Objective To investigate the effect of reinforcing spleen and kidney method on adriamycin-induced CKD in rats and to explore its possible mechanism. Methods Totally 36 Sprague-Dawley rats were randomly divided into a Adriamycin-induced model group and a control group. The model group was further divided into five groups:the Adriamycin-induced model control group,bennazepril-treated group, and TCM treated low, moderate, and high dose groups. The level of serum creatinine, urea nitrogen,24hours urine protein and urine creatinine were measured at 14,28,42 days after establishing the model rats. And the protein expression of transforming growth factor-β1 (TGF-β1) and cyclin-dependent kinase inhibitor p21cip1 (p21)were detected by immunohistochemistry. Results The proteinuria was observed on the seventh day after injection of adriamycin in adriamycin nephropathy model group, and reached summit on the fourteenth day. Both TCM treated groups and benazepril group reduced the level of urine protein within 24 hours (P<0.05),the reduction was most remarkable in the TCM high dose group. The expression of p21 and TGF-β1 (p21 288627.66±97021.65,TGF-β1 98405.14± 19216.89) in kidney increased in the model groups, while the TCM treated high dose group (p21 518886.35±6810.89,TGF-β1 222012.95± 50484.73) was significantly lower than the model control group (P< 0.05).Conclusion Reinforcing spleen and kidney method could decrease the level of urine protein within 24 hours by regulating the expression of p21 and TGF-β1,so thus to protect renal function and delay progress of kidney disease.

Background and purpose:Twist has been identifi ed as tumor metastasis promoter transcription factor and KiSS-1 has been identified as tumor metastasis suppressor gene, and both of them have been identified to be associated with the metastatic potential of non-small cell lung cancer(NSCLC) .Our aim was to identify the expression of both Twist and KiSS-1 in NSCLC and analyze their correlation with patients’ survival. Methods:Immunohistochemical staining using monoclonal Twist and KiSS-1 antibody were performed on paraffi n embedded specimens from 61 patients diagnosed with NSCLC,and 15 specimens of tumor surrounding lung tissue were used as control. The association with clinicopathologic data and prognosis of NSCLC were analyzed. Results:The expression of Twist was significantly higher in NSCLC than in tumor surrounding lung tissue(P

Objective To investigate the emergency treatment and correlation factors of aqueous misdirection during glaucoma and cataract surgery in glaucoma patients.Design Retrospective case series.Participants 126 cases(132 eyes)of angle-closure glaucoma patients with aqueous misdirection during surgery,100 cases(100 eyes)of angle-closure glaucoma patients without aqueous misdirection during surgery and 100 cases(100 eyes)of senile cataract patients. Methods We performed pars plana vitreous water-bag aspiration or anterior vitrectomy on glaucoma patients with aqueous misdirection during surgery. All the factors in observation group and control groups were compared and analyzed by logistic regression.Main Outcome Measures Preoperative highest intraocular pressure(HIOP),intraocular pressure after maximum dose(IOP),anterior chamber depth(ACD),lens thickness(LENS),axial length (AXL),mean keratometry(K),intraocular lens diopter within-0.5D corrected by SRK-T formula(IOLD).Results Among all the 132 angle-closure glaucoma eyes whose posterior chamber pressures were increased during surgery,we performed pars plana vitreous water-bag aspiration in 123 eyes(93.18%),and anterior vitrectomy in 9 eyes(6.8%).Postoperative complications included shallow anterior chamber in 20 eyes(7 eyes with malignant glaucoma,9 eyes with choroidal detachment,4 eyes with delayed local suprachorodal hemorrhage),and vitreous local hemorrhage in 1 eye.Means of all the observation indexes on three groups showed significant difference(all P

Objective To determine the content of astragaloside Ⅳin Qi Zao Granules. Methods Equipped with the evaporative light scattering detector (ELSD), a RP-HPLC method was established. The chromatographic column was C18 and the column temperature was at 30 ℃.The drift tube temperature was at 40 ℃and the evaporative gas was nitrogen with pressure of 3.5 bar.The mobile phase was acetonitrile-water(30∶70) and the flow rate was 1mL/min. Results This method showed a good linear relationship in the range of 2.51~50.2 ?g astragaloside Ⅳ. The average recovery was 98.13 %with RSD being 1.28 %. Conclusion The method is sensitive and accurate and can be used for the quality control of Qi Zao Granules.

Objective To assess regional cerebrovascular reactivity (CVR) and cognition impairment of subcortical ischemic vascular disease (SIVD). To show whether CVR affects cognitive impairment of SIVD patients. Methods Arterial spin-labeling (ASL) by MR image was applied in measuring regional cerebral blood flow (rCBF) of frontal lobe, temporal lobe, parietal lobe, occipital lobes after 5% CO2 inhalation, and the CVR was demonstrated by increase rate of rCBF in all the subject areas. Results (1)The patients with SIVD had reduced CVR in cortex of frontal lobes, white matter of frontal lobes, temporal lobes and occipital lobes(P < 0.05). (2) CVR in the SIVD patients with cognitive impairment decreased in frontal cortex and white matter when compared with the patients without cognitive impairment(P < 0.01). Conclusion These results showed that CVR decreased significantly in cortical gray matter and white matter in elderly patients with SIVD. Also the reduction of CVR in the frontal cortex and white matter was associated with cognitive impairment.

Journal of Clinical Pediatrics 2015;33(5):445-449
Objective To investigate the interventional effect of multidimensional strategy to reduce the incidence of neonatal ventilator-associated pneumonia. Methods The patients who were admitted to the NICU department and received mechanical ventilation (MV) for more than 48 hours from October 2012 to September 2014 were recruited. The control group received the experienced interventions from October 2012 to September 2013, neonates from October 2013 to September 2014 were recruited as the intervention group receiving multidimensional controlling strategy, including bundle care, education, pro-cess and outcome surveillance and feedback on the practices. The compliance of before-after implementation of interventions were quantitatively evaluated, and the rate of VAP was compared between the two groups. Results The compliance rate of hand hygiene and the qualiifed rate of sputum suction, oral care, drain condensation from ventilator circuit, semi-recumbent posi-tion, and preventing of stress-ulcers were increased 17.0%, 11.4%, 14.7%, 18.2%, 37.5% and 56.3% respectively after implemen-tation of multidimensional strategy, and had statistical difference (χ2=36.47-294.36,P<0.01). But the qualiifed rate of antibiotic use only was 66.1% in the post-VAP bundle phases, and showed no statistical difference before and after(P>0.05). The VAP rate was 41.7 cases per 1000 MV-days during control group and 19.7 cases per 1000 MV-days during intervention group, had statis-tical signiifcance (P<0.05). But the rate of ventilator application showed no statistical difference between two group (P>0.05). ConclusionThe multidimensional strategy can effectively prevent the incidence of VAP.
