1.Relationship between the fever caused by infection and the damage in acute cerebral infarction

ObjectiveTo investigate relationship between the temperature rising caused by infection in acute period of cerebral infarction and the prognosis.Methods68 patients with temperature rising caused by infection in acute period of cerebral infarction was observed as the observation group.The level of neurological deficit, space of infarct, location and prognosis in the study group was compared with normal temperature as the control group.ResultsThere was significant difference between observation group and control group in the level of neurological deficit,space of infarct,location and prognosis(P<0.01).ConclusionFever caused by infection can deteriorate cerebral ischemia.
2.Study on the factors of heparin potency determined by sheep plasma method and methods improved

Objective To study on the factors of sheep plasma method detect heparin potency and improved methods.Methods Acorrding to sheep plasma method, to study the bubbles, solids, pH and NaCl concentration on the heparin potency determination.Results The bubbles impact on the heparin potency determination was less.Solids were the main factors on heparin potency determination, containing solids protein hydrolysates heparin potency determination was (2.87 ± 0.01 ) U/mL, after removing solids potency was 24.60 U/mL, raised about 8.5 times.In the removed solids sample, when pH 7.5 to 9.5 had little effect on heparin potency determination, and pH 6.5 and 7.0 had smaller effect on the heparin potency determination.The presence of solids in the sample, as the sample pH rised, heparin potency determination also rised, at potency of sample at pH 9.0 was 3.4 times than that at pH 6.5.Enzymatic hydrolyzate mainly contain: 47.86% protein, 39.16% NaCl, 4.12% fat, wherein the protein was 91.34% of small peptides.NaCl concentration had no effect on the heparin potency determination.The improved method of recovery assay value was 98.92%~101.53%.Conclusion The method is reliable, the heparin output value calculated by measuring value in accordance with the process of practical production output.This method can be used for the determination the production process of each link in heparin potency of heparin.
3.Change of alveolar epithelial type Ⅱ cells and pulmonary surfactant protein A in young rats with acute lung injury

Objective To study the temporal changes of alveolar epithelial type Ⅱ cells and surfactant pro-tein A in young rats with acute lung injury induced by lipopolysaecharide. Method Totally 110 SD young rats (male:53, female : 57) were randomly divided into ALI and normal control groups (six subgroups in each group).LPS(4 mg/kg) was given intraperitoneally in ALI group. The same amount of normal saline was given in the con-trol groups. Eight rats in each subgroup were sacrificed at 6, 12, 24, 36, 48 and 72 hours after the injection.Lung samples were taken for transmission electron microscope examination. RT-PCR was epmloyed for the mea-surement of SP-A mRNA. Western blot was used for the detection of SP-A in the lung tissue. ANOVA and homo-geneity of variance test were performed by SPSS 12.0. Results The microvilli disappeared at 24 hours after the injection of LPS. The number of lamellar body (LBs) was provisionality increased at 24 hours and 48 hours. The ring-like an'angement of LBs around nucleus and the giant LB with vacuole-like deformity were found as the main characteristics of AEC- Ⅱ in ALI at 48 hours. The number of LBs reduced and broken and residual LB remained at 72 hours. SP-A elevated greatly from 24 to 48 hours (P < 0.01), reached peak at 36 hours (6.94 ± 0.80, P <0.01),reached the lowest level(3.87 ±0.50, P <0.01)at 72 hours. Conclusions The pathological changes of AEC-Ⅱ and SP-A in lung tissue wiht ALI are time-dependent. The typical alterations of AEC- Ⅱ occurs at 48 hours accompanied by the compensatory increase of SP-A. AEC- Ⅱ is seriously injuried with the typical changes of LBs and the diminishing of SP-A in lung tissue.
4.Preparation of glycerol reference material

