Aim To investigate the potential molecular basis of Epimedium and to clarify the molecular mechanism of Epimedium in the treatment of osteoporosis. Methods The method of network pharmacology was used to determine and screen the known compounds corresponding to Epimedium, and predict the drug-related gene/protein targets, and combined with bioinformatics, the specific target of osteoporosis prevention and treatment of Epimedium was determined, and then the signal pathway enrichment was analysed, the molecular mechanism of Epimedium in treatment of osteoporosis was further explored. Results A total of 130 corresponding compounds were retrieved in the TCMID database from Epimedium.According to the values of OB and DL,23 blood transfusion components were screened,in which 101 targets were obtained using the related target prediction technique.Through the secondary mining of the gene chip of GEO chip database,we obtained a total of 124 significantly different genes. And a total of 355 known target genes closely related to the development of osteoporosis were retrieved in the disease-related database.Using the cytoscape to construct and synthesize the protein-protein interaction network of active ingredients and diseases,221 key genes were screened out by network topological analysis.Using ClueGO analysis,it was shown that Epimedium was directly engaged in the signal pathways involved in the key nodes of osteoporosis, which were mainly related to the direct regulation of bone metabolism, such as Wnt signaling pathway,TGF-β signaling pathway and Notch signaling pathway, and that Epimedium regulated other signal pathways, such as PI3K-Akt signaling pathway,VEGF signaling pathway and thyroid hormone signaling pathway. Conclusions Epimedium has the characteristics of multiple targets and multicomponent in the treatment of osteoporosis, which could not only be directly involved in bone cell differentiation and regulation of the balance of osteogenesis and osteoclasts, but also affect and interfere with the bone microenvironment through other systemic systems, such as circulatory system, nervous system, which is consistent with the current mechanism of treatment of osteoporosis.

OBJECTIVE:To stu dy in vitro lipid-lowering effect of ganoderic acid C 2(GAC2),and to investigate its potential mechanism on the basis of S 6K/SREBPs signaling pathway. METHODS :Using human liver cells HL- 7702 as objects ,MTT assay was used to test relative cell viability after treated with low ,medium and high doses (5,10,20 μmol/L,hereinafter)of GAC 2. Using lovastatin as positive control ,ELISA method was used to detect the contents of TC and TG in cells after treated with low , medium and high doses of GAC 2. Nile red staining was used to observe the accumulation of lipids in cells. After transfected SREBPs report gene plasmid ,using 25-HC as positive control ,relative viability of SREBPs luciferase in cells were determined by luciferase assay after treated with low ,medium and high doses of GAC 2. Using 25-HC as positive control ,real-time fluorescent quantitative PCR was used to measure the mRNA expression of SREBPs and their downstream genes in cells after treated with medium and high doses of GAC 2. Using SREBPs inhibitor (25-HC)and S 6K inhibitor (rapamycin)as control ,Western blotting assay was adopted to determine the expression of SREBP- 1 and SREBP- 2(in the case of n-SREBPs ),relative expression ratio of phosphorylated S 6K to S 6K(p-S6K/S6K ratio ). AutoDock 4.0 and other softwares were used for molecular docking of S 6K and GAC2. RESULTS :There was no significant effect of low , 0.05). Compared with blank control group ,the content of TC qq.com in lovastatin group and GAC 2 high-dose group as well as thecontent of TG in lovastatin group , GAC2 medium- and 床应用。电话:0371-65962746。E-mail:whui3697@126.com high-dose groups were decreased significantly (P<0.05 or P< 0.01);the number of lipid droplets in the cells of all medication groups decreased. Compared with blank control group ,relative viability of SREBPs luciferase in 25-HC group ,GAC2 low-,medium- and high-dose groups were decreased significantly ;mRNA expression of HMGCS1,MVK,SCD,HMGCR gene in 25-HC group and GAC 2 medium-,high-dose groups ,mRNA expression of DHCR7 gene in 25-HC group ,mRNA expression of SREBP-2 gene in GAC- 2 high-dose group as well as mRNA expression of DHCR24 and MSMO2 gene in 25-HC group and GAC 2 high-dose group were all decreased significantly ;relative protein expression of n-SREBP- 1 in 25-HC group ,GAC2 low-,medium- and high-dose groups ,relative protein expression of n-SREBP- 2 in 25-HC group and GAC 2 high-dose group as well as p-S 6K/S6K ratio in rapamycin group and GAC 2 groups were decreased significantly (P<0.05 or P<0.01). The molecular docking results showed that GAC 2 could bound to amino acid residues Arg 335,Arg330 and Ala332 of S 6K via hydrogen bond. CONCLUSIONS :GAC2 can reduce the lipid level of HL- 7702 cells, which may be associated with inhibiting the expression of S 6K/SREBPs signaling pathway.

