Objective To analyse the unqualified cause of unpaid blood donation blood test in Dianjiang County .Methods The screening results of ALT ,HBsAg ,HCV antibody ,HIV antibody ,TP antibody ,of blood donors ,and their personal information from 2008 to 2012 were collected .The statistics method was used to comparative analysis .Results Rate of unqualified of unpaid blood donation blood test was 4 .06% ;Rate of unqualified of ALT was 2 .37% ,anti-TP was 0 .85% ,HBsAg was 0 .77% ,anti-HCV was 0 .15% ,anti-HIV was 0 .07% .The rate of unqualified was higher among the blood donors ,young male person ,whose level of educa-tion was junior high school and below junior high school .Conclusion It should strength preliminary screening of blood donors and screening of high-risk donors .Collecting flood from low-risk and regular donors in case to improve the quality of blood donors and decrease the waste of blood .

ObjectiveTo evaluate the clinical efficacy,untoward reaction of bruceolic oil emulsion combined GP program in treatment of advanced non-small-cell lung cancer(NSCLC).MethodsOne hundred and thirty-six patients with NSCLC were divided into observation group and control group with 68 cases each by random digits table,the two groups were received GP program,the observation group added bruceolic oil emulsion( 1 time a day).The effective rate and untoward reaction between two groups were compared.ResultsThe objective effective rates of observation group and control group were 32.4％(22/68)and 17.6％( 12/68 ),the subjective effective rates of observation group and control group were 52.9％(36/68 )and 32.4％ (22/68),there were significant differences between two groups(P ＜0.05).There was no significant difference in CD3+,CD4+,CD8+,CD56+ between two groups before treatment (P＞ 0.05 ).CD3+,CD4+,CD8+,CD56+ in observation group were better than those in control group after treatment(0.552 ± 0.072 vs.0.468 ± 0.080,0.368 ± 0.062 vs.0.288 ± 0.061,0.268 ± 0.052 vs.0.212 ± 0.044,0.226 ± 0.044 vs.0.168 ±0.042),there were significant differences between two groups (P ＜ 0.05).There was no statistical difference in Ⅲ+ Ⅳ degree untoward reaction between two groups(P ＞ 0.05).ConclusionBetter therapeutic efficacy and less untoward reaction are expected by combined bruceolic oil emulsion with GP program in treatment of advanced NSCLC.

Objective To compare the effects of sedation induced with midazolam and propofol on blood pressure(BP), heart rate(HR) and heart rate variability (HRV) during operation on lower extremity preformed under continuous epidural anesthesia in patients with the type Ⅱ diabetes. Methods Forty patients with type Ⅱ diabetes scheduled for lower limb surgery were randomized to receive either midazolam (group M n - 20) for propofol (group P n = 20) for sedation during operation, all patients were classified as ASA physical status Ⅱ or Ⅲ . The patients were unpremeditated. Epidural catheter was inserted at L2-3 or L3-4 . The local anesthetic used was 2% lidocaine. MAP,HR,HRV and SpO2 were monitored during anesthesia and operation. After onset of epidural analgesia a loading dose of midazolam(0.03 mg?kg-1 ) or propofol (0.7 mg?kg-1 ) was given i.v. followed by intermittent i.v. boluses of 1 /4-1 /2 loading dose to maintain the depth of sedation at OAA/S score of 3 (respond to name outer if called repeatedly) . Changes in HRV before and during sedation were recorded and compared between the two groups. Results The demographic data including sex, age and body weight were comparable between the two groups. Objective To investigate the effect of Ginkgo biloba extract on serum divalent metal transporter1 ( DMT1 ) , glucose regulated protein 75(grp75) and nerve function in patients with Parkinson disease.Methods 38 cases of patients loith parkinson disease according to different drugs were divided into experimental group and control group, 19 cases in each group.Control group was treated with levodopa and Benserazide tablet, experimental group on the basis of control group, was given Ginkgo biloba extract tablets, treatment for 4 weeks.After treatment, DMT1, grp75 and cognitive function of all patients in substantia nigra were detected.ResuIts Compared with before treatment, two groups of patients with lower DMT1 level (P<0.05), compared with control group, experimental group of patients with lower DMT1 levels (P<0.05).Compared with pre-treatment, two groups of patients grp75 level was higher (P<0.05), compared with control group, experimental group after treatment grp75 level was higher (P<0.05).Compared with before treatment, the two groups of patients with MoCA scores were higher (P<0.05), HAMD scores were lower (P<0.05).Compared with control group, experimental group after treatment MoCA scores were higher (P<0.05), HAMD scores were lower (P<0.05).ConcIusion Ginkgo biloba extract can significantly reduce the level of DMT1 in the substantia nigra of Parkinson patients, increase the level of grp75, and improve the cognitive function.

