1.Analysis of the therapeutic effects of orthodontic therapy with mandibular first molar extraction

Objective:To evaluate the therapeutic effects of orthodontic therapy with mandibular first molar extraction.Methods:77 cases of malocclusion were divided into 3 groups:Bilateral mandibular first premolar extraction group(A,30 cases),unilateral man-dibular first molar extraction group(B,27cases)and bilateral mandibular first molar extraction group(C,20 cases).The OGS indexes were measured before and after orthodontic treatment and statistically analysed.Results:Remarkable occlusal improvement was seen in all three groups after treatment(P<0.01 ),but the variation of OGS indexes had no significant difference among the 3 groups(P>0.05).OGS indexes among the three groups after treatment were significantly different(P<0.05).The improvement in group A was more significant than that in group C(P<0.01)and the improvement in group C was more significant than that in group B(P<0.01). Conclusion:Orthodontic therapy with bilateral mandibular first molar extraction is effective in orthodontic treatment of malocclusion, but is not as effective as that with bilateral mandibular first premolar extraction.

3.Eccrine spiradenocarcinoma: a case report

A 70-year-old female patient presented with several cutaneous masses on the left neck and trunk for 40 years. Some masses were removed by surgical operation more than 10 years prior to the presentation, but recurred subsequently along with an increase in lesion number. Persistent dull pain emerged at the lesion sites 2 months prior to the presentation and the masses on the left neck ulcerated 10 days prior to the presentation. Histopathology showed tumor cell clumps in the dermis with benign eccrine spiradenoma components in the centre region and carcinomatous components in the periphery. The carcinomatous components included slightly atypical cells and hyaline degeneration. Immunohistochemically, carcinoembryonic antigen and epithelial membrane antigen were observed in the tumor tissues. The case was diagnosed as eccrine spiradenocarcinoma. The masses were surgically removed, but recurred 1 month later, increased in size and number and ulcerated 3 months later, and the patient died 6 months after the surgery.
4.Laparescopic surgical technique training program in urology

Objective To validate a new standardized training program for urological surgeons to improve their laparoscopic surgical skills. Methods The laparoscopic surgical training program was carried out by using the traditional mechanical simulators and animal models. Thirty-three trainees participated the urological laparoscopic surgical training program. Initially, the novices were assigned to practise basic laparoscopic skills step by step on the simulator with fixed trocar positions. After a period of basic training, they were allowed to practise on animal models for some particular proce-dures. Results All trainees (33/33, 100.0%) participated were able to perform all basic techniques skillfully and completed laparoscopic anastomosis accurately after the training. The time required for performing the partial nephrectomy, dismembered pyeloplasty and ureteral reimplantation on animal models declined from 64.0±18.4, 127.54±17.5 and 75.84±11.6 min at the beginning to 30.94±3.8, 65.2±7. 5 and 37.7±7.2 min after practicing these procedures 8 times (P<0.01). They could un-derstand the crucial procedures of the laparoseopic surgeries after 6 to 8 special trainings on animal models. Fifteen trainees (15/33, 45.5%) had started to carry out laparoscopic surgeries after-finish- ing the training program. Conclusions Our program enables the participants to improve their techniques in complicated laparoscopic surgeries. The challenging parts of reconstructive laparoscopic surgeries such as laparoscopic dismembered pyeloplasty can be taught by using animal models. This program could be incorpo-rated easily by all urological departments developing a laparoscopie surgical training program.
5.Expression in Escherichia coli, purification and enzymatic properties of porcine urate oxidase.

The aims of this research were to construct prokaryotic expression vector containing the gene of porcine urate oxidase (pUOX), optimize the conditions of the expression of pUOX in recombinant Escherichia coli BL21(DE3), and analyze the in vitro activity and the enzymological properties of pUOX. The pUOX gene was amplified by RT-PCR from the extracted total RNA of porcine liver, and was inserted into the prokaryotic expression vector pET30a(+) to construct a recombinant expression vector pET30a(+)/pUOX. We identified the recombinant vector by endonuclease digestion and sequence analysis. The pUOX gene was amplified and cloned into the vector pET30a(+) successfully. And then the recombinant vector was transformed into E. coli BL21(DE3). The expression of pUOX with a molecular of approximately 41 kD was induced by IPTG. We also optimized the expression conditions of the recombinant protein. The recombinant protein was mostly located in the cytoplasm and it was insoluble. After the inclusion body was solved in 8 mol/L urea and refolding in 2 mol/L urea, the recombinant protein was collected and purified by Ni2+-NTA column. This recombinant protein had a specific activity of 50.61 IU/mg and showed similar properties of optimum temperature and thermal stability, base on the enzymatic assay and analysis of enzymological properties. These results would help to analyze the in vivo activity by testing animal.

