Objective: Through analyzing the level, construction and operation efficiency of revenue, to analyze the basic status and change trend of current economics operation of county-level hospitals in this area, provide former basic information for making comprehensive reform evaluation on public hospitals. Methods: To collect data of annual health financial report, collect and analysis the data. Results: The overall level of revenue and expenditure of couty-level hospitals in Hainan increased rapidly , but there were rapid fall in 2012, the balance gap increased obviously; the proportion of financial assistance increased in 2011, while the construction of income and expenditure remained relatively stable. Conclusion: The general economic operation efficiency of county-level hospitals improved, but the comprehensive reforms still has not appeared, which needs further implementation of compensation mechanism for ensuring the successful implementation of comprehensive reform.

To probe into problems in the existing essential medicine system in China and the necessity of legislature in this regard, and propose to build a legal framework for such a system. Objective:To evaluate the therapeutic effect of neoadjuvent chemotherapy for grade Ⅲ and Ⅳ carcinoma in the digestive tracts,Methods: A total of 30 cases of carcinoma in the digestive tracts were collected;including 9 cases in grade Ⅲa,13 cases in grade Ⅲb,and 8 cases in grade Ⅳ admitted to our hospital.Through gastroscope、enteroscope and CT investigation after admission,20 cases had been determined as advanced carcinoma in the digestive tracts before operation.Then the cases underwent chemotherapy twice,were rechecked by CT,and finally accepted operation treatment.The changes of the cancer volume,labelling micovessel index CD105 and the content of the inhibitory protein of apoptosis survivin from excisional tumor tissue were compared after operation. Results: 1.The change of tumorvolumebeforeandafterchemotherapyshowedstatisticalsignificance(P0.05)between intervenient chemotherapy group and systematical chemotherapy group. None died from operation. Conclusion: Surgical therapy after neoadjuvent chemotherapy for advanced cancer in the digestive tracts can improve the rate of exairesis and therapic effect.

Objective:To detect expression of tumor necrosis factor-converting enzyme(TACE)on the mouse uterine epithelial cell and effect of implantation in vitro. To analyze its role in blastocyst implantation and to investigate the relation TACE withimplantation of mouse embryo. Methods:Co-culturing endometrial epithelial cells and blastocyst of the pregnant 4-day mouse to establish the implantation model in vitro. Epithelial cells was identified by Immunocytochemistry. RT-PCR and Immunocytochemistry were used to detect the expression of TACE gene and protein in the endometrial epithelial cells. Different concentrations of TACE antibody were added to the implantation model to observe effect of TACE antibody on mouse blastocyst implantation in vitro. Expression of TACE protein was detected by Immunocytochemistry in each antibody interference system. Results:Endometrial epithelial cells growed vigorously in vitro. Keratin antibody showed positive expression,meanwhile,Vimentin antibody showed negative expression in the endometrial epithelial cells. PCR showed that the expression of TACE gene was high and results of Immunocytochemistry was consisted with PCR. Objective To explore the early diagnosis of placental abruption and the control measures.Methods Retrospective analysis of our hospital from January 2001 to January 2011 during the 62 patients admitted to the placental abruption.Results Gestational hypertension,trauma and sexual intercourse,premature rupture of membranes for the three main causes; clinical manifestations of lumbar pain,vaginal bleeding,abnormal fetal heart rate and fetal movement,Palace board hard and tenderness,bloody amniotic fluid,etc.;take natural childbirth 9 cases,seven cases of vaginal delivery,cesarean section 42 cases,hip midwifery 4;43 cases of postpartum hemorrhage,26 cases of uteroplacental apoplexy,4 routine hysterectomy; normal newborn delivery,31 cases of neonatal asphyxia,22 cases of fetal death in 3 cases,perinatal child death in 6 cases,perinatal child mortality 9.7％.Conclusion Emphasis on pathogenesis and clinical manifestations,combined with auxiliary B ultrasonic examination to help the early diagnosis of placental abruption.Aggressive treatment of hypertensive disorders in pregnancy,to avoid injury prevention and treatment of placental abruption and premature rupture of membranes is an effective measure.

Objective:To investigate the incidence of hypertension and its risk factors among male bus drivers.Methods:A total of 512 male bus drivers in Chongqing were surveyed.Blood pressure,weight and height were examined.Result:The incidence of hypertension among male bus drivers was 23.63%,which was higher than that in general people.The age,family hypertension history and BMI were the main risk factors.Their OR were 2.014,2.490 and 3.530,respectively.Conclusion:Hypertension was the result of the interaction between inheritance and environment.Therefore,the strategy to prevent and control hypertension among bus drivers should be comprehensive.

Carcinomatous meningitis (CM) represents a devastating metastatic complication of malignant tumour, and the incident of CM is increasing in recent years. The diagnosis is made by both cerebrospinal fluid by lumbar puncture and radiological investigation, mainly magnetic resonance imaging. The therapeutic management of CM includes intrathecal administration of chemotherapy, radiotherapy, surgery and palliative care, but the treatment of CM remains controversial Individualized treatment and combined modality therapy are recommended at present, and the utilization of targeted drug may increase therapeutic effect of CM.
