Enhanced recovery after surgery (ERAS) is a multimodal perioperative care pathway designed to achieve early recovery for patients undergoing major surgery.The ERAS programs aim to reduce traumatic stress and postoperative complications,improve full recovery of patients or combine and coordinate evidence-based perioperative care interventions that support standardizing and optimizing surgical care.The international working group assembled by the ERAS Society composed the 2014 Consensus guidelines for enhanced recovery after gastrectomy Enhanced Recovery After Surgery (ERAS(@)) Society recommendations in July 2014.This guideline is designed for optimal perioperative care for patients undergoing gastrectomy.This paper presented the understanding of hot issues of the guidelines in order to provide the clinical evidence of evidence-based medicine for the surgical field in China.

Background and purpose:Twist has been identifi ed as tumor metastasis promoter transcription factor and KiSS-1 has been identified as tumor metastasis suppressor gene,and both of them have been identified to be associated with the metastatic potential of non-small cell lung cancer(NSCLC) .Our aim was to identify the expression of both Twist and KiSS-1 in NSCLC and analyze their correlation with patients’ survival.Methods:Immunohistochemical staining using monoclonal Twist and KiSS-1 antibody were performed on paraffi n embedded specimens from 61 patients diagnosed with NSCLC,and 15 specimens of tumor surrounding lung tissue were used as control.The association with clinicopathologic data and prognosis of NSCLC were analyzed.Results:The expression of Twist was significantly higher in NSCLC than in tumor surrounding lung tissue(P

5.A Rapid and Efficient Method for Directed Screening of Lipase-producing Burkholderia cepacia Complex with Organic Solvent Tolerance from Plant Rhizosphere

Lipase from Burkholderia cepacia complex is one of the most versatile biocatalyst and is used widely in many biotechnological application fields including detergent additives, the resolution of racemic compounds, etc. Based on the known whole genomic information of B. cepacia, both ampicillin and kanamycin were added to the TB-T media, the traditional selective media, to screen B. cepacia complex strains from rhizosphere soil samples. The single colonies on the plates with the modified TB-T media were then qualitatively determined the ability to produced the extracellular lipase in the rhodamine B-olive oil agar plates. Thirty-five strains of lipolytic pseudo-B. cepacia complex were isolated and the positive rate of lipolytic bacteria was 65%. Among them, 15 pseudo-B. cepacia complex strains with the tolerance to benzene, n-hexane and n-heptane at the concentration of 10% (V/V) were selected and identified by the recA gene sequence. All of the 15 lipolytic bacteria belonged to the B. cepacia complex.
6.Expression and Clinical Significance of Twist Protein in Non-small Cell Lung Carcinoma

Objective To study the clinical significance and the expression of Twist protein in non-small cell lung cancer(NSCLC).Methods The expression of Twist protein was examined in formalin-fixed paraffin-embedded tissue samples of 61 NSCLC by S-P immunohistochemical method.Results The expression level of Twist in NSCLC was significantly higher than that of normal lung tissue;the positive expression of Twist protein was related to the histotype,clinical stage and lymph node metastasis;the expression level of Twist protein in the cases whose survival time was less than 4 years was significantly higher than that in the cases whose survival time more than 4 years;the Cox analysis revealed that smoking and lymph node metastasis were the noticeable affective factors on the death of the NSCLC patients after surgery.Conclusion s Twist gene played an important role in the genesis and development of NSCLC,and was related to the prognosis.
7.High-density micropulse photocoagulation combined with intravitreal injection of ranibizumab for diabetic macular edema

Objective To investigate the clinical effects of high-density micropulse photocoagulation (HD-SDM) combined with intravitreal injection of ranibizumab for diabetic macular edema (DME).Methods Thirty-one patients (31 eyes) with DME were randomly divided into two groups.Group A (15 eyes) received HD-SDM combined with intravitreal injection of ranibizumab.Group B (16 eyes) only received intravitreal injection of ranibizumab.The best corrected visual acuity (BCVA) and central macular thickness (CMT) of the two groups before and after treatment were analyzed,and the annual injection times of the two groups were compared.Results The average annual injection times was 3.67 ± 1.11 in group A,and 9.12 ±2.63 in group B.The difference was significant between the two groups (t =2.05,P < 0.05).There were significant differences in CMT before and after treatment in both groups (all P < 0.05).There was no significant difference in CMT between the two groups(t =1.19,P > 0.05).There were significant differences in BCVA before and after treatment in both groups (all P < 0.05),but there was no significant difference before and after treatment between the two groups(all P > 0.05).Conclusion Both HD-SDM combined with intravitreal injection of ranibizumab and single intravitreal injection of ranlbizumab are effective for DME,but the combining treatment can remarkably decrease the annual injection times and had a good compliance of patients,is a good choice for DME patients.
8.Fatal case caused by H7N9 avian influenza A virus in Shanghai: a case report and literature review

Objective To understand the epidemiology and clinical features of avian influenza,improve the prophylaxis and treatment.Methods Clinical data of a fatal case caused by H7N9 avian influenza A virus in Shanghai was retrospectively reported and analyzed,literature on avian influenza A virus infection in human was reviewed.Results A severe case of H7N9 avian influenza was reported,with typical clinical characteristics.The epidemiology history of the patient was unclear,all the contacts were tested negative for H7N9 avian influenza A virus.Literature search,H7 subtype of avian influenza in 2012 was only sporadic,the majority of patients with mild symptoms.People did not have immunity against avian influenza.Conclusions Severe case of H7N9 avian influenza progress quickly and its infection pattern is not clear up-to the time point.It needs further exploration and discovery.
10.Use of an enzyme-linked immunospot assay for diagnosing latent tuberculosis infection in rheumatic diseases

Objective To compare the efficacy of the conventional PPD skin test and a new enzymelinked immunospot assay(TSPOT-TB)for diagnosing latent tuberculosis infection(LTBI)in patients with rheumatic diseases.Methods Two hundred and sixty rheumatic patients were enrolled,and all were screened for LTBI based on clinical history,chest X-ray,PPD skin test or TSPOT.Results The positive rate of TSPOT assay was 24.1%and that of PPD skin test was 39.4%.The overall concordance rate between the 2tests was 61.0%.Among PPD negative patients (n=149).29 were TSPOT(+)(19.5%).Among PPD(+)patients(n=98),69 were TSPOT(-)(70.0%).The patients who got BCG vaccination or had history of tuberculosis infection showed a significantly higher rate of positive result of PPD skin test than those who did not (P<0.05 or P<0.01).While in TSPOT assay,the BCG vaccination or history of tuberculosis infection did not show influence on TSPOT results(P>0.05).Of the 127 patients who received biological agents after screening for LTBI,9 patients were pretreated with isoniazide.Twenty-seven patients stopped biological agent treatment because of the positive results of PPD or TSPOT.Twenty three patients who had positive PPD but negative TSPOT results received biological agent treatment without isoniazide,and none of them developed active tubereulosis after 6 to 18 months of follow-up.Conclusion BCG vaccination affects the result of PPD test in rheumatic patients,but has no influence on TSPOT results.The infection rate of latent tuberculosis of rheumatic patients in our research is 23.8%detected by TSPOT.
