1.Clinical utility of mammography,ultrasound and ~(18)F-FDG dual-head coincidence imaging for diagnosis of primary breast cancer

Objective To assess the diagnostic value of mammography,ultrasound and 18F-fluorodeoxyglucose(18F-FDG) dual-head coincidence(DHC) imaging in the detection of primary breast cancer. Methods The results of 54 female patients with 57 breast lesion sites examined by mammography,ultrasound and 18F-FDG dual-head coincidence(DHC) imaging were analysed and compared with pathologic findings.The sensitivity of mammography was compared with combined mammography with ultrasound or triple-tests,and the sensitivity of 18F-FDG DHC imaging was compared with combined mammography and ultrasound. Results The individual sensitivities of mammography,ultrasound and 18F-FDG DHC imaging in the diagnosis of primary breast cancer were 89.13%,91.30% and 91.30%,respectively,those for specificities were 72.73%,72.73% and 63.64%,respectively,and those for accuracies were 85.96%,87.72% and 85.96%,respectively.The sensitivity,specificity and accuracy of combined mammography with ultrasound were 100%,63.64% and 92.98%,respectively,and those of triple-tests were 97.83%,81.82% and 94.74%,respectively.Combined mammography with ultrasound and triple-tests were more sensitive than mammography(P0.05).Triple-tests were more sensitive than combined mammography with ultrasound(P

A new sesquiterpenoid, 8α-hydroxy-6β-methoxy-1-oxoeremophila-7 (11), 9 (10) -diene-12, 8-olide (1) and five known compounds, petasin (2), caffeic acid (3), hepta-cosanol (4), β-sitosterol (5) and β-daucosterol (6) have been isolated from the roots of Ligularia intermedia. The compounds were isolated by column chromatography on silica gel and Sephadex LH-20, and identified based on spectral analyses (MS, 1H-NMR, 13C-NMR).

A new eremophilane derivative, (3aR,4R,5S,7S,7aS)-2-acetyl-7,7a-dihydroxy-3a,4-dimethyl-3a,4,5,6,7,7a-hexahydro-3H-inden-5-yl acetate (1) and three known compounds, 10beta-hydroxy-eremophil-7 (11)-en-12,8alpha-olide(2), beta-sitosterol (3) and beta-daucosterol(4) have been isolated from Ligularia intermedia. 4.To strengthen the prevention and treatment of normocalcemic primary hyperparathyroidism and parathyroid hyperfunction

With the development of various laboratory testing techniques such as serum calcium and parathyroid hormone (PTH) , a large number of asymptomatic or normocalcemic primary hyperparathyroidism (PHPT) can be diagnosed early. PHPT has become the third most common endocrine disease affecting human health. Currently, most PHPT, especially normocalcemic primary hyperparathyroidism, are not primary diseases and may be related to vitamin D deficiency/insufficiency and/or insufficient calcium supplementation. That is, the relative hypocalcemia caused by long-term vitamin D deficiency/insufficiency and/or insufficient calcium supplementation leads to parathyroid hyperfunction, stimulates parathyroid hyperplasia, and secretes excessive parathyroid hormone to compensate for the regulation of calcium and phosphorus balance. When it is in the initial reversible stage, it can be cured by internal medicine; if it progresses freely, long-term hypocalcemia stimulation will lead to excessive parathyroid hyperplasia and even tumor occurrence with the formation of so-called PHPT and parathyroidectomy has to be performed. Therefore, routine screening of bone mineral density, calcium, magnesium, phosphours, 25-hydroxyvitamin D, parathyroid hormone and other bone metabolism indicators in the physical examination of general population is beneficial to the prevention and treatment of bone metabolism diseases, urinary stones and hyperparathyroidism. At the same time, attention should be paid to identifying the stage of prehyperparathyroidism in which vitamin D deficiency/insufficiency and insufficient calcium supplementation will stimulate parathyroid hyperfunction. Active intervention on prehyperparathyroidism is an effective way to avoid the development of primary hyperparathyroidism.

6.Preliminary study of "erythroblast island" in the bone marrow of hematocytopenic patients with positive BMMNC-Coombs test.

OBJECTIVETo explore the mechanism of 'erythroblast island (EI)' formation in the bone marrow of patients with immun-related hemocytopenia (IRP).
METHODSThe category of BM-auto antibody (au Ab) in 48 patients with IRP was detected with FCM. The BM-au Ab in the 'EI' of these cases were explored with immuonhistofluorescence (IF). Clinical and laboratory characteristics of these cases were also analyzed retrospectively.
RESULTSIgG could be detected in the 'EI' on the BM smear of 14 cases (29.17%), BM-au Ab mainly deposited at the edge/membranes between macrophage and erythroblasts rather than cyto plasm. Positive reaction were seen in all the cases with GlycoAIgG. The red blood cell count [(1.8 ± 0.5) × 10(12)/L] and hemoglobin level [(59.6 ± 16.2)g/L] were significantly lower than that in the IF(-) group [(2.5 ± 0.9) × 10(12)/L and (83.4 ± 25.0) g/L] (P < 0.05). The percentage of reticulocyte [(2.0 ± 0.8)%], serum level of IBIL [(9.4 ± 4.7) µmol/L], percentage of erythroblats in sternum BM (0.441 ± 0.139) and response rate to therapy (85.7%) in IF(+) group were significantly higher than that in IF(-)group [(1.3 ± 1.0)%, (6.6 ± 6.7)µmol/L, 0.298 ± 0.082, 61.3%, respectively] (P < 0.05).
CONCLUSIONMacrophage was connected with erythroblasts through autologous IgG in the 'EI's of some patients with IRP. 'EI' were the places where macrophages devoured and destroyed erythroblasts rather than erythroid development and differentiation. The pathogenetic mechanism of IRP might be associated with macrophages phagocytosing and destroying BM hematopoietic cells.

