1.Clinical Characteristics, Imaging Findings and Surgical Outcomes of Chiari Malformation Type Ⅰ in Pediatric and Adult Patients

A growing number of children and adolescents are being diagnosed as Chiari malformation type Ⅰ (CM-Ⅰ) for behavioral disorders,developmental delay,seizures,or abnormal orpharyngeal function.The aim of this study was to compare the clinical characteristics,imaging findings and surgical outcomes of CM-Ⅰ in pediatric and adult patients.Between January 2014 and June 2017,84 patients with CM-Ⅰ underwent surgical treatment in our department.We divided the patients into two groups:pediatric group (n=1 1,age <18 years)and adult group (n=73,age ≥18 years).Data on clinical characteristics,imaging findings,surgical outcomes,and prognosis were retrospectively reviewed and compared between these two groups.For clinical presentation,scoliosis (36.4%) and developmental delay (36.4%) were more common in pediatric patients,whereas,sensory disturbance (58.9%) and motor weakness (41.1%) were more common in adult patients.Imaging findings showed that the incidence of hydrocephalus and craniovertebral junctional abnormalities was significantly higher in pediatric group than in adult group (P<0.05).Compared to adult group,pediatric group showed a better improvement or resolution of syrinx and tonsillar herniation after surgical treatments (P<0.05).The total Chicago Chiari Outcome Scale (CCOS) score in pediatric patients at the last follow-up was significantly higher than that in adult patients (P=0.002).In conclusion,the clinical characteristics and imaging findings appeared to be different in pediatric and adult patients with CM-Ⅰ.The surgical outcomes of pediatric patients were shown to be significantly better than those of adult patients.
2.Effects of morphine on K+ currents in caudate nucleus of neonatal rat.

AIMThe effects of morphine on the potassium ionic currents of caudate nucleus neurons of neonatal rat were studied.
METHODSUsing of whole cell voltage clamp technique on caudate nucleus neurons, applied morphine chronically or acutely on it. In order to research the effects of morphine for voltage-gated of potassium ionic currents.
RESULTSThe amplitude of potassium ionic currents are increased by applied morphine acutely in caudate nucleus from (2.6 +/- 0.4) nA to (3.3 +/- 0.5) Na, naloxone can block the effect of morphine on K+ current and the currents are decreased to (2.4 +/- 0.4) nA. If applied morphine in caudate nucleus chronically, the amplitude of potassium ionic currents are increased from (2.6 +/- 0.4) nA to (3.1 +/- 0. 5) nA. After applied naloxone, the currents are decreased to (2.4 +/- 0.4) nA.
CONCLUSIONThe effects of morphine increased potassium ionic currents by micro-opioid receptor mediated and induced the hyper polarization of neurons, leading to inhibition of neural activity.

3.Investigation and analysis of effect of teaching competition on young teachers' teaching ability

Objective To investigate current situation of teaching competition and to put forward some feasible suggestions for improving the teaching competition of theoretical course for young teachers. Methods Questionnaire survey was conducted for 126 teachers including the effect of teaching competi-tion on effective teaching atmosphere,comprehensive teaching results,teaching ability and recommenda-tions for improvement. SPSS software was used to do statistical analysis andχ22 test was applied. P<0 . 05 stands for statistically significant difference. Results Investigation results demonstrated that teaching competition of theoretical course was conducive to forming effective teaching atmosphere,achieving better teaching effect and improving teaching ability of teachers . A χ22 test was performed on the cognition of the cultivating of classroom management ability;results demonstrated that there were significant differ-ences between teachers with more than 10 years education age and those with less than 10 years education age(χ22=4.375,P=0.0365). Conclusions Teaching competition of theoretical course is an effective way to cultivate young teachers teaching ability. Teaching competition should be combined with training and feedback and evaluation should be more scientific and objective in order to improve young teachers' teaching ability.
4.Protective effect of Salvia chinensis Benth. polysaccharides on lipopolysaccharide and D-galactosamine induced acute liver failure in mice