Objective To prepare a glycerol reference material.Methods The material was prepared and characterized according to the primary standard substance technological specification(JJG 1006- 1994).Glycerol was dissolved in water containing 0.5% sodium azide and dispersed to glass ampules.The homogeneity and stability of this material were tested with an HPLC method.Glycerol concentration was determined by a titration method as specified in the Pharmacopoeia of China.Results The three time measuring result of glycerol reference material was 1.297 5?0.014 3,1.302 0?0.008 9,1.313 7? 0.007 8,respectively.Statistical analysis showed that this material was homogeneous (F=1.462,P=0.166) .It was stable for at least 4 years at 4℃.The assigned reference value was 0.103 6 g/g and the expanded uncertainty was 0.000 4 g/g.Conclusions This material meet the technical requirements of national primary standard reference material.It is approved as the Certified Reference Material (GBW 09149) by General Administration of Quality Supervision,Inspection,Quarantine of the People's Republic of China in May,2006.
5.The value of CT annular calcification in identifying benign and malignant thyroid nodules

Objective To evaluate the value of annular calcification in CT in the diagnosis and differential diagnosis for benign and malignant thyroid nodules.Methods CT findings of 67 nodules in 67 patients pathologically diagnosed with annular calcifications were retrospectively analyzed to identify 49 benign nodules and 18 malignant nodules.The interior or boundaries of annular calcification before and after contrast-enhancement were compared,and the degree of enhancements of both interior annular calcification and thyroid tissues were observed.After contrast-enhancement,the numbers of lesions showing clearer boundaries and higher degree of enhancement were summarized.Statistic analysis was conducted by using x2 test.Results Among 67 nodules with annular calcifications,clearer boundaries after contrast-enhancement were observed in 61.2% (30/49) benign nodules and 16.7% (3/18) malignant nodules,showing significant statistical difference (x2 =10.457,P < 0.05).The sensitivity and specificity of clearer edge after contrast-enhancement for benign nodules were 61.2% (30/49) and 83.3% (15/18) respectively.Higher degree of enhancement were observed in 16.3% (8/49) in benign nodules and 0(0/18) malignant nodules,showing no significant statistical difference (x2 =3.337,P > 0.05).The sensitivity and specificity of higher degree of enhancement for benign nodules were 16.3% (8/49) and 100.0% (18/18) respectively.The combination of clearer boundaries and higher degree of enhancement after contrast-enhancement was observed in 77.6% (38/49) benign nodules and 16.7% (3/18) malignant nodules,showing significant statistical difference (x2 =20.549,P < 0.05).Meanwhile,the sensitivity and specificity of such combination for benign nodules were 77.6% (38/49) and 83.3% (15/18) respectively.Conclusions CT is important in the diagnosis of thyroid nodules with annular calcification.Clearer boundary after contrast-enhancement and its combination with higher degree of enhancement are helpful for the diagnosis of benign nodules.Boundaries that similar to or more obscure than that of plain scan indicate malignant nodules.
6.Value of CT hyperenhancement sign in diagnosis and differential diagnosis of benign and malignant nodules in thyroid

Objective To assess the value of CT hyperenhancement sign in diagnosis and differential diagnosis of benign and malignant thyroid nodules.Methods CT findings of 2926 nodules in 1676 patients were retrospectively analyzed,among which 2174 nodules were benign and 752 nodules were malignant.All the patients had pathological diagnosis.The degrees of enhancement were divided into hyperenhancement and iso/hypoenhancement.The distribution of hyperenhancement in benign and malignant nodules were summarized.The sensitivity,specificity,positive and negative predictive value and accuracy of hyperenhancement for benign nodules and adenomatoid nodules were observed.Results Hyperenhancement was more common in benign nodules than in malignant nodules (10.3% vs 0.7%,x2 =70.259,P < 0.05),and its sensitivity,specificity,positive and negative predictive value and accuracy was 9.8%,99.5%,98.2%,27.6% and 32.8%,respectively.Hyperenhancement was more common in adenomatoid nodules than in nonadenomatoid benign nodules (67.4% vs 6.1%,x2 =525.025,P < 0.05),and its sensitivity,specificity,positive and negative predictive value and accuracy was 67.4%,93.9%,41.6%,97.8% and 92.3%,respectively.Conclusion Hyperenhancement sign is an important sign which can effectively differentiate benign nodules from malignant nodules,and adenomatoid nodules from nonadenomatoid benign nodules,and it is helpful for diagnosis of benign nodules and adenomatoid nodules.
7.Anti-platelet aggregation bioassay based quality control for XST capsules.

