1.Therapeutic Efficacy of Xinyi Biyan Pills Combined with Budesonide in the Treatment of Allergic Rhinitis and Its Effects on Related Inflammatory Factors

OBJECTIVE:To observe therapeutic efficacy of Xinyi biyan pills combined with budilaide in the treatment of aller-gic rhinitis and its effects on related inflammatory factors. METHODS:A total od 120 patients with allergic rhinitis were randomly divided into observation group and control group,with 60 cases in each group. Control group was treated with Budesonide nasal spray,spraying once each nostril(64 μg/spray),2 times a day. Observation group was additionally treated with Xinyi biyan pills 3 g,3 times a day. Treatment course lasted for 4 weeks. Clinical efficacies of 2 groups were observed and compared. The changes of total score of clinical symptoms and signs were observed before and after treatment. The contents of IgE and LTE4 were deter-mined,and the occurrence of ADR was recorded. RESULTS:Three patients of observation group and 4 patients of control group fell of the study. Total response rate of observation group was significantly higher than that of control group,with statistical signifi-cance (84.21% vs. 73.21%,P<0.05). Before treatment,there was no statistical significance in total score of clinical symptoms and signs,IgE and LTE4 levels between 2 groups (P>0.05). After 2 and 4 weeks of treatment,total score of clinical symptoms and signs,IgE and LTE4 levels in 2 groups were significantly lower than before treatment,and the observation group was signifi-cantly lower than control group,with statistical significance (P<0.05). No obvious ADR was found in 2 groups. CONCLU-SIONS:Xinyi biyan pills combined with budesonide show good therapeutic efficacy for allergic rhinitis,improve clinical symp-toms and signs significantly and reduce inflammatory factors levels with good safety.

Objective To study the expression of phosphorylated myosin light chain kinase(P-MLCK) in human pulmonary arterial endothelial cell(HPAEC) induced by lipopolysaccharide(LPS). Methods HPAECs were cultured in vitro and treated with LPS(2 ?g/mL) and normal saline for 1 h,respectively.Immunofluorescence method and western blotting were used to detect P-MLCK. 4.Gender-related differences in metabolism of the enantiomers of trans tramadol and trans O-demethyltramadol in rat liver microsomes.

AIMTo investigate the gender-related differences in the metabolism of trans tramadol (trans T) enantiomers and the glucuronidation of trans O-demethyltramadol (M1) enantiomers.
METHODSIn vitro, trans T or M1 were separately incubated with liver microsomes of male or female rats. The concentrations of the enantiomers of trans T and M1 were determined by an HPCE method.
RESULTSCompared with (+)-enantiomers, (-)-trans T was preferentially metabolized, and (-)-M1 was produced faster in rat liver microsomes. (+)-M1 and (-)-M1 were preferentially glucuronidated in the liver microsomes of male and female rats, respectively. Compared with those in male rat liver microsomes, the enantiomeric ratios of CLint for M1 formation and M1 glucuronidation were more deviated from 1 in female rat liver microsomes.
CONCLUSIONIn vitro, trans T metabolism, M1 formation and M1 glucuronidation were found to be stereoselective in rat liver microsomes. There were gender-related differences in the stereoselectivity in M1 formation and M1 glucuronidation, with a larger extent in female rat liver microsomes.

5.Design, synthesis and antiproliferative activity in cancer cells of novel 18β-glycyrrhetinic acid derivatives.

To investigate the anticancer effects of ring C in 18β-glycyrrhetinic acid (GA), a series of GA derivatives featured with 9(11)-ene moiety in ring C were designed and synthesized. The structures were confirmed by IR, LC-MS and 1H NMR. Their inhibitory effects towards human prostate cancer PC-3 and leukemia HL-60 cell lines were determined. Most of the derivatives displayed stronger antiproliferative activities than GA. Particularly, compound 14 showed promising anticancer activity with the GI50 values of 4.48 µmol · L(-1) and 1.2 µmol · L(-1) against PC-3 and HL-60 cells respectively, which is worth further study.

Objective To evaluate the safety of interventional radiotherapy with X-ray angiography. Methods To compare X-ray radiation levels with and without radioprotect with X-ray protective monitor by simulating operators' body's parts. Results The difference is significant. Conclusion Interventional radiotherapy with angiography with effective protect is safe.
7.Clinical Features and Outcomes of Cardiac Resynchronization Therapy in 16 Patients With Dilated-phase Hypertrophic Cardiomyopathy

