1.Sequence analysis for full length genomes of human enterovirus 71 strains isolated in Linyi, Shandong Province

Objective To isolate enterovirus 71 from a death children,and analyze whether the neurovirulence was related to the variation of nucleotide and amino acid. Methods Enterovirus 71 was isolated from throat swabs which were colleted from Shandong Linyi People's Hospital. The full length genome was sequenced by amplification with RT-PCR and sequencing of 9 overlapped gene fragments covering full length of the genomes. The nucleotide and amino acid sequenced was aligned by BLAST, Bioedit and MEGA 4. Results A strain of enterovirus 71 was isolated and named as SDLY107. The full length was 7405 bp. The results of homology analysis of overall nucleotide sequence showed that strain Fuyang. Anhui. P. R. C/17.08/2 had highest homology (98.6%)with strain SDLY107, and the homology was 80.0% between strain SDLY107 with prototype strain BrCr/70,and 86. 5% between strain SDLY107 with nerve strain MS/87. Phylogenetic analysis showed that the phylogeny was close between SDLY107 with some isolated strains from Chinese Mainland, such as Beijing, Henan, Guangxi, Sbenzhen, Lanzhou, Fuyang, Chongqing and Zhejiang strains, which was clustered for C4 subtype. The results of amino acid sequence analysis showed that there were 2 mutations, E947D and K1873R, for strain SDLY107. Conclusion SDLY107 belonged to C4 subtype, amino acid mutations E947D and K1873R of which may be relevant to the pathogenicity of EV71.
2.Changes of serum concentration of amino-terminal propeptide of procollagen type Ⅲ(P Ⅲ NP)of post-burn hypertrophic scar at excessive stages

Objective To explore the level and the significance of amino-terminal propeptide of procollagen type Ⅲ(P Ⅲ NP)in the serum of hypertrophic scar patients at excessive stages.Methods The serum samples of 74 inpatients admmitted in Affiliated Fosham Hospital of Sun Yat-sen University from August 2007 to August 2009 suffering from long-persisting post-burn hypertrophic scar at various stages were collected.Serum samples of 12 were normal persons were also collected and used as control group.Hypertrophic scar group were divided into 4 subgroups according to the phase of scar and the serum concentrations of P Ⅲ NP were detected using the sensitive RIA method.Results The level of PⅢ NP increases gradually during the process of immature hypertrophic scar to mature scar before it peaking in 4 to 6 months scar subgroup.The concentration of PⅢ NP degreased gradually with the maturation of hypertrophic scar.Conclusions The high concentration of PⅢNP can reflect the high level of the synthesis of type Ⅲ collagen in the tissue of post-burn hypertrophic scar.The level of P Ⅲ NP is synonymous with the ongoing process of hypertrophic scar.PⅢ NP may be a satisfactory marker in discerning the growth and development of post-burn hypertrophic scar.
3.A clinical study on composite transplantation of meshed split-thickness autograft and heterologous dermal matrix.

OBJECTIVETo investigate the feasibility of the composite transplantation of 1:3 meshed split-thickness autograft and acellular heterologous (porcine) dermal matrix.
METHODS9 inpatients with full thickness skin burn or hypertrophic scar were selected in this study. After the eschar or scar was excised, the wound was covered with acellular heterologous dermal matrix. Then the meshed (1:3) split-thickness autologous skin sheet was grafted on the dermal matrix. Before dressing up, the radiated pigskin was placed on the composite transplants.
RESULTSThe composite transplantation was successfully used in 9 cases. The meshed split-thickness autograft was expanded 3 times and covered the dermal matrix tightly. The clinical results of the composite transplantation were similar to that of intermediate split thickness skin graft or full thickness skin graft.
CONCLUSIONThe composite transplantation of meshed (1:3) split-thickness autograft and acellular heterologous (porcine) dermal matrix allowed the expansion of the autologous skin sheet to 3 times. The clinical results were similar to that of intermediate split thickness skin graft or full thickness skin graft.

4.Application of electro-acupuncture-anesthesia device in brachiplex nerve blocking.

