1.Acupuncture treatment of migraine research progress based on cerebral blood flow study

Migraine is a kind of nerve vascular disease, characterized by hemialgia, moderate and severe, pulsating pain. Acupuncture as a safe and effective analgesic therapy, has been widely used in more than 160 countries and regions around the globe. Treating migraine with acupuncture has certain good features as effective, non-toxic side effects and untoward effect.But its mechanism is still not very clear.The cerebral blood flow study ( CBF study) is currently the most credibility of migraine pathogenesis theory.Now based on the theory of CBF study, this paper discusses the acupuncture treatment of migraine which can provide the basis for clinical treats.
2.The mechanism of synapse changes in the region around a cerebral infarct after mobility training

Objective To investigate how mobility training affects the synapses and their functioning in the region around a cerebral infarct.Methods One hundred and fifty rats were randomly divided into 5 groups:a mo- tor training group,a normal saline group,an Ara-c inhibition group,a mevastatin group and a control group.Cere- bral infarcts were surgically induced in all 150 rats,and the level of either glial fibrillary acidic protein(GFAP)or synaptophysin,and the cholesterol content around the infarct were observed at 7,21 and 42 days using immunohisto- chemical staining and high performance liquid chromatography.Results At 7,21 and 42 days after the infarct model was induced,significant differences in the optical density of either GFAP or synaptophysin and in cholesterol levels were noted between the motor training group and the control group.Ara-c inhibition was also significantly high- er in the controls.The optical density of synaptophysin and the cholesterol levels were significantly lower in the me- vastatin group than in the motor training group.Conclusion Motor skill training can improve synapse redefinition in rats with a model of acute cerebral infarct.Astrocytes may play a crucial role by means of the secretion of choles- terol in the region around the infarct.
3.HPLC fingerprint of Xuanbo Shuangsheng Granule

Objective To establish the HPLC fingerprint of Xuanbo Shuangsheng Granule for its quality control.Methods The components were separated on an Agilent TC-C18 column (150 mm ×4.6 mm,5 μm) at 25 ℃,with a gradient elution in 0-60 min at the flow rate of 1 mL/min using 0.2% acetic acid aqueous solution and methanol as the mobile phase.The detection wavelength was 278 nm.By detecting 11 batches of Xuanbo Shuangsheng Granule,the HPLC fingerprint was established using Similarity Evaluation System for Chromatographic Fingerprint of TCMs (Version 2004A)and the common peaks were analyzed and identified in raw herbal material.Results The HPLC fingerprint of Xuanbo Shuangsheng Granule was obtained with similarity all over 0.9.Totally 27 common peaks were confirmed,and each common peak could be found in raw herbal medicines.Based on the reference substances,5 common peaks were identified,including phellodendrine (peak 11),liquiritin (peak 22),angoroside C (peak 25),cinnamic acid (peak 26) and harpagoside (peak 27).Conclusions This method is simple,accurate,repeatable and reliable,which could be applied in the quality control of Xuanbo Shuangsheng Granule.
4.A comparison between opening total extraperitoneal herniorrhaphy and traditional anterior approach on tension-free repair of inguinal hernia

Objective To evaluate the effects of the two different operations, such as open total extraperitoneal herniorrhaphy or traditional anterior approachs on tension-free repair of inguinal hernia. Methods In a prospective randomized controlled study, 165 cases with inguinal hernia were allocated randomly to either the anterior approach group (82 cases)or open total extraperitoneal herniorrhaphy group( 83 cases ).The index including the operation time, hospital stay, mean expense, time for returning to normal activity,and the recent or long-term operative complications and recurrence rate, were observed to evaluate the curative effect of open total extraperitoneal herniorrhaphy approach. Results The follow-up rate were 98.79% ,after (20.52 ± 1.54) months in average follow-up in the anterior approach group and (21.63 ± 1.76) months in the TEP group, and no significant differences were recorded between the two groups in the operation time, hospital stay, time for returning to normal activity, recurrence rate(3.66% vs 1.22% ) and urinary retention ( P > 0. 05 ), but operative complications in TEP group were significantly less than that in anterior approach group (P< 0.05). Conclusions The operative complications or postoperative unwell decrease significantly through the open total extraperitoneal herniorrhaphy approach in repairing inguinal hernias. Its curative effect was confirmed and deserves to be clinically popularized.
5.Abnormal expression of metallothionein and FasL in rectal carcinoma tissues

