1.Analysis of position of one-piece soft intraocular lens after implantation of intraocular lens with Pentacam

Background The aspheric intraocular lenses(IOLs)can reduce ocular spherical aberration to some degree.However,the clinical effect depends more on the IOL proper alignment.It becomes more important to study the IOL position in eye,Objective This study was to analyze the position alteration of IOL after phacoemulsification combined with implantation of one-piece soft IOLs.Methods In this prospective control study,80 eyes of 40 patients with age-related cataract were enrolled.The phacoemulsification with IOL implantation was performed in all the eyes.Decentration and tilt of IOL in the nasal superior,superior temporal,inferior temporal and nasal inferior quadrants(the intersection point of the system optical axis and the IOL maximum cross plane were regarded as the ordinate origin)were measured by rotating Scheimpflug camera(Pentacam Oculus)in 3 months postoperatively under the mydriasis condition.Written informed consent was obtained from each subject prior to this trial.Results In the right eye group,the IOL decentered toward temporal in 26 eyes(65%)and infratemporal in 16 eyes(40%).IOLs tilted temporally in the horizontal plane in 37 eyes(92.5%)and tilted inferiorly in the vertical plane in 34 eyes(85.0%).In the left eye group,IOLs decentered temporally 33 eyes(82.5%)and 20 IOLs (50%)infratemporally,IOLs tilted temporally in the horizontal plane in 37 eye(92.5%)and 36 IOLs(90%)tilted inferiorly in the vertical plane.There was no statistical difference for the intercomparsion of horizontal/vertical decentration in various quadrant in the right eye(F =0.221,0.792,P>0.05).The obvious elevated horizontal decentration was found in the supertemporal and infratemporal quadrants compared with supernasal quadrant in the left eyes but there was no significant difference in the vertical decentration among 3 quadrants(F=0.576,P>0.05).Decentrations were positively correlated with the tilt in both horizontal and vertical plane(right eye horizontal plane:r=0.374,P=0.002;right eye vertical plane:r=0.402,P=0.001 ;left eye horizontal plane:r=0.377,P=0.002;left eye vertical plane:r=0.347,P=0.002).Conclusions The one-piece soft IOLs(Adapt AO)decenter toward temporal mostly in 3 months after surgery,especially infratemporally in the eye.And the optical axis of the IOL tilt toward infratemperol mostly in both right and left eyes.The decentration and tilt are consisted in the corresponding direction between the right and left eyes.The position of the IOLs showed mirror symmetry between right and left eyes.The IOLs decentration show the positively correlation to tilt whatever in horizontal and vertical plane.
2.Fluorescent carbon dots and the application in biomedicine.

As a new type of carbon nanomaterials, fluorescent carbon dots (fluorescent CDs) have many advantages when compared with the traditional fluorescent probes. They are photoluminescence stable and resistance to photo bleaching. Moreover, they are excellent in biocompatibility, low-toxic and easy to modify. All these above make them a promising optical image material as a probe in optical image. This article reviews structure, the common carbon sources, the preparation methods, and the light-emitting principles of the carbon dots. We also introduce the research progress of fluorescent carbon dots in biomedicine, and the problems need to be resolved in the study of fluorescent CDs.

3.The treatment of acoustic neuroma with microsurgery by retrosigmoid approach

Objective To investigate the operative methods and clinical significance of the treatment for large acoustic ncuroma with microsurgery by retrosigmoid approach.Methods 15 cases of large acoustic neuroma treated with microsurgery by retrosigmoid approach were systematic analyzed,including the operative approach,microsurgical technique,disposal after operation,prevention and cure of complications.Results Tumors were totally removed in 12 cases and were subtotally removed in 3 cases.Facial nerve was kept ana- tomic intact in 13 cases(86.7%) and acoustic nerve was kept anatomic intact in 6 cases(40.0%).The short period complications happened in 3 cases and no patient died in this series.Conclusion Treatment for large acoustic neuroma with microsurgery by retrosigmoid approach is a safe method,which give small hurt brain tissue and benefit to increase the total removal rate and protect effectively the function of facial nerve and acoustic nerve.
4.miR-143 suppresses the proliferation and migration of SGC7901 gastric cancer cells

