1.Reperfusion Therapy and White Blood Cell Count in Patients with Acute Myocardial Infarction

Objective To study the relationship between blood cell count and cardiac events in acute ST elevation myocardial infarction patients treated with reperfusion in the early stage. Methods In this study, we assigned 151 patients to whom reperfusion therapy had been delivered within 3 hours of STEMI symptom into two groups:primary percutaneous cor-onary intervention group and thrombolysis therapy group. Differences of cardiac events and white blood cell count in these two groups were analyzed in the first 4 days. Results In the second day and the forth day, cardiac events rate and white blood cell count were significantly lower in the primary percutaneous coronary intervention group than those in the thromboly-sis therapy group (P<0.05). In the second day, white blood cell count has a positive correlation with cardiac events rate (r=0.226, P<0.05). Conclusion Primary percutaneous coronary intervention decreased blood cell count and cardiac events rate. In patients with acute myocardial infarction, increasing white blood cell was related to cardiac events in the near future.
2.Correlation between myocardial infarction and angiotensinogen T174M

BACKGROUND: A majority scholars views that polymorphism of angiotensinogen T174M gene is one of the susceptible factors of inheritance of coronary heart disease, hypertension and myocardial infarction.OBJECTIVE: To probe into the relationship between the variation of angiotensinogen T174M gene and myocardial infarction.DESIGN: Case-controlled verified experiment.SETTING: Department of Cell Biology, Biochemistry and Molecular Biology, Biological Science Faculty of North China Coal Medical College,and Department of Cardiology in Affiliated Hospital of North China Coal Medical College.PARTICIPANTS: Fifty-five cases of myocardial infarction were collected from outpatients and inpatients in Department of Cardiac Vascular Internal Medicine of Worker's Hospital Affiliated to North China Coal Medical College in Tangshan from September 2002 to September 2003, of which,29 cases were males and 26 cases females, aged (60±8) years. At the same time, 60 cases (health control) were selected from the people who received clinical physical health check (without repeated physical check), of which,32 cases were males and 28 cases females, aged (60±10) years. The cases selected had no manifestation of coronary heart disease, without history myocardial infarction or cerebral infarction in family and the participants were in the know of the research.INTERVENTIONS: Polymerase chain reaction was used to amplify the genetic sequence of No.174 DAN residue involved in No.2 exon of angiotensinogen gene. Electrophoresis was used after variated with Nco I restriction endonuclease.Analysis of restriction fragment length polymorphism was carried on angiotensinogen genotype. Simultaneously,the relevant risk factors of coronary heart disease were detected in two groups, such as blood pressure, body mass, blood lipid, fasting blood glucose, etc.MAIN OUTCOME MEASURES: ① Distribution of genotype, frequency of genotype and frequency of allele in two groups. ② Analysis on risk factors of two groups.RESULTS: Totally 115 cases of objects all accomplished the design and entered result analysis. ① Frequency of angiotensinogen genotype: in myocardial infarction group, TT 75% (41/55), TM 18% (10/55), MM 7% (4/55) and in control group, TT 83% (50/60), TM 15% (9/60), MM 2% (1/60). Frequency of allele of M174 and T174 were 16% (18/110), 84% (92/110) and 9% (11/120), 91% (109/120) in myocardial infarction group and control group respectively. Frequency of allele of M174 in myocardial infarction group was significantly higher than that of control group (x2=5.79,P < 0.05). By the division of sex, the frequency M and T alleles of both male and female in experiment group was basically identical to control group. Angiotensinogen 174MM genotype in myocardial infarction group was significantly higher than control group (x2= 7.55, P < 0.025). ② Comparison of risk factors: The percentage of smoking history in myocardial infarction group was significantly higher than control group (P = 0.006). After correction of essential risk factors of coronary heart disease, angiotensinogen 174MM gene still increased significantly the risk of myocardial infarction (Odds ratio was 3.66, P= 0.018).CONCLUSION: Angiotensinogen genotype is related to the occurrence of myocardial infarction. M allele is one of the susceptible factors of inheritance of myocardial infarction and T allele prevents from myocardial infarction. The attack of myocardial infarction is not relevant to sex, but angiotensinogen 174TM gene is one of the essential risk factors of myocardial infarction.
3.Application of measurement of lung volumes from MSCT images in evaluation of pulmonary function of patients with chronic obstructive pulmonary disease

