Objective To explore the stooling patterns and colonic motility after transanal onestage pull through operation for Hirschsprung' s disease in children. Methods Follow-up of more than 6 mos was obtained in all 58 children. The stooling pattern was investigated through questionnaire. Barium enema and colonic transit time with the simplified radio opaque markers were performed. Results Frequent stools was found in 4 cases, postoperative soiling in 9, constipation in 5 and Hirschsprungassociated enterocolitis in 3. No patients suffered from postoperative incontinence. Barium enema showed normal shape of the colon in most children, the sigmoid loops decreased or disappeared and was believed to be related to the length of removed colonic segment. Defecography showed the anorectal angle was larger and fixed than that of preoperative and control group[(123?15)?vs. (85?8)?vs. (79?12)?,P 168 h, P 0. 05 ]. TCTT and RSTT in symptomatic group were significantly shorter [ (25?6) h vs. (29?8) h,P

Objective To explore the stooling patterns and sphincter function after transanal one-stage pull through operation for Hirschsprung's disease in children. Methods In this study, 73 children patients averagely aging at 24. 7 months underwent transanal one-stage pull through operation for common type Hirschsprung' s disease. Fifty-eight postoperative patients were followed up for an average of 15. 8 months. Stooling pattern was investigated with the informed questionnaire, according the result patients were divided into symptomatic and nonsymptomatic group. The rectoanal inhibitory reflex ( RIR) and vector manometry including the maximal anal pressure in resting and squeezing, the vector volume ( VV) and vector symmetric index ( VSI) were performed to explore the postoperative stooling patterns and sphincter function. Results Stooling patterns were fine in most patients, postoperative soiling was found in 9 cases, constipation in 5 and Hirschsprung-associated enterocolitis in 3, there was no incontinence. The rectoanal inhibitory reflex recovered in 5 children. The vector manometry showed the maximal anal pressure of the constipated group in resting and squeezing were both higher than those of nonsymptomatic and control group (167?36vs. 157 ?47 vs. 152 ?33,P

Objective · To investigate the clinical effects of photo-activated disinfection (PAD)-assisted non-surgical therapy on the treatment of chronic periodontitis. Methods · Eleven patients with chronic periodontitis (totally 118 selected sites) were randomly enrolled. A split-mouth self-control method was adopted. Selected sites with the probing depth (PD) of bilateral homonymous teeth ≥ 5 mm were randomly assigned to the group A (receiving scaling and root planning, SRP) and the group B (receiving SRP+ PAD). Periodontal clinical indexes, inflammation indexes, and periodontal pathogens at selected sites were examined at baseline and 6 and 12 weeks after treatment. Results · Both SRP alone and SRP+PAD improved the periodontal inflammation significantly. The positive rate of bleeding on probing (BOP) 12 weeks after treatment was remarkably lower in the group B than in the group A (37.3% vs 57.6%, P=0.042). Both SRP and SRP+PAD effectively decreased matrix metalloproteinase-8 (MMP-8) and interleukin-1β (IL-1β) levels in gingival crevicular fluid. The IL-1β levels 6 and 12 weeks after treatment were significantly lower in the group B than in the group A [(17.99±14.26) pg/mL vs (32.64±30.69) pg/mL, P=0.007; (11.37±10.25) pg/mL vs (23.85±15.55) pg/mL, P=0.000]. Both treatments decreased the ratio of P. gingivalis and F.nucleatum in total bacteria. Conclusion · PAD can serve as a potential adjunctive therapy to conventional SRP in the treatment of chronic periodontitis with better improvement of bleeding gums and control of periodontal inflammation.

Objective: To investigate the correlation between admission blood glucose (ABG) level and long-term prognosis in patients with acute coronary syndrome (ACS) without diabetes mellitus (DM). Methods: A total of 585 ACS patients received percutaneous coronary intervention (PCI) in our hospital from 2014-01 to 2015-10 were consecutively enrolled and no one combined with DM. Based on ABG levels, the patients were divided into 2 groups:ABG≥7.0 mmol/L group,n=133 and ABG<7.0 mmol/L group,n=452. Demographic, clinical data and treatment conditions were collected to conduct follow-up study and to assess the prognosis. Results: The median follow-up time was 350 days and all cause death in ABG≥7.0 mmol/L group and ABG<7.0 mmol/L group was 2.4% vs 0.9%,P>0.05. Compared with ABG<7.0 mmol/L group, the patients in ABG≥7.0 mmol/L group had the higher incidences of MACE, PCI complication, recurrence of angina and more re-admissions for cardiac event, all P<0.05.Cox regression mode lanalysis in dicated that in addition to acute STEMI (HR=2.016), malignant arrhythmia (HR=3.028) and previous MI history (HR=2.293), blood glucose≥7.0mmol/L was also an independent risk factor for poor long-term prognosis in relevant patients. Conclusion: ABG≥7.0 mmol/L implies poor long-term prognosis in ACS patients and special attention is necessary in secondary prevention.
