2.Langerhans cell histiocytosis: clinical analysis of 126 cases

Objective To analyze lesional and clinical characteristics of Langerhans cell histiocytosis in children.Methods A clinical retrospective study was performed on 126 patients with Langerhans cell histiocytosis collected from 2006 to 2011 at the Hunan Children's Hospital.Results Of the 126 patients,the youngest was 2months old,and the oldest was 9 years old.The ratio of male to female was 2.5 ∶ 1.Clinical manifestations included eczematid,seborrheic lesions,hemorrhagic maculopapules,yellow nodules and white macules.Of the three clinical phenotypes of Langerhans cell histiocytosis,Letter-Siwe disease was the most prevalent,and most cases of LetterSiwe disease were associated with hepatosplenomegaly,abnormal chest X-ray,impaired hematopoietic function and multifocal bone injuries.The clinical grade was mainly Ⅲ and Ⅳ in patients with Letter-Siwe disease,Ⅰ in patients with eosinophilic granuloma,and varied from Ⅰ to Ⅳ in patients with Hand-Schuller-Christian disease with Ⅱ as the most common.Of these patients,those with eosinophilic granuloma had the oldest average age with bone as the only affected organ,while those with Letter-Siwe disease had the youngest average age with the greatest number of affected organs.The treatment of Langerhans cell histiocytosis included surgical operation and combined chemotherapy.Conclusions Langerhans cell histiocytosis has characteristic skin lesions and diverse clinical manifestations.Pathology has diagnostic significance to Langerhans cell histiocytosis.Therapy strategies and curative effects are dependent on the severity of,and the organs affected by Langerhans cell histiocytosis.
3.Protective effect of nerve growth factor (NGF) from the Chinese cobra venom on spinal neurons after sciatic nerve lesion

AimTo investigate the protective effect of nerve growth factor (NGF) from the Chinese cobra venom on spinal neurons after sciatic nerve lesion. Methods Fourty Wistar rats were divided into 2 groups each containing 20 animals. After surgical crush lesion on the bilateral sciatic nerve, the rats were injected NGF into the target muscle every day. On the d 5 and d 10 after operation, enzyme histochemistry technique was used to show acetycholinesterase(AchE) activity of the motoneurons in L5~S2 spinal cord anterior hornandfluorideresistantacidphophatase (FRAP) activity of substantia gelatinosa in L5~ S2 spinal cord posterior horn. The activity of enzyme was analysed with computer image analysis system. Results Compared with the control group, the activity of FRAP in NGF group was higher on the d 5 and d 10 after operation and the activity of AchE in NGF group was higher on the d 10 after operation. ConclusionIt is demonstrated that NGF could protect the mononeurons in spinal cord anterior horn and the sensory neurons in dorsal root ganglia after axotomy.
4.The application of cardiac operation under direct vision through lower median ministernotomy

Objective To explore the application of cardiac operation under direct vision through Lower median ministernotomy,and to search the new methods to improve the orthodox cardiac operation under direct vision.Methods The method of the cardiac operation under direct vision through lower median ministernotomy was used to 296 cases with heart disease who needed cardiac surgery,including 250 cases with congenital heart disease,40 cases with rheumatic heart disease,5 cases with left atrial myxom and 1 case with coronary heart disease.Results All cases performed the cardiac operation under direct vision through lower Median ministernotomy successfully without death and any residual.Conclusion The cardiac operation under direct vision through lower median ministernotomy can be used in majority cardiac operation under direct vision,it has well surgical and cosmetic result,but cannot be used in treating some patients with complex cardiac deformity or large heart.
5.Histological and enzymohistochemical observation of experimental muscle sprain

Method We used biomechanical methods in controlling strain of anterior tibial muscles of rabbits .Objective To observe the histological and enzymohistochemical change after sprain.Result Experimental model of muscle sprain could be made with yield load which was about 128% of the body weight extracting anterior tibial muscle of the rabbits.Some of the muscle fibers broke near the junction between muscle and the tendon. Conclusion Fibrosis of endomysium and scar formation at the injury might be an important cause of frequent recurrence of muscle sprain.
6.Effect of using laparoscopic trocar for rapidly closed thoracic drainage on serious pneumothorax

Objective To explore a rapid,simple and effective therapy for serious pneumothorax which could be used in pre-hospital and in-hospital first-aid.Methods Sixty-seven patients were randomly divided into the observation group and control group .Patients in the observation group were treated using laparoscopic trocar for rapidly closed thoracic drainage,and patients in the control group were treated by using the traditional large caliber drainage tube and the intercostal incision method of conventional closed thoracic drainage.The operation time,remaining time of drainage,length of stay,effective rate,and complications,including of postoperative pain,hemorrhage,subcutaneous emphysema and infection were observed in both groups. Results The total effective rate was 94.1%(32/34) in the observation group,which was significantly higher than that in the control group(90.9%,30/33)(x2=1.876,P>0.05).No significant difference was found on the remaining time of drainage and length of stay between the two groups(remaining time of drainage:[4.56±1.65]d vs.[6.26±3.45]d;length of stay:(6.0±2.6)d vs.(6.7±2.2)d ,t=1.335 and 0.779,respectively,Ps>0.05).The operation time of using laparoscopic trocar was significantly lower than that of the control group((5.00±1.28)min vs.(15.00±4.03)min,t=3.031,P<0.05).The incision length was(0.95±0.11)cm in the observational group,which was significantly lower than that in the control group((2.41±0.52)cm ,t=2.585,P<0.05).Postoperative pain occurred in 14.7%(5/34) of patients in the observational group,which was significantly lower than that in the control group(87.9%(29/33))(t=2.983,P<0.05).In the observational group no hemorrhage and infection occurred,whereas in the control group the hemorrhage and infection rate was 36%(12/33) and 33%(11/33),respectively(x2=5.880 and 3.687,respectively,Ps<0.05). Conclusion The use of laparoscopic trocar for rapidly closed thoracic drainage in the treatment of serious pneu-mothorax is simple,easy,convenient,effective and reliable,with few complications.This therapy is suitable for using in pre-hospital and in-hospital first-aid.
7.Inferior epigastric artery angiography applied in the transplantation with the deep inferior epigastric perforator free flap

