Chinese Journal of Radiology 2011;45(10):942-946
ObjectiveTo investigate the clinical and MRI features of multiple sclerosis in children, including the clinically isolated syndrome (CIS) and relapse.MethodsIn total,16 cases of pediatric multiple sclerosis were included in this study.Of them,11 patients were female and 5 were male, with the mean onset age of 10.1 years. They were followed up for 4 months to 7 years and found to have 1-5 relapses. The clinical manifestations of CIS and relapse were analyzed by a pediatric neurologist. An experienced neuroradiologist reviewed the MRI images of CIS and relapse.Information on the location, size, and pattern of the lesions was gathered. The location of lesions included subcortical, central, and periventricular white matter, cortex, deep gray matter, brain stem, and cerebellum.ResultsCIS episode presented acute onsets in 13/16 cases, with symptoms of cortice in 10 cases and visual impairment in 6 cases.Relapse occurred in 14/16 cases within one year. The incidence of symptoms of cortice was less frequent and severe in the second episode of MS,whereas the visual impairment had a high incidence. All patients had full recovery after the last episode.MRI of CIS showed confluent subcortical white matter lesions in 13/16 cases, abutting on central white matter lesions. The most frequently involved brain part was the frontal lobe, followed by the parietal lobe.Cortical involvement was observed in 9/16 cases. In 6 cases, periventricular white matter lesions were detected.Bilateral deep gray matter was abnormal in 4 cases.Other abnormalities included brain stem lesions in 5 cases, cerebellum lesions in 3 cases, optic nerve involvement in 3 cases, and pyramidal tract lesions in 2 cases.MRI of relapse revealed more small lesions in the subcortial and periventricular white matter in the patients. In the second episode, only 2 cases presented cortical involvement.Lesions were found in the brain stem in 4 cases and in the cerebellum in 5 cases.Pyramidal tract involvement was identified in 6 cases, serving as “railway track sign”.ConclusionsCIS in children is characterized by large confluent lesions in subcortical white matter coalescing with central white matter.MRI of relapse showed distinctive “railway track sign”.It is important to integrate MRI and clinical features in the diagnosis of pediatric multiple sclerosis.

DMD/BMD is a X-linked recessive hereditary disease. It predominantly affects males. While female carriers do not have symptoms, due to their inactive X chromosome make it present mosaic.Recently, more and more papers reported that a clinically significant proportion of DMD/BMD female carriers have symptoms. They presented variable degrees of symptoms. But the mechanism of the pathogencity is still not clear. Most of the research considered that the dominating reason is the skewed X inactivation. It means that the predominant expression of the DMD mutant allele make the normal one have weak expression, thus no function dystrophin proteins could be generate, manifested as DMD/BMD.In this paper,we mainly summarized the relationship between skewed X inactivation and pathogenicity of the symptomatic DMD female carriers.

Objective To investigate the effects of long-term low-dose radiation (LDR) of γ-rays on the proliferation and radiosensitivity of human lymphoblast cells HMy2.CIR (HMy) and to elucidate the underlying mechanism. Methods HMy cells were divided into control group and long-term LDR group. For the long-term LDR treatment,HMy cells were fractionally exposed to a low dose of γ-rays,which could enhance cell proliferation,3 times per week for 4 weeks. After the long-term LDR exposure, part of the control and long-term LDR exposed cells were further irradiated with a challenging dose (2 Gy) of γ-rays.Then cell proliferation and radiosensitivity were assayed by CCK-8 kit, cell apoptosis, and γ-H2AX formation was measured by flow cytometry.Gene expressions of cyclinD1,PCNA,bcl-2 and bax were detected by RT-PCR.Results The long-term LDR significantly increased cell proliferation (t =9.607,P < 0.01) accompanied with up-regulation of cell cycle regulation gene cyclinD1 (t =6.869,P < 0.01),proliferation regulation gene PCNA (proliferating cell nuclear antigen) (t =9.229,P < 0.01) and bcl-2 gene (t =2.662,P < 0.05),but decreased the expression of pro-apoptotic gene bax (t =19.908,P <0.01) in HMy cells.Compared to untreated cells, the long-term LDR decreased cell radiosensitivity (t =8.896,P < 0.01),including apoptosis induction (t =4.762,P < 0.01) and γ-H2AX formation (t =10.264,P<0.01).Conclusions The long-term LDR promoted cell proliferation by up-regulating cell cycle related genes, while it reduced the radiosensitivity of HMy cells with acquisition of apoptotic resistance.
