1.Quality assessment of iodized salt and a survey of urinary iodine level in Shijiazhuang city from 2004 to 2008

Objective To assess the quality of iodized salt and investigate the urinary iodine level of pupils and women after implementation of universal salt iodization in Shijiazhuang city. Methods Between 2004 and 2008, nine salt samples were randomly collected from wholesale enterprises of 5 different directions of east,south, west, north and center in Shijiazhuang city every month. Meanwhile, eight salt samples, four urine samples of women, were randomly collected in each of the 8 households of 210 villages selected from 23 counties. Between 2004 and 2008, twenty urine samples of 8 - 10 year old pupils were collected in each of 160 schools from the 23 counties. Direct titration method was used for salt iodine determination, iodine concentration in urine was detected by the method of ammonium persulfate digestion-As3+-Ce4 + catalytic spectrophotometry. Results At wholesale level, the qualified rate of iodized salt was above 99%. At household level, the consuming rate of iodized salt was above 95%, and the rate of consuming non-iodized salt was less than 5%. The rate of qualified iodized salt was above 90% in every year except 2004(87.10%), and the rate of consuming qualified iodized salt was above 90% in those years except 2004(83.08%). Urinary median iodine level of both the pupils and the women was higher than 100 μg/L, and the rate of urinary iodine level that less than 50 μg/L was below 10%. Conclusions The quality assessment of iodized salt from wholesale companies and related indices of urinary iodine level of pupils, women and household iodized salt have already reached the national standard for eliminating iodine deficiency disorders.

3.Value of real-time three-dimensional echocardiography in patients with restrictive cardiomyopathy and constrictive pericardits

Objective To investigate value of real-time three-dimensional echocardiography timingexcursion parametric index and 17 segment volume curves index in patients with restrictive cardiomyopathy and constrictive pericardits.MethodsSeventeen patients with restrictive cardiomyopathy (proven by biopsy) ,six patients with constrictive pericardits (proven by CT or surgical),twenty subjects with normal left ventricular(LV) function were examined by Philips iE33 with X3-1 probe.Results Parameter index of Tmsv 16-SD,Tmsv 12-SD Tmsv 6-SD,Tmsv 16-Dif,Tmsv 12-Dif,Tmsv 6-Dif,Tmsv 16-SD(%) ,Tmsv 12-SD(%),Tmsv 6-SD(%),Tmsv 16-Dif(%),Tmsv 12-Dif(%),Tmsv 6-Dif(%) was significantly higher in patients with restrictive cardiomyopathy than that in subjects with normal LV function(all P <0.05).Average and maximum value and minimum of excursion was significantly lower in patients with restrictive cardiomyopathy than that in subjects with normal LV function (all P <0.005).Whereas,compare with subjects with normal LV function,the parametric indexes of timing-excursion and 17 segment volume curves were not significantly difference in patients with constrictive pericardits(all P >0.05).Conclusions Realtime three-dimensional echocardiography can evaluate and diagnose fastly restrictive cardiomyopathy and constrictive pericardits.
4.Evaluation of the hepatic uptake and excretion function with 99mTc-EHIDA hepatobiliary scintigraphy in neonatal intrahepatic cholestasis caused by citrin deifciency

Objectives To investigate the value of hepatobiliary scintigraphy in evaluation of the hepatic uptake and excre-tion function in neonatal intrahepatic cholestasis caused by citrin deifciency (NICCD). Methods Hepatobiliary scintigraphy with SPECT was used to detect the hepatic uptake and excretion function of 12 patients with NICCD conifrmed by SLC25A13 gene analysis, and the results were compared with the results of blood biochemical tests. Results The hepatic uptake and excretion function were obviously impaired in all of 12 NICCD patients in the initial scintigraphy. The scintigraphy were performed again in 5 patients in the follow-up after treatment, and showed that the hepatic uptake and excretion function was recovered. It was sug-gested that the hepatic uptake and excretion function was consistent with the level of liver enzymes and the degree of cholestasis. Conclusions Hepatobiliary scintigraphy is of value in evaluation of the hepatic uptake and excretion function in NICCD patients.

6.Evaluation of the Performance of 7 homogeneous direct reagents for HDL-cholesterol

Objective To evaluate the analytical performance of 7 homogeneous HDL-cholesterol reagents. Methods An altracentrifugation-HPLC method was used as the comparison method. Fourty fresh patient samples were analyzed by homogeneous methods and the comparison method. The homogeneous methods were all performed on a Hitachi 7170A chemistry analyzer according to the manufacturer's instructions, Precision, accuracy and total errors were analyzed. Results The homogeneous assays typically demonstrated within run coefficient variance(CV) of < 1%, and total CV of < 3%. Methods A, B and D showed average bias, bias at the medical decision points and total errors all within the NCEP performance criteria and method C and F unacceptable biases (-19.74% and 11.46%, respectively) and total errors according to the NCEP criteria. However, all the homogenous methods (A-F) had total errors of < 30%, as required by the US Clinical Laboratory Improvement Amendment (CLIA). Conclusions Homogeneous HDL-C assays have been shown to be reasonably precise, but discrepant results have been observed with some of the assays. Clinical laboratories should pay more attention on selecting and validating homogeneous HDL-C reagents.
7.Study on potential effect of Dioscorea nipponica in intervening peripheral system of rats based on metabonomic analysis.

