4.Is fire needle superior to Western medication for herpes zoster? A systematic review and meta-analysis

Objective:To compare the effectiveness of fire needle versus Western medicine in the treatment of herpes zoster. Methods:Randomized controlled trials comparing fire needle with Western medicine in the treatment of herpes zoster were identified using 8 databases. A meta-analysis was performed using RevMan 5.3 software. Results:Eight trials involving 569 patients were included in this meta-analysis, and the results showed that fire needle was superior to Western medicine comparing the effective rate [risk ratio (RR)=1.13, 95% confidence interval (CI): 1.06 to 1.20;P=0.0002], the visual analog scale (VAS) score [mean difference(MD)=–7.95, 95% CI: –10.71 to –5.20;P<0.00001], time of pain disappearance (MD=–7.61, 95%CI: –9.38 to –5.84;P<0.00001), time of blister-stop (MD=–1.34, 95%CI: –1.51 to –1.18;P<0.00001), time of crusted scab (MD=–2.92, 95%CI: –3.62 to –2.23;P<0.00001), and time of scab off (MD=–4.64, 95%CI: –5.83 to –3.46;P<0.00001). In addition, a significantly lower incidence of postherpetic neuralgia was found in the fire needle group in 30 d (RR=0.23, 95%CI: 0.11 to 0.51;P=0.0002) and 60 d (RR=0.33, 95%CI: 0.12 to 0.91; P=0.03) after treatment. Conclusion:Fire needle has a favorable effect in increasing the effective rate, relieving pain, recovering skin lesions and decreasing incidence of postherpetic neuralgia in the treatment of herpes zoster. However, considering the limitations in this study, the findings should be interpreted cautiously.
5.Variations in the composition of Fructus Evodiae after processing with Radix Glycyrrhizae extract.

OBJECTIVETo evaluate the changes in the concentrations of five components in Fructus Evodiae used in Chinese medicine, including evodiamine and glycyrrhizic acid, during processing of Fructus Evodiae with Radix Glycyrrhizae extract by using high performance liquid chromatography (HPLC) and to provide a scientific basis for different clinical uses of processed and unprocessed Fructus Evodiae.
METHODSThe concentrations of the Fructus Evodiae components in processed or unprocessed Fructus Evodiae were evaluated by HPLC using a YMC J'sphere ODS-H80 column (4.6 mm×250 mm, 5 μm) with acetonitrile-water-tetrahydrofuran-acetic acid (41:59:1:0.2, v/v/v/v) as the mobile phase. The detection wavelength was 225 nm, the column temperature was 35°C, the flow rate was 1.0 mL/min, and the injection volume was 10 μL. The concentrations of the Radix Glycyrrhizae components were determined by HPLC with a Kromasil-C₁₈ column (4.6 mm×250 mm, 4 μm) and a gradient elution of acetonitrile (A) and 0.05% aqueous phosphoric acid (B) as the mobile phase. The detection wavelength was 237 nm, the column temperature was 35 °C, the flow rate was 1.0 mL/min, and the injection volume was 10 μL.
RESULTSThe calibration curves of evodia lactone, evodiamine, rutaecarpine, liquiritin, and glycyrrhizin showed good linear relationships (r>0.99). The recoveries of evodia lactone, evodiamine, rutaecarpine, liquiritin, and glycyrrhizin were 96.59%, 104.18%, 101.91%, 97.75%, and 97.95%, respectively. The concentrations of the components in processed Fructus Evodiae were obviously different to those in unprocessed Fructus Evodiae.
CONCLUSIONSThe developed method is rapid and accurate. The results provide a reference for processed Fructus Evodiae and the changes that could be expected in its effects compared to unprocessed Fructus Evodiae.

6.Genetic polymorphisms of 19 STR loci in Shandong Han population.

OBJECTIVE:
To investigate the genetic polymorphisms of 19 STR Loci in Shandong Han population in order to provide the genetic data for paternity testing.
METHODS:
The genotypes of 205 unrelated individuals in Shandong Han population were typed by Goldeneye 20A kit to get the allele frequencies and population genetic parameters of 19 STR loci. Four kits, Identifiler kit, SinoFiler kit, PowerPlex 16 kit, and Goldeneye 20A kit, were compared with each other and used in the analysis of a special paternity test case.
RESULTS:
The population genetic parameters of 19 STR loci in Shandong Han Population were obtained. The cumulative discrimination power (CDP) and cumulative probability of exclusion (CPE) ranked from high to low were Goldeneye 20A kit, SinoFiler kit, PowerPlex 16 kit and Identifiler kit, respectively. As duo case, the result of the real case showed that Identifiler kit had no excluding loci, and none of the SinoFiler kit, PowerPlex 16 kit or Goldeneye 20A kit could exclude fatherhood.
CONCLUSION
Compared with Identifiler kit, SinoFiler kit, and PowerPlex 16 kit, Goldeneye 20A kit shows the higher efficiency than the others, but is not completely satisfied for duo cases.

