3.Evaluation of cystatin C with two kinds of detection systems

Objective To evaluate the performance of Cys C results among two detection system.Methods The particle-enhanced immunonephrometic assay was used in Dade Behring BNII. Immunoturbic assay was used in Hitachi 7170 to evaluate the JING' YUAN reagents.We compared the precison,linearity,interference,correlation,and calibrators agreement with Dade Behring BNII.Results The total CV of the samples that contain 0.6-5.0 mg/L was less than 10%.The Dade Behring and JING'YUAN method showed good linearity.Haemoglobin(10 g/L),Bilirubin(300 mg/L), Vitamin C(5 g/L)in the tested sample had no significant interference in the assay(interference 0.05) between JING' YUAN and Dade Behring reagents.Values were slightly lower than that from the Dade Behring BNII method,the mean bias was-0.16.The bias range was 1.1%-23% between JING'YUAN and Dade Behring for one sample.Conclusions The precision,linearity and interference test were suitable for routine Cys C measurement on automated biochemistry analyzer,but results has bias.
4.Study of the difference of the expectations on comprehensive quality of nursing staff

Objective To investigate the differences of the expectations of hospital patients, nurses and doctors on comprehensive quality of nursing staff so as to pertinently improve the comprehensive quality of nursing staff. Methods A total of 260 patients, nurses and doctors were enrolled from a 3A-level hospital. Selfdesigned questionnaires were used to investigate their expectations on the comprehensive quality of nursing staff.Results Hospital patients, nurses and doctors had different expectations on the comprehensive quality of nursing staff. The differences were statistically significant ( F = 19. 720,3. 377,26. 054,10. 076,7. 322 ; P <0. 05). Conclusions There are different expectations from hospital patients, nurses and doctors on the comprehensive quality of nursing staff. Comprehensive quality of nursing staff is patients oriented and should be promoted for the needs of patients.
5.Comparison of serum creatinine,Cystatin C and estimated glomerular filtration rate on evaluation of glomerular filtration function in chronic kidney disease patients

Objective To compare the coherence of serum creatinine,creatinine clearance(Ccr), Cystatin C,and estimated glomerular filtration rate(eGFR)in each stage of chronic kidney disease(CKD) patients.Methods Creatinine in serum and urine were determined by Jaffe method;serum Cystatin C was measured by particle enhanced turbidimetric method,while eGFR was calculated using the abbreviated Modification of Diet in Renal Disease(MDRD)equation which was mainly based on the serum creatinine concentration.According to the American national kidney foundation-Kidney Disease Outcome Quality Initiative(NKF-K/DOQI)guideline,all cases were grouped by eGFR into 5 stages.Results In these 228 cases,as eGFR decreased gradually,the average levels of creatinine and Cystatin C increased,while Ccr decreased.The level of each items showed a statistic difference among each stage(P0.05);in eGFR 60-89 ml/min group,the average level of creatinine was 83.3 ?mol/L,the abnormal rate was only 6.8%,it was not a sensitive marker to detect the slightly damaged GFR,the levels of Ccr and Cystatin C showed a marginal decrease and increase,with an abnormal rates of 70% and 86%,there was a statistic difference among the three abnormal rates(P

7.Pterygium conjunctive reverse transplantation combined with amniotic membrane transplantation on recurrent pterygium

To discuss the effective method of decreasing the postoperative recurrence rate of recurrent pterygium.
●METHODS:Totally 126 cases (126 eyes) with recurrent pterygium were randomly divided into A group (56 cases) and B group ( 70 cases ). Group A was treated by pterygium conjunctive reverse transplantation combined with amniotic membrane transplantation, group B was treated by amniotic membrane transplantation. The followed-up time after surgery was 6-24mo.
●RESULTS:ln group A, postoperative 5-7d (average 5. 62± 1. 38d), cornea epithelium was repaired. ln group B, postoperative 7- 10d ( average 7. 38 ± 1. 12d), the corneal wound was healed. There was statistical significant difference between two groups (t = 4. 307,P<0. 05). Three cases recurrence were noted in A therapeutic group (56 cases), the recurrent rate was 5. 4%; Twelve cases recurrence were noted in B compared group (70 cases), the recurrent rate was 17. 1%. There was statistical significant difference between two groups(P<0. 05).
●CONCLUSlON: lt is suggested that pterygium conjunctive reverse transplantation combined with amniotic membrane transplantation is effective in the treatment of recurrent pterygium.
8.18F-FDG PET/CT in diagnosis of malignant pericardial lesions

