3.Changes in visual field and retinal nerve fiber layer thickness after Ex - press glaucoma shunt implantation

AIM:To observe and analyze the change of visual field and retinal nerve fiber layer thickness of primary open-angle glaucoma ( POAG ) after the Ex-press glaucoma shunt implantation.
●METHODS:A total of 14 patients (24 eyes) with POAG were underwent Ex-press glaucoma shunt implantation. Visual acuity, intraocular pressure ( lOP ) , mean defect ( MD) , pattern standard deviation ( PSD ) , retinal nerve fiber layer thickness ( RNFLT ) , and corneal endothelial cell number were collected preoperatively. Visual acuity and lOP were collected in 1wk, 1 and 3mo postoperatively respectively. Also, MD, PSD, RNFLT, and corneal endothelial cell number were collected preoperatively and 3mo postoperatively respectively. Complication, the additional treatment, success rate were analyzed.
●RESULTS:There was no significant difference (P>0. 05) in visual acuity preoperative and postoperative 1wk, 1, 3mo. Visual acuity did not significantly reduce after operation. Compared with preoperative, there was significant decreased (P<0. 05) in lOP after 1wk, 1, 3mo postoperative, respectively. lOP keep stead in postoperative 3mo. The reduction of RNFLT had significant difference (P<0. 05) between preoperative and postoperative 3mo. Compared with preoperation, MD and PSD had no significant difference ( P > 0. 05 ) after postoperative 3mo. Corneal endothelial cell number decrease had significant difference (Z=-2. 585, P=0. 01) between preoperative and postoperative 3mo. Success rate: complete success: 79. 2% ( 19 eyes ); partial success:8. 3% (2 eyes);failure:12. 5% (3 eyes).
●CONCLUSION: Ex-press glaucoma shunt implantation could efficiently decrease the lOP in POAG patients. Although it results in reduction of RNFLT in short time. The visual acuity and visual field keep stable after operation. lt is a safe and effective device for treating primary open-angle glaucoma.
4.A preliminary study on the occlusal contact changes during retention in adolescent patients

Objective To evaluate the changes of the occlusal contact characteristics in adolescent patients during 12 months after active orthodontic treatment Methods Twenty adolescent patients with Hawley retainers after active orthodontic treatment were divided into none occlusal interference group and occlusal interference group. The occlusion of the patients was examined with T-Scan Ⅱ system directly after the appliance removal(Tl) and after an average retention period of 12 months(T2). The changes of occlusal contact characteristics were observed. Results The disclusion time during protrusion, left and right lateral movements reduced significantly. The average disclusion time decreased [ from (1.07 ±0.87) , (0. 91 ± 0.47), (0.76±0.43) s to (0.43 ±0. 25), (0.67±0.41), (0.50±0.27) s] significantly (P< 0.05). The occlusal interference disappeared in 4 patients and 1 patient with occlusal interference showed masticatory muscle symptom. The dynamic occlusion [ from (1. 25 ±1.11), (0.84±0.15), (0.52± 0.49) sto (0.35 ±0.15), (0.36±0.15), (0.33±0.11) s ] improved significantly (P < 0. 05) in none occlusal interference group after retention and no statistical differences were found in the occlusal interference group(P>0. 05). Conclusions The overall dynamic occlusion improved after retention in patients with retainers. The presence of occlusal interference affected the self-improvement process and increased the chance of the disorders of stomatognathic system, such as mandibular abnormal movements. Therefore, functional occlusion evaluation and final detailing were needed before appliance removal.
5.Regularity of drugs compatibility of anti-hepatoma traditional Chinese medicine ancient prescriptions and risk evaluation of anti-hepatoma new drug research and development.