Objective To study the expression of metallothioneiu (MT)and Fas ligand (FasL) in rectal carcinoma and their association with metastasis to lymph node and liver. Methods Immunohistochemistry method and quantitative RT-PCR technique were used to assay the expression of MT and FasL at protein and mRNA levels in 85 cases of rectal carcinoma. The data of each group were compared and analysed by statis-tics. Results The rates of MT expression in primary foci, normal rectal mucosa, lymph node metastasis and hepatic metastasis were 57.3% ,29.6% ,79.5% and 61.8% respectively. And the rates of FasL expression were 45.8%, 17.8% ,63.5% and 90.3%, respectively. The positive rates of MT and FasL in primary foci, hepatic and lymph node metastasis were higher than that in normal mucosa (X~2 =33. 1322,56. 7142,P < 0.01). Among clinical stages, the positive rates of MT and FasL in C and D were higher than that in A and B (X~2 = 18. 8372,21. 5823 ,P <0.01). And higher rates of MT and FasL expression were detected in low differentiation adenocarcinoma and mucus adenocarcinoma than in middle-high differentiation adenocarcino-ma(X~2 = 11.2146,9.3136,P < 0. 05). High MT mRNA level was found in lymph node metastasis and high FasL mRNA level in hepatic metastasis. Conclusions The detection of MT and FasL expression was useful in clinic to predict lymph node metastasis and early diagnosis of liver metastasis of rectal carcinoma respec-tively. Assay of MT and FasL expression has prognostic values for rectal carcinoma patients.
6.Therapeutic Effect of Lai's Acupuncture Therapy Combined with Chinese Herbal Decoction in Treating Infertility with Ovulatory Dysfunction

Objective To observe the therapeutic effect of Lai's acupuncture therapy combined with Chinese herbal decoction for the treatment of infertility with ovulatory dysfunction, and to summarize the experience of Professor Lai Xin-Sheng in treating infertility. Methods Ninety-six infertility patients with ovulatory dysfunction were randomized into treatment group and control group, 48 cases in each group. The treatment group was treated with Lai's Tongyuan acupuncture therapy for regulating Governor Vessel to tranquilize mind, inducing meridian Qi to original Qi, and was also given oral use of Chinese herbal decoction of Lai's Yiyun Recipe for promoting gestation. The control group was treated with Chinese herbal decoction alone. The treatment began exactly after the completion of menstruation period and lasted 15 days for one treatment course, covering 3 courses. And then the patients of the two groups were followed-up for 3 months. Therapeutic effect and pregnancy rate were evaluated after treatment, serum levels of estradiol and progesterone were examined, and abdominal ultrasonography was used for monitoring the maximum diameter of the dominant follicles and endometrial thickness before and after treatment. Results (1) Three cases from the treatment group and 5 cases from the control group were dropped out, and the final case number in the treatment group and control group was 45, 43 respectively. (2) The total efficiency of the treatment group was 88.9%, and that of the control group was 81.4%. The effect of the treatment group was better than that of the control group (P < 0.05). (3) The pregnancy rate of the treatment group was 73.3%, higher than that of the control group (51.2%), the difference being significant (P < 0.05). (4) After treatment, the maximum diameter of the dominant follicles and endometrial thickness in the two groups were obviously improved(P < 0.05 compared with those before treatment), and the improvement in the treatment group was superior to that in the control group (P < 0.05). (5) After treatment, the serum levels of estradiol and progesterone in the two groups were obviously improved(P < 0.05 compared with those before treatment) , and the improvement in the treatment group was superior to that in the control group(P < 0.05). Conclusion The therapeutic effect of Lai's acupuncture therapy combined with Chinese herbal decoction is superior to that of Chinese herbal decoction alone, showing an obvious synergistic effect by shortening the course of treatment and enhancing pregnancy rate.
7.Clinical Observation of Acupuncture plus Medication in Treating Chronic Pelvic Inflammatory Disease

Objective To observe the clinical efficacy of acupuncture plus medication in treating chronic pelvic inflammatory disease. Method By using the random number table, sixty-eight patients with chronic pelvic inflammatory disease were randomized into an acupuncture-medication group of 34 cases and a medication group of 34 cases. The clinical efficacies were compared after 2 courses of treatment, and the symptoms and body signs scores and syndrome score of traditional Chinese medicine (TCM) were also compared. Result There was a significant difference in comparing the therapeutic efficacy between the acupuncture-medication group and the medication group (P<0.05). After the treatment, the symptoms and body signs scores and TCM syndrome score dropped significantly in both groups (P<0.05), indicating that the two groups both had improvement in the symptoms, body signs and TCM syndrome; there were significant between-group differences in comparing the score differences in the symptoms and body signs scores and TCM syndrome score after the treatment (P<0.05), and the acupuncture-medication group was higher than the medication group. Conclusion Acupuncture plus medication can better ameliorate the symptoms and body signs and TCM syndrome in chronic pelvic inflammatory disease.
8.Changes in MLS-BAEP in newborn piglets with hypoxic-ischemic brain damage during selective moderate head cooling therapy.