A in vitro platelet aggregation bioassay was developed for the quality control of XST capsules. The in vitro anti-platelet aggregation effect in rats was observed to detect the bioactivity of XST capsules. Panax notoginseng saponins and Xuesaitong lyophilizedpowder for injection were taken as standard control substances to determine the potency. According to the results, XST capsules showeda significant inhibitory effect on thrombin-induced platelet aggregation in a dose-dependent manner. The in vitro anti-platelet activity oflyophilized powder for injection was stabler than that of Panax notoginseng saponins, and so suitable to serve as a standard control substance. The biological potency of XST capsules compared with standard control substance was detected by using parallel line assay. According to the results, the established bioassay method had a good repeatability (RSD 2.92%). The sample test results could pass thereliability test(linear deviation P > 0.05, parallel deviation P > 0.05). This bioassay method could be used as one of the complementary quality control methods for XST capsules.

8.Preliminary investigation of the molecular mechanisms of imipenem-resistance in clinical isolates of Acinetobacter baumannii in Xi'an.

OBJECTIVETo investigate the molecular mechanism of carbapenem resistance in the clinical isolates of Acinetobacter baumannii from Xi'an and their profile of carbapenemase production.
METHODSA total of 146 Acinetobacter baumannii strains were isolated from 6 general hospitals in Xi'an. Antimicrobial susceptibility test was performed for all the strains, followed by detection of imipenem resistance using E-test for metallo-beta-lactamase (MBL) and NaCl inhibition test for OXA type carbapenemase. Bla(OXA-23)and bla(OXA-58) were amplified by PCR, and the positive products were sequenced.
RESULTSFrom the collected strains, 15 non-repetitive imipenem-resistant Acinetobacter baumannii strains were identified, among which 14 yielded negative results in E-test for MBL production. All the resistant strains showed increased sensitivity to imipenem after NaCl inhibition, suggesting the presence of carbapenemase production. Eleven of the strains harbored OXA -23 type gene and 1 harbored OXA -58 type gene. The concordance rate of the results by NaCl inhibition test and PCR was 85.7%.
CONCLUSIONSProduction of OXA-type carbapenemase is the most important reason for carbapenem resistance in Acinetobacter baumannii in Xi'an. The OXA-58 type gene is a novel carbapenemase genotype in China. NaCl inhibition test is a convenient and cost-effective method for detecting carbapenemase in Acinetobacter baumannii.

9.Relationship between fractional esterification rate of high density lipoprotein cholesterol and coronary artery disease.

OBJECTIVETo assess the relationship between fractional esterification rate of high density lipoprotein cholesterol (FER(HDL)) and coronary artery disease.
METHODSA total of 131 hospitalized patients underwent coronary angiography due to chest pain were included in the study and patients were divided into CAD group (n = 76) and non CAD group (n = 55) according to coronary angiogram. Clinical and laboratory data including biochemical laboratory, FER(HDL) and lipid subclasses were analyzed.
RESULTSThe FER(HDL) value of CAD group was significantly higher than that of the non CAD group (21.70 ± 8.73 vs. 18.65 ± 6.26, P < 0.05). There was an increased trend of FER(HDL) with numbers of diseased coronary arteries, significant difference was evidenced between non CAD group and 3-vessel group (18.65 ± 6.26 vs. 24.00 ± 9.22, P < 0.05). FER(HDL) was positively correlated with TG (r = 0.647, P < 0.001), LDLb-C(r = 0.441, P < 0.001) and negatively correlated with HDL-C (r = -0.708, P < 0.001) and HDL(2)-C (r = -0.748, P < 0.001).
CONCLUSIONOur data showed that the values of FER(HDL) were significantly increased in CAD patients and correlated with the severity of the CAD.
