2.Effect of HIF-1αon proliferation of pathological scar fibroblasts and its mechanism

Objective To explore the effect of hypoxia-inducible factor 1α(HIF-1α)in pathological scar and its specific mechanism,and the therapy target of pathological scar.Methods Real time PCR and Western blot was used to test the expression of HIF-1αin normal tissue and pathological scar,meanwhile to detect the effect of hypoxic environment on the expression of HIF-1α.Fibroblasts activity in pathological scar and normal tissue under different oxygen concentration(20%,10%,5%,2% and 1%)was determined by MTT method.Determine the different expression of HIF-1αmRNA and protein in normal environment(20% oxygen) and hypoxic environment(5%oxygen).The changes of the number of fibroblasts after the silence of HIF-1αby HIF-1αshRNA virus using flow cytometer.Results The Real time PCR and Western blot results showed that the expressions of HIF-1αmRNA and protein in fibroblasts of pathological scar was significantly higher than that in normal tissue(P<0.05),and the activity of the keloid fibroblasts in hypoxia environment(5%,2% and 1%)was also higher than that in normal tissue(P<0.05).The expressions of HIF-1αmRNA and protein in keloid fiber cells were higher in hypoxia environment(5% oxygen)than those in normal environment(20% oxygen),but when silence the expression of HIF-1αin keloid fibroblasts,the apoptosis of fibroblast in hypoxia environment(5%oxygen)were significantly higher than control group[(0.021 ±0.001)%vs.(3.739 ± 0.039)%,P<0.05].Conclusion HIF-1αincreases significantly in the pathological keloid fibroblasts and could promote the proliferation of fibroblasts and its vitality,which could accelerate the formation of pathological scar.
3.Clinical effect of Kang Shuling gel on the inflammatory factors in Ⅱ degree burns

Objective To investigate the effect of Kang Shueling gel on the degree of inflammation in the patients with Ⅱ degree burns. Methods 86 patients with Ⅱ degree burns from July 2014 to July 2016 in our hospital were divided into the control group and the experimental group by lottery method with 43 cases in each group. The control group was treated with silver sulfadiazine, the experimental group was treated with Kang Shuling gel, the two groups of clinical curative efficacy, inflammatory factors, visual analogue scale (VAS), wound healing rate and wound healing time, wound infection rate and adverse drug reactions were compared. Results After treatment, the total effective rate of the experimental group was 95.34% higher than that of the control group 72.09%, the difference was statistically significant (P<0.05). The tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6) levels of the experimental group was lower than the control group, the difference was statistically significant (P<0.05). The VAS of the experimental group was lower than the control group, the difference was statistically significant (P<0.05).The wound healing rate of the experimental group was higher than that of the control group on the seventh day and the fourteenth day, and the wound healing time was better than that of the control group, the difference was statistically significant (P<0.05). The wound infection rate of the experimental group was lower than that of the control group (P<0.05). There was no significant difference in adverse drug reactions between the two groups. Conclusion The curative effect of Kang Shuling gel on the treatment of Ⅱ degree burns patients worthy of affirmation, could reduce serum inflammatory factors level.
4.Correlation analysis of homocysteine and umbilical arterial with color doppler ultrasound with preeclampsia

Objective To analyze the relationship between the levels of plasma total homocysteine (Hcy) and umbilical arterial with color doppler ultrasound with the severity of preeclampsia(PE).Methods A total of 70 cases of the third trimester of normal pregnancy, 72 cases of mild PE, and 66 cases of severe PE were selected.Then plasma Hcy levels and umbilical arterial with color doppler ultrasound were detected.Results Hcy levels were statistically significant difference among the normal control group,mild PE group,and severe PE group((8.950±0.585), (11.116±0.615), (14.648±0.620) μmol/L,P<0.05).Umbilical artery blood flow parameter S/D values in severe PE group was significantly higher in mild PE group and in contrg.1 group (3.43 ±0.72 vs.2.86 ± 0.82 vs 2.75 ± 0.56, P< 0.05), while the control group, mild PE group had no significant difference(P>0.05).Resistance index(RI) ,Pulsatility index(PI) in mild PE group and severe PE group were significantly higher than control group(0.60±0.05,0.78±0.07 vs.0.57±0.06;1.24±0.21,1.47±0.64 vs.0.67±0.35), and the differences among three groups were significant(P<0.05), while the mild PE, severe PE group had no significant difference(P>0.05).Hcy of mild PE group was positively correlated with S/D, RI and PI(r=0.548,P=0.009;r=0.587,P=0.008;r=0.324,P=0.02).Hcy of severe PE group was positively correlated with S/D,RI and PI(r=0.752,P=0.001;r=0.627,P=0.003;r =0.438,P=0.024).Hcy and the severity of PE was positively correlated (r =0.450, P < 0.05), S/D and the severity of PE was positively correlated(r=0.316, P =0.002).RI, PI and the severity of PE was no correlation (r =0.024, P =0.726;r =0.054,P =0.649).Conclusion The levels of Hcy and S/D were related to the severity of PE.To reduce Hcy,and monitor S/D were new ways for the diagnosis and treatment of pre-eclampsia.
5.Significance of human papilloma virus subtype detection in oppor-tunistic screening for cervical cancer in Uygur and Han women