Objective To explore the application value of lung volumes from 64 multi-slice CT (MSCT) images in patients with chronic obstructive pulmonary disease (COPD) and study the correlation between lung volumes measured by MSCT and pulmonary function test (PFT) results. Methods 24 patients clinically diagnosed with COPD (COPD group) and 22 healthy people (control group) were selected and underwent both chest MSCT scans and PFT within one week. 4.Effects of insulin-like growth factor-1 on the nerve function and expression of basic fibroblast growth factor after cerebral ischemic reperfusion in rats

Objective To observe dynamically the influence of Insulin-like growth factor-1 (IGF-1) on the nerve function and expression of bFGF protein and Basic fibroblast growth factor(bFGF) mRNA after cerebral ischemic reperfusion in rats.Methods Seventy-two SD rats were randomly divided into sham-operated group,cerebral ischemia group,and IGF-1 treated group.The rat model of middle cerebral artery occlusion (MCAO) and reperfusion was performed.The evaluation of etiology was performed with mNSS at 12 h,24 h,3 d,7 d after ischemia-reperfusion,expression of bFGF protein was determined with immunohistochemical technique and expression of bFGF mRNA was determined with RT-PCR.Results The ratings of mNSS in IGF-1 treated group((8.67± 1.21),(7.50± 1.52),(4.33± 1.03),(3.67± 1.37)) were lower than those in ischemia group((11.0±1.26),(9.83±1.33),(7.83±1.17),(7.17±1.72) at 12 h,24 h,3 d or7 d after reperfusion(P<0.05).For the IGF-1 treated group,the expression level of bFGF protein was higher than that of the cerebral ischemia group (P<0.05),especially at 12 h after reperfusion (P<0.01).The expression level of bFGF mRNA in the IGF-1 treated group was higher than that of the cerebral ischemia group (P< 0.05),especially at 24h after reperfusion (P< 0.01).Conclusion IGF-1 treatment has a protective effects on cerebral ischemia injury,which may contribute to its action on regulating expression of bFGF protein and bFGF mRNA.

Cyprinid herpesvirus 3 (CyHV-3) is the causative agent of an extremely contagious and aggressive disease afflicting common corp Cyprinus carpio L. termed koi herpesvirus disease (KHVD). Since it was first reported in 1997, the virus has spread worldwide rapidly, leading to enormous financial losses in industries based on common carp and koi carp. This review summarizes recent advances in CyHV-3 research on the etiology, epidemiology, pathogenesis, diagnosis, prevention, and control of KHVD.

6.Normal Representations of Permanent Teeth and Alveolar Bone at Multi-slice Spiral CT

Objective To explore the spiral the normal representations of permanent teeth and alveolar bone of multi-slice spiral(MSCT). Methods 30 adults with normal teeth were studied by multislice spiral CT scanner,window width and window level was 3000~5000 HU and 1200~1500 HU respectively.MSCT findings were analysed using multiplanar reformation,dental panorama and 3D reconstruction were done in all cases.Results The tooth were showed as multilaryer structures with different density at MSCT because of the mineral content of different part of tooth,from high to low density was enamel,circumpulpal dentin,outside dentin,cementum and pulp.The periodontal membrane was low density located beween the cementum and lamina dura and the thickness was homogeneous.The bone cortex of alveolar bone was successive,the bone trabecula appeared as wirelike or reticulate.Conclusion The structures of permanent teeth and alveolar bone can be displayed clearly by MSCT.Multiplanar reformation,dental panorama and 3D reconstruction are helpful to display the structures of teeth and alveolar bone as well as the relationship around structures.
7.Outcome analysis of breast diseases screening in 773 women in Tangshan area

Objective To analysis the occurrence status of breast disease in married women in Tangshan area,and to provide basis for early prevention of breast cancer in this region.Methods 773 married women in Tangshan area were given mammography examination.The tissue biopsy or surgical treatment was recommended for patients with above grade 3 of breast imaging reporting and data system (BI-RADS) classification to obtain pathological diagnosis.Results In 773 women,482 cases (62.35 %) were diagnosed as BI-RADS grade 2 or below,256 cases (33.12 %) were BI-RADS grade 3,33 cases (4.27 %) were BI-RADS grade 4,2 cases (0.26 %) were BI-RADS grade 5.The distribution of each BI-RADS grade among different age group was statistically significant (x2 =17.90,P < 0.001),35 cases got pathological diagnosis by biopsy or surgical treatment,including 30 cases of mammary gland hyperplasia,3 cases of breast fibroadenoma,and 2 cases of breast cancer.Conclusion Women's mammography screening is important for the prevention and early detection of mammary gland diseases.
8.Correlation Analysis Between Admission Blood Glucose Level and Long-term Prognosis in Patients With Acute Coronary Syndrome