Objective:To investigate the role and mechanism of miR-143 in the proliferation and migration of gastric cancer (GC) cells. Methods:Western blot was performed to detect the expression level of avian erythroblastosis oncogene B-3 (ERBB3) in GC tissues, paired non-cancerous tissues, and SGC7901 GC cells. RT-qPCR was conducted to determine the mRNA and miR-143 of ERBB3 quantita-tively. Bioinformatics tools were used to predict the target gene of miR-143. Luciferase reporter assay was carried out to confirm the predicted target gene. Transwell and EdU assays were applied to observe the migration and proliferation of SGC7901 GC cells transfect-ed with miR-143 mimics/inhibitor/NC mimics/inhibitor. Results:Compared with the expression levels of ERBB3 and miR-143 in the paired non-cancerous tissues, the expression level of ERBB3 was upregulated and the expression level of miR-143 was downregulated in GC tissues. In the prediction of the potential target gene, miR-143 could bind to a specific sequence of the 3′-untranslated regions (UTR) of the mRNA of ERBB3. This finding was supported by luciferase reporter assay results. In vitro, ERBB3 protein expression and cell migration and proliferation were suppressed significantly in the SGC7901 cells transfected with miR-143 mimics. By contrast, these processes were remarkably enhanced when the cells were transfected with miR-143 inhibitor. Conclusion:miR-143 can suppress the migration and proliferation of GC cells by downregulating the expression of ERBB3.
5.Diagnosis and treatment of serious pediatric airway obstruction.

OBJECTIVETo investigate the diagnosis and treatment of serious pediatric airway obstruction.
METHODSSeventy three pediatric cases with serious airway obstruction, admitted to Shantou hospital from 1995 to 2005, were analyzed retrospectively.
RESULTSAmong the 73 cases, 28 cases (38.4%) with inflammatory disease and 33 cases (45.2%) with foreign body. In addition, 8 cases of laryngeal papilloma, 3 cases of laryngotracheobronchial spasm and 1 case of pharyngeal dysembryoma were also included in this study. The patients who had endotracheal intubation or tracheotomy were 39 cases (53.4%) and 27 cases (36.9%) respectively. Ten cases received non-surgery treatment. Seventy two cases diagnosis was confirmed and the related serious airway obstruction condition got stable within 12 hours. While serious complications occurred in 23 cases (31.5%) and operative complications occurred in 3 cases (4.1%). Four cases ( 5.5%) died and 68 cases (93.2%) were cured. One case refused to be treated.
CONCLUSIONSThe condition of serious pediatric airway obstruction patients is usually critical, the related causes could be complicated, quick diagnosis and timely removal of the obstruction factors are imperative.

6.Expressions and correlation with radiotherapy's effect of five kinds of genes in nasopharyngeal carcinoma.

OBJECTIVETo investigate the expressions of E-cadherin, CD44H, matrix metalloproteinase-3 (MMP-3), nm23H1 and vascular endothelial growth factor (VEGF) in nasopharyngeal carcinoma and its relationship to the effect of radiotherapy. Furthermore, to analyze the predict value of radiotherapy effect.
METHODSThe expressions of E-cadherin, CD44H, MMP-3, nm23H1 and VEGF in 62 patients with nasopharyngeal carcinoma were determined by immunohistochemical SP method. There were 62 patients (17 to 70 years old) with nasopharyngeal carcinoma which were treated by radiotherapy from March 1995 to October 1995 and the period of follow-up had full 10 years.
RESULTSThe expressions of CD44H (chi2 = 18.739, P = 0.028) and VEGF (chi2 = 18.523, P = 0.030) were closely related with short-term effect after radiotherapy in nasopharyngeal carcinoma. The short-term effect was descent along with enhancement of their expressions. In the group of low expressions in CD44H and nm23H1, 3-year overall survival rate were 65.5% and 45.5%, and 5-year overall survival rate were 47.3% and 22.7%. In the group of high expressions in CD44H and nm23H1, 3-year overall survival rate were 54.6% and 75.9%, and 5-year overall survival rate were 27.8% and 53.2%. There were respectively significant difference between two group of expressions in CD44H (chi2 = 7.31, P = 0.0069) and nm23H1 (chi2 = 15.64, P = 0.0001).
CONCLUSIONSThese findings indicated that to detect the expressions of CD44H and VEGF gene may predict short-term effect of radiotherapy. Furthermore, to detect the expressions of CD44H and nm23H1 gene may predict long-term effect of radiotherapy in nasopharyngeal carcinoma.

7.Clinical apllication of indocyanine green fluorescence imaging in patients with hepatocellular carcinoma

Objective To explore the clinical application of indocyanine green (ICG) fluorescence imaging in patients with hepatocellular carcinoma.Methods 65 hepatocellular carcinoma patients,all were with single lesion,were divided into two groups:preoperative ICG administration group (n =28) and intraoperative ICG administration group (n =37).In preoperative ICG administration group ICG was given 2 to 14 days before surgery,while in the intraoperative ICG administration group it was given during surgery.ICG mediated near-infrared imaging system was used to observe the tumor and determine surgical margin.Results In the preoperative group,tumors were bright fluorescent imaging,in 6 cases 10 new small lesions were found,with cancer diagnosed in 4 patients,cirrhotic liver nodule in 5 patients,fatty degeneration in 1 patient.In the intraoperative group,tumors were clearly demarcated shadow,12 new small lesions were found in 8 cases,with cancer being diagnosed in 5 patients,cirrhotic liver nodule in 4 patients,fatty degeneration in 2 patients,hemangioma in 1 patient.Conclusion Both methods can find small lesions,preoperative administration group was better in identifying the nature of the tumor,while the intraoperative administration group was more valuable in determining the boundaries of the tumor.
8.Window partial laryngectomy for the treatment of stage T2-3 glottic laryngeal carcinoma

