2.Comparison of disinfection effect of two disinfection methods on X-ray protective clothing

Objective To compare the effect of GammaTM wet wipes and 75% alcohol on the disinfection of X-ray protective clothing,and provide reference for choosing rational disinfection methods.Methods 62 pieces of X-ray protective clothing were selected,30 were from operating rooms,32 from catheterization room.Protective clothing were randomly divided into trial group and control group,each had 31 pieces.Trial group disinfected clothing with GammaTM wet wipes,control group disinfected clothing with 75% alcohol,disinfection effect,drying time,and cost between both groups were compared.Results Average bacterial colony counts in trial group and control group before disinfection were 2.38 CFU/cm2 and 2.36 CFU/cm2 respectively,qualified rates were 80.64% and 83.87%respectively.Bacteria were not detected in both groups after disinfection.The average drying time in trial group was shorter than control group([352.35±9.93]s vs[676.13±10.62]s,t=123.98,P <0.01).The cost of disinfecting X-ray protective clothing in trial group was 29.60% less than control group.Conclusion Effect of GammaTM wet wipes and 75% alcohol on disinfecting X-ray protective clothing are both reliable,compared with 75% alcohol, GammaTM wet wipes are without corrosion,have shorter drying time,and save more cost.
3.Application of task-driven teaching methodology based on action research in cardiopulmonary resuscitation training

Objective To explore the effect of task-driven teaching methodology based on the action research on cardiopulmonary resuscitation training. Methods The 80 nurses recruited in 2011 were assigned in the control, receiving routine training while another 80 nurses recruited in 2012 were in the study group, where task-driven teaching methodology was used based on the action research. The two groups were compared in terms of results of theoretical and operational examinations. Result The study group was significantly better than the control group in the scores of theoretical and operational examinations ( P < 0 . 05 ) . Conclusion The task-driven teaching methodology based on the action research not only increases the examination results, but also improves the effect of teaching.
4.Influencial Factors of Activities of Daily Living for Stroke Patients in Community-based Rehabilitation

ObjectiveTo explore the related factors influencing effect of community-based rehabilitation (CBR) on activities of daily living (ADL) in stroke patients in.Methods202 stroke patients were randomly divided into the CBR group (n=103) and control group (n=99). The patients of the CBR group treated with rehabilitation and following-up including risk factors controlled with drug, rehabilitation, health education and mental treatment, those of the control group only treated by following-up. Before and five months after treatment, patients of two groups were assessed with Barthel Index (BI), clinical never function limitation score, cognitive item of Functional Comprehensive Assessment (FCA). Multielement regression analysis was applied, the ADL score of last assessment was taken as dependent variable, group information, location, smoking, sex, age, course of diseases, education, drinking, sleeping, the first scores of FCA and BI were taken as independent variable.ResultsThe improvement of ADL of stroke patients was related with group information, drinking, course of diseases, the first scores of FCA, BI, and etc.ConclusionEarly CBR can significantly improve the ADL of the stroke patients. Cognitive deficit also has influence on ADL.
5.A ten-consecutive-years study of pathogenic bacteria distribution and drug resistance in children with sepsis

Objective To determine the pathogenic bacteria distribution and drug resistance in children with sepsis. Methods From 2002 to 2011, a ten consecutive years of monitoring of pathogenic bacteria distribution, drug resistance in pediatric patients with sepsis were conducted. Results From 2002 to 2011, 2 493 strains of pathogenic bacteria were detected in 68 419 specimens of blood culture. The positive rate was 3.64%. Among them, 1 913 strains (76.73%) were the gram-positive bacteria (G+bacteria), 562 strains (22.54%) were gram-negative bacteria (G-bacteria) and 18 strains (0.72%) were fungi. From 2002 to 2006, 959 strains (4.73%) of pathogenic bacteria were detected in 20 287 specimens of blood culture. Among them, the G+bacteria was 731 strains (76.23%), G-bacteria was 228 strains (23.77%). From 2007 to 2011, 1 534 strains (4.73%) of pathogenic bacteria were detected in 48 132 specimens of blood culture, G+ bacteria was 1 182 strains (77.05%), G-bacteria was 334 strains (21.77%), fungi was 18 strains (1.17%). The positive rate of blood culture, the pathogenic bacteria distributions of G+, G-, and fungi were signiifcantly different between the ifrst and the second 5 years (P<0.01). The positive rate of G+bacteria tended to increase and the positive rate of G-bacteria tended to decrease. Com-paring between the ifrst and second 5 years, the positive rate of coagulase-negative staphylococci, Klebsiella, Alcaligenes, fungi tended to increas, and the positive rate of Staphylococcus aureus, Enterococcus, Pseudomonas, Salmonella tended to decrease. The distribution of pathogens in different age groups of children with sepsis also were signiifcantly difference (P<0.001). Over the 10 years, G+positive bacteria were highly resistant to penicillin and erythromycin, but not resistant to Vancomycin. ESBLs positive E. coli strains and Klebsiella Trevisan became highly resistant to antibiotics. Salmonella was sensitive to commonly used antibiotics. Conclusions Coagulase negative staphylococcus was the most common pathogenic bacteria in children with sepsis in the last 10 years, multiple pathogenic bacteria also show a growing trend in drug resis-tance.
6.Repetitive variable deceleration with a short interval in labor and neonatal acidosis

Objective To discuss the relationship between repetitive variable deceleration with a short interval (RDSI) in labor and neonatal acidosis.Methods One hundred and seventy-seven electronic fetal heart monitoring (EFM) patterns within one hour preceding delivery in term singleton pregnancies were collected in Peking University Third Hospital between February 2011 to October 2013.Continued EFM were recorded before delivery.Analysis was conducted on general information of both mothers and babies,including pregnant complications,duration of labor,cord and placental factors,nature of amniotic fluid,Apgar score and neonatal cord blood gas.RDSI was defined as that over 50% intervals between two decelerations (the end of the last deceleration to the beginning of the next one) ≤ 60 s which appeared repetitevly in a period of 20 minutes.Independent sample t test,rank sum test,Chi-square test and Fisher's exact test were applied for statistics and receiver operating characteristic curve was obtained from the information of those with RDSI.Results Twenty-four of the 177 women with RDSI were assigned to the study group (24 cases,13.6%) and the rest 153 cases without RDSI to the control group.Background information of women in the two groups was comparable according to the maternal age,gestational weeks at delivery and duration of labor (all P > 0.05).The occurrence of meconium stained amniotic fluid in the study group was higher than that of the control group [16.7% (4/24) vs 5.2% (7/153),x2=5.204,P=0.045],while the pH and base excess value of the neonatal blood gas in the study group were lower [7.20 (7.13-7.28) vs 7.29 (7.25-7.33),Z=-4.490;-6.10 (-4.67 to-9.62) mmol/L vs-3.20 (-4.90 to-1.55) mmol/L,Z=-5.044;P ≤ 0.01] resulting a higher rate of neonatal acidosis [50.0%(12/24) vs 7.8% (12/153),x2=31.456,P < 0.01].No significant difference was found in the incidence of neonatal asphyxia between the two groups.The area under the curve was 0.774 (95%CI:0.579-0.969).Conclusion RDSI in labor might indicate a high risk of neonatal acidosis.