6.Effect of Notch1 on biological behavior of bladder cancer cells

Objective To investigate the in vitro effects of bladder cancer cell proliferation after silencing Notch1 gene. Methods The siRNA eukaryotic expression vector of Notch1 (psiRNA1)was constructed and transfected into bladder cancer cell lines T24 and BIU-87. Methabensthiazuron (MTT) and flow cytometry (FCM) assays were used to detect bladder cancer cells line growth, cell cycle and apoptosis after the transfection. RT-PCR and Western blotting were used to determine the expression changes of Notch1 in these cell lines. Results After transfection for 72 h, the rate of G0/G1 phase cells inceased from (23.89±1.32) % to (80.13±2.69)% in T24 cell line, and increased from (24.63±1.68)% to (69.44±2.41)% in BIU-87 cell line (both P<0.05). In addition, apop-totic cell index in T24 and BIU-87 cell lines increased from (1.28±0.14)% to (13.75±1.23)%, from (1.01±0.27)% to (8.72±1.01)%, respectively(both P<0.05). The growth of T24 and BIU-87 cell lines was obviously inhibited 24 h after the transfection, and the inhibitory effects lasted until 96 h after the transfection. Notch1 mRNA and protein significantly downregulated after transfection compared to the control(P<0.05). Conclusions Silencing Notch1 expression can inhibit the prolif-eration of bladder cancer cell lines. Notch1 gene might act as a tumor gene in bladder cancer.
7.Sebaceoma:a clinicopathological analysis of 31 cases

Objective To analyze clinicopathologic features of sebaceoma. Methods Clinical, pathologic and immunohistochemical findings from 31 cases of sebaceoma were retrospectively analyzed. The clinicopathologic features of sebaceoma were investigated. Results There were 9 males and 22 females. The patients′ age was 53.90 ± 15.40 years, and the clinical course was 9.41 ± 13.75 years. Sebaceoma predominantly affected the face. The common lesion of sebaceoma was red, yellowish?red, skin?colored or slight brown papules, with no subjective symptoms in most cases. Histopathologically, neoplasms had symmetric structures, and were located in the dermis. Epidermal involvements were found in 9 cases. The neoplasm cells were mainly composed of basaloid cells, a few mature sebocytes and some transition cells. The proportion of mature sebocyts was less than 1%in 26 cases, less than 20%in 2 cases, and 20%-40%in 3 cases. Mitoses were occasionally found in 5 cases. One patient was complicated by eccrine poroma. Varying amounts of ducts were found in all the patients. Immunohistochemical staining showed that epithelial membrane antigen was expressed on ducts and mature sebocytes in all the patients, while epithelial antigen was undetected in any of the patients. Carcinoembryonic antigen, androgen receptor and D2?40 were found in 20, 24 and 28 patients with sebaceoma, respectively. Conclusions The diagnosis of sebaceoma mainly depends on histopathological examination. Combined immunohistochemical detection of epithelial membrane antigen, androgen receptor and D2?40 is beneficial to its differential diagnosis.
8.Association of polymorphisms in aldosterone synthase and 11 beta-hydroxylase genes with the risk of primary aldosteronism

Objective To determine the association of mutations in aldosterone synthase (CYPllB2)and 11 beta-hydroxylase(CYP11B1)genes with primary aldosteronism(PA).Methods Five mutations of CYP11B2 and CYP11B1 genes were analyzed in patients with PA and normal population.Among them,intron 2 was detected by 2 independent PCR reactions,and the others were analyzed using Taqman probes.The Haploview 4.0,SNPassoc 1.5-3 and Haplo.stats 1.3.8 were used to analyse the association between polymorphisms and PA.Results All the selected mutations were successfully genetyped.Only rs64lO allelic frequencies in patients with aldosterone-producing adenoma (APA)and idiopathic hyperaldosteronism(IHA)were significantly different with those in controls (P<0.05).There was a relative excess of AA homozygotes and AG heterozygotes of rs6410 allele in APA group compared with control group(P<0.01).There were significantly different genotypes AA and AG of rs6410 allele between patients with IHA and controls only after adjusted for age,gender,eeptible haplotype AAAWT was identified to be significantly associated with APA(OR=1.44,95%CI 1.19-1.76).Three susceptible haplotypes AAAWT,AGGWT and AGAWC were identified to be significantly associated with IHA(OR=1.55,95%CI 1.23-1.96;OR=1.49,95%CI 1.17-1.89;OR=1.40,95%CI 1.04-1.88).In contrast,1 protective haplotype GGAWT showed significant difference between patients with APA and controls(OR=0.73,95%CI 0.55-0.97).Conclusion There is a significant association between genetic variations in CYP11B2 and CYP11B1 genes and genetie predisposition to PA.
9.Construction of EZH2 gene site-directed knock-in Hut78 cells by CRISPR/Cas9 system

