1.Experimental study on preventive effect of Yougui drink on femoral head necrosis in rats under micro CT.

OBJECTIVETo explore the preventive effect of Yougui drink on femoral head necrosis in rats under micro CT.
METHODSTwenty-five SD rats were divided into steroid hormone group (group A, 10 rats ), Yougui drink group (group B,10 rats) and normal group (group C,5 rats)with random number table. Endotoxin were injected into abdominal cavity of rats in group A and B for 2 days, methylprednisolone sodium succinate were injected by gluteus for twice a week continued for 6 weeks; group B were gavaged by Yougui drink (veryday for 8 weeks; group C did not do any processing. All rats were killed on the 10th weeks,m icro CT were used to scan femoral head in vitro and preventive effect of Yougui drink (n femoral head necrosis in rats.
RESULTSThere was statistical significance in BMD, BV/TV, Tb.N, Tb, Th, Thb, Sp, BS/TV and DA but no significance in SMI between group A and B. Comparison between A and C, there was significant meaning in BMD, BV/TV, Tb.N, Tb, Th, Tb, Sp, BS/TV, DA and SMI.
CONCLUSIONYougui drink on femoral head necrosis in rats under micro CT has preventive effect from BMD BV/TV, Tb.N, Tb, Th, Tb, Sp, BS/TV and DA.

2.Research progress on clinical diagnosis and treatment for external snapping hip.

External snapping hip(ESH) is a vague term used to describe palpable or auditory snapping with hip movements with or without pain. The pathogenesis of ESH is related to the specific anatomical structure and friction factor. The clinical symptom is auditory snapping during activities, physical examination, X-ray, magnetic resonance imaging(MRI), dynamic ultrasound and other imaging techniques can be used to diagnose. Conservative medical management includes rest, avoidance of aggravating activities, and antiinflammatory medications. Treatment Patients with mild symptoms can achieve good results by medication, rest and physiotherapy. Surgical treatment for patients with ineffective conservative treatment was performed. All kinds of open surgery method can achieve good clinical curative effect, arthroscopic surgery is gradually been promoted due to small trauma, less complications. Besides, there are some reports that traditional treatments such as massage, acupuncture and acupotomology have achieved good clinical results, which deserve further study and promotion.
3. Relationship between thyroid nodules and lifestyle characteristics in Chinese centenarians

Objective:
To study the correlation of lifestyle characteristics with thyroid nodules in a population-based sample of centenarians in Hainan.
Methods:
The study was based on China Hainan Centenarian Cohort Study (CHCCS) conducted in 18 cities and counties in Hainan province from 2014 to 2016. A group of multidisciplinary team interviewed and examined local centenarians with structured questionnaires and ultrasonography procedures. A total of 918 centenarians were analyzed after excluding those who refused ultrasonographic examinations or had relevant missing data. Thyroids of centenarians were examined by 3-year experienced sonographer, details on lifestyle characteristics and dietary habits were collected by standard procedure.
Results:

4.Network Meta-analysis of Efficacy of Five Classical Prescriptions for Acute Attack of Asthma in Children

ObjectiveTo compare and evaluate the clinical efficacy of five classical prescriptions for acute attack of bronchial asthma (BA) and cough variant asthma (CVA) in children, and to further compare and assess the effect of them on cold-induced asthma or heat-induced asthma. MethodRandomized controlled trials (RCT) on the treatment of acute attack of asthma with five classical prescriptions (Sanzi Yangqintang, Maxing Shigantang, Shegan Mahuangtang, Xiao Qinglongtang, and Dingchuantang) were retrieved from China Science and Technology Journal Database (VIP), China National Knowledge Infrastructure (CNKI), and Wanfang Data (from establishment to August 15, 2021). The eligible RCT were evaluated and the data were extracted for network Meta-analysis by Stata 16.0. ResultA total of eligible 47 RCT were screened out, involving 5 114 children with acute attack of asthma and 10 intervention measures. Among them, 16 RCT (1 912 children, 6 intervention measures) were about the cold-induced asthma and 10 RCT (1 054 cases, 4 intervention measures) focused on the heat-induced asthma. According to the Meta-analysis, among the 10 interventions, Maxing Shigantang + routine treatment of western medicine demonstrated the most significant effect, and the effect of the interventions was in the following order: Maxing Shigantang + routine treatment of western medicine > routine treatment of western medicine, Shegan Mahuangtang + routine treatment of western medicine> Xiao Qinglongtang + routine treatment of western medicine > Shegan Mahuangtang > Dingchuantang + routine treatment of western medicine. For the cold-induced asthma, the effect of Shegan Mahuangtang + routine treatment of western medicine was remarkable, and for the heat-induced asthma, the corresponding intervention was Dingchuantang + routine treatment of western medicine. Shegan Mahuangtang was outstanding in improving the percentage of forced expiratory volume in the first second in predicted value (FEV1%). ConclusionThe combination of western medicine with the five prescriptions was more effective than the western medicine alone, particularly the combination with Maxing Shigantang. The combination of Shegan Mahuangtang and western medicine was outstanding in the treatment of cold-induced asthma, while the corresponding intervention for heat-induced asthma was the combination of Dingchuantang and western medicine. However, a large number of RCT with scientific design and higher quality are still needed to verify the conclusion.
5.Progress on femorotibial mechanical axis of total knee arthroplasty.