Objective: To analyze the clinical features and outcomes of cardiac resynchronization therapy (CRT) in patients with dilated-phase hypertrophic cardiomyopathy (DHCM). Methods: A total of 16 DHCM patients received CRT in our hospital from 2007-03 to 2016-01 were retrospectively studied to analyze their clinical features and outcomes. Results: There were 12 male and 4 female patients at the mean age of (53.3±13.5) years. Pre-operative QRS duration of ECG was (158.7±32.2) ms, left ventricular ejection fraction (LVEF) was (33.6±6.3) %, the patient with NYHA class I, II, III and IV were 1, 5, 8 and 2 respectively. 13 patients received new CRT device, 3 received upgraded device and 8 (50%) combining atrial fibrillation (AF). The patients were followed-up for (2.56±2.13) years, 5 of them died including 3 of heart failure, 1 of sudden death and 1 of stroke. At 6 months follow-up time, 7 patients had the response to CRT which was defined by the improvement of NYHA class≥1 and the absolute elevation of LVEF≥5%; NYHA class improved from (2.69±0.79) to (2.38±0.89), P=0.02; LVEF increased from (33.6±6.3) % to (40.03±9.83) %, P=0.01. Conclusion: DHCM patients with CRT indication had the higher incidence to suffer from AF, those were more in patients with traditional pacemaker or ICD upgrading. DHCM patients with CRT had the poor general prognosis, while there was still certain proportion of patients had the response to CRT.
8.Relative factors in home safety evaluated by multilevel statistical models

Objective To understand the situations of home establishments and the related domestic behaviors that causing injuries. Methods A total of 9760 families with regular residents in a community in Shanghai were investigated. Questionnaire was designed bascd on the Guidelines for Conducting Community Surveys on Injuries and Violence and International Classification of External Causes of Injuries. Results Inside the home settings, relative factors were found on issues as fire protection and using of electricity. In terms of household settings, 14.85% of the families had smoke alarm systems in the kitchen; 40.75% had no windows set for emergence. 50% to 70% of the residents had the idea of safe behaviors, including 35.93% of the families stored cleaning products,other chemicals or medical substances in alternative containers, such as used drinking bottle. Only 1.81% of the people being investigated thought that home was also a high risk place where injury might occur and it was placed number 9 in a multiple choice questionnaire. Data from the multiple level model analysis showed that factors as number of family members, space, education, profession etc. were closely related to the situation of home settings and their resident's behaviors. Conclusion Many injury related factors were found related to home establishments and people' s daily behaviors at home which called for interventions to be taken.
9.Arthroscopically minimal invasion asisted percutaneous cannulated screw fixation of longitudinal fracture of the patella

Objective To evaluate the effectiveness of arthroscopically assisted percutaneous reduction and internal fixation with eannulated screws.Methods The fracture was reduced by closed manipulation or percutaneous leverage force by using the Kirschner wire.Then the patella was temporarily fixed by using a large size towel clamp or Kirschner wires.Under the guidance of knee arthroscopy,a micro-incision was made at the size of cannulated screw placement,the pilot holes were drilled at a proper depth,and the thread was configured.Two titanium screws were inserted parallelly.Results Following-up chekups for 4~24 months in 18 cases showed a satisfactory recovery of knee functions.According to the Bostman' standard,excellent effects were obtained in 16 cases and good effects in 2 cases.Conclusion Treatment of patellar fractures by percutaneous cannulated screw fixation under arrhroscope of- fered advantages of minimal invasion,accurate reduction,reliable fixation,and excellent recovery of joint functions.
10.Predictor Analysis for Super-response to Cardiac Resynchronization Therapy

To analyze the predictors and prognosis for super-response to cardiac resynchronization therapy (CRT) in patients with different etiology. Methods: A total of 181 patients received CRT in our hospital from 2012-01 to 2016-01 were enrolled. The patients were divided into 3 groups: Non-response group, n=63, Response group, n=62 and Super-response group, n=56. The patients were followed-up at 6 months after CRT. Results: There were 30.9% (56/181) patients having super-response. Compared with the other 2 groups, Super-response group had more patients with NYHA II-III and less NYHA IV, the patients were with the smaller LAD, LVESD, LVEDD andless patients had CRT-D implantation. The baseline cardiac function was obviously improved at 6 months after CRT in all 3 groups. The basic LVEDD, LVESD, CRT-D implantation, non-ischemic cardiomyopathy (NICM) and NYHA IV were the independent predictors for super-response occurrence. In addition, compared with ischemic cardiomyopathy (ICM), NICM patients had the higher ratio for super-response occurrence (37.6% vs 7.5%), P<0.001. Survival analysis indicated that NICM patients had the lower risk of all cause mortality (HR=0.31, 95% CI 0.14-0.80), cardiac death (HR=0.27, 95% CI 0.09-0.48) and combined endpoints (HR=0.36, 95% CI 0.27-0.78). Conclusion: At baseline condition, the patients with less degree of left ventricular reconstruction, CRT-D implantation, NICM and NYHA IV had more chance to suffer from super-response after CRT. NICM patients had the better response and prognosis to CRT.
