OBJECTIVETo evaluate therapeutic effect of acupuncture at "Kaiyin point No. 1" on submucosal hemorrage of vocal cords.

METHODSOne hundred and sixty cases were randomly divided into a test group and a control group. The test group were treated by acupuncture at "Kaiyin point No. 1" with reducing method in reinforcing or reducing method by manipulating the needle in cooperation with the patient's respiration, combined with deep respiratory movement of the glottis laryngeal cavity, once daily; and the control group by spray inhalation of western medicine, once daily, 7 days constituting one therapeutic course.

RESULTSThe cured and markedly effective rate and the total effective rate were 85.0% and 97.5% in the test group and 60.0% and 85.0% in the control group respectively, with a significant difference between the two groups. After treatment of the test group,the acoustical parameters maximum phonation time (MPT), frequency perturbation quotient (FPQ), amplitude perturbation quotient (APQ), ratio of harmonic to noise (H/N) objectively reflected improvement of voice quality.

CONCLUSIONAcupuncture at "Kaiyin point No. 1" as main method is an ideal therapy for submucosa hemorrhage of vocal cords.

Objective To construct a decision-making aid for rehabilitation training in patients with ischemic stroke, providing structured guidance and support for patients'decision-making. Methods Based on literature review and semi-structured interviews, a decision-making aid for rehabilitation training in patients with ischemic stroke was initially developed. The Delphi expert consultation method was used to conduct a two-round consultation for 15 experts. Results The response rate and effective rate of the questionnaires in the two rounds of expert consultation were both 100%, indicating a high enthusiasm among the experts; the authority coefficients of the experts in the two rounds of consultation were 0.88 and 0.89, respectively, indicating a high level of expert authority; the Kendall's W coefficients of concordance in the two rounds of expert consultation were 0.298 and 0.398, respectively(P < 0.001), indicating a high degree of consistency among expert opinions. The final decision-making aid for rehabilitation training in patients with ischemic stroke included 4 primary indicators, 9 secondary indicators, and 25 tertiary indicators. Conclusion The decision-making aid for rehabilitation training in patients with ischemic stroke constructed in this study is both scientific and practical, and can help improve the quality of decision-making of patients.

Background Histone deacetylase inhibitors can regulate gene expression through modulation of the degree of acetylation of histone and non-histone,thus affecting cell proliferation,survival and chemosensitivity.Histone deacetylase inhibitors combined with paclitaxel may enhance the inhibitory effect of drugs on lung cancer cells.This study aimed to observe the effect of trichostatin A (TSA)/paclitaxel on the proliferation and apoptosis in human A549 lung adenocarcinoma cells,and to investigate its mechanism.Methods A549 cells were cultured in Dulbecco modified Eagle's medium (DMEM) in the presence of paclitaxel and the histone deacetylase inhibitor TSA,and the growth curve was obtained by trypan-blue exclusion assay and cell count.Apoptosis was assessed using Hoechst 33258 staining and flow cytometry analysis,and cell cycle was detected by flow cytometry analysis.The proteins poly ADP-ribose polymerase (PARP),caspase-3,survivin,and tubulin acetylation were detected by Western blotting.Results A significant reduction of proliferation was observed in A549 lung adenocarcinoma cells treated by paclitaxel or TSA.Combined treatment with TSA/paclitaxel caused the greatest inhibition of cell proliferation.The combined treatment with TSA and paclitaxel induced more severe apoptosis,and significantly more cells were arrested in Gz/M phase (P ＜0.05) then with a single drug.Using Western blotting,we demonstrated that treatment with TSA/paclitaxel led to synergistic increase in acetylated tubulin,PARP,caspase-3,and reduced the expression of survivin.Conclusion TSA and paclitaxel have a synergistic activity that can inhibit cell growth and induce apoptosis.