7.Prognostic significance of systemic immune-inflammatory index in patients with acute respiratory distress syndrome

Objective:To evaluate the relationship between systemic immune-inflammatory index (SII) and prognosis of acute respiratory distress syndrome (ARDS).Methods:ARDS patients from the I Medical Information Mart for Intensive CareⅢ were selected as the study objects. Patients were followed up for one year. The primary endpoint was the 30-day mortality rate, and secondary endpoints were the 90-day and one-year mortality rates. Cox proportional hazard regression analysis was used to assess SII as an independent risk factor for ARDS patients, with propensity score matching to control for confounding factors.Results:A total of 723 ARDS patients were included in this study. Patients with SII≥ 3655 had older age, lower SpO 2 levels, and higher simplified acute physiology scoreⅡ (SAPSⅡ) and sequential organ failure assessment (SOFA) scores compared to those with SII <3655. Additionally, the 30-day, 90-day, and one-year mortality rates were higher in patients with SII ≥3655. Cox proportional hazard regression analysis showed that high SII level was an independent risk factor for the prognosis of ARDS at 30 days ( HR=1.68, 95% CI: 1.19-2.36, P=0.0028), 90 days ( HR=1.46, 95% CI: 1.07-1.99, P=0.0170), and one year ( HR=1.34, 95% CI: 1.01-1.77, P=0.0425). Propensity score matching analysis further confirmed the relationship between SII and the prognosis of ARDS patients. Conclusions:SII, as a simple and easily measurable index, is an independent risk factor for the prognosis of ARDS patients.
8.Prevention against and treatment of doxorubicin-induced acute cardiotoxicity by dexrazoxane and schisandrin B.

In this study, it is to compare the effectiveness of prevention against and treatment of doxorubicin (DOX) induced cardiotoxicity by dexrazoxane and schisandrin B (Sch B) in rats. Sprague-Dawley (SD) rats were randomly divided into the following 6 groups: normal saline group, DOX group, DOX+DEX group, DOX+Sch B (80 mg x kg(-1)) group, DOX+Sch B (40 mg x kg(-1)) group and DOX+Sch B (20 mg x kg(-1)) group. The results showed that Sch B could combat the increase of myocardial enzymes in peripheral blood, decrease of the enzyme activity of myocardial tissue antioxidant enzymes and disorders of systolic and diastolic function of heart in rats intravenously injected with doxorubicin (15 mg x kg(-1)). Sch B was better than DEX in protecting rat against DOX-induced the symptoms. Sch B could protect rat against DOX-induced acute cardiomyopathy and has clinical potential applications.

9.Determination of synephrine and N-methyltyramine in Pericarpium Citri Reticulatae viride by HPLC.

OBJECTIVETo establish a quantitative method for determination of synephrine and N-methyltyramine in Citri Reticulatae.
METHODSamples were extracted with 30% methanol. ODS column was used with methanol-water-sodium dodecyl sulfate (55:45:0.1) as mobile phase. Detection wavelength was 285 nm.
RESULTSynephrine and N-methyltyramine in sample solution were well separated. Linearity of synephrine was good (r = 0.9999) in range of 0.35-11.24 microg. The average recovery was 97.1%, and RSD of repeatability was 1.9%.
CONCLUSIONThis method can be used for quality control of Citri Reticulatae.

10.Clinical study on simvastatin treatment of pulmonary hypertension in patients with coal worker's pneumoconiosis.

OBJECTIVETo observe simvastatin treatment of pulmonary hypertension in patients with coal workers pneumoconiosis (CWP).
METHODS96 CWP patients with pulmonary hypertension were randomly divided into treatment and control groups. The control group was treated with 2.5 mg warfarin, once a day for four months; the treatment group was treated with 20 mg simvastatin, taken in evening, for 4 months. 6 min walking distance (6MWD) test and inspection pulmonary artery pressure were measured by echocardiography before and after treatment.
RESULTSIn the treatment group, the 6MWD were (258 ± 26) m after treatment and (225 ± 19) m before treatment, respectively. Compared with control group, pulmonary artery pressure was (41 ± 9) mm Hg in the treatment group before treatment, (36 ± 3) mm Hg in the treatment group after treatment, and (39 ± 5) mm Hg in control group, respectively, the difference was statistically significant (P < 0.05).
CONCLUSIONSSimvastatin can improve pulmonary hypertension in coal workers pneumoconiosis, and shows a definite curative effect.