Objective To observe the effect of inferior epigastric artery angiography applied in the transplantation with the deep inferior epigastric perforator free flap.Methods Seven patients who had undergone the deep inferior epigastric perforator free flap transplantation,received angiography of the inferior epigastric artery.The value of the angiography was discussed.Results All patients were successful in angiography without any adverse reaction.Mll patients were successful in transplantation except one because of personal reason.Conclusion Inferior epigastric artery angiography facilitates the transplantation with the deep inferior epigastrie perforator free flap.
8.Total flavonoids of litsea coreana decreases the cytotoxicity of oxaliplatin in TM3 Leydig cells via enhancing the function of gap junction.

OBJECTIVETo investigate the effects of total flavonoids of Litsea Coreana (TFLC) on the gap junction (GJ) intercellular communication in TM3 testicular Leydig cells and whether TFLC can reduce the cytotoxicity of oxaliplatin (OHP) in vitro.
METHODSWe detected the effect of TFLC on the dye spread of the in vitro cultured TM3 cells by parachute assay, observed changes in the expression of connexin 43 (Cx43) total protein in the TFLC-treated TM3 cells by Western blot, and determined the effects of TFLC on the expression of Cx43 on the membrane of the TM3 cells by immunofluorescence assay and on the cytotoxicity of OHP by MTT assay.
RESULTSTFLC obviously enhanced the GJ function with the increasing of the TFLC concentration in the TM3 cells. Western blot and immunofluorescence assay confirmed that TFLC significantly enhanced the expression of Cx43 total protein and Cx43 expression on the membrane of the TM3 cells. MTT assay showed that at a high cell density (confluent with GJ formation), 20 microg/ml TFLC enhanced the GJ function of the TM3 cells and reduced the cytotoxicity of OHP (P < 0.05), while at a low density (preconfluent with no GJ formation), TFLC exhibited no effect on the cytotoxicity of OHP (P > 0.05).
CONCLUSIONTFLC increases the Cx43 expression and GJ function in normal TM3 Leydig cells, and the enhancement of GJ function reduces the cytotoxicity of OHP.

9.Osteogenic action of human osteoblasts and alloplastic decalcified cancellous bone

Objective To study osteogenic courses and features of human osteoblasts cultured in vitro and seeded into alloplastic decalcified cancellous bone.Methods Human osteoblasts isolated from normal human perios- teum were cultured and amplified in vitro.Thy configuration and developing manners of osteoblasts were observed. Then osteoblasts were seeded into alloplastic decalcified cancellous bone.Thy composites were implanted and cultured in thy body of athymic mice.The specimens were obtained 8 weeks later.Osteogenic characteristics were observed by scanning electron microscope.Results Osteoblasts isolated from human periosteum developed well and proliferated rapidly in vitro.Island new bone formation could be observed histologically 8 weeks after the composites of os- teoblasts and alloplasric decalcified cancellous bone were implanted and cultured in the body of athymic mice.Conclu- sion Calcified bone can be formed by seeding osteoblasts into alloplastic decalcified cancellous bone,which may be a new kind of bone graft source to construct bone defects.
10.Clinical performance of hepatitis B surface antigen quantification with high sensitivity chemiluminescent enzyme immunoassay technology

Objective To assess the clinic application of a new-style automated immunoassay system HISCL ( high sensitivity chemiluminescent enzyme immunoassay ) with high sensitive chemiluminescence substrate CDP-Star?, bind-free separate technology and filter conversion technology.Methods The performance verification test evaluated HISCL′s specification including reproducibility, functional sensitivity, linearity, accuracy, and sensitivity for HBsAg seroconversion.To evaluate the specificity , 1 007 HBsAg-negative specimens , 82 potentially interfering specimens were from conventional specimens in West China Hospital , Sichuan University between October and December of 2014.At the same time, with these results to determine the rate of the specimens that their results are in negative grey zone.259 HBsAg-positive specimens and 27 weakly-positive specimens were tested for the comparison between the HISCL and ECLIA ( electrochemiluminescence immunoassay ) HBsAg quantification.A linear regression model , Pearson′s correlation and Bland-Altman analysis were used as statistical methods.Results The between day and within-lot variable coefficient of two level samples are 1.55%, 2.02%, 0.34%, 1.34%separately.The functional sensitivity of the HISCL HBsAg assay reaches 0.007 IU /ml.There is a good linearity (y=3.262x+0.082,r=0.994; y=2 303.608x-33.006,r=0.999) within range of 0-2 300 IU/ml.HISCL obtain the accurate results for the CAP control material , the agreement rate is 100%.The sensitivity for the detection of HBsAg seroconversion is high.The specificity is 99.91%.The negative grey zone rate is 1.66%.The results of these two methods have good correlation and uniformity , r=0.995, LOA:-0.3 -0.19 log10 IU/ml.Conclusions HISCL HBsAg detection system shows good detection performance.Be of the function of both qualitative and quantitative detection.And the negative “grey zone”specimen rate is low.It is wholly capable of wide linearity and quick assays for quantifying serum HBsAg levels.HISCL could contribute to improve HBsAg quantitative detection process for clinical laboratory.