Objective To assess the value of integrated 18 F-fluorodeoxyglucose (FDG) PET/CT in differentiation of malignant and benign pericardial effusion. Methods 18F-FDG PET/CT were performed in 23 patients with pericardial effusion. The detected soft tissue tumor or nodulous lession in pericardium or the thickened pericardium, with the maximum standardized uptake value( SUVmax ) ≥2.5, was defined as PET/CT-positive. The invaded lession in pericardium with SUVmax ≥2.5 was also as the positive. The difference of SUVmax of benign and malignant lesions was analyzed with two-independent-sample test of nonparametric tests. The final diagnosis was confirmed by biopsy or post-operative pathology. Results The diagnosis were confirmed with 14 malignant and 9 benign lesions. The median of SUVmax was 6.0 in malignancy group and 2.2 in benign group (z= -3. 279, P =0.001 ). According to the pathology results, there were one false negative case and two false positive cases with PET/CT imaging interpretation. The sensitivity, specificity,accuracy, positive predictive value ( PPV ) and negative predictive value ( NPV ) of 18 F-FDG PET/CT in diagnosis of benignity or malignance of pericardium effusion were 92.9% ( 13/14), 7/9, 87.0% (20/23),86.7% (13/15) and 7/8, respectively. Conclusion For the patients with pericardium effusion 18F-FDG PET/CT may be a helpful modality for malignancy differentiation

Objective To study the association between the plasma homocysteine level and coronary artery disease(CAD), and the methylenetetrahydrofolate reductase (MTHFR) A1298C polymorphism, the methionine synthase (MS) A2756G polymorphism and their associations to the plasma homocysteine level and CAD in the elderly . Methods One hundred and twenty-nine elderly patients with CAD documented by coronary angiogram and 48 elderly patients with normal coronary angiographic results were included in this study. Plasma homocysteine level were measured by fluorescence polarization immunoassay (FPIA) method and polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method was used to analyse the MTHFR A1298C and MS A2756G genotypes. Objeetive To explore the mechanism of drug resistance of New Delhi metallo-β-lactamase-1 (NDM-1) producing Enterobacteriaceae,and to investigate the characteristics of blaNDM-1 carrying plasmid and its gene environment.Methods A total of 48 strains of carbapenem-resistant Enterobacteriaceae were successively collected from six general hospitals in south China during August 2011 and January 2013.Escherichia coli J53 was used for plasmid conjugation.Modified Hodge test was performed,and PCR method was used for the detection of carbarpenase-related genes.The relative molecular mass of the blaNDM-1 carrying plasmid was determined using pulsed field gel electrophoresis (PFGE)assay,and enzyme digestion was performed to investigate the homology and incompatibility group of the plasmid.Clinical feature of blaNDM-1 producing Enterobacteriaceae infection was also investigated.Results Among 48 strains of carbapenem-resistant Enterobacteriaceae,43 were positive in modified Hodge tests.blaVIM,blaGIM and blaSPM genes were negative in all strains,while blaNDM-1 was positive in 19 strains including 3 strains of Escherichia coli,5 strains of Klebsiella pneumoniae,6 strains of Enterobacter cloacae,3 strains of Citrobacterfreundii,1 strain of Klebsiella oxytoca and 1 strain of Providencia rettgeri.All the 19 strains were resistant to imipenem,cefotaxime,ceftazidime,cefepime,aztreonam and piperacillin/tazobactam,47.3% strains were resistant to ciprofloxacin and levofloxacin,but 68.4% strains were sensitive to amikacin.Conjugation experiment showed that,blaNDM-1 carrying plasmids in 13 strains were transmitted to the Escherichia coli J53.The conjugants were resistant to imipenem,ceftazidime,cefotaxime and piperacillin/tazobactam,but were sensitive to amikacin,ciprofloxacin and levofloxacin.All genes in conjugant J-FR90 (Providencia rettgeri) were negative,while the remaining 12 conjugants carried blaNDM-1,blaSHV and aac-(6')-Ib genes.PFGE showed that,the sizes of all blaNDM-1 carrying plasmids were about 50 kb,and more than 80% of their macrorestriction maps were similar.The plasmid belonged to incompatibility group IncX3,and exhibited 100% passage stability after 500 generations of propagation.Among 19 patients infected with NDM-1 producing Enterobacteriaceae,6 died and 13 survived.Conclusions NDM-1 producing Enterobacteriaceae is emerging in south China,and blaNDM-1 is transmitted to Enterobacteriaceae through IncX3.Patients infected with NDM-1 producing Enterobacteriaceae usually have good prognosis.