To study the potential effect of Dioscorea nipponica(DN) in intervening peripheral system of rats based on metabolomic analysis. The identification of the potential intervention targets of DN in peripheral system may facilitate its safe application and therapeutic potential exploitation. Totally 20 male SD rats were randomly divided into the blank group and the DN-treated groups, with 10 rates in each group. The DN-treated group was orally administrated with DN extracts once a day for 5 days, with the dose of 80 mg x kg(-1) (equivalent to 15 g crude drug in human), and the blank group was given equal volume of saline once a day for 5 days. Heart, liver, spleen, lung, and kidney tissues and serum samples were collected from each rat 24 h later after the last administration. The ultra-performance liquid chromatography/quadrupole time-of-flight-mass spectrometry based metabolomics was used to investigate the effect of DN in intervening peripheral system of rats. After the treatment with DN, 5 modulated metabolites in heart tissue, 6 in liver tissue, 5 in spleen tissue, 3 in lung tissue, 5 in kidney tissue and 6 in serum sample were identified and considered as the potential intervention targets of DN. Effect of DN in regulating some endogenous metabolites was beneficial for protecting peripheral system, while that in other endogenous metabolites produced potential toxicity to peripheral system. The metabolomic analysis revealed the coexistence of protective and toxic effects of DN on peripheral system, which may be a practical guidance for its safe application and beneficial to the expansion of its application scope.

8.MR perfusion weighted imaging of the swines with experimental chronic pancreatitis

Objective To characterize pancreatic perfusion in swines with experimental chronic pancreatitis(CP)by dynamic contrast-enhanced MRI and to correlate it with the severity of CP.Methods Twenty-two swines were randomly selected and made CP by ligation of main pancreatic duct.Six healthy swines served as control.MR perfusion study with dynamic contrast-enhanced MRl was carried out on the swines under anesthesia.The animals were euthanized after MR perfusion exam.The swines were divided into 4 groups according to the histological results of pancreas:normal group,group of CP of grade 1,group of CP of grade 2,and group of CP of grade 3.The maximum perfusion slope rate,the time-to-peak value and the maximum enhancement rate was compared between the groups using variance analysis,and the relationship between each perfusion parameters and severity of CP was made by correlative analysis.Results Twenty-one swines underwent MR perfusion study,including 7 swines in normal group,7 in group of grade 1 CP,3 in group of grade 2 CP,and 4 in group of grade 3 CP.On signal intensity time curve(SI-T)derived from MR perfusion weighted imaging,the m~imum slope rate was(10.88±1.20)%,(10.59±1.02)%,(6.67±1.31)%,and(5.48±1.97)%for normal swines,swines with grade 1 CP,swines with grade 2 CP and swines with grade 3 CP,respectively(F=20.51,P<0.05).The time-to-peak value was(13.82±3.09)s,(12.31±5.52)s,(20.55±3.79)s and(37.26±14.56)s respectively(F=10.68,P<0.05).The maximal signal enhancement rate was(62.95±20.20)%,(60.44±20.00)%,(46.33±22.70)%,and(67.65±32.66)%,respectively(F=0.53,P>0.05).The highest slope value(r=-0.72,P<0.05)and the time-to-peak(r=0.54,P<0.05)of the SI-T curve were correlated to the severity of CP,respectively.However there was no linear correlation between the maximal signal enhancement rate and the severity of CP.Conclusion MR perfasion weighted imaging has a potential to diagnose and to grade experimental chronic pancreatitis.
9.Application of intraoperative ultrasound in the surgical treatment of hemangioblastoma

Objective To investigate the value of intraoperative ultrasound in the resection of hemangioblastoma.Methods Intraoperative ultrasound was applied in 17 patients who underwent resection. The size,number,location ,depth,feeding arteries and draining veins of the tumors were clearly displayed.Results Intraoperative ultrasound could real-time locate the tumor with high accuracy. Totally there were 35 tumors in 17 patients, and intraoperative ultrasound found 32 of them.The diameter of thr smallest tumor was 7 mm. Intraoperative ultrasound could displayed clearly the feeding arteries and draining veins in bigger tumors. Conclusions Intraoperative ultrasound should be routinely used in the operation of hemangioblastoma for its high detection rate.
10.Construction of a Saccharopolyspora erythraea mutant producing erythromycin B as its major fermentation product

Objective:To obtain a large amount of erythromycin B and to investigate the activity site in eryK. Methods:The key sequence of the BC loop region in eryK gene was knocked out and the eryK gene with 101 bp deleted was amplified by overlapping PCR,and cloned into vector pWHM3 to construct recombinant plasmid. The Saccharopolyspora erythraea mutant AK17 was constructed through chromosomal homologous recombination technique.Results and Conclusions:The S.erythraea mutant AK17 was constructed. The results of TCL and MS analysis showed that the major fermentation product of AK17 is erythromycin B.