OBJECTIVE To explore the protective effect of polysaccharides from Salvia Chinensis Benth. (PSSC) on lipopolysaccharide (LPS) and D- galactosamine (GalN)- provoked mouse acute liver failure (ALF) and the possible molecular mechanism. METHODS Kunming mice were randomly divided into four groups: normal control, model, model+PSSC 30 and 100 mg·kg- 1 groups. PSCC was given once a day and for a week. To establish an ALF model, mice of model and PSSC groups were ip injected with LPS 10 μg·kg-1 and GalN 700 mg·kg-1 at the end of PSSC treatment. The microscopic structure of the liver was detected by HE staining. Serum and hepatic biochemical parameters of glutamic-oxalacetic transaminase (GOT), glutamic- pyruvic transaminase (GPT), catalase (CAT), superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), malondialdehyde (MDA), and glutathione (GSH) were detected by colorimetric methods. The relative content of hepatic reactive oxygen species (ROS) was measured by DCFH-DA fluorescent probes. Levels of cytokines tumor necrosis factor-α (TNF-α), interleukin 1β(IL-1β) and IL- 6 in the serum and liver were detected by ELISA. Activity of caspase 3 in liver homogenates was detected by aspase 3 activity assay kit. RESULTS Compared with normal control group, in the liver of model group, hepatocytes were arrayed in disorder, cytoplasm of hepatocytes shrank, and boundaries between cells were fuzzy, the infiltration of a large number of inflammatory cells and tissue hemorrhage could be detected, pathological scores were elevated significantly (P<0.01), levels of MDA and ROS in the liver of ALF model mice were elevated to 2.2 and 4.3 times that of the normal control, respectively (P<0.01), the level of GSH decreased to 51% (P<0.01), and the activities of SOD, CAT and GSH- Px declined to 74%, 36% and 42%, respectively (P<0.01). Levels of TNF- α, IL- 1β and IL- 6 in the serum and liver of model group were increased (P<0.01), and caspase 3 activity was increased to 5.3 times that of the normal control (P<0.01). Compared with the model group, the number of surviving mice in PSSC groups increased, liver pathological scores declined (P<0.01), levels of MDA and ROS increased (P<0.01), levels of GSH, TNF-α, IL-1β and IL-6 in the liver and serum declined (P<0.01), and caspase 3 activity decreased (P<0.01). CONCLUSION PSSC is able to alleviate LPS and GalN-induced ALF in mice. Inhibition of hepatic oxidative stress, inflammatory response, and cell apoptosis is possibly implicated in the protective effect of PSSC.
5.Effect of early postoperative enteral nutritional support in abdominal surgery

Objective To evaluate the feasibility and the effect of the early postoperative enteral nutrition on improving the nutritional status, cost of nutrition and hospital stay after operation in abdominal surgery.Methods 48 patients who underwent abdominal operation were allocated into 2 groups: enteral nutrition group(EN group,23 cases) and parenteral nutrition group(PN group,25 cases). It was isonitrogenous and isocaloric intake in both groups. Each day nitrogen intake (0.18±0.02)g/kg,104.6—125.52 kJ(25—30kCal/kg). EN was given at postoperation 8 hours by naso intestinal tube which was placed 30 cm distal to ligament of Treitz or jejunal export loop. PN was provided by peripherally inserted central venous catheters(PICC). The nutritional indices were investigated. A visual analogy scale was used to estimate the infection related complication, cost of nutrition and hospital stay. These parameters were evaluated preoperatively and on the 8th day after operation.Results Early postoperative enteral nutrition showed significant nutrition effect in the patients undergoing abdominal surgery. In the EN group,the nitrogen balance became positive at the postoperative day 4,whereas the nitrogen balance still remained negative in the PN group. The EN group decreased their hospital stay and cost of nutrition compared with that of the PN group(P<0.05). Conclusion The early postoperative enteral nutrition is safe and feasible. It cannot only improve the nutritional status and intestinal function obviously, but also decreased the hospital stay and cost of nutrition compared with parenteral nutrition.
6.Effect of sildenafil on ABR thresholds shift to noise-induced hearing loss in guinea pigs.

