1.A clinical observation on Yishen-Tiaodu moxibustion combined with bladder training in the treatment of neurogenic bladder after spinal cord injury operation

objective To evaluate effect of the Yishen-Tiaodu moxibustion combined with bladder training to treat neurogenic bladder after spinal cord injury operation.Methods 72 patients met the inclusion criteria of neurogenic bladder were divided into two groups according to random number table: a control group was treated with bladder training, and a treatment group on the basis of the control group was treated withYishen-Tiaodu moxibustion therapy. Both groups were treated for 3 months. The urodynamics as well as the change of urinalysis and kidney function were observed in both groups.Results The original bladder feeling (90.2 ± 6.9 mlvs. 53.1 ± 6.9 ml,t=22.812)、maximum urinary output(302.1 ± 37.8 mlvs. 207.7 ± 40.9 ml, t=10.170), the maximum bladder capacity (399.8 ± 74.1 mlvs. 330.9 ± 50.2 ml,t=4.619) in the treatment group were higher than the control group after treatment (P<0.01). Residual urine volume (82.9 ± 32.9 mlvs. 109.8 ± 51.8 ml,t=2.630) in the treatment group was lower than the control group (P<0.01); the 24h urinary protein (70.8 ± 9.1 mgvs. 96.3 ± 14.9 mg,t=8.763), Urea (5.7 ± 2.1 mmol/Lvs. 9.3 ± 2.9 mmol/L,t=6.033), Scr (100.8 ± 19.6μmol/Lvs. 119.2 ± 19.7μmol/L,t=3.973), BUN (5.2 ± 1.8 mmol/Lvs. 6.7 ± 2.4 mmol/L,t=3.000) in the treatment group were significantly decreased than the control group(P<0.01).Conclusions Yishen-Tiaodu moxibustion combined with bladder training could effectively improve urodynamics for the neurogenic bladder in patients with SCI, and decrease the incidence of urinary tract infections.
2.Rotational DSA assessment of intracranial aneurysms

Objective To compare rotational DSA with routine DSA in the assessment of intracranial aneurysms. Methods Conventional anteroposterior, lateral and oblique views of DSA and rotational DSA were respectively performed in 20 patients suspected of intracranial aneurysms in subarachnoid hemorrhage using DSA (Philips Integris V 3000DSA system). Images were compared for localization of the aneurysm, shape of the aneurysmal neck and vascular branch anatomy. Results Among 20 patients, only 18 intracranial aueurysms were found by convertional DSA and the other two were only by the rotational DSA. Furthermore, conventional DSA examination could not clarify the relationships of 6 (6/18) aneruysms with their parent arteries and with the vascular branch anatomy including the aneurysmal neck could not be clarified in 7 (7/18) patients. However, shape of the aneurysms and visibility of the aneurysmal neck could be clarified after using the rotational DSA, and meanwhile, vascular anatomy can be indentified with the rotational DSA. Conclusion Rotational DSA often allows better visibility of vascular anatomy and aneurysmal neck compared with conventional DSA, offering more important information for operation.
3.Curative effect of laser sub-epithelial keratomileusis on low and midrange myopia in aged patients

Objective To study the characteristic of operation design and analyze curative effect of laser sub-epithelial keratomileusis (LASEK) on low and midrange myopia in aged patients. Method Fiftyeight patients ( 114 eyes) no less than 35 years old were treated with LASEK by monoblepsis. The subjects were examined and analyzed at 3 days, 7 days, 1 month,3 months and 6 months after operation. Results Seventy-one corneas(62.28%) recovered well at 3 days after operation. At 1 month after operation, the acuity of vision of all patients achieved or exceeded corrected vision before operation,and lasted to 6 months after operation. The break up time of 63 eyes were lower than normal at 3 months after operation,but 6 months after operation all of them recovered to normal. The satisfaction of patients raised gradually. Conclusions LASEK is effective for no less than 35 years old myopic patients. Make operation plan refer to monoblepsis and patients' needs can receive more satisfaction.
4.Comparison of disinfection effect of two disinfection methods on X-ray protective clothing

Objective To compare the effect of GammaTM wet wipes and 75% alcohol on the disinfection of X-ray protective clothing,and provide reference for choosing rational disinfection methods.Methods 62 pieces of X-ray protective clothing were selected,30 were from operating rooms,32 from catheterization room.Protective clothing were randomly divided into trial group and control group,each had 31 pieces.Trial group disinfected clothing with GammaTM wet wipes,control group disinfected clothing with 75% alcohol,disinfection effect,drying time,and cost between both groups were compared.Results Average bacterial colony counts in trial group and control group before disinfection were 2.38 CFU/cm2 and 2.36 CFU/cm2 respectively,qualified rates were 80.64% and 83.87%respectively.Bacteria were not detected in both groups after disinfection.The average drying time in trial group was shorter than control group([352.35±9.93]s vs[676.13±10.62]s,t=123.98,P <0.01).The cost of disinfecting X-ray protective clothing in trial group was 29.60% less than control group.Conclusion Effect of GammaTM wet wipes and 75% alcohol on disinfecting X-ray protective clothing are both reliable,compared with 75% alcohol, GammaTM wet wipes are without corrosion,have shorter drying time,and save more cost.
5.Separation and purification of Naomaitong Granules by AB-8 macroporous absorption resin