OBJECTIVETo investigate the effect of electro-acupuncture-anesthesia device (EAAD) as an auxiliary localizing method for brachiplex nerve blocking.
METHODSOne hundred and twenty patients scheduled for operation in upper limbs were randomly divided into group A and group B, 60 in each group. Bupivacaine 25 ml (0.375% in concentration) was used to block intermuscular plexus brachialis in the operation for anesthesia and EAAD was used for adjunctive localizing in group A, while in group B paresthesia elicitation was used.
RESULTSIn group A, the blocking was complete in 56 cases and incomplete in 4, the anesthesia effect was superior to that in group B, complete in 46, incomplete in 9, and of inefficient in 5 (P < 0.01).
CONCLUSIONEAAD provides a localizing method for plexus brachialis blocking more precise than paresthesia elicitation.

5.Progress analysis of pancreatic jejunal anastomosis in laparoscopic pancreaticoduodenectomy techniques

There are many methods of pancreaticojejunostomy in laparoscopic pancreaticoduodenectomy. It is suggested that the formation of "sinus healing" by pancreaticojejunostomy benefits pancreatic juice extravasation and intestinal fluid reflux. The healing of fibrous layer is based on mechanical connection, and there are various forms of connection. Our team summarized the experience since laparoscopic pancreaticoduodenectomy, using the improved "double needle pancreaticoenterostomy" and combined with "variable diameter measurable drainage catheter" to support the pancreatic duct to form "sinus healing" at the pancreaticointestinal anastomosis.The main techniques and experiences are summarized as follows.
6.A retrospective comparative study on optional timing of removal of abdominal drains after laparoscopic pancreaticoduodenectomy based on the enhanced recovery after surgery concept versus conventional practice

Objective:To study the optional timing of removal of abdominal drains after laparoscopic pancreaticoduodenectomy (LPD) based on the enhanced recovery after surgery (ERAS) concept versus conventional practice.Methods:The clinical data of patients who underwent LPD at the Second Hospital of Hebei Medical University and the First Hospital of Hebei Medical University from January 2020 to June 2021 were retrospectively analyzed. Of 127 patients included in this study, there were 74 males and 53 females, with age of (58.68±8.65) years old. Then patients were divided into two groups according to the timing of removal of abdominal drains based on the ERAS concept (the ERAS group, n=61), and conventional clinical practice (the control group, n=66). The abdominal drains in the ERAS group was removed based on 2 criteria: (1) no discharge of bile, gastrointestinal contents, pus, and active bleeding in the abdominal drains on the first day after operation; (2) amylase in abdominal drainage fluid was less than 5 000 U/L on the first day after operation. The abdominal drains in the control group was removed after meeting the following criteria: (1) no discharge of bile, gastrointestinal contents, pus, and active bleeding in the abdominal drains; (2) from the first day after operation, amylase levels in the drain fluid was measured once everyday, and the concentrations of the amylase were less than 5 000 U/L for 2 consecutive days; (3) the volume of drainage was less than 100 ml/24 h. The postoperative recovery and other clinical data of the two groups were also compared. Results:LPD was successfully performed in the 2 groups, and there was no perioperative death. The timing of removal of abdominal drains [1 vs. 7(5, 9) d], the first passage of flatus [3(2, 4) vs. 3(3, 5) d] and the postoperative hospital stay [14(10, 18) vs. 17(14, 22) d] in the ERAS group were significantly shorter than the control group, and the hospitalization cost was also significantly less [10.33(9.64, 11.52) vs. 11.22(10.38, 13.58) wan yuan] (all P<0.05). Conclusion:The ERAS concept in guiding the timing of removal of abdominal drains after LPD was safe and feasible. The enhanced recovery after surgery concept is worthy of further promotion and application.
7.Clinical and genetic analysis of IL11RA-related Crouzon-like syndrome