Traditional Chinese ancient prescriptions have been used for treatment of liver cancer for a long history and the scientific and rational compatibility is a great wealth for modern research and development (R&D) of new drugs. The research and development of new drugs are often accompanied with a large investment, a long cycle and a high risk, especially for the anti-tumor drugs R&D which are facing more risks and lower successful rate. In this research, the regularity of compatibility of drugs was analyzed from 124 anti-hepatoma ancient prescriptions by computer program. The results can offer help to the R&D of anti-hepatoma new drugs and reduce the risk of drug screening. In addition, we surveyed 22 companies in this field from six provinces such as Beijing, Shanghai, Tianjin and so on and obtained 240 risk assessment questionaires. Then we used qualitative analysis method to interpret the greatest impacts for the risks in the process of R&D, production and sales of anti-hepatoma new drugs. The study provides a basis for anti-liver cancer drugs R&D researchers, who can take effective measures to reduce the R&D risks and improve successful rate.

Risk monitoring of new Chinese patent anti-hepatoma drugs is tracking recognized risks and residual risks, identifying emerging risk and ensure the implementation of the plan, estimating the process of reducing effectiveness. The paper is mainly through understanding the status of Chinese patent anti-hepatoma drugs, the content, characteristic and analysis method of dynamic risk monitoring, and then select the risk control indicators, collect risk information. Finally, puts forward the thought of anti-hepatoma drugs listed evaluation in our country, and try to establish the model of dynamic risk management of anti-hepatoma drugs.

7.Organization and implementation of aeromedical evacuation of US patients with Ebola virus infection from West Africa

In 2014,serious Ebola fever epidemic broke out in West Africa .Afterwards, the US transported two Ebola patients from Liberia by air, which aroused world wide concern .During the aeromedical evacuation , there were lots of tech-nical problems and management procedures involved .First, the US government toke emergency measures for the patients in a local hospital ,which made evacuation possible .Secondly , the US CDC and other government departments coordinated this matter during and after the evacuation .Finally,upon arrival at home the two patients received careful isolation and treat-ment.Thus, the successful aeromedical evacuation is instructive for us .
8.Cloning and Preliminary Analysis of the Stem Specific Promoter of Sugarcane

Sugarcane stem is an ideal organ for producing foreign pharmaceutical proteins and chemicals by genetic engineering.A perfect promoter driving foreign gene to express strongly and specifically in sugarcane stem is necessary for this purpose.In order to isolate a Sugarcane stem-specific promoter,a fragment of 1968bp nucleotide sequence(Ppst2a)upstream 5' of sugarcane pst2a gene,which was demonstrated to express specifically in sugarcane stem previously was isolated by using chromosomal walking.Bioinformatical analysis of this sequence shows that the sequence contains some typical elements of a promoter.To identify the stem-specific of this promoter,a construct was derived from pCAMBIA1301,which original CaMV 35S promoter was replaced by the 1968bp nucleotide sequence,and named as pCAMBIA1900.Transformations of pCAMBIA1900 and pCAMBIA1301 to leaves and stem pieces of sugarcane were carried out by using particle bombardment.The transient expression of gus showed that the gus expressed specifically in sugarcane with a little higher level compared with CaMV 35S.It is the first report that pst2a promoter is a potential stem-specific promoter which can further be used in transgenes into sugarcane.
9.Preparation and Characterization of Polycolnal Antibody of Galectin-7

