower extremity assessment (FMA-L), the timed up-and-go test (TUGT) and the modified Barthel index (MBI). Results:After the treatment, significant improvement was observed in all of the measurements in both groups. The improvements in the average BBS, FMA-L and MBI scores and in the TUGT times of the observation group were significantly better than those of the control group.Conclusion:Function-oriented training can improve the balance and the ability in the activities of daily living of stroke survivors more effectively than the routine therapy.