Objective:This study investigates the significance of human papilloma virus (HPV) subtype detection in opportunis-tic screening for cervical cancer in Uygur and Han women. Methods:Flow-through hybridization gene chip and thin-prep cytology test were used to detect HPV in cervical cell samples from 1140 females. A total of 428 patients had undergone cervical biopsies through colposcopy. The diagnostic results of the HPV subtype test for cervical lesions were evaluated on the basis of histology. Results:Total HPV infection rate for the 1140 cervical samples was 30.3%. The most common HPV subtypes were HPV16, HPV58, HPV52, HPV18, and HPV45. HPV52 infection rate was higher in Han women than in Uygur women, with statistically significant differences between the two (χ2=8.737, P=0.003). Among these cervical samples, the single HPV infection rate was 22.4%(255/1140), whereas the multiple HPV infection rate was 6.1% (69/1140). The sensitivity and specificity of the HPV subtype test for cervical lesions were 86.4% and 24.5%, respectively. The positive and negative predictive values were 58.5%and 59.3%. Conclusion:HPV infection subtypes in Ugyur and Han women have unique characteristics. Subtype detection is important in opportunistic screening for cervical cancer.
6.Posterior pedicle subtraction osteotomy at the apical vertebra with transpedicular instrumentation in the treatment of rigid angular kyphosis of thoracolumbar spine:a medium-term curative effects

BACKGROUND: During spinal orthopedic repair, the main difficulty is to maximize the correction of the deformity, simultaneously, to reduce the incidence of trauma and complications, especial y to avoid the corresponding spinal nerve injury. OBJECTIVE: To evaluate the curative effect of one-stage posterior pedicle subtraction osteotomy of apical vertebra with transpedicular instrumentation in the treatment of rigid angular kyphosis of thoracolumbar spine. METHODS: We retrospectively analyzed the data of 42 cases of rigid angular kyphosis of thoracolumbar spine that were treated by one-stage posterior pedicle subtraction osteotomy of apical vertebra with transpedicular instrumentation. Al patients received detailed imaging examination before and after operation. Kyphosis angle, Frankel grading and perioperative complications were recorded in al patients. X-ray films or CT films showed the bone graft fusion during fol ow-up. RESULTS AND CONCLUSION: Patients were fol owed up for 10-36 months after treatment. The average time of bone graft fusion was 5.1 months. Cobb’s angle of kyphosis was corrected from 78.4° (38°-110°) preoperatively to 7°(-8°-24°) at 10 days after treatment. The correction rate was 90%. The average angle during final fol ow-up was 7.9°, with an average loss of 0.9°. In 16 paresis patients, Frankel grading results showed grade B in 0 case, grade C in 3 cases, grade D in 5 cases, and grade E in 8 cases during final fol ow-up, showing significant improvement as compared with that pre-treatment (P < 0.05). Among 42 patients, 5 cases had complications. At 7 months after treatment, there were screw and titanium rod loosening at the distal end of the fusion segment in 1 case, cerebrospinal fluid leakage in 2 cases, transient double lower limb weakness in 1 case, pain in one side of lower limb in 1 case, and no severe complications appeared. These results verified that one-stage posterior pedicle subtraction osteotomy of apical vertebra with transpedicular instrumentation could achieve satisfactory clinical outcomes in rigid angular kyphosis of thoracolumbar spine. The internal fixation was stable and with a high fusion rate and few complications. The medium-term effect was satisfactory.
7.Anemia in patients on combined androgen block therapy for prostate cancer.

