1.The transcription of interleukin-8 in gastric epithelial cells downregulated by ORF10 of cag Ⅱ of Helicobacter pylori

Objective Helicobacter pylori (H. pylori )contains more than 30 genes and many of them in cag pathogenicity island (cag PAI) composed of cag Ⅰ and cag Ⅱ are involved in eliciting the transcription of interleikin 8 (IL 8) and the induction of IL 8 protein secretion in gastric epithelial cells, although some genes are not involved. This study was designed to observe the effect of ORF10 of H. pylori cag Ⅱ on the transcription of IL 8 in gastric epithelial cells and to test the possibility that certain genes in the cag PAI also downregulate (modulate) the transcription and expression of IL 8 in gastric epithelial cells. Methods Three strains of H. pylori mutants with deletion of ORF10 gene (ORF10, namely 28 1, 28 2, 28 3) and L5F11 cells containing IL 8 reporter gene were constructed. The constructed L5F11 cells were co cultured with the above strains and the luciferase activity (IL 8 transcription) was measured in a scintillation counter and the concentration of IL 8 protein was assayed by enzyme linked immunosorbent assay (ELISA). Results The activities of luciferase induced by 3 strains of H. pylori mutants with deletion of ORF10 gene were much higher than that of the mother strain 26695 [ 28 1:(1.49 ? 0.27)?10 6 cpm vs. (0.67 ? 0.08 )? 10 6 cpm, P
2.The optimal administration time of Qingyi Decoction and clyster with rhubarb in patients with severe acute pancreatitis

Objective To investigate the optimal initial timing of Qingyi Decoction and clyster with rhubarb in patients with severe acute pancreatitis (SAP). Methods Patients with SAP were randomly divided into 2 groups. The group A (n = 34) received clyster with rhubarb and Qingyi Decoction after 12 h of SAP onset and the group B (n = 27) received samel therapy after 72 h of onset of SAP. The serum levels of TNF-α, CRP and APACHE Ⅱ scores, time to abdominal pain cessation, length of hospital stay and medical costs were compared. Results The serum levels of TNF-α and CRP of patients in group A were (265±66)U/ml, (32.1 ±7.1) mg/L, and the score of APACHEⅡ were 6. 3±2.0, time to abdominal pain cessation was (4±2) d, length of hospital stay was (18±5)d, medical costs was (42 000±18 000) yuan; while the corresponding values in the group B were (491±81)U/ml, (43.5±11.0) mg/L, 9.1±1.8, (8±3)d, (34±8)d, (71 000±26 000)yuan, and the difference was statistically significantly (P<0.05). Conclusions Qingyi Decoction and clyster with rhubarb should be given in the early phase of SAP in order to achieve better outcomes.
3.Clinical analysis of 39 cases of chronic pancreatitis with mass

Objective To investigate the clinical characterisitics of chronic panereatitis(CP) with mass.Methods The clinical features,radiologieal and pathologic records of 39 cases of chronic pancreatitis with mass confirmed pathologically between January 2005 to December 2007 were retrospectively reviewed,and compared with 17 surgical pathologically confirmed patients with pancreatic cancer.Results The jaundice was found in 14 of 39 patients with pancreatits and 1 of 17 patients with pancreatic cancer,with significant difference between two groups(χ2=0.111,P=0.045).Elevated serum CEA and CA19-9 were found in 0 and 12 patients with pancreatitis.respectively,and 3 and 11 patients with pancreatic cancer,respectively(P=0.025 and=0.018,respectively).CT examination showed that the atrophy pancreas and the infiltration of surrounding and blood vessels were found in 0 and 5 patients with pancreatitis or 3 and 8 patients with pancreatic cancer,respectively(all P values<0.05).The magnetic resonance cholangiopanereatography (MRCP) examination revealed that dilation of bile and pancreatic ducts,and calculus in pancreas duct were found in 14,2 and 15 patients with pancreatitis,or 11,6 and 2 patients with pancreatic cancer,respectively(all P values <0.05).Pancreatic cancer was confiremed by endoscopic ultrasonography guided fine needle aspiration(EUS-FNA)in 10 of 14 patients with pancreatic cancer,but none in 18 patients with pancreatitis.Conclusions It is difficult to diagnose CP with mass,but it will helpful ifassists diagnosis with clinical features,tumor markers and image examinations,especially with biopsyexamination.
4.Research progress on the relationship between cognitive impairment in diabetes and NF-kappaB and its re-lated factors

Cognitive impairment in diabetes ( CID) is one of the complications of diabetes .The features of mild or moderate cognitive disorder and the decread abilities in memory and studying are the main symptoms of CID .But it′s mechanism is still un-known .NF-κB is the original signal activator , it can activate other signal pathway of cell dysfunction .With the influence of hyperglyce-mia and oxidative stress, the expression of NF-κB is enhanced, that leads to the changes of the expression of NOS , MMP-9, TNF-α, then, these changes lead to the apoptosis of hippocampus neuron cell .This review focuses on NF-κB in order to provide evidences in studying CID by reviewing the relationship between NF-κB and it′s relative factors of NOS, MMP-9, TNF-αand CID.
5.Expression of TREM-1 in mice with acute necrotizing pancreatitis