Objective: To investigate the role of miR-19a/b in myocardial ischemia reperfusion injury (R/I) with its clinical significance. Methods: Our research included in 2 parts. Part 1: in H9c2 cells. miR-19a/b expression in H9c2 cells with 10 h hypoxia and 2 h re-oxygenation was detected by real-time PCR, miR-19a/b potential target gene was assessed by luciferase reporter activity assay. Part 2: in natural person. Control group,n=40 healthy subjects and AMI (acute myocardial infarction) group,n=40 relevant patients. Peripheral blood levels of miR-19a/b were detected as Part 1, cTnI were measured by chemiluminescence immune analysis and CK-MB were assessed by immune inhibition method. Results: Part 1: Compared with normal H9c2 cells, miR-19a/b expressions were increased 5.876 times and 2.761 times in H9c2 cells with 10 h hypoxia and 2 h re-oxygenation, bothP<0.01. With respectively transfected miR-19a/b mimic and inhibitor, miR-19a/b expression was up-regulated and down-regulated respectively. miR-19a/b and chromosome-10 deleted phosphatase, tensing homolog gene (PTEN) had the targeting effect. With up-regulated miR-19a/b expression, PTEN level was decreased and with down-regulated miR-19a/b expression, PTEN level was increased. Part 2: Compared with Control group, AMI group had elevated blood level of miR-19a/b,P<0.01. In AMI patients, miR-19a/b was increasing at 0-3 h of chest pain and reaching the peak at 6-12 h; CK-MB enzyme activity and cTnI content were elevating at 2-6 h of onset and reaching the peak at 24 h. Conclusion: miR-19a/b expression was up-regulated by myocardial ischemia reperfusion in H9c2 cells, PTEN was the potential target gene of miR-19a/b. Circulating miR-19a/b might be used as a new non-invasive biological marker for myocardial ischemia R/I diagnosis.

Objective: To investigate the chemical constituents from the whole plants of Gynura procumbens. Methods: The chemical constituents were isolated and purified by repeated silica gel column chromatography, Sephadex LH-20 gel column chromatography, medium pressure column chromatography, and semi-preparative HPLC, and their structures were elucidated by chemical properties and spectroscopic analyses. Results: Sixteen compounds were identified to be quercetin (1), apigenin (2), luteolin (3), kaempferol (4), astragaline (5), kaempferol-5-O-(6″-O-acetyl)-β-D-glucopyranoside (6), negletein (7), 4-methoxycinnamic acid (8), benzyl-O-β-D-glucopyranoside (9), 2-phenylethyl-O-β-D-glucopyranoside (10), 3,5-dicaffeoylquinic acid methyl ester (11), 3,5-dicaffeoylquinic acid ethyl ester (12), 3,4-dicaffeoylquinic acid methyl ester (13), 4,5-dicaffeoylquinic acid methyl ester (14), protocatechuic acid (15), and eugenol glucoside (16). Conclusion: Compounds 7, 8, 12, 14, and 16 are obtained from the plants in Gynura Cass. for the first time, and compounds 3, 6, 9-11 and 13 are obtained from this plant for the first time.

