2.Historical Evolution and Modern Research Overview of Traditional Chinese Medicine Simmering Method

Simmering method is one of the traditional processing methods of Chinese materia medica, which has been documented in the herbal literature and medical books of the past dynasties and has a great variety, but at present, there are not many specific varieties of Chinese materia medica involved, and there are few related researches. By reviewing the ancient and modern related information, the authors have organized and analyzed the historical evolution, processing purpose, modern representative Chinese materia medica(processing technology, quality evaluation, pharmacological research) of simmering method. After sorting out, it was found that the simmering method was widely used in ancient times, which was first seen in Huashi Zhongzangjing of the Eastern Han dynasty, and was enriched and developed through the Tang, Song, Jin, Yuan, Ming and Qing dynasties, and entered its heyday in Ming and Qing dynasties along with the economic prosperity and development of the Ming dynasty, involving as many as 159 varieties of Chinese materia medica, and gradually perfecting the processing theory of the simmering method. However, the number of varieties that still use the simmering method in modern times significantly decreased. The main purposes of using simmering method in modern Chinese materia medica are to reduce adverse reactions, moderate medicinal properties, enhance therapeutic effects, remove non-medicinal parts, and facilitate further processing, etc. This paper combed the key information of simmering methods for Chinese materia medica from ancient to modern times, which can provide a literature basis for the clinical application and modern research of simmered products of Chinese materia medica.

4.Choroidal vascular changes in high myopic patients using ultra-widefield SS-OCTA

Objective:To evaluate the three-dimensional choroidal vascularity index (3D-CVI) and choriocapillaris blood flow density (CCD) changes in patients with high myopia using ultra-widefield swept-source optical coherence tomography angiography (SS-OCTA), and to investigate the relationship between axial length (AL) and choroidal vascular structure changes in high myopia.Methods:A cross-sectional study was performed.A control group of 168 eyes of 168 normal subjects, and a high myopia group of 182 eyes of 182 high myopia patients were enrolled from November 2021 to October 2022 at the Peking University People's Hospital.All subjects underwent an ultra-widefield SS-OCTA scan with an area of 24 mm × 20 mm.Full-field average and central area 3D-CVI and CCD were compared between the two groups.Correlation analysis and receiver operating characteristic curve analysis were used for 3D-CVI, CCD and AL, respectively.The AL cut-off values were determined when 3D-CVI and CCD were reduced.The study followed the Declaration of Helsinki, and the study protocol was approved by the Ethics Committee of Peking University People's Hospital (No.2021PHB058-001).All participants voluntarily signed an informed consent form.Results:The mean 3D-CVI and central 3D-CVI in the high myopia group were (31.37±7.85)% and (29.63±9.00)%, respectively, which were significantly lower than (32.86±4.25)% and (35.01±3.96)% in the control group ( t=2.241, 7.337; both at P<0.05).The mean CCD in the high myopia group was (45.59±3.63)%, which was significantly lower than (46.47±1.36)% in the control group ( t=3.038, P=0.003).Correlation analysis showed that mean 3D-CVI and CCD were negatively correlated with AL in the high myopia group ( r=-0.547, -0.657; both at P<0.001).Receiver operating characteristic curve analysis of 3D-CVI, CCD and AL suggested that the AL cut-off value of mean 3D-CVI-reduction was 28.93 mm and the area under the curve of 0.883 ( P<0.001, sensitivity=0.690, specificity=0.944).The AL cut-off value of mean CCD-reduction was 28.26 mm and the area under the curve was 0.884 ( P<0.001, sensitivity=0.793, specificity=0.832). Conclusions:3D-CVI and CCD are both reduced in patients with high myopia.As AL increases, CCD changes earlier than 3D-CVI.
5.Mechanism of hippocampal CRHR1 regulating chronic stress-induced learning and memory impairment in early aged mice