Objective To investigate the technological feasibility,efficacy and morbidity of cr guided ~(125)Ⅰseed implantation for recurrent rectum cancer.Methods Twenty-three patients with recurrent rectum cancer were treated with cr guided interstitial ~(125)Ⅰseed brachytherapy.In 20 patients the procedure was performed under epidural anesthesia and 3 patients under local anesthesia.Treatment planning system was used to calculate the number of seeds,the space distribution and the introduction of the seeding needles.Matched peripheral dose (MPD) of ~(125)Ⅰseed implantation ranged from 90-120 Gy for patients who had had external radiotherapy,and 140- 160Gy for those who had not.The planning target volume(PTV)was clinical target volume(CTV)plus 1 cm margin.The range of radioactivity of the ~(125)Ⅰseeds was 18.5-25.9 MBq.All these 23 patients had CT scan at 5 mm intervals after implantation for quality evaluation,together with routine chest,pelvic X-ray films within 24-48 hours after seed implantation.Three patients received three-dimensioual conformal radiation therapy(3DCRT) to a total dose of45-50 Gy,with 2-3 Gy/f.Follow-up time was from 3 to 28 months.Results All patients was able to tolerate seed implantation well.Complete pain relief was observed in 12/15,and partial relief in 2/15 and no response in 1/15,with a response rate of 93%.The local control rate was 87%.The 1-and 2-year survival rate was 93% and 50% respectively.Two of four patients have died of dissemination to the lung after 8 and 12 months. One seed has migrated into the pelvis without causing any untoward morbidity.Conclusion CT guided ~(125)Ⅰseed implantation for recurrent rectum cancer is safe,minimally invasive,causing only mild morbidity.It possesses a high efficacy,yet it should be given in combination with external beam radiation and chemotherapy,should distant metastasis be observed.

Objective To observe the combined poisonous effects of fluoride and aluminum on sex hormone of male rats.Methods Sixteen weaned SD healthy male rats aged two week were selected and divided into control group,aluminum group,fluoride group,fluorine-aluminum group,four rats in each group.All rats in the experimental groups were fed with corn collected from the prevailng areas containing different fluorine contents respectively for 90 days.Serum testosterone(T)and estradiol(E2)were detected.Results Compared separatelv with the control group[(3.317±0.635)μg/L],serum T level of fluorine-aluminum group[(15.994±6.558)μg/L]was higher(P＜0.05),but aluminum[(8.134±3.134)μg/L]and fluorine[(1.868±0.367)μg/L]groups had no significant differences(P＞0.05).Compared separately with the control group[(0.319±0.072)nmol/L],E2 level of the fluorine group[(0.172±0.030)nmol/L]being lower(P＜0.05),and it was not significant differences(P＞0.05)in the control group when compared with aluminum group[(0.282±0.012)nmol/L],and fluorine-aluminum group[(0.265±0.047)nmol/L].Fluorine and aluminum interacted with each other(F=9.82,P＜0.05).Conclusion The combined poisonous effects of fluorine and aluminum may influence sex hormone levels of male rats.

Objectives To investigate the relationship between single nucleotide polymorphism (SNP)H558R in SCN5A gene and chronic Keshan disease (KSD) complicated with hypertension,and the relationship between H558R and occurrence of arrythmia in chronic KSD complicated with hypertension.MethodsThirty nine patients with chronic KSD complicated with hypertension and 63 geographical region matched hypertension control subjects were recruited in our study in Fuyu county,Qiqihaer city,Heilongjiang province between 2006 and 2010.H558R polymorphism in case and control groups was genotyped using the polymerase chain reaction single-strand conformation polymorphism(PCR-SSCP) and sequenced,and electrocardiography(ECG) characteristics were examined in the two groups.Case-control study analytical methods were applied to analyze the relationship between H558R and chronic KSD complicated with hypertension,and the relationship between H558R and occurrence of arrythmia in chronic KSD patients complicated with hypertension.Results Subjects of genotype 558 TC in the case group had a decreased risk of chronic KSD complicated with hypertension with odds ratio of 0.288[95％ confidence interval (CI):0.104 - 0.794],and subjects of genotype TC in chronic KSD complicated hypertension patients had a decreased risk of QRS prolongation with odds ratio of 0.061 (95％CI:0.006 - 0.612).Conclusions Polymorphism H558R in SCN5A gene may be a predisposition factor of chronic KSD complicated with hypertension and occurrence of arrythmia in chronic KSD complicated with hypertension.