OBJECTIVETo investigate the protective effects of sildenafil to noise-induced hearing loss in guinea pigs.
METHODSGuinea pigs were randomly divided into control group, noise exposure group and the sildenafil treatment group, with 10 in each group. One week after the exposure of 110 dB (A) white noise, sildenafil [10 mg/(kg×d)] and normal saline [4 ml/(kg×d)] were injected into guinea pigs of noise plus sildenafil group (NSG) and noise plus normal saline group(NNG) respectively. One week after noise exposure to four weeks continuous administration. ABR thresholds were measured respectively prior to the experiment, 1 week post-noise, 1, 2 and 4 weeks post-drugs. The changes of cochlea hair cells were also observed by scanning electron microscope (SEM).
RESULTSAfter noise exposure, the ABR threshold shifts in NSG were significantly fewer than that in the NNG. An average of 19.1 dB in NNG compared with 19.8 dB in NSG. Four weeks after exposure, the threshold shifts were become larger to 22.0 dB in NNG while become smaller to 4.8 dB in NSG. Compared NNG with NSG, in addition to noise exposure time point, there were statistically significant differences in the rest time points after administration of the ABR threshold (P<0.05). SEM showed that the inner and outer hair cells in NNG displayed mess, fusion and imperfections. In NSG, the hair cells displayed slight pathological changes, there was no significant difference when compared with control group.
CONCLUSIONSildenafil is able to reduce the ascending of ABR thresholds shift, and it can significantly protect against noise-induced hearing loss.

7.Surgical treatment for ventricular septal defect in infants under 5 kg of body weight.

OBJECTIVETo review the indication and timing of surgery,surgical techniques and perioperative management of ventricular septal defect (VSD) repair on infants under 5 kg of body weight.
METHODSFrom January, 2000 to December, 2005, 134 children patients with VSD and associated anomalies, who were under 5 kg of body weight and aged (3.9+/-1.9) months, were diagnosed and treated in our department.
RESULTSOne case died of arrhythmia and heart failure in the early stage of postoperation (mortality 0.7%). The most common postoperative complications were pneumonia (14), pulmonary hypertensive crisis (6), arrhythmia (4), low cardiac output (4), dropsy of thoracic cavity(1). Followed up for 6 m-6 y, the development of all 133 cases was well. Two children with residual shunt got a spontaneous closure confirmed by echocardiography one year postoperatively.
CONCLUSIONThe clinical results of operations on VSD infants with low body weight are satisfactory with attention to the indication and timing of surgery, proper CPB management, rational operations and perioperative management.

8.Efficacy of combination treatment of the inhibitor of phosphatidyl inositol-3-kinase/protein kinase B pathway BEZ235 and the inhibitor of extracellular regulated protein kinase/mitogen-activated protein kinase pathway U0126 in a tumor cell model.

OBJECTIVETo study the inhibitory effect of the dual usage of BEZ235 and U0126, the inhibitor of phosphatidyl inositol-3-kinase/protein kinase B pathway and extracellular regulated proteinkinase/mitogen-activated protein kinase pathway, respectively, on cell proliferation.
METHODSPhosphatase and tensin homolog knockout mouse embryonic fibroblast (PTEN-/-MEF) cell lines were used as the cellular model for malignant tumors. BEZ235, the dual inhibitor of phosphatidyl inositol-3-kinase and mammalian target of rapamycin, and U0126, the inhibitor of mitogen-activated protein kinase were used to treat the cells individually and in a combination manner. The inhibitory effects to cell proliferation were monitored by MTT.
RESULTSBoth BEZ235 and U0126 suppressed PTEN knockout cell proliferation, and their half inhibitory concentrations were 6.257 nmol/L and 22.85 μmol/L, respectively. However, the combination treatment of the two drugs showed antagonistic rather than synergistic effect on cell proliferation.
CONCLUSIONBEZ235 and U0126 are not suitable for a combined target therapy regimen.