OBJECTIVETo study the effect of selective moderate head cooling therapy on maximum length sequences brainstem auditory evoked potential (MLS-BAEP) in newborn piglets with hypoxic-ischemic brain damage.
METHODSSixteen newborn piglets aged 5-7 day old were randomly divided into three groups: normothermic control (n=4), HI (n=6) and mild hypothermia-treated (n=6). HI was induced through temporary occlusion of both carotid arteries, followed by mechanical ventilation with low concentration of oxygen (FiO2=0.06) for 30 minutes. Mild hypothermia was induced by equipment via circulating water. MLS-BAER was recorded before HI and at 12 hours, 24 hours, 36 hours, 48 hours, 60 hours, 72 hours, 4 days, 7 days, 10 days, 13 days and 15 days after HI.
RESULTSCompared with the normothermic control group, all latencies and intervals tended to increase significantly at 72 hours in the HI group and reached peak values on day 7. From day 10, all latencies and intervals tended to decrease, but apart from wave I latency, still differed significantly from those of the normothermic control group. MLS-BAER variables did not reach normal values until day 15. Ⅲ latency, Ⅰ-Ⅲ interval and Ⅰ-Ⅴ interval were significantly reduced in the hypothermia-treated group between 60 and 7 days after HI compared with the HI group (P<0.05). V latency and Ⅲ-Ⅴ interval in the hypothermia-treated group were also reduced compared with the HI group between 72 hours and 7 days after HI (P<0.05).
CONCLUSIONSBoth peripheral and central auditory systems are disturbed by HI, which shows as a significant increase in MLS-BAER variables (all latencies and intervals) in newborn piglets. Involvement in central brainstem auditory system reaches a peak on day 7 after injury. MLS-BAER variables still cannot reach to normal values until day 15. Selective moderate head cooling therapy can significantly reduce brainstem damage induced by HI.

9.Expression of eosinophil major basic protein mRNA in bronchial asthma.

OBJECTIVETo investigate the expression of eosinophil major basic protein (MBP) mRNA in bronchial asthma and explore its significance.
METHODSPeripheral blood eosinophil MBP mRNA levels were measured in 40 patients with asthma and 20 normal controls by semi-quantitative RT-PCR. The association of MBP mRNA levels with eosinophil count and pulmonary function was also analyzed.
RESULTSCompared with the normal control group, MBP mRNA level were significantly increased in asthma patients (0.37-/+0.11 vs 0.17-/+0.04, P<0.001), so was the eosinophil count (0.86-/+0.52 vs 0.21-/+0.10, P<0.001). MBP mRNA levels in patients with moderate persistent asthma (0.42-/+0.05) and those with severe persistent asthma (0.47-/+0.05) were significantly higher than those in patients with mild persistent asthma (0.25-/+0.06, P<0.001), and the difference in MBP mRNA levels between moderate persistent asthma patients and severe ones was also significant (P<0.05). Among the asthma patients, MBP mRNA levels showed an inverse correlation with pulmonary function (r=-0.7490, P<0.001).
CONCLUSIONIncreased MBP mRNA expression level may correlate with the severity of asthma. MBP may play an important role in the development of asthma.

10.The value of MRCP combined with DWI in T-staging of gallbladder cancer

Objective To investigate the value of conventional MRI,MR cholangiopancreatography(MRCP) and diffusion weighted imaging(DWI) in T-staging of gallbladder cancer(GBC).Methods 56 patients with histologically proven GBCs were analyzed retrospectively.Staging was performed according to MR findings and compared with pathological findings.The accuracy of the two methods with or without DWI sequence was also compared.Results The accuracy of staging of conventional MRI and MRCP was 87.5% (49/56).The sensitivity,specificity and accuracy of conventional MRI and MRCP in the stage of lesions as T1 vs ≥T2,≤T2 vs ≥T3 and ≤T3 vs T4 were 83.3 %/98.0 %/96.4 %,92.3 %/97.7 %/96.4 % and 93.1%/88.8 %/91.1%,respectively.The accuracy of staging of conventional MRI and MRCP combined with DWI was 91.1% (51/56).The sensitivity,specificity and accuracy of conventional MRI and MRCP combined with DWI in the stage of lesions as T1 vs ≥T2,≤T2 vs ≥T3 and ≤T3 vs T4 were 83.3%/98.0%/96.4%,92.3%/97.7%/96.4% and 93.8%/100%/96.4%,respectively.There was no statistical difference between the two groups(P>0.05).Conclusion Conventional MRI and MRCP provides excellent T-staging accuracy for GBC.The accuracy of T-staging can be improved by conventional MRI and MRCP combining with DWI,especially for T4 stage.