Objective To investigate the correlation between white matter hyperintensities (WMH)and hyperintense vessel sign (HVS) on fluid-attenuated inversion recovery (FLAIR) magnetic resonance imaging(MRI)in old adults and to explore the risk factors and pathogeneses of WMH.Methods We retrospectively collected imaging and clinical data of patients who had received both head and neck CTA and brain MRI within one month at our hospital from 2013 to 2016.The Fazekas visual scale was used to evaluate periventricular white matter hyperintensity(PWMH)and deep white matter hyperintensity(DWMH)in each brain hemisphere.According to the presence or absence of HVS in a cerebra[hemisphere,patients were assigned into an HVS-positive group or an HVS-negative group.Clinical data,PWMH,and DWMH differences were compared between the two groups.Results A total of 271 patients(542 cerebral hemispheres)were included in this study.HVS-positive imaging occurred in 79(14.6％)cerebral hemispheres and negative imaging was observed in 463 (85.4％) cerebral hemispheres.There was a significant difference between the HVS-positive and negative groups in the ipsilateral CIA stenosis(x2 =126.840,P＜0.01).The incidence of ipsilateral severe carotid artery stenosis in the HVS-positive group was 62.0％ (49/79),which was significantly higher than 9.9％ (46/463)in the HVS-negative group.The incidence of moderate-severe DWMH was 65.8％(52/79) in the HVS-positive group,which was higher than 34.8％ (161/463)in the negative group(x2 =34.962,P ＜20.01).Nevertheless,the incidences of moderate-severe PWMH in the two groups were 65.8％ (52/79) and 55.5％ (257/463),respectively,without a significant difference between them (x2 =6.944,P =0.074).After adjusting for age,gender,ipsilateral ICA stenosis,hypertension,diabetes,etc.multivariate analysis suggested that HVS-positive imaging was still an independent risk factor for DWMH(OR =2.653,95％CI:1.489-4.726,P =0.001).Conclusions HVS-positive imaging is an independent risk factor for DWMH in the elderly,but no clear correlation with PWMH is found.It suggests that hypoperfusion is a possible mechanism for the development of DWMH in the elderly.

It's difficult to diagnose precancerous lesion and early cancer for a long time, because both of them haven't typical morphological characteristics. As a novel diagnostic modality, fluorescence endoscopy can accurately reflect minimal changes in human's tissue, thus making a meaningful progress for cancer diagnosing. 200 patients were examined by fluorescence endoscopy to evaluate the diagnostic value. The overall accuracy, sensitivity and specificity for detecting malignant gastrointestinal tumor was 94.0%, 94.6% and 93.5%, respectively. Thus, fluorescence endoscopy can be used to diagnose malignant gastrointestinal tumors with high validity and reliability, and is advantageous over conventional white light endoscopy especially in detecting the atypical and suspicious lesions. Furthermore, fluorescence endoscopy can also guide target biopsy, is significant to improve the early cancer detection rate, has a broad development prospect.

Objective To investigate the impact of chronic renal insufficiency on serum levels of protein induced by vitamin K absence or antagonist‐Ⅱ(PIVKA‐Ⅱ) and sialic acid (SA) .MethodsThe levels of serum PIVKA‐Ⅱ ,SA ,urea and creatinine(Cr) were detected in 127 cases of chronic renal insufficiency ,32 cases of renal disease with normal renal function ,57 healthy controls un‐dergoing the physical examination and 120 cases of hepatocellular carcinoma (HCC ) by using the chemiluminescent and enzymatic methods respectively .The serum urea and creatinine levels were also measured .The estimated GFR (eGFR) was calculated .Results The serum PIVKA‐Ⅱ level had no statistical difference in among the healthy control group ,renal disease with normal renal func‐tion group and renal disease with renal insufficiency group (H=2 .902 ,P> 0 .05) ,moreover significantly lower than that in the HCC group (U=319 .5 ,203 .00 ,665 .50 respectively ,P<0 .001) .Serum PIVKA‐Ⅱ level had no statistical difference among vari‐ous stages in the disease with renal insufficiency group (H=3 .991 ,P>0 .05) .However ,serum SA levels had statistical differences among the healthy control group ,disease complicating normal renal function group and disease complicating renal insufficiency group( H = 63 .685 ,P<0 .001) ,and among the various stages in the disease complicating renal insufficiency group (H=64 .689 , P<0 .001) .The serum SA level was negatively correlated with eGFR level (r= -0 .705 ,P<0 .001) ,and positively correlated with Urea and Cr levels (r= 0 .599 ,r= 0 .704 ,P<0 .001) .The SA level in the HCC group was significantly increased compared with that in the stage 1-4 of chronic kidney disease(CKD) (U=126 .00 ,163 .50 ,247 .00 ,715 .00 respectively ,P<0 .001) ,but had no obvious change compared with the stage 5 of CKD(U=419 .00 ,P>0 .05) .Conclusion The renal insufficiency has no obvious im‐pact on serum PIVKA‐Ⅱexpression ,but could significantly increase the SA expression level ,moreover is closely related with the se‐verity of renal function impairment ,thus indicating that the SA level increase not only has the assisted diagnosis value on HCC and multiple malignant tumors ,but also better reflects the renal function status in the patients with chronic renal insufficiency .