9.Is fire needle superior to Western medication for herpes zoster? A systematic review and meta-analysis

Objective:To compare the effectiveness of fire needle versus Western medicine in the treatment of herpes zoster. Methods:Randomized controlled trials comparing fire needle with Western medicine in the treatment of herpes zoster were identified using 8 databases. A meta-analysis was performed using RevMan 5.3 software. Results:Eight trials involving 569 patients were included in this meta-analysis, and the results showed that fire needle was superior to Western medicine comparing the effective rate [risk ratio (RR)=1.13, 95% confidence interval (CI): 1.06 to 1.20;P=0.0002], the visual analog scale (VAS) score [mean difference(MD)=–7.95, 95% CI: –10.71 to –5.20;P<0.00001], time of pain disappearance (MD=–7.61, 95%CI: –9.38 to –5.84;P<0.00001), time of blister-stop (MD=–1.34, 95%CI: –1.51 to –1.18;P<0.00001), time of crusted scab (MD=–2.92, 95%CI: –3.62 to –2.23;P<0.00001), and time of scab off (MD=–4.64, 95%CI: –5.83 to –3.46;P<0.00001). In addition, a significantly lower incidence of postherpetic neuralgia was found in the fire needle group in 30 d (RR=0.23, 95%CI: 0.11 to 0.51;P=0.0002) and 60 d (RR=0.33, 95%CI: 0.12 to 0.91; P=0.03) after treatment. Conclusion:Fire needle has a favorable effect in increasing the effective rate, relieving pain, recovering skin lesions and decreasing incidence of postherpetic neuralgia in the treatment of herpes zoster. However, considering the limitations in this study, the findings should be interpreted cautiously.
10.Analysis on SARS‑CoV‑2 contamination in the environment exposure to the SARS‑CoV‑2 cases in Huangpu District of Shanghai in 2022

Abstract: Objective To analyze the detection of SARS‑CoV‑2 in household environment and public place environment of Huangpu District, and describe the feature of SARS‑CoV‑2 contamination in the environment exposure to the infected cases, so as to support the control strategies such as disinfection and health communication. Methods The results of RT-PCR test for the environmental samples exposure to the cases infected by SARS‑CoV‑2 during February 1 to March 31 2022 in Huangpu District of Shanghai were collected as the research data. Pearson χ2 was used to test the significance of the differences between positive rates of SARS‑CoV‑2 contamination. Results From February 1 to March 31, household environment samples had a higher positive rate (6.47%, 234/3 618) of SARS‑CoV‑2 contamination while the public place samples had a lower one (1.22%, 47/3 582) in Huangpu District of Shanghai (χ2=141.908, P<0.01). Among the household buildings, the lane houses of old style representing poorer living condition had the highest positive rates (8.31%, 96/1 155) of SARS‑CoV‑2 contamination while the apartments representing better living condition had the lowest (3.59%, 22/612) (F=5.25,P<0.05). Among the samples from household environment, samples regarding sewerage had the highest positive rates (13.30%,58/436) of SARS‑CoV‑2 contamination, while samples regarding the tool of cooking and sweeping had the lowest (3.10%,17/548) (F=9.84,P<0.01). Among the samples from public place environment, samples regarding entertainment tools had the highest positive rates (13.33%, 2/15) of SARS‑CoV‑2 contamination, while samples regarding the tool of cooking and sweeping had the lowest (0.62%, 4/646) (F=4.22,P<0.01). Conclusion In the environment exposure to the SARS‑CoV‑2 infected cases, the disinfection, ventilation and cleaning should be intensified strictly. SARS‑CoV‑2's surviving in sewage environment should be evaluation dynamically. More health communication should be pushed to people of poorer living condition.