AIM: To study the purification of Naomaitong Granules(Radix et Rhizoma rhei,Rhizoma chuanxiong,Radix Puerariae lobatae.etc) by macroporous absorption resin. METHODS: Naomaitong Granule was purificated by macroporous absorption resin AB-8.UV spectrothotometry was used to determine the contents of total anthraquinones,total ginsenosides,total alkaloids,and the content of puerarin was determined by HPLC.The technic of purification was optimized according to the content above. RESULTS: The optimized technological conditions consisted of eoncentranon of original sample 120 mg/mL,the diameter and height was in proportion of 1∶10,the ratio of maximum adsorption to resin volume was 1∶6,water elution was 2B multiple of resin volume,8B multiple of resin volume 50% alcohol was the elution. CONCLUSION: AB-8 macroporous absorption resin can be used to purify Naomaitong Granules.
6.Feasibility of reducing artifacts of spine metal implants at 3.0 T MRI

Objective To investigate the feasibility of reducing spine metal artifacts with metal artifacts reduction technique (WARP) at 3.0 T MRI.Methods This study included 15 cervical and 14 lumbar spine cases.The image quality of WARP sequences and conventional sequences were compared (5 score evaluation scale) as well as the signal to noise ratio (SNR) and contrast noise ratio (CNR) of the image artifacts.The scanning time was recorded.Paired-t test and Mann-Whitney test were used respectively to compare the SNR and CNR,and qualitative scoring between the two sequences.P<0.05 was considered to indicate a significant difference.Results The image distortion and blur of the WARP sequences were obviously reduced as compared to the conventional sequences.The SNR and CNR of artifacts of the WARP sequences were lower than that of the conventional sequences (All P<0.05).The image quality scores of WARP sequences in cervical and lumbar spines[4(3 to 5) and 4(3 to 5)] were higher than that of conventional sequences[3(2 to 4),3(2 to 4)](P<0.05).The scanning time of cervical spines in WARP sequence(14 min 9 s) was increased by 64 s (8.2%),and the time of lumbar spines (13 min 41 s) decreased by 9 s (1.1%).Conclusion The WARP sequences at 3.0 T could effectively reduce the artifacts of metallic prosthesis in cervical and lumbar spine without prolonging the scanning time at 3.0 T MRI.

8.Influencial Factors of Activities of Daily Living for Stroke Patients in Community-based Rehabilitation

ObjectiveTo explore the related factors influencing effect of community-based rehabilitation (CBR) on activities of daily living (ADL) in stroke patients in.Methods202 stroke patients were randomly divided into the CBR group (n=103) and control group (n=99). The patients of the CBR group treated with rehabilitation and following-up including risk factors controlled with drug, rehabilitation, health education and mental treatment, those of the control group only treated by following-up. Before and five months after treatment, patients of two groups were assessed with Barthel Index (BI), clinical never function limitation score, cognitive item of Functional Comprehensive Assessment (FCA). Multielement regression analysis was applied, the ADL score of last assessment was taken as dependent variable, group information, location, smoking, sex, age, course of diseases, education, drinking, sleeping, the first scores of FCA and BI were taken as independent variable.ResultsThe improvement of ADL of stroke patients was related with group information, drinking, course of diseases, the first scores of FCA, BI, and etc.ConclusionEarly CBR can significantly improve the ADL of the stroke patients. Cognitive deficit also has influence on ADL.
9.Effect on threshold of triggered angiographic acquisition of renal artery CTA by different iodine flow rate

Objective To evaluate the effect on threshold of triggered angiographic acquisition by different iodine flow rate on renal artery CTA using Smart Prep tracking technique.Methods A total of 420 patients were randomly divided into A-D group with iodine flow rate of 0.90 gI/s,1.11 gI/s,1.20 gI/s,1.48 gI/s on average respectively.And each group were divided 7 subgroups with threshold 140-<150 HU,150-< 160 HU,160-< 170 HU,170 <180 HU,180 <190 HU,190-<200 HU,200-210 HU respectively.The quality of the images were graded from 1 5 by 2 radiologists.The best image quality subgroups were achieved,and the scores were compared among the best image quality subgroups.Results Image quality of renal CTA in each group were best performed with a threshold of 170-< 190 HU,160-< 190 HU,170-< 180 HU,160-< 180 HU respectively.When the threshold were 170-<180 HU,image quality were good with all iodine flow rate groups.There was no significant difference among the scores of the best image quality subgroups (all P>0.05).Conclusion Different iodine flow rate with appropriate threshold can acquire better image quality of renal CTA.
10.Simulation analysis of 9033 cases of second trimester maternal serum screening for Down's syndrome

Objective To reduce the screening positive rate (SPR) and improve clinical efficiency of maternal serum screening for Down's syndrome.Methods Nine thousand and thirty-three cases of second trimester maternal serum screening for Down's syndrome were included from Apr.2013 to Apr.2014 in the present study.The screening results,all basic data and equation curves were analyzed retrospectively.Based on the data from the authors' laboratory,the important adjustment parameters were simulated.Combined with postnatal follow-up results,the quality and clinical performance of second trimester serum screening for Down's syndrome were evaluated.Results The SPR of second trimester serum screening for Down's syndrome was 6.69%(604/9033),the detection rate (DR) was 75%(3/4),and FPR was 6.65%(601/9033).The median multiple of median (MOM) of alpha-fetoprotein (AFP) was low and SPR was high,and MOM of free human chorionic gonadotropin β subunit (free hCGβ) were high and SPR was high,while MOM of unconjugated estriol (uE3) were a little bit low,and SPR was slightly high.Considering these three factors,it is believed that the screening positive rate is high.By the simulation adjustments of MOM value equations (AFP and free hCGβ) and weight correction equation,the SPR reduced to 4.11%(371/9033) after recalculating the risk,FPR declined to 4.07%(368/9033),and no more Down's syndrome fetus were missed compared with postnatal follow-up results.Conclusion Based on a localized setting depending on the local laboratory data,we suggest that the MOM value distributions(AFP,free hCGβ and uE3) and maternal weight should be regularly adjusted since it is a useful way to reduce the false-positive rate and improve clinical efficiency of maternal serum screening for Down's syndrome.