Objective :
To explore the mechanism of hippocampal corticotropin-releasing hormone ( CRH) receptor type 1 ( CRHR1 ) in chronic stress-induced learning and memory impairment in early aged mice.
Methods:
C57BL /6J mice aged 12 -14 months were divided into two groups according to gender,and then divided into wild type (WT) group and hippocampal CRHR1 conditional gene knockout (KN) group according to genotype.Mice in each group were randomly divided into control group and stress group,and the stress group was subjected to chronic unpredictable stress ( CUS ) for 30 days. Genotyping of mice was performed using polymerase chain reaction ( PCR) ,agarose gel electrophoresis and real-time fluorescence quantitative PCR (RT-qPCR) .The new object rec- ognition experiment and Morris Water maze measured learning and memory ability.Golgi-Cox staining was used to observe damage to hippocampal neuronal dendrites. The protein expressions of target protein of rapamycin (mTOR) ,p-mTOR (Ser2448) ,ribosomal protein S6 kinase ( p70S6K) and p-p70S6K ( Thr389 / Thr412 ) were detected by Western blot.Serum levels of corticotropin releasing hormone ( CRH) were measured by ELISA.
Results :
Compared to mice without chronic stress,the cognitive coefficient of WT stress groups decreased after chron- ic stress,and the difference was statistically significant (P <0. 05) ,while there was no significant difference in cognitive coefficient of KN stress groups before and after chronic stress.Compared with the WT stress group,the escape latency of the WT control group was shortened (P<0. 05) ,and the number of crossing the platform and tar- get quadrant increased (P <0. 01) ,and there was no significant difference in the KN groups above. Compared with the WT control group,the WT stress group had a significant reduction in the neuronal complexity in the hipp- ocampal CA1,CA3 and DG regions (P <0. 05) and significant reductions in the expression of p-mTOR and p- p70S6K in the hippocampus (P<0. 05) .There was no significant difference in the expression of p-mTOR between the KN stress group and the KN control group (P>0. 05) ,except that the expression of p-mTOR in the hippocam- pus of the female group decreased (P<0. 05) .In addition,the serum level of CRH in the stress group was higher than that in the control group (P<0. 01) .
Conclusion
Hippocampal CRHR1 regulates learning and memory im- pairment and neuronal dendrite damage in early aged mice induced by chronic stress.The mechanism may be that high levels of CRH induced by chronic stress cannot bind to CRHR1 receptor,thereby enhancing the expression of down-regulated mTOR / p70S6K signaling pathway.

Objective To investigate the incidence rate,clinical characteristics,and prognosis of neonatal stroke in Shenzhen,China.Methods Led by Shenzhen Children's Hospital,the Shenzhen Neonatal Data Collaboration Network organized 21 institutions to collect 36 cases of neonatal stroke from January 2020 to December 2022.The incidence,clinical characteristics,treatment,and prognosis of neonatal stroke in Shenzhen were analyzed.Results The incidence rate of neonatal stroke in 21 hospitals from 2020 to 2022 was 1/15 137,1/6 060,and 1/7 704,respectively.Ischemic stroke accounted for 75%(27/36);boys accounted for 64%(23/36).Among the 36 neonates,31(86%)had disease onset within 3 days after birth,and 19(53%)had convulsion as the initial presentation.Cerebral MRI showed that 22 neonates(61%)had left cerebral infarction and 13(36%)had basal ganglia infarction.Magnetic resonance angiography was performed for 12 neonates,among whom 9(75%)had involvement of the middle cerebral artery.Electroencephalography was performed for 29 neonates,with sharp waves in 21 neonates(72%)and seizures in 10 neonates(34%).Symptomatic/supportive treatment varied across different hospitals.Neonatal Behavioral Neurological Assessment was performed for 12 neonates(33%,12/36),with a mean score of(32±4)points.The prognosis of 27 neonates was followed up to around 12 months of age,with 44%(12/27)of the neonates having a good prognosis.Conclusions Ischemic stroke is the main type of neonatal stroke,often with convulsions as the initial presentation,involvement of the middle cerebral artery,sharp waves on electroencephalography,and a relatively low neurodevelopment score.Symptomatic/supportive treatment is the main treatment method,and some neonates tend to have a poor prognosis.
7.The prospect and challenges of injectable hydrogel in the treatment of chronic heart failure

Heart failure is the leading cause of mortality in cardiovascular diseases and represents the ultimate common manifestation of most cardiovascular conditions,impacting over 60 million individuals globally.Currently,heart transplantation remains the standard treatment for heart failure patients.Adherence to fundamental pharmacotherapy can improve quality of life and extend survival time for heart failure patients.However,due to the complex mechanism of heart failure and numerous complications,the limitations of conventional heart failure treatment strategies in clinical work are gradually magnified.In recent years,interventional therapy has emerged as an innovative approach for managing heart failure,attracting significant attention and achieving substantial breakthroughs that offer new hope for affected individuals.Injectable hydrogel has garnered considerable interest in biomedicine due to its minimally invasive nature and capacity for efficient therapeutic drug delivery.In the context of chronic heart failure,injectable hydrogel finds application primarily in tissue regeneration,drug delivery,and immunotherapy.This review mainly describes the application and research progress of injectable hydrogel in the treatment of heart failure.
8.Alterations of choroidal vasculature after submacular fluid absorption in central serous chorioretinopathy using ultra-widefield swept-source optical coherence tomography angiography