Objective To investigate the operative effect and value of window partial laryngectomy for the treatment of stage T2-3 glottic laryngeal carcinoma.Methods From Oct.2000 to Dec.2006,the stage T2-3 glottie laryngeal carcinoma of 48 patients by properly selected were performed with window partial laryngectomy and laryngeal reconstruction.Twenty nine males and 19 females were included.Their ages ranged from 43 to 78 years(median 57.0 years).Before operation and in the 6 months after operation,these targets including auditory mental evaluation of hoarse degree,active degree and symmetry of vocal cord,glottie width in the time of quiet breathing,glottie closing degree in the time of phonating,respiratory function and swallowing function were surveyed.In addition,the things of recurrence and metastasis of tumors together with survival time of patients were following investigated. Results All patients were decannulated successfully and incisions were healed smoothly.No operative complication occurred.Except active degree of vocal cord(P=0.343),there were respectively significant difference between two group targets of auditory mental evaluation of hoarse degree(all P<0.01),symmetry of vocal cord(P=0.000),glottic width in the time of quiet breathing(P=0.001),glottie closing degree in the time of phonating(P=0.001)and respiratory function(P=0.001)those were investigated before operation and after operation.The swallowing function wasn't influeneed(P=0.310).There were laryngostenosis in 1 case,recurrence in 1 case(2.1%),cervical lymph node metastasis in 1 ease and hepatic metastasis in 1 case.Two cases died.3-year and 5-year overall survival rate were respectively 96.9% and 88.9%.Conclusions The study showed that window partial laryngectomy wag successful for treating stage 32-3 glottic laryngeal carcinoma by properly selected.This operation was effective for reducing surgical invasion and beneficial to resume respiratory and vocal function.

Diabetes,a chronic metabolic disease with hyperglycemia,has a high incidence in the modern time that put many patient families into troubles. At present,insulin or oral hypoglycemic agents are used to treat diabetes,though several side effects such as hypoglycemia and various complications mainly caused by these drugs. Antibody with high selectivity and high therapeutic index is employed in many disease treatments. In this paper,the recent study of different antibodies against certain targets of T cells,B lymphocytes,glucagon receptors and vascular endothelial growth factor are involved,which could supply new insight into the treatment of diabetes.
10.Association between metformin therapeutic efficacy and SLC47A1 polymorphism in systemic lupus erythematosus

Objective:To evaluate the association between the efficacy and safety of metformin and the influence of variants in SLC47A1 rs2289669 G>A polymorphism in the treatment of systemic lupus erythematosus (SLE).Methods:A multicenter, randomized, double-blind, placebo-controlled trial was conducted. Patients were consented at enrollment for blood donation for genotyping, and their peripheral blood were used to detect the distribution frequency of SLC47A1 mutations. The major or mild/moderate flares defined by modified safety lupus erythematosus national assessment (SELENA)-systemic lupus erythematosus disease activity index (SLEDAI) Flare Index (SFI) and adverse events were recorded at 12 months of follow-up. The correlation between efficacy/safety and genotype was analyzed. Student's t test and χ2 test was used to assess the continuous variables and categorical variables. Results:Between May 24, 2016, and Dec 13, 2017, a total of 31 patients in the metformin group and 35 in the placebo group were detected. There were no statistical significant differences in the clinical manifestations, SELENA-SLEDAI scores, and therapy of the participants at baseline. There was no significant difference in the frequency of AA genotype, GA genotype, and GG genotype of SLC47A1 rs2289669 distribution between the metformin group and the placebo group. In the metformin group, patients who flared had a lower frequency of A alleles than those non-flared [25%(4/16) vs 61%(28/46), χ2=6.116, P=0.019 8]; the flare rate was significantly lower in patients with AA genotype than in GG genotype [0%(0/8) vs 57%(4/7), χ2=6.234, P=0.012 5]. The infection rate was lower in the metformin group than that in the placebo group [38%(12/31) vs 69%(24/35), χ2=5.913, P=0.015 0], but there was no significant difference among different genotypes in the metformin group. Compared to GG geno-type, AA genotype showed a trend of decrease in infection rate[38%(3/8) vs 72%(5/7), χ2=1.727, P=0.188 8]. Conclusion:Metformin has a favorable safety profile and may reduce the frequency of flares in SLE patients with low-grade lupus disease activity. The metformin therapeutic efficacy in SLE is relevant to the SLC47A1 gene polymorphism. Patients of the AA genotype may benefit most from metformin than those of the GG and GA genotypes.