OBJECTIVETo study the effect of combined androgen block therapy on hemoglobin (Hb) and hematocrit value (Ht) in patients with prostate cancer.
METHODSOne hundred and thirty-six patients with adenocarcinoma of the prostate were treated with combined androgen block (orchiectomy and flutamide 250 mg, Tid). Complete blood counts were detected before initiation and after 1, 2, 3, 6, 9 and 12 months of therapy.
RESULTSHb level declined significantly in all patients from a mean baseline of (136 +/- 14) g/L to (126 +/- 16) g/L, (121 +/- 14) g/L, (120 +/- 15) g/L, (113 +/- 12) g/L, (121 +/- 13) g/L and (123 +/- 15) g/L at 1, 2, 3, 6, 9 and 12 months. Ht decreased from a mean baseline of 0.424 +/- 0.041 to 0.390 +/- 0.038, 0.381 +/- 0.042, 0.378 +/- 0.038, 0.366 +/- 0.041, 0.384 +/- 0.039 and 0.387 +/- 0.040. The differences between Hb, Ht before and after treatment were significant (P < 0.05).
CONCLUSIONPatients with prostate cancer being treated with combined androgen block would develop a significant degree of anemia. Hemoglobin and hematocrit level should be monitored periodically. This kind of anemia can be treated by recombinant human erythropoietin.

8.The effect of acute urinary retention on serum prostate specific antigen concentration.

OBJECTIVESTo study the effect of acute urinary retention on the serum prostate-specific antigen (PSA) concentration.
METHODSBlood samples from 34 benign prostatic hyperplasia (BPH) patients with acute urinary retention were drawn immediately before suprapubic cystomy and 48 hours after relief of urinary retention. Serum PSA concentrations were measured with radioimmunoassay.
RESULTSThe mean serum PSA levels of BPH patients with acute urinary retention was (24.6 +/- 16.1) micrograms/L (range from 2.6 micrograms/L to 45.8 micrograms/L). Forty-eight hours after relief of urinary retention, the mean serum PSA levels declined to (9.4 +/- 6.3) micrograms/L (range from 1.7 micrograms/L to 16.6 micrograms/L). The difference was significant (P < 0.01).
CONCLUSIONSAcute urinary retention could dramatically increase the serum PSA value of patients with BPH. After relief of the urinary retention, the patients had a great than 50% decreased of PSA values.

9.Surgical correction of penoscrotal transposition with hypospadias: experience with 83 cases.

OBJECTIVETo investigate the techniques of surgical correction of penoscrotal transposition with hypospadias.
METHODSWe retrospectively studied 83 cases of penoscrotal transposition with hypospadias treated by surgery from January 2003 to June 2009, and analyzed the surgical techniques and follow-up results.
RESULTSThe patients underwent urethroplasty with simultaneous or staged surgical correction of penoscrotal transposition. Postoperative follow-up was conducted for 0.5-5 years. Eighty-one of the patients were satisfied with the appearance of the reconstructed penis and scrotum, and satisfactory outcomes were achieved in the other 2 with severe hypospadias after a second surgical correction of penoscrotal transposition.
CONCLUSIONUrethroplasty with simultaneous or staged surgical correction of penoscrotal transposition is recommendable for its resultant penile straightness, desirable penoscrotal appearance, good surgical prognosis and few postoperative complications.

10.Effect of OATP1B1 521T --> C heterogenesis on pharmacokinetic characterstics of rosuvastatin in Chinese volunteers.

This study is to report the effect of OATP1B1 gene mutation in the 521T --> C in Chinese human on the pharmacokinetics of rosuvastatin and guide the reasonable clinical application of rosuvastatin by the feature of genetic polymorphism of OATP1B1. Plasma samples were determined with LC-MS: the analyte and internal standard pitavastatin were both analyzed by MS in the ESI, m/z was 480.0 for rosuvastatin and 420.0 for the IS, separately. Genotyping of OATP1B1 was determined with the method of polymerase chain reaction--amplification refractory mutation system targeted at 40 healthy volunteers and showed that there were 7 subjects with 521T --> C mutant, accounting to 17.5% of total and wild type homozygote accounted to 82.5%. It was found that there were significant differences between OATP1B1 mutation in the 521T --> C and wild type homozygote for rosuvastatin pharmacokinetic process in Chinese human. In contrast to OATP1B1 wild type group, OATP1B1 mutation group's absorption degree increased, elimination process decreased. The OATP1B1 mutation should be noted for guiding the reasonable application of rosuvastatin during its clinical use.
