1.The MRI study of correlation between patella location and chondromalacia

ObjectiveTo discuss the relationship between patella location and chondromalacia patella,explore its mechanism and clinical significance.Methods Knee joint MRI was performed in 1052 patients (506 men and 546 female).Among them,there were 299 patients( 100 men and 199 female)with chondromalacia patella.They were divide into the group of 1-19,the group of 20-39,the group of 40-59 years old and the group of older than 60 years to compute the positive rate respectively.Insall-Salvati method was used to measure the length of chondromalacia ligaments(L) and the length of chondromalacia path(P).The relationship between patella location and chondromalacia was tested by using t test and x2 test.ResultsThe total positive rate of chondromalacia patella was 28.4% (299/1052).In female it was 36.4% (199/546) and in men was 19.8% (100/506).The group of 1-19 years old had 16 patients ( 16.8%,16/95).The group of 20-39 years old had 71 patients( 17.9%,71/396).The group of 40-59 years old had 116 patients ( 33.2%,116/349 ).The group of older than 60 years had 96 patients (45.3%,96/212).The positive rate of chondromalacia patella increased with age.The L/P value of normal group and chondromalacia patella group were 1.15 ±0.15 and 1.24 ±0.17 respectively.The L/P value of normal group of men and women were 1.13 ± 0.15 and 1.17 ± 0.14 respectively.The L/P value of chondromalacia patella group of men and women were 1.20 ±0.17 and 1.26 ±0.16 respectively.The InsallSalvati index of age groups showed significant differences.The L/P value of pathological changes group and normal group of 1-19 years old were 1.38 ± 0.25 and 1.24 ± 0.16 respectively.The L/P value of pathological changes group and normal group of 20-39 years old were 1.24 ± 0.17 and 1.15 ± 0.16 respectively.The L/P value of pathological changes group and normal group of 40-59 years old were 1.24 ±0.16 and 1.12 ±0.12 respectively.The L/p value of pathological changes group and normal group of older than 60 years were 1.21 ±0.16 and 1.12 ±0.12 respectively.All of them had significant differences (P < 0.01 ).ConclusionsThe positive rate of chondromalacia patella for female is higher than that for men,which also increases with age.High patella location is relevant to chondromalacia patella.
2.Relationship between Temperature and Change of Serum Electrolytes in Patients with Spinal Cord Injury

Objective To explore the relationship between temperature and change of serum electrolytes (potassium, natrium, chlorine,and calcium) in patients with spinal cord injury. Methods 321 patients with spinal cord injury (SCI) following fever were regarded as observation group, while 313 SCI patients in normal temperature as control group. The level of serum electrolytes (potassium, natrium, chlorine, and calcium) in both groups was analyzed. Results There was no significant difference in the level of serum potassium (P>0.05), but was in the serum natrium, chlorine, and calcium (P<0.05) between 2 groups. The level of serum potassium didn't correlated with their temperature, and the level of serum natrium, chlorine, and calcium correlated with the temperature negatively. Conclusion The level of serum natrium, chlorine, and calcium may decrease in SCI patients following fever while the level of serum natrium is unchanged.
3.Influence mechanism and intervention strategies of psychological resilience in patients with cerebral infarction

Objective To analyze the effect of self efficiency,social support and coping style on the psychological resilience.Methods A total of 386 patients with cerebral infarction in a grade 3A hospital of Tangshan City were served as the subjects.The resilience scale,self efficiency scale,social support scale and medical coping questionnaire were taken as the survey tools.The Pearson correlation analysis,multiple linear regression model and structure equation model(SEM) were applied.Results The resilience score was (59.72 ±12.17) points,the resilience was positively correlated with the self efficiency,social support,facing coping style and avoidance coping style(P<0.01),and negatively correlated with the surrender coping style(P<0.01).The total efficiency coefficients of self efficiency sensation,subjective support,facing coping style and surrender coping style for affecting the resilience were 0.720,0.264,0.121 and-0.092 respectively.Conclusion Self efficiency,social support and coping style have significant influences on the psychological resilience.
4.Effect of Herbal Tanreqing-injection Combined with Vancomycin in Treatment of MRSA Pneumonia and Antibiotic Resistance of the pathogens