Objective: To investigate the correlation between admission blood glucose (ABG) level and long-term prognosis in patients with acute coronary syndrome (ACS) without diabetes mellitus (DM). Methods: A total of 585 ACS patients received percutaneous coronary intervention (PCI) in our hospital from 2014-01 to 2015-10 were consecutively enrolled and no one combined with DM. Based on ABG levels, the patients were divided into 2 groups:ABG≥7.0 mmol/L group,n=133 and ABG<7.0 mmol/L group,n=452. Demographic, clinical data and treatment conditions were collected to conduct follow-up study and to assess the prognosis. Results: The median follow-up time was 350 days and all cause death in ABG≥7.0 mmol/L group and ABG<7.0 mmol/L group was 2.4% vs 0.9%,P>0.05. Compared with ABG<7.0 mmol/L group, the patients in ABG≥7.0 mmol/L group had the higher incidences of MACE, PCI complication, recurrence of angina and more re-admissions for cardiac event, all P<0.05.Cox regression mode lanalysis in dicated that in addition to acute STEMI (HR=2.016), malignant arrhythmia (HR=3.028) and previous MI history (HR=2.293), blood glucose≥7.0mmol/L was also an independent risk factor for poor long-term prognosis in relevant patients. Conclusion: ABG≥7.0 mmol/L implies poor long-term prognosis in ACS patients and special attention is necessary in secondary prevention.
9.Silencing IL-23 expression by a small hairpin RNA protects against asthma in mice.

To determine the impact of IL-23 knockdown by RNA interference on the development and severity of ovalbumin (OVA)-induced asthmatic inflammation, and the potential mechanisms in mice, the IL-23-specific RNAi-expressing pSRZsi-IL-23p19 plasmid was constructed and inhaled into OVA-sensitized mice before each challenge, as compared with that of control mice treated with alum or budesonide. Inhalation of the pSRZsi-IL-23p19, significantly reduced the levels of OVA-challenge induced IL-23 in the lung tissues by nearly 75%, determined by RT-PCR. In addition, knockdown of IL-23 expression dramatically reduced the numbers of eosinophils and neutrophils in BALF and mitigated inflammation in the lungs of asthmatic mice. Furthermore, knockdown of IL-23 expression significantly decreased the levels of serum IgE, IL-23, IL-17, and IL-4, but not IFNgamma, and its anti-inflammatory effects were similar to or better than that of treatment with budesonide in asthmatic mice. Our data support the notion that IL-23 and associated Th17 responses contribute to the pathogenic process of bronchial asthma. Knockdown of IL-23 by RNAi effectively inhibits asthmatic inflammation, which is associated with mitigating the production of IL-17 and IL-4 in asthmatic mice.

10.Clinical research in the endoscopic management of digestive fistula

Objective To evaluate the therapeutic value of endoscopic jejunal tube placement, endoscopic clipping, and over the scope clip ( OTSC) for digestive fistula. Methods Data of 38 patients with digestive fistulas at the First Affiliated Hospital of Soochow University admitted from July 2015 to July 2017 were retrospectively analyzed. Treatments were chosen according to the size and the site of the fistulas. Thirteen patients underwent jejunal tube placement ( the jejunal tube group ) , 20 underwent endoscopic clipping( the endoscopic clipping group) , and 5 underwent OTSC( the OTSC group) . The technical success rate, clinical cure rate and postoperative hospital stay were analyzed. Results All patients received the endoscopic operation successfully with no significant complications. In the jejunal tube group, 4 patients′fistulas fully healed, lesion was smaller after treatment in 3 patients, lesion didn′t change in 5 patients, and 1 patient died. The complete cure rate was 30. 8% (4/13), and the postoperative hospital stay was 47. 4± 14. 1 days. For the endoscopic clipping group, 16 patients′ fistulas fully healed, lesion was no smaller compared with that before treatment in 3 cases, and 1 patient died. The complete cure rate was 80. 0% ( 16/20) , and the postoperative hospital stay was 17. 9 ± 8. 9 days. Total patients in the OTSC group were completely cured, with 100. 0%( 5/5) of complete cure rate. One patient with refractory esophageal fistula underwent OTSC repeatedly with endoscopic clipping, and the healing time of fistula was 102 days. The postoperative hospital stay of 4 others was 5. 3±1. 7 days. The cure rate of fistula was higher (P=0. 03, P<0. 001) and the postoperative hospital stay was shorter ( P=0. 04, P<0. 001) in the OTSC group compared with the clipping group and the jejunal tube group. Conclusion Endoscopic management is safe and effective for digestive fistulas with less trauma, easy performance and short time of healing.