Objective To construct the Hut78 cell line with EZH2 gene knocked into by CRISPR/Cas9 system. Methods The EZH2 expression vector pMD-18T-EZH2 with homologous arm and the sgRNA expression vector pSpCas9 (BB)-2A-Puro-sgRNA, which could cut the double stranded genomic DNA, were constructed, and the two vectors were co-transfected into Hut78 cells. Then the expression of EZH2 mRNA was detected by qPCR, and the expressions of EZH2 and H3K27me3 proteins were detected by Western blot assay. Results The pMD-18T-EZH2 and pSpCas9(BB)-2A-Puro-sgRNA recombinant vectors were confirmed by DNA sequencing. When Hut78 cells were transfected with the two recombinant plasmid, qPCR results showed that the expression of EZH2 mRNA was significantly increased, and Western blot analysis showed that the expressions of EZH2 and H3K27me3 proteins were significantly increased. Conclusion EZH2 gene is successfully knocked into Hut78 cells by CRISPR/Cas9 system.
10.Clinical efficacy of combined use of No ha la hu and Ni ru ha in the treatment of incomplete intestinal obstruction

Objective To investigate the clinical effect of No ha la hu(Mongolian medicine abdominal massage)combined with Ni ru ha(Mongolian medicine enema)in the treatment of incomplete intestinal obstruction.Methods A total of 80 patients with incomplete intestinal obstruction admitted to the department of emergency and critical care medicine of Inner Mongolia International Mongolian Hospital from April 2020 to April 2022 were selected as the study objects,they were randomly divided into control group and observation group,40 cases in each group.The control group was treated with routine modern medicine,and the observation group was treated with mongolian medicine No ha la hu combined with Ni ru ha on the basis of routine treatment.Specific methods:the patient supine position,both lower limbs flexion,natural calm breathing,the doctor stands on the patient's right side,applies a little butter to the palm of his hand,rub hands,massages the abdomen with the middle finger and ring finger at acupoints such as ileocecal point,small intestine point,small intestine point,small intestine point and lower clockwise direction point.Massage the abdomen with the palm for about 20 minutes,once a day for 3 days.No ha la hu was given after the treatment of Ni ru ha(Rhubarb 250 g,Cold water stone 150 g,Sanai 100 g,Facial alkali 100 g,Hyacinth 50 g,Terminalia chebula 50 g),the patients should lie on the left side,raise the buttocks about 10 cm,insert the enema into the rectum 15-20 cm,take the Mongolian medicine Liuweian powder 12 g and warm water 50-100 mL for enema,once a day for 3 days.The difference of relieving time of abdominal pain and distension,time of exhaustion,time of fasting,and time of hospital stay between the two groups were compared,and the clinical effect was observed.Results Relieving time of abdominal pain,abdominal distension,time of exhaust and time of hospital stay in the observation group were significantly shorter than those in the control group[abdominal pain relief time(hours):5.3±1.8 vs.8.1±1.6,abdominal distension relief time(hours):3.2±1.5 vs.6.5±1.9,time of exhaust(days):2.6±1.7 vs.4.1±2.1,hospital stay(days):8.3±2.9 vs.10.1±2.1,all P<0.05],time of fasting in the observation group were shorter than those in the control group(days:2.9±0.5 vs.3.1±0.6,the difference was not statistically significant(all P>0.05).The effective rate of the observation group was significantly higher than that of the control group[92.5%(37/40)vs.75.0%(30/40),P<0.05].Conclusion On the basis of routine treatment of western medicine,the treatment of incomplete intestinal obstruction by Mongolian medicine No ha la hu combined with Ni ru ha is more effective,which can effectively restore gastrointestinal function,relieve abdominal pain and other symptoms,and shorten the time of exhaust.