OBJECTIVE To investigate(meticillin-resistant Staphylococcus aureus MRSA) the antibiotics resistance and curative effect of herbal Tanreqing-injection combined with vancomycin in the treatment of MRSA pneumonia.METHODS All data of microbiology about sputum culture between 2001-2007 were statistically analyzed with a retrospective method in a hospital,in order to understand the MRSA prevalence trends and their resistance.From 100 cases of MRSA pneumonia,50 cases were treated by vancomycin alone,another 50 cases were treated by Tanreqing injection plus vancomycin.RESULTS There were 3082 MRSA strains and 162(meticillin-sensitive S.aureus MSSA) strains in 3244 S.aureus strains;nosocomial infection cases accounted for 78.07% in MRSA-positive sputum samples;MRSA wasn't resistant to vancomycin,but resistant to other antibiotics.Resistance rates had a significant difference between MRSA and MSSA(P
5.Factors Related with Length of Stay for Inpatient Rehabilitation after Stroke

Objective To investigate the trend of length of stay (LOS) and identify variables that predict LOS for inpatient rehabilitation after stroke. Methods A total of 3049 inpatients who completed the stroke rehabilitation program at Beijing Bo'ai Hospital from January 2002 to December 2011 were reviewed and analyzed with nonparametric rank sum test and binary Logistic regression analysis. Results The median of LOS was 82 days, and tended to decrease obviously in 2010 and 2011. On Logistic regression analysis, there were 13 variables associated with LOS, in which operation, muscle spasm, gain of activities of daily living (ADL), onset admission interval, nosocomial infection,complications, speech disorder, the type of stroke, the number of hospitalization, methods of payment were the risk factors, and age,functional ambulation classificateion and Fugl-Meyer Assessment were protect factors. Conclusion Reducing LOS must base on rehabilitation efficacy, and the improvement of hospital management process, the prevention of complications and nosocomial infection control, and early rehabilitation intervention can reduce the LOS.
6.Study of supervised aerobic exercise on physical activity, quality of life and psychological status in patients with systemic lupus erythematosus

Objective To evaluate the efficacy of medical supervised aerobic exercise on physical activity, quality of life and psychological status in patients with systemic lupus erythematosus (SLE). Methods SLE patients who fulfilled ACR criteria were recruited and divided into 2 groups: exercise group (n=24) and control group (n=25). The patients in the exercise group were supervised to have aerobic exercises. The intensiveness of exercise was determined by 20%-40% of maximum heart rate reservation. Visual analog scale (VAS), SLE disease activity index (SLEDAI) score, physical working capacity (PWC170), SF-36 and profile of mood states (POMS) of the two groups were used to evaluate the changes at the baseline, 1 month, 3 months and 6 months after this study. Results The 2 groups were homogeneous and comparable in disease activity at baseline. 1, 3 and 6 months after the study, the VAS, PWC170, POMS and SF -36 of SLE patients were improved in certain degrees in both groups, while the improvement of VAS (P<0.05), PWCITO (P< 0.01 ) and social function of SF-36 (P<0.05) of exercise group were significantly more evident than those of the control group in 6 months after study without any impact on disease condition. There was a high negative correlation between VAS and 5 categories of POMS (r=-6.26~-0.393, P<0.01 ) and a more relevant positive association between VAS and 2 categories of POMS (r=0.534~0.611, P<0.01). Conclusion The data demonstrate that the supervised aerobic exercise can ameliorate physical capacity, improve quality of life and psychological and emotional status in the state SLE patients without aggravating disease per se.
7.Publications of general practice education in China mainland 1993-2012: a bibliographic review

Objective To review the publication of general practice education in China mainland in 20 years from 1993 to 2012.Methods The research articles of general practice education published between January 1,1993 and December 31,2012 were searched from CNKI and Wanfang databases, and analyzed with bibliometric and literature management software NoteExpress 2, Epidata 3.1 and SPSS 13.0.Results Total 4 281 articles were retrieved and 241 education relevant articles were selected.29.9% (72/241) of the 241 articles were on on-campus education, 67.6% (163/241) were on continuing medical education and 4.5% (11/241) were on post-graduation education.As to the published institutions, 58.5% (141/241) were from the medical colleges and research institutions, 25.7% (62/241 were from the tertiary hospitals, 3.7% (9/241) were from primary care institutions, 1.7% (4/241 were from the secondary hospitals and 5.8% (14/241) were from the government departments.The general practice education grants increased from 1 in 2001 to 46 in 2012;20 of which were supported by provincial and ministerial level funds.Conclusion The contents of general medical education literature in China is mainly on on-campus education and continuing medical education;most of which were from the tertiary hospitals and the medical colleges and research institutions.The amount of research grants of general practice education were increasing year by year during 1993-2012, indicating that more attention had been paid to general practice education in mainland China in the last two decades.
8.Lung fibrosis model made by repeated low - dose of paraquat administered intraperitoneally in mice