Objective:To investigate the related risk factors of surgical appendectomy after endoscopic retrograde appendicitis therapy (ERAT) in children. Methods:From September 2019 to November 2023 at the Children's Hospital Affiliated to Shandong University, the data from all related children with appendectomy after ERAT were analyzed. The general situation and main clinical manifestations of the children were collected. According to the effect of ERAT,the patients were divided into transfer to surgical group and appendicitis recurrence group by the reasons and time of transfer to surgical treatment. The indexes such as ERAT and reoperation process and pathological types of appendix after operation were collected, and the clinical experience was summarized. Results:Among the 242 children who underwent ERAT,32 cases underwent appendectomy again, including 19 males and 13 females, with an average age of(9.16±2.77)years,and the reoperation rate was 13.2%. The clinical manifestation was abdominal pain (32 cases,100.0%),mainly right lower abdominal pain (28 cases,87.5%),and the main duration was less than 3 months (30 cases,93.8%). Abdominal ultrasound or CT before ERAT mainly indicated appendicitis (11 cases,34.4%) and appendicitis complicated with appendiceal calculus (11 cases,34.4%). There were 9 children in transferred to surgical group,of which 5 cases were transferred to surgery because of ERAT intubation failure, and 4 cases were transferred to surgery because of appendix perforation or abscess. Postoperative pathology showed acute suppurative appendicitis in 5 cases and acute gangrenous appendicitis in 4 cases. There were 23 children in appendicitis recurrence group, whose abdominal pain was relieved after ERAT,but their symptoms were repeated after discharge. After clinical evaluation, they were diagnosed as recurrent appendicitis and underwent surgical appendectomy. Most of them were reoperated within 6 months after ERAT (21 cases,91.3%).Postoperative pathology was mainly chronic appendicitis (10 cases,43.5%).Conclusion:Acute suppurative appendicitis and acute gangrenous appendicitis are prone to catheterization or endoscopic failure during ERAT,and there is a high risk of surgical transfer. ERAT may relieve the symptoms of chronic appendicitis in children. If the symptoms are repeated, surgery is still needed to remove the appendix.

OBJECTIVETo investigate the effect of Huanshuai Recipe Oral Liquid ([characters: see text], HSR) on retarding the progression of renal dysfunction in patients with atherosclerotic renal artery stenosis (ARAS).
METHODSA total of 52 ARAS patients with the Chinese medicine (CM) syndrome of qi deficiency and blood stasis, phlegm and dampness retention were recruited and randomly assigned into the treatment group (36 cases) and the control group (16 cases). Both groups received a basic treatment (high-quality low-protein diet, blood pressure control, lipid-lowering, correcting the acidosis, etc.). In addition, the treatment group received 20 mL HSR and the control group received placebo, 3 times a day for 6 months. Renal function (serum creatinine, blood urea nitrogen and uric acid) and blood lipids (cholesterol, triglycerides and low density lipoprotein) were examined monthly. The estimated glomerular filtration rate (eGFR) and CM syndrome score were compared between groups.
RESULTSAfter treatment, compared with the control group, the serum creatinine level, uric acid level and CM syndrome score of the treatment group were significantly decreased (P<0.05 or P<0.01), and the eGFR in the treatment group were significantly increased (P<0.05).
CONCLUSIONHSR can effectively improve the renal function and clinical symptoms of ARAS patients.