Objective:To analyze the associations between the choroidal vasculature and submacular fluid (SMF) in central serous chorioretinopathy (CSC).Methods:A retrospective study. A total of 29 CSC patients (31 eyes) with complete records who visited the Department of Ophthalmology in Peking University People's Hospital from August 1, 2021 to March 1, 2023 were included in this study. The patients were divided into complete absorption and incomplete absorption groups according to the status of SMF in the last visit. All the patients underwent ultra-widefield swept-source optical coherence tomography angiography (UWF SS-OCTA) with a scanning range of 24 mm × 20 mm. The UWF SS-OCTA images were automatically analyzed in 9 regions (superotemporal, superior, superonasal, temporal, central, nasal, inferotemporal, inferior, and inferonasal). Alterations of choroidal vasculature in the nine subfields after SMF absorption were described, including choroidal thickness (CT), flow density of choriocapillaris layer, vessel density of large choroidal vessel layer, three-dimensional choroidal vascularity index (CVI), the mean choroidal vessel volume (mCVV), and the mean choroidal stroma volume (mCSV). The relevant factors affecting the complete absorption of SMF were additionally evaluated.Results:At baseline, CT ( Z=2.859, P=0.004), mCVV ( t=2.514, P=0.018), and mCSV ( Z=2.958, P=0.003) in the superotemporal region of the affected eyes in the incomplete absorption group were significantly higher than those in the complete absorption group. Compared with baseline, at the last visit, the proportion of asymmetric vortex veins in the complete absorption group was significantly decreased ( χ2=6.000, P=0.014), CVI in the superotemporal, superonasal, temporal, central, nasal, inferotemporal, and inferonasal regions ( t=-4.125, t=-3.247, Z=-3.213, t=-2.994, t=-3.417, t=-3.733, t=-3.795; P=0.001, 0.006, 0.001, 0.010, 0.005, 0.003, 0.002), the mCVV of 9 regions ( t=-2.959, t=-2.537, t=-2.235, t=-3.260, t=-3.022, t=-2.796, t=-2.747, Z=-2.107, t=-2.935; P=0.011, 0.025, 0.044, 0.006, 0.010, 0.015, 0.017, 0.035, 0.012) were significantly decreased. Compared to the complete absorption group, the choroidal blood flow changes in the non-complete absorption group were more limited, and CT in the upper region increased significantly at the last follow-up ( t=2.272, P=0.037). Multivariate logistic regression analysis revealed that baseline CT in the superotemporal region may be an independent risk factor affecting the complete absorption of SMF (odds ratio=0.981, 95% confidential interval 0.965-0.997, P=0.021). Conclusions:In the process of SMF absorption in CSC, significant reductions of choroidal blood flow were found in the large choroidal vessel layer, and there may be a locally compensatory increase in CT. In addition, baseline CT in superotemporal region is an independent risk factor affecting SMF absorption.
9.Advances in application of optical coherence tomography angiography for quantitative analysis in central serous chorioretinopathy

Central serous chorioretinopathy (CSC) is one of the representative pachychoroid spectrum disease. Although fundus fluorescein angiography and indocyanine green angiography can be used as the gold standard for the diagnosis of CSC, they are invasive examinations, which may bring certain risks in clinical application and cannot help us obtain quantitative parameters. Optical coherence tomography angiography (OCTA), as a non-invasive and quantitative examination, is an important imaging tool for understanding the pathogenesis, diagnosis and treatment of CSC. With the advancement of OCTA, the swept-source OCTA has a satisfying scanning depth, a wider scanning range and a higher resolution. The development of OCTA broadens the horizons of the pathogenesis of CSC, promotes the understanding of the pathophysiology of CSC, and sheds new light for its clinical diagnosis and treatment. Based on OCTA, the choroid and retina in eyes with CSC are presented with qualitative and quantitative changes in vascular system. OCTA-guided CSC treatment and the discovery of prognostic markers based on OCTA challenge the application of traditional imaging techniques in CSC. With the continuous improvement and progress of OCTA technology, traditional angiography combined with OCTA will bring great benefits to the diagnosis and treatment of CSC. This review summarizes the quantitative application of OCTA in the pathogenesis, diagnosis and treatment of CSC.
10.Technical status and development trends of electronic tongue

Electronic tongue is one kind of bionic detection technologies, which can objectively reflect the taste of drugs based on electrochemical principle. In this paper, the development histories of electronic tongue both of potential type and voltammetry type were introduced, including their detection principles and key innovation technologies. In order to comprehensively improve the understanding of electronic tongue, its technological progresses, such as the study of dedicated sensors or biosensors for specific tastes, and the development of miniaturized or hybrid devices, were also discussed in detail. And the challenges and countermeasures in the application of electronic tongue were analyzed to provide some suggestions for its further technology promotion.