Objective:To investigate the clinical manifestation, genetic characteristics, treatment and prognosis of Crouzon-like syndrome.Methods:Clinical data of one case of Crouzon-like syndrome diagnosed in Children′s Hospital Affiliated to Zhengzhou University in May 2019 were collected, including clinical test, treatment plan, follow-up outcomes. The clinical characteristics and the mutation characteristics of IL11RA-related Crouzon-like syndrome were analyzed combined with the literature.Results:The male proband, five years and four months old, was admitted with the main clinical manifestations including headache, vomiting, exophthalmos, ocular hypertelorism, nasal root flat and scaphocephaly. CT showed that the cerebellar tonsil moved down slightly, the occipital magnum was full, the bilateral cranial plates were locally thinner, the bilateral cranial diameters were increased, and the cranial seams were closed. Magnetic resonance imaging showed ChiariⅠmalformation. The mutation c.40_63del and splice site mutation c.811-2A>G of the patient′s IL11RA gene were screened by whole exome sequencing. Sanger sequencing showed that the mutations are compound heterozygous and both are first reported. The mutation c.811-2A>G was derived from the patient′s mother, and the other one is de novo.Conclusions:The main clinical manifestations of Crouzon-like syndrome are craniosynostosis and midface hypoplasia and ocular deformity. The study identified two novel mutations in the Crouzon-like syndrome related IL11RA gene. Genetic sequencing is helpful for accurate diagnosis and timely surgical treatment.
8.Association between ABO blood group and acute myocardial infarction.

OBJECTIVETo explore the association between the ABO blood group and the risk of myocardial infarction in Chinese people.
METHODSWe retrospectively recruited 1 988 consecutive patients with acute myocardial infarction (AMI) and 1 856 non-coronary artery disease (non-CAD) subjects who hospitalized in our hospital between January 2013 and December 2013. The clinical features and ABO blood group were analyzed.
RESULTSBlood group distribution was A (27.1%, 539/1 988), B (34.4%, 684/1 988), AB (10.8%, 215/1 988), O (27.7%, 551/1 988) in patients with AMI and A (26.7%, 496/1 856), B(32.2%, 598/1 856), AB(10.8%, 200/1 856), O (30.4%, 564/1 856) in non-CAD group. The single factor analysis showed that blood group O tended to be more common in the non-CAD group than in AMI group (P = 0.06). After adjustment for common cardiovascular risk factors such as age, gender, hypertension, diabetes, smoking and serum cholesterol level, the A, B, and AB blood groups were associated with increased risk of AMI compared with O blood group, and the difference was significant with A blood group (OR = 1.229, 95% CI 1.019-1.482, P = 0.031) and B blood groups (OR = 1.214, 95% CI 1.017-1.449, P = 0.032). In addition, non-O blood group remained significantly associated with the increased risk of AMI than O blood group after logistic regression analysis (OR = 1.223, 95% CI 1.048-1.426, P = 0.01).
CONCLUSIONOur results suggest that non-O blood group is associated with the increased risk of AMI.

9.Comprehensive Proteomic Profiling of Patients' Tears Identifies Potential Biomarkers for the Traumatic Vegetative State.

The vegetative state is a complex condition with unclear mechanisms and limited diagnostic, prognostic, and therapeutic methods. In this study, we aimed to explore the proteomic profile of tears from patients in a traumatic vegetative state and identify potential diagnostic markers using tears-a body fluid that can be collected non-invasively. Using iTRAQ quantitative proteomic technology, in the discovery phase, tear samples collected from 16 patients in a traumatic vegetative state and 16 normal individuals were analyzed. Among 1080 identified tear proteins, 57 were upregulated and 15 were downregulated in the patients compared to the controls. Bioinformatics analysis revealed that the differentially-expressed proteins were mainly involved in the wound response and immune response signaling pathways. Furthermore, we verified the levels of 7 differentially-expressed proteins in tears from 50 traumatic vegetative state patients and 50 normal controls (including the samples used in the discovery phase) using ELISA. The results showed that this 7-protein panel had a high discrimination ability for traumatic vegetative state (area under the curve = 0.999). In summary, the altered tear proteomic profile identified in this study provides a basis for potential tear protein markers for diagnosis and prognosis of the traumatic vegetative state and also provides novel insights into the mechanisms of traumatic vegetative state.