Objective To investigate the effects of HMGB1 small interference RNA (siRNA) on retinal vascular endothelial cells.Methods siRNA was used to inhibit the expression of HMGB1,followed by the application of CCK8 assay,Hochest33342 staining and flow cytometry to observe the effects of HMGB1 siRNA on retinal vascular endothelial cells in high glucose environment.Meanwhile,the expression of proteins related to apoptosis was detected by Western blot.Results The transfection of HMGB1 siRNA down-regulated the protein expression level of HMGB1 by 73％ in siRNA group compared with normal control (NC) group (P ＜ 0.05),and the protein expression level of HMGB1 in siRNA group was decreased by 75％ compared with scr-siRNA group (P ＜ 0.05).There was no significant difference between NC group and scrsiRNA group (P ＞ 0.05).The total apoptotic rate of NC group,high-glucose group,scrsiRNA group and siRNA group was (0.40 ± 0.03)％,(49.80 ± 3.50)％,(47.60 ±1.98) ％ and (23.60 ± 2.40) ％ by flow cytometry.Compared with NC group,the apoptotic rates of high-glucose group,scr-siRNA group and siRNA group were increased (all P ＜ 0.05).Compared with scr-siRNA group,the apoptotic rate of HRECs in siRNA group was reduced,with significant statistical difference (P ＜ 0.05).There was no significant difference in the rate of cell apoptosis between scr-siRNA group and high-glucose group (P ＞ 0.05).Compared with the NC group,the protein expression level of cleavedcaspase3 protein in high-glucose group and scr-siRNA group were increased by (233 ±10) ％ and (266 ± 22) ％,respectively (both P ＜ 0.05);compared with scr-siRNA group,the protein expression level of cleaved-caspase 3 in siRNA group was reduced by (43 ±3) ％ (P ＜ 0.05);and there was no significant difference in the protein expression of cleaved-caspase 3 in high-glucose group and scr-siRNA group (P ＞ 0.05).Compared with the NC group,the protein expression of B-cell lymphoma-2 (Bcl-2) in high-glucose group and scr-siRNA group was decreased by (32 ± 2) ％ and (29 ± 3) ％,accordingly (both P ＜ 0.05);compared with scr-siRNA group,the protein expression level of Bcl-2 in siRNA group was increased by (42 ± 2) ％ (P ＜ 0.05);and there was no significant difference in the protein expression of Bcl-2 in high-glucose group and scr-siRNA group (P ＞ 0.05).Conclusion HMGB1 siRNA can reduce the apoptosis of retinal vascular endothelial cells in high glucose environment by inhibiting the activation of cleavedcaspase3 and increasing the expression of Bcl-2.

Background:Successful antiretroviral therapy (ART) has been demonstrated to be effective in reducing the infectivity of human immunodeficiency virus (HIV).We conducted a study to predict the potential effect of ART on the spread of HIV in Chaoyang District,Beijing,China,using the Asian Epidemic Model (AEM).Methods:The AEM baseline workbook was used to determine the current infection status and to project the future spread of HIV under current conditions.We changed the input on the ART coverage from 2014 to 2025 and also modified the treatment eligibility in the AEM intervention workbook,in order to allow for analysis of the projected downstream impact of ART.Results:By gradually increasing the ART coverage rate from 29.7％ (rate of 2013) to 40.0％,50.0％,60.0％,70.0％,80.0％,and 90.0％ (at CD4+ ≤350 cells/μl),and by changing the dates of coverage from 2014 to 2020,the number of new infections showed a cumulative decline of 0.60％,1.59％,2.94％,5.33％,9.32％,and 14.98％,respectively.After 2020,the projected rates of infection rebounded slightly,so with the exception of the years with very high coverage (90.0％),new infections continued to decrease.When we changed the initial threshold of therapy to CD4+ cell counts ≤500 cells/μl,new infections decreased 6.00％,11.64％,15.92％,2 1.11％,26.92％,33.05％,and 38.75％,respectively,under varying ART coverages.Conclusion:Our study demonstrates that the early initiation of ART for people living with HIV/acquired immune deficiency syndrome (AIDS) has a positive effect in slowing the spread of HIV.