Objective To prepare the mouse anti recombinant human Galectin-7 antibody and the antibody was characterized in bladder cancer.Methods The gene coding for Galectin-7 was amplified by PCR from the cDNA of human foreskin cells and cloned into prokaryotic expression vector pET28a.Then the recombinant plasmid pET28a/Galectin-7 was transformed into E.coli BL21 (DE3)and expressed under IPTG induction.The recombinant Galectin-7 was purified through Ni2+-NT agarosegel column and the purified Galectin-7 used as imunogen to imunize the mouse.The titer and specificity of the anti-Galectin-7antibody from the mouse were analyzed by ELISA,Western blot and immunohistochemistry,respectively.Results The recombinant Galectin-7 was successfully expressed and purified,and the polyclonal ani-Galectin-7 antibody was suc-cessfully prepared.The titer of the antiserum was 1∶32 000 by ELISA.Western blot analysis showed this antiboday reacted specifically with Galectin-7.Immunohistochemistry analysis showed the antibody could recognize the native Galectin-7 in the human bladder cancer tissue.Conclusion The preparation recombinant Galectin-7 protein was as immunogen in rabbits.It was successful to produce high titer and high specificity of anti Galectin-7 polycolonal antibody.
10.Effects of optic disc topography and retinal nerve fiber layer thickness measurement by spectral-domain OCT on diagnosis of glaucoma

BackgroundGlaucoma is an optic neuropathy caused by structural damage of the optic nerve,and its early diagnosis is critical for arresting the irreversible damage of visual function. Optical coherence tomography (OCT) allows an early diagnosis of glaucoma by the measurements of the optic disc and retinal nerve fiber parameters. Objective This study was carried out to evaluate the effects of optic disc tomography and the measurement of the retinal nerve fiber layer (RNFL)thickness by spectral-domain OCT on the diagnosis of glaucomatous eye. MethodsIt was a noninterventional, cross-sectionalstudy. The optic disctopographic parameters and total and regional RNFL thickness were measured by RTVue OCT in 62 normal eyes and 67 glaucomatous eyes. The area under the receiver operating characteristic curve( ROC ) was used to assess the ability to differentiate glaucoma eyes from normal eyes of each testing parameter. This trial complied with the Helsinki Declaration and was approved by the Clinical Trial Ethic Committee of Beijing TongrenHospital. All of the participants signed the written informed consent before any medical examination. Results In the comparison of demography ,the ages of patients, the mean deficiency( MD ) and pattern standard difference( PSD ) of perimetry were obviously larger in the glaucoma group, primary open angle glaucoma ( POAG ) group and primary closure-angle glaucoma(PACG) group than those of normal controls( P<0. 01 ). No significant differences were found in the disc area between a total glaucoma group, POAG group or PACG group and normal group ( P =0. 101,0. 741 and 0. 652, respectively) ;however, the average RNFL thickness between normal eyes and glaucomatous eyes were significantly different( 109. 758 μm versus 79. 539 μm, P<0. 01 ). Among the eight regions around the optic disc, the thickest RNFL located at the inferotemporal( 150. 109 μm) and superotemporal( 146. 105 μm) regions in normal eyes,and at the superotemporal( 104. 354 μm) and inferotemporal( 102. 436 μm) regions in glaucomatous eyes. Both in normal and glaucomatous eyes,the thinnest RNFL located at the nasal(NU+NL) and temporal(TU + TL) regions. For optic disc topographic parameters,the highest ROC were observed in rim volume( ROC--0. 850,0. 841 and 0. 862 in total glaucoma,POAG and PACG, respectively) and vertical cup/disc ratio( ROC =0. 840,0. 849 and 0. 830 in total glaucoma,POAG and PACG,respectively), and the sensitivities for specificity cutoff set at 80% were 73.1% and 76. 1% in total glaucoma,73.0% and 81.1% in POAG and 73.3% and 70.0% in PACG, respectively. For RNFL thickness ,the highest ROC was observed in average RNFL( ROC =0. 925,0. 910 and 0. 942 in total glaucoma, POAG and PACG,respectively) ,and the sensitivities for specificity cutoff set at 80% were 89. 6% ,89.2% and 90. 0% in total glaucoma,POAG and PACG, respectively. Among the eight regions around the optic disc, RNFL thickness of region IT achieved the highest ROC, RNFL thickness of region TU and TL had the lowest ROC. Conclusions RTVue OCT appears to be of fair discriminating ability in distinguishing normal from glaucomatous eyes. RTVue OCT shows promise for the diagnosis of glaucoma.