Femorotibial mechanical axis (FTMA) is one of important factors influencing clinical effect after total knee arthroplasty (TKA). It is generally believed that the range of lower limb alignment after TKA is controlled within neutral FTMA ± 3 °, which has more advantages in improving joint function, prolonging prosthesis survival rate and reducing revision rate, and obtain better clinical results. Therefore, neutral FTMA is also considered to be the gold standard for TKA. However, with the application of computer-assisted surgery and other technologies, the alignment of FTMA is more accurate than before, but the clinical effect after surgery has not significantly improved. Some scholars have begun to question the necessity of neutral alignment of FTMA, and proposed alignment methods such as kinematics and retained residual deformity, which could achieve better clinical effects. In recent years, it has been reported that FTMA might not be the most important factor influencing postoperative clinical effects, and it is suggested that the arrangement and measurement of lower limbs and the effects on adjacent joint functions could affect clinical effect after TKA. The paper reviews neutral FTMA alignment is still an important factor for success of TKA. After a thorough evaluation according to the patient's condition, it should be appropriately applied in the case of neutral FTMA alignment; the operator should explore other factors which affect clinical outcome after TKA, and improve it to achieve the best therapeutic effect.

6.Ultrasound-guided closed reduction and kirschner wires internal fixation for the treatment of KilfoyleⅡand Ⅲ medial condylar fracture of humerus in children.

OBJECTIVE:
To explore clinical effectiveness and safety of ultrasound-guided closed reduction and K-wires internal fixation in treating of Kilfoyle Ⅱand Ⅲ medial condylar fracture of humerus in children.
METHODS:
Clinical data of 32 children with medial condylar fracture of humerus treated with closed reduction and internal fixation with K-wires under the guidance of ultrasound were retrospectively analyzed from January 2014 to August 2019, including 23 males and 9 females, age ranged from 3.2 to 12.8 years old with an average of (8.3±2.1) years old;According to classification of Kilfoyle, 12 patients classified to typeⅡ and 20 patients were type Ⅲ;5 patients combined with elbow dislocation;the time from injury to operation ranged from 1 to 5 days with an average of (3.1±1.3) days. Radiological evaluation of treatment results and complications were observed. At the final follow up, Mayo elbow performance score(MEPS) was used to evaluate elbow function. And humerus-ulna angle on the affect side and healthy side were measured and compared.
RESULTS:
All patients were followed up from 8 to 26 months with an average of(19.3±5.5) months. All fractures were healed well, the healing time ranged from 4 to 6 weeks with an average of (4.5±0.5) weeks. No infection, vascular and nerve injury, bone nonunion, trochlear necrosis, cubitus varus or valgus deformity were occurred. According to Mayo scoring, all patients were assessed as excellent. There was no significant difference in angle of humerus-ulna between affectedside (9.5±3.6)° and healthy side (9.1±3.5)°, and no difference in MEPS scores between affected side(95.3±2.5) and healthy side(96.3±2.2)(
CONCLUSION
For Kilfoyle typeⅡand Ⅲ medial condylar fracture of humerus in children, closed reduction and internal fixation with K-wire under ultrasound guidance is a safe and effective method, and could promote in further.

7.Mechanism of Xiaochuanning Granules in Treatment of Bronchial Asthma via PI3K/Akt Signaling Pathway Based on Network Pharmacology and Experimental Verification

ObjectiveTo explore the underlying molecular mechanism of Xiaochuanning granules in the treatment of bronchial asthma based on the network pharmacology and experimental verification through the phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) signaling pathway on ovalbumin (OVA) sensitization-induced bronchial asthma model in rats. MethodThe main active ingredients and targets of Xiaochuanning Granules were screened out from Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) and Bioinformatics Analysis Tool for Molecular Mechanism of Traditional Chinese Medicine (BATMAN-TCM). The targets related to bronchial asthma were obtained from five disease databases such as GeneCards and Online Mendelian Inheritance in Man (OMIM). The common targets were screened out through the Venn diagram. STRING was used to construct the protein-protein interaction (PPI) network of "compound-disease", and Cytoscape 3.8.0 was used to establish a network of key active ingredients of Xiaochuanning granules and core target genes ("ingredient-gene" network). Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses were performed through DAVID. The bronchial asthma model was induced by OVA stimulation in rats. Bronchial and lung tissue inflammation was observed by hematoxylin-eosin (HE) staining, and the enrichment analysis results of the network pharmacology were verified by Western blot. ResultIn this experiment, 232 active ingredients and 4 687 related targets of Xiaochuanning granules were screened out, and 233 common targets of Xiaochuanning granules and bronchial asthma were collected, including eosinophil-derived neurotoxin 1 (EDN1), cyclic AMP response element-binding protein 1 (CREB1), cyclin-dependent kinase inhibitor 1A (CDKN1A), epidermal growth factor receptor (EGFR), mitogen-activated protein kinase 14 (MAPK14), and Akt1. KEGG pathway analysis revealed 186 related signaling pathways, indicating that the PI3K/Akt signaling pathway presumedly played a key role in the treatment of bronchial asthma by Xiaochuanning granules. The animal experiment showed that Xiaochuanning granules relieved the airway inflammation and smooth muscle hyperplasia in rats and down-regulated the gene expression of PI3K and Akt as compared with the conditions in the model group (P<0.05). ConclusionXiaochuanning granules have the characteristics of multi-component, multi-target, and multi-pathway synergistic effect in the treatment of asthma. Xiaochuanning granules may exert anti-inflammatory effects by regulating the expression of genes related to the PI3K/Akt signaling pathway. The present study is expected to provide a theoretical basis for follow-up in-depth research on the complex mechanism of Xiaochuanning granules in the treatment of bronchial asthma.