7.Association between circulating vaspin levels and the risk of gestational diabetes mellitus: A systematic review and Meta-analysis

Objective To review the existing literature and quantitatively evaluate the association of circulating vaspin levels and the risk of gestational diabetes mellitus ( GDM) . Methods We systematically searched the PubMed,EMBASE,Web of Science,China National Knowledge Infrastructure,and WanfangData databases up to June 2019. Pooled standardized mean differences ( SMDs) with 95% confidence intervals ( CIs) were calculated using random- or fixed-effects models based on the heterogeneity of studies. Subgroup analyses,Meta-regression,sensitivity and publication bias were assessed to analyze the heterogeneity and the robustness of the results. All statistical analyses were performed using STATA 12.0. Results Nine articles ( 11 comparisons) published from 2013 to 2019 were included in our final Meta-analysis,covering a total of 738 patients with GDM and 661 normal pregnant women. There was significant difference in the overall maternal circulating vaspin levels between GDM patients and healthy pregnant women ( SMD= 0.613,95% CI: 0.044-1.182,P= 0.035) . Subgroup analyses stratified by trimester in which vaspin was measured and whether BMI was matched suggested the similar trend to the overall result. Subgroup analysis according to ethnicity found that circulating vaspin level might not be related to GDM in " European" subgroup; sensitivity analysis by excluding moderate-quality studies and BMI-unmatched studies found that circulating vaspin levels were still related to GDM risk. Conclusions Our Meta-analysis indicated that maternal circulating vaspin levels might be positively correlated with the risk of GDM in Asians.
8.Preoperative psychological nursing of patients in minimally invasive spine surgery in one-day hospital ward

Objective To explore the specific model of preoperative psychological nursing of patients in minimally invasive spine surgery in one-day hospital ward. Methods The characteristics of preoperative psychological nursing of 584 patients in minimally invasive spine surgery in one-day hospital ward were summarized and effective psychological nursing were performed on them. Results Patients under the local anesthesia could initiatively cooperate in the surgery, recognize the outcomes of operations and accept training at home after surgery. Conclusions Good perioperative psychological nursing can effectively promote the surgical safety, help patients to build confidence, improve their initiative in the operation and surgical efficacy, increase patients' satisfaction.
9.HMGB1/SREBP-1 mediated IFN-gamma-induced lipid deposition in mouse mesangial cells.

OBJECTIVETo explore the possible mechanism of lipid deposition induced by interferon-gamma (IFN-gamma).
METHODSThe mouse mesangial cells (MMC) were randomly divided into control group, stimulation group, stimulation + control vector group (sh-HMGB1) and stimulation+ specific sh-vector group (sh-SREBP-1). RT-PCR was used to detect the expression of HMGB1, SREBP-1 and fatty acid synthetase (FAS) mRNA; the protein expression was determined by Western blot.
RESULTSThe Oil Red O staining revealed that the mouse mesangial cells showed significant lipid droplet in IFN-gamma group. IFN-gamma up-regulated the expression of HMGB1, SREBP-1, FAS mRNA and protein time-dependently; Transfection of MMC with HMGB1 siRNA resulted in the suppression of SREBP-1, FAS protein levels induced by IFN-gamma, following with decrease of lipid deposition. Stimulation with HMGB1 markedly induced expression of SREBP-1, FAS expression and peaked at 8 h, decreased at 12 h compared with that at 8 h. Sh-SREBP-1 decreased the lipid deposition induced by HMGB1 in MMC.
CONCLUSIONIFN-gamma might induce lipid deposition in mouse mesangial cells partly by up-regulating the expression of HMGB1/SREBP-1/FAS.

10.Clinical observation and study of mechanisms of needle-picking therapy for primary infertility of abnormal sperm.

OBJECTIVETo search for the best method for increasing clinical therapeutic effect on primary abnormality of sperm.
METHODSOne hundred and sixty-eight cases of infertility were randomly divided into a treatment group of 85 cases and a control group of 83 cases. The treatment group were treated with needle-picking at bilateral Shengzhi points, Dicong Shenjing points and L2 Shenjing points as main. The control group were treated with oral administration of Wuzi Yanzong Pills [symbol: see text]. Their therapeutic effects were observed in 3 hospitals.
RESULTSThe total effective rate of 83.5% and the pregnancy rate of the patient's wife of 78.8% in the treatment group, and corresponding figures were 54.2% and 43.4% in the control group, with a very significant difference between the two groups (P < 0.01); and reproductive hormones improved significantly after treatment in the two groups (P < 0.01); after treatment, superoxide dismulase (SOD) activity and Zn content in semen were elevated and cadmium level decreased significantly in the treatment group (P < 0.05).
CONCLUSIONNeedle picking therapy can significantly improve and regulate endocrine function, increase quality of semen and elevate pregnancy rate of the patient's wife for the patient of primary abnormal sperm.