Objective To detect the expression of triggering receptor expressed on myeloid cells-1 (TREM-1) in mice with acute necrotizing pancreatitis (ANP). Methods Male kunming mice (n = 50) were randomly divided to control group, ANP 24, 48, 72, 96 h group. ANP model was induced by intraperitoneally injection with 20% L-arginine at a dose of 4 mg/g each, 1 h apart. Mice in control group received intraperitoneally injections of same amount of normal saline. Serum amylase, creatinine, and ALT were examined and pathological evaluation of pancreatic tissues was performed. The expression of TREM-1 mRNA in peripheral blood leucocyte was determined by RT-PCR. The expression of TREM-1 protein in pancreatic tissue was detected by immunohistochemistry. Results Serum amylase, creatinine, and ALT in ANP 24 h were (9439 ± 1273)U/L, (84.8 ±75.9) μmol/L, ( 158.1 ± 122. 1 ) U/L, which were significantly higher than those in control group [ ( 2412 ± 297 ) U/L, (29.2 ± 19. 1 ) μmol/L, (41.4 ± 7.9 ) U/L) ]. The pathological scores of the pancreas in ANP group increased corresponding to time. The expressions of TREM-1 mRNA in ANP 24, 48,72, 96 h group were 15.55, 30.36, 15.77, 28.32, and the expressions of TREM-1 mRNA in ANP 48 h group was significantly higher than that in other groups ( P <0.01 ). The expressions of TREM-1 protein in the pancreas did not significantly change corresponding to time. Conclusions TREM-1 may be involved in the development of ANP by triggering other inflammatory factors.

Triggering receptor expressed on myeloid cells-1(TREM)-1,a recently identified molecule,is involved in monocytic activation and inflammatory responses.TREM-1 is a member of natural killer cell receptor family and is expressed on neutrophils,mature monocytes and macrophages,and it can enhance the inflammatory responses mediated by Toll-like receptor-2 and -4,showing a role of inflammation promotion.Reportedly sTREM-1 was detectable in the body fluids in the presence of bacterial or fungal infection; therefore it might be used as a marker for inflammation.In addition,recombinant sTREM-1 may also be a new method for anti-inflammatory therapy.Recently,it has been found that TREM-1 not only promoted the acute inflammatory responses,but also participated in the development of chronic inflammation.This review summarizes the role of TREM-1 in the inflammation-associated dieases.
7.Effect of triptolide combined with gemcitabine on proliferation and apoptosis of pancreatic cancer cells

Objective To observe the effect of triptolide combined with gemcitabine on proliferation and apoptosis of pancreatic cancer cells ,and to analyze the relevant mechanisms .Methods After treated with TPL ,GEM or TPL combined with GEM in vitro , PANC-1 cells proliferation was accessed by MTT assay and the interaction between the two drugs was calculated .Apoptotic mor-phological changes and apoptosis rate of the cells were investigated by Hoechst 33258 staining and flow cytometry ,respectively .The expression of signal transduction and transcription factor 3(STAT3) ,cysteine aspartate specific proteases-3(caspase-3) protein were detected by Western blot analysis .Results TPL ,GEM or TPL combined with GEM could significantly inhibit the prolifera-tion of PANC-1 cells ,and the combination of the two drugs had a synergistic effect .The cells of the TPL group ,GEM group ,as well as the combined group showed typical apoptotic morphological changes .Compared with the TPL group and GEM group ,the number of apoptotic cells of the combined group increased significantly .Compared with the control group ,the cells apoptosis rate of the TPL group ,GEM group and combined group was significantly increased (P< 0 .05) ,and the apoptosis rate of the the combined group was significantly higher than that of the monotherapy group(P<0 .05) .TPL combined with GEM synergistically inhibited p-STAT3 protein expression and activated caspase-3 protein expression .Conclusion TPL combined with GEM can synergistically in-hibit proliferation and induce apoptosis of pancreatic cancer PANC-1 cells ,its mechanism is related to the inhibition of STAT 3 sig-naling pathway ,promotion the expression of caspase-3 protein .
8.BIODEGRADABLE POLY (D,L-LACTIDE-CO-GLYCOLIC ACID) MICROSPHERESUSED AS ADJUVANT IN DNA IMMUNIZATION OF HEPATITIS B VIRUS S GENE

Study the immunological adjuvant function of biodegradable microspheres for DNA immunization. Empty poly(D,L-lactide-co-glycolic acid) microspheres were prepared using the water-inoil-in-water(w-o-w)technique; A plasmid DNA pRc-CMV encoding hepatitis B virus S antigen was constructed;The mixture of the microspheres and the plasmid DNA was prepared by incubation method. The mixture was administered to Balb/c mice by intramuscular injection. Result: The high antibody titer (1:1600) of intramuscular injection of the mixture of microspheres and the plasmid DNA was obtained, similar to that of intramuscular injection of the mixture of AL(OH)3 and hepatitis B virus S antigen; while intramuscular injection the plasmid DNA elicited no serum antibody respones. Conclusion: biodegradable microspheres may be used as an good adjuvant for DNA immunization.

10.Study on the status of catastrophic disease medical assistance under NRCMS in A City at Hubei Province and B County at Guizhou Province

Objective:This paper designs to analyze the status of catastrophic disease medical assistance in A and B regions, and put forward feasible suggestions to improve the medical assistance level and financial effects. Methods:We selected A city at Hubei province and B county at Guizhou province as sample areas, and obtained 872 copies of questionnaires in the field survey. Then, out-of-pocket health expenditure per year, the incidence, and rel-ative gap of catastrophic health expenditure before and after the financial aid were calculated. Results: Catastrophic disease medical assistance plays a limited role in relieving patients' economic burden. Low financial aid rate, narrow range, high deductibles and low ceiling level are responsible for much lower effective financial aid rate. Conclusion:Raise financial aid rate and widen the scope of medical assistance gradually;Establish effective link-up between cata-strophic disease medical assistance and catastrophic disease insurance / basic medical insurance;Put outpatient OOP into the scope of catastrophic disease medical assistance.