Objective To study the differences between the animal model of pulmonary injury/ fibrosis induced by using paraquat and that induced by using bleomycin in mice in order to establish an ideal mouse pulmonary fibrosis model.Methods Thirty healthy and 8 ~ 10 weeks old male C57BL/6J (C57) mice were randomly (random number) divided into paraquat group (n =10),bleomycin group (n =10),and control group (n =10).Paraquat ( 10 mg/kg) was given to mice intraperitoneally once every three days for 5 times in paraquat group.Bleomycin was injected into trachea of mice in a dose of 3 mg /kg in bleomycin group.The mice were sacrificed 7 days,14 days and 21 days after administration of drug.The general physical condition,body weight and pulmonary pathological changes were observed.Data were analyzed with SPSS13.0 statistical package.The comparison was made between two groups with mann -whitney U- test.Results Both agents could induce pulmonary injury and fibrosis.After comparison of survival rate,body weight,pulmonary histopathological change and rate of successful modelling,the repeated low - dose of paraquat injected intraperitoneally was proved to be a method of more simple and effective with high success rate of modeling in comparison with the conventional technique of intratracheal injection of bleomycin.Conclusions By the comparison between two methods of establishing pulmonary injury and fibrosis models in mice,the method of repeated low - dose intraperitoneal injection of paraquat is superior over the bleomycin - induced method in respect of higher rate of successful modelling.
9.Study on the influencing factors of quality of life in children with congenital anorectal malformation

Objective To analyse the influencing factors of quality of life (QOL)among postoperative children with congenital anorectal malformation (ARM), so as to provide evidence for developing comprehensive interventions and improve QOL of children with ARM.Methods The QOL of 100 children aged at 2 and 5 years old were evaluated by using the Chinese version of PedsQLTM4.0 scale, and the total and 4 dimensions scores of QOL were calculated.Univariate and multiple stepwise regression model was applied to analyse the influencing factors of the QOL.Results The results of univariate analysis among ARM children aged at 2 years old showed that the clinical classification, bowel disorders and family residence have different influence on QOL scores of the 4 dimensions and total scale (P < 0.05).The results of univariate analysis among ARM children aged at 5 years old showed that the clinical classification, bowel disorders and maternal education had different influence on QOL scores of the 4 dimensions and total scale (P < 0.05).The results of multiple stepwise regression model among ARM children aged at 2 years old showed that the major influence factor of QOL scores in physical function was family residence and bowel disorders(P < 0.05), the major influence factor of QOL scores in social functioning was bowel disorders(P < 0.05), the major influence factor of the total scores was bowel disorders and family residence(P <0.05).The results of multiple stepwise regression model among ARM children aged at 5 years old showed that the major influence factor of QOL scores in physical function was maternal occupation and clinical classification(P < 0.05), the major influence factor of QOL scores in emotional functioning was bowel disorders(P < 0.05) ,the major influence factor of QOL scores in social functioning was clinical classification and bowel disorders (P < 0.05), the major influence factor of QOL scores in school performance was maternal occupation(P < 0.05), the major influence factor of total scores was bowel disorders and clinical classification(P < 0.05).Conclusion Family residence and bowel disorders have effct on QOL of ARM children aged at 2 years old.Maternal occupation, clinical classification and bowel disorders have effct on QOL of ARM children aged at 5 years old.We should take positive interventions on these influencing factors to improve QOL of children with ARM.
10.Application and research of acupuncture in military.

Acupuncture has remarkable effects of pain relieving and functional restoration on injuries of soft tissue and joint due to military training. As more and more attention has been attached to the impact of psychological states and biorhythm disorder on the fighting ability of military staff, acupuncture has found its place in treating chronic fatigue, combat stress reaction, traumatic brain injury and post-traumatic stress disorder as well as regulating circadian rhythms. The therapeutic effect of acupuncture in military training-related physical damage and psychological trauma has already been proved by numerous clinical practices and researches. It is held that using acupuncture as an alternative could not only save medical resources, but also enhance the fighting ability of the army. However, the current clinical studies is facing the problem of limited sample size. Therefore, randomized controlled trials in large scale and multiple centers should be further carried out toward military staff, so as to provide more speaking evidences to the prevention and treatment of physical and psychological diseases.
